Major Histocompatibility Complex (MHC)

Human leukocyte antigen (HLA)
• โมเลกุล MHC class I จะแสดงอยูบนผิวเซลลที่มีนิวเคลียสทุกเซลลและเกล็ดเลือด ควบคุมดวยยีน locus A, B และ C เพ่ือกําหนดการสราง HLA-A, HLA-B และ HLA-C ตามลําดับ
• โมเลกุล MHC class II จะแสดงอยูเฉพาะบนผิว APC คือ macrophage, dendritic cell และ B cell ควบคุมดวย locus D ซึ่งแบงไดอีกเปน 3 บริเวณยอย คือ DP, DQ และ DR เพ่ือกําหนดการสราง HLA-DP, HLA-DQ และ HLA-DR ตามลําดับ

• Are expressed on all nucleated cells and platelets
• MHC I molecules bind to peptide derived from proteins synthesized within the cells (viral antigens)
• CD8+ cytotoxic T cells can recognize viral peptide only if presented with MHC I molecule,
“Class I MHC restricted”
Class II MHC molecules
• Encoded by HLA-DP, -DQ, -DR gene loci
• Cells normally expressing MHC II are APCs (monocyte, macrophage, dendritic cell), and B cells
• Bind to peptides derived from proteins synthesized outsidethe cell (extracellular microbes, soluble proteins)
• CD4+ helper T cells can recognize peptide only if presented with MHC II molecule,

Natural Killer Cells (NK cells)
• 10-15% of peripheral blood lymphocytes
• large granular lymphocytes
• Contain abundant granules and able to lyse a variety of tumor cells, virally infected cells, and some normal cells without previous sensitization (innate immunity)

• Macrophage กระตุนใหเกิดการตอบสนองแบบ cell-mediated โดยเฉพาะ delayed-type hypersensitivity reaction (type IV)
• โดยเมื่อจับกินและยอย antigen และจะสงใหกับ T cell โดยผานทาง MHC class II จากนั้น T – cell จะตอบสนองโดยการเพิ่มจํานวน จากสารท่ี macrophage หล่ังออกมา (macrophage – derived monokines) ยกตัวอยางเชน IL-1 ที่หลั่งจาก macrophage ชวยเพิ่ม IL-2 receptor ของ T cell ทําใหมีการเพิ่มจํานวน ของ T – cell มากข้ึน

คุณสมบัติท่ัวไปของ cytokine1. Cytokine ชนิดเดียวกัน อาจสรางจากเซลลหลายชนิด เชน IL-1 สรางจาก
เซลลเม็ดเลือดขาว เซลลเย่ือบุหลอดเลือดและ fibroblast
2. Cytokine มีการทํางานแบบ Pleiotropic คือ cytokine ชนิดหนึ่ง อาจออกฤทธ์ิไดหลายอยางในเซลลที่ตางกัน เชน IL-2 มีฤทธิ์ให T cell เจริญเติบโตและยังออกฤทธ์ิตอ B cell ใหเติบโตและเปลี่ยนแปลง นอกจากน้ีพบวา cytokine ตางชนิดกันสามารถออกฤทธิ์เหมือนกัน หรือคาบเกี่ยวกันได เรียกคุณสมบัตินี้วา Redundant
3. Cytokines ออกฤทธ์ิได 3 แบบ คือ • Autocrine effect : IL-2 ที่สรางจาก T cell สามารถกระตุนการ
เจริญเติบโตของตัวเองได • Paracrine effect : IL-7 ท่ีสรางจากไขกระดูก และเซลลค้ําจุน ในตอม thymus มีฤทธิ์ทําใหเซลลตนกําเนิดของ B cell ในไขกระดูกและ
เซลลตนกําเนิดของ T cell ในตอม thymus เจริญเติบโตไดตามลําดับ• Endocrine effect: IL-1 และ TNF ท่ีออกฤทธ์ิใหเกิดไขในระยะการอักเสบ

Figure 5-5 Adaptive immune responses consist of sequential phases: recognition of antigen by specific lymphocytes, activation of lymphocytes (consisting of their proliferation and differentiation into effector cells), and

โรคภูมิคุมกันบกพรองท่ีเกิดข้ึนภายหลัง
• Acquired immunodeficiency syndrome (AIDS)เปนโรคเร้ือรังท่ีมีความรุนแรงถึงตายได สาเหตุเกิดจาก human immunodeficiency virus (HIV) 1 และ 2 โดยอาการของโรคคือ ความบกพรองทางภูมิคุมกันโดยเฉพาะ cellular immunity ผูปวยสวนใหญติดเชื้อ HIV-1 ซ่ึงในระยะตอมาจะพบการติดเช้ือฉวยโอกาส ลักษณะเดนของโรคคือเช้ือ HIV เขาไปใน CD4+ (helper) T lymphocyte ทําใหเซลลถูกทําลายและมีจํานวนลดลงจนเกิดความบกพรองของภูมิคุมกัน เปนผลทําใหผูปวยเสียชีวิตจากการติดเช้ือฉวยโอกาส นอกจากนี้โรคเอดสยังมีความเกี่ยวของกับมะเร็งตอมนํ้าเหลืองและ Kaposi sarcoma

พยาธิกําเนิดของโรคเอดส• เช้ือโรคท่ีเปนสาเหตุของโรคเอดสคือ
HIV-1 ซึ่งเปน enveloped RNA retrovirusท่ีมี reverse transcriptase อยูภายใน สวนของ RNA core ถูกคลุมดวย phospholipid สองช้ันท่ีมี glycoprotein อยูภายในโดยเฉพาะ gp120 และ gp41 แมวา HIV-1 จะสามารถติดเช้ือในเซลลตางๆ เชน B lymphocyte macrophage glial cell และเซลลเย่ือบุผนังลําไสไดก็ตาม แตเซลลเปาหมายคือ CD4+ T lymphocyte ซ่ึงมีผลทําใหจํานวน CD4+ lymphocyte ลดลงเปนอยางมาก

3. Integration : HIV DNA สายคูท่ีถูกสรางข้ึนใหมเขาสูนิวเคลียสของเซลล เปาหมายที่ซึ่งเชื้อ HIV ใชเอนไซม integrase แทรก HIV DNA เขาไปใน DNA ของเซลลเปาหมายรวมเรียกวา provirus ซึ่ง provirus เหลานี้จะคงอยูโดยไมมีการสรางเชื้อ HIV เพิ่มเปนเวลาหลายป
4. Transcription : เมื่อเซลลเปาหมายไดรับสัญญาณใหทํางาน provirus จะใชเอนไซมของเซลลเปาหมายท่ีช่ือวา RNA polymerase เพ่ือสรางสารพันธุกรรมของ HIV และ messenger RNA (mRNA) ซึ่ง mRNA เหลาน้ีใชเปนตนแบบในการสรางโปรตีนสายยาวของเช้ือ HIV

1. Initially, HIV infects T cells and macrophages directly or is carried to these cells by Langerhans cells.
2. Viral replication in the regional lymph nodes leads to viremia and widespread seeding of lymphoid tissue.
3. The viremia is controlled by the host immune response (not shown), and the patient then enters a phase of clinical latency.
4. viral replication in both T cells and macrophages continues unabated. There continues a gradual erosion of CD4+ cells by productive infection (or other mechanisms, not shown).
5. Ultimately, CD4+ cell numbers decline and the patient develops clinical symptoms of full-blown AIDS.

Autoimmunity arises from the inheritance of susceptibility genes that may interfere with self-tolerance, in association with environmental triggers (infection, tissue injury, inflammation) that alter the display of self antigens, promote lymphocyte entry into tissues, and enhance the activation of self-reactive lymphocytes.

โรค Systemic Lupus Erythematosus (SLE)
• สําหรับ autoantibudies ที่สําคัญที่สุดในการวินิจฉัยโรคนี้ คือ – antinuclear antibodies (ANA)– antibody ตอ double–stranded DNA และ Sm antigen
• ซึ่ง Ab เหลาน้ีไมไดทําอันตรายตอเซลลโดยตรง แตเกิดจาก antigen –antibody complex ที่เกิดในกระแสเลือด และยึดเกาะที่เนื้อเยื่อ แลวทําใหเกิดการบาดเจ็บในลักษณะตาง ๆ เชน vasculitis , synovitis และ glomerulonephritis (กรวยไตอักเสบ) จากเหตุผลดังกลาว โรค SLE จึงจัดเปนตัวอยางของ type III hypersensitivity reaction

พยาธิกําเนิดสาเหตุของ SLE น้ันยังไมเห็นชัดเจน ลักษณะเดนของโรคคือ มี autoantibodies
จํานวนมาก โดยเฉพาะ antinuclear antibodies ซ่ึงมีผลทําใหกลไกปกติของระบบภูมิคุมกันเสียไป และเก่ียวของกับหลายปจจัย เชน
1. Hormonal factor พบวา 90% พบในหญิงอายุ 12 – 40 ป และพบวาโรค autoimmune ทุกโรคพบในหญิงมากกวาชายอยางชัดเจน2. Genetic factor พบวาผูปวย autoimmune มีความเก่ียวของกับระบบ MHC เชน ในกรณีของโรค SLE เกี่ยวของกับ DR2 และ DR3
3. Type III hypersensitivityมีสาเหตุท่ีเช่ือไดวาการบาดเจ็บของเน้ือเย่ือในผูปวย lupus เกิดจาก immune complex ท่ีเกิดจากการตอตานตัวเอง โดยเฉพาะตอ DNA

Genetic susceptibility and exposure result in failure of self-tolerance and persistence of nuclear antigens. Autoantibodies serve to internalize nuclear components, which engage TLRs and stimulate IFN production. IFN may stimulate B and T cell responses to the nuclear antigens.

Host-Versus-Graft Reactionลักษณะทางพยาธิของ graft rejection เห็นไดชัดเจนในกรณีของการเปล่ียนถายไต ชนิดของ rejection แบงตามระยะเวลาไดเปน 3 ชนิดดังตอไปน้ี
1. Hyperacute Rejectionเปนการเกิดปฏิกิริยาภายในไมกี่นาทีถึงหลายชั่วโมงหลังการเปลี่ยนถายอวัยวะ
ลักษณะทางคลินิกคือปสสาวะนอยลงอยางกระทันหันพรอมๆกับมีไขและปวดบริเวณไต ซ่ึงในกรณีน้ีตองรีบผาตัดเอาไตน้ันออกอยางรวดเร็ว (เกิดจาก antibody ท่ีมีอยูแลวในกระแสเลือดทําปฏิกิริยากับเน้ือเย่ือของอวัยวะท่ีปลูกถาย)
2. Acute Rejectionเกิดข้ึนภายในไมก่ีสัปดาหหรือเดือนหลังการเปล่ียนอวัยวะ ลักษณะทางคลินิก
คือ พบ azotemia และปสสาวะนอยลงอยางเฉียบพลันซ่ึงอาจมีไขหรือปวดท่ีไตรวมดวย (ทั้ง Humoral และ Cell-mediated immunity มีบทบาทในการเกิดโรค)
