reprimand placed on her board record and was ordered to pay a civil penalty of $1,500 and costs of
investigation of $52.52 because she maintained any place for the practice of esthetics without first having
obtained a license, employed unlicensed persons in the practice of nail technology and/or cosmetology,
and aided and abetted unlicensed activity in the practice of nail technology and/or cosmetology.

Washington County, was suspended for a period of two years, stayed in favor of probation after a period
of thirty days of active suspension retroactive to April 1, 2015, subject to the Board's terms and
conditions; accepted the voluntary surrender of Respondent’s restricted II anesthesia permit No.
DN019018A; and ordered to attend and successfully complete at least six hours of remedial education on
the topic of prescribing and/or pharmacology within one hundred eighty days after approval of the
Consent Agreement, because he committed acts of gross negligence, malpractice or incompetence or
repeated acts of negligence, malpractice or incompetence; engaged in unprofessional conduct by
departing from, or failing to conform to, the standard of acceptable and prevailing dental practice; failed
to maintain a dental record for each patient which accurately, legibly and completely reflects the
evaluation and treatment of the patient, including information with regard to the administration of nitrous
oxide/oxygen analgesia; and did not abide by the duties required of a restricted permit II holder in the
administration of nitrous oxide/oxygen analgesia. (9/11/15)

reasonable skill and safety by reason of addiction, and the respondent violated the board’s final order

suspended for a period of no less than three years retroactive to December 4, 2014, based on findings that
