The major focus of The University of Melbourne Department of Surgery, Austin Health is the development and delivery of high quality surgical education, research and training. The Department provides a comprehensive surgical education program to both undergraduate and postgraduate students in parallel with the clinical surgical training delivered by the members of the Division of Surgery. The department also provides opportunities for scientific research in a wide variety of fields in a warm, friendly and collegial atmosphere. Successful development of Masters programs in education (Master of Surgical Education, Master of Surgical Sciences) further strengthen the departments educational portfolio. Modern facilities and easy access to transport ensure a pleasant working environment.

Welcome Message
Welcome to the General Surgery Education Program for the year 2018. This program is aimed at providing continuing surgical education and training delivered through a series of tutorials, seminars, journal clubs and workshops. It also provides participation to all members of the Division of Surgery through the Surgical Forums and the Annual Surgical Unit Audits. The current level of excellence of this program is predominantly due to the commitment and dedication by the members of the Division of Surgery who continue to volunteer their valuable time for the benefit of our trainees.
The program continues to evolve each year, increasingly passing on a level of responsibility to our trainees. The Austin General Surgical Trainees Education Committee (ASTEC) established in 2014 has continued to progressively take on the responsibility for developing and delivering the program.
Changes have continued to be incorporated in to the program to reduce the demand on trainees and surgeons while concentrating the teaching in to manageable segments. We have endeavoured to limit surgeon involvement to a maximum of one Saturday and one Tuesday for the majority of surgeons.
The Saturday Surgical Seminars held once a month on Saturday afternoons continue to be our flagship education module. Participation of our colleagues from The Northern Hospital has been an enormous success. The development of the Northern Hospital education program under the direction of Krinal Mori complements the program at Austin.
Inviting previous Austin trainees presently working in other centres to participate in the education process has been universally embraced by the invited surgeons. The format continues to change and in 2018 introduces a section on clinical trials and critical publication update.
A paradigm shift in our educational activities took place in 2014 with the introduction of advanced surgical workshops which we continue in 2018 aimed at junior SET and JDOCs.
The program is open to anyone who is interested from the Austin and other centres, including all Surgical Trainees, Unaccredited Registrars, Surgical HMOs, Interns medical students and International Medical Graduates. The Saturday sessions in particular are also open to allied health members.
Thank you to all the members of the Division, Surgeons from The Northern Hospital, External Invited Speakers, Specialty Fellows and Surgical Trainees who participate in the surgical education program and we look forward to another successful year.

Surgical Education Program
INTRODUCTION: All General Surgical Trainees are expected to participate in the Surgical Education Program. This program provides a continuous series of educational activities for trainees at all levels.
SURGICAL EDUCATION CO-ORDINATORS: Two trainees based at the Austin Hospital are chosen to co-ordinate the Surgical Education Program. It is the co-ordinator’s responsibility to communicate with the relevant tutors in advance to confirm the sessions or make alternative arrangements if necessary.
ATTENDANCE: Attendance is strongly encouraged for all SET & NSET trainees based at the Austin Hospital. All trainees including those on regional rotations are expected to attend the Saturday Surgical Seminars. Surgical HMOs, Interns and medical students are also strongly encouraged to participate.
ABSENCE: Trainees who are unable to attend a particular session are expected to inform the Trainee Co-ordinators so that apologies may be noted. If trainees are unable to attend a session where they are expected to present, they must make alternate arrangements.
VIDEO CONFERENCING: The trainees committee (ASTEC) has arranged for the weekly morning tutorials to be video linked to NH. This may expansd to other centres with time.

Information Sources
The Department of Surgery publishes a number of regular publications relating to surgical education, research and training. These are aimed at providing all relevant information to trainees and surgeons alike and encourage participation in the educational and training activities. They may be readily downloaded from the departmental web site or may be requested as hard copies from the departmental office.
Austin Surgery Education Program: This is printed as an A5 sized 44 page full colour booklet for ease of storage. It contains all relevant details about the surgical education program, surgical rosters and details of departmental activities.
All trainees, surgical HMOs and surgeons are provided a copy of the booklet. The elec-tronic version available on the departmental web site is continuously updated.
Pathways to Discovery: This is the flagship publication for the research arm of the department. It contains details of all undergraduate and post graduate research degrees offered by the department along with information on research groups and research projects available surgical train-ees. The document is avalable in hard copy format as well as electronic which can be downloaded from the Department Web Site.
Austin Research Prize Book:This booklet is the official publication for the Annual Austin Surgery Research Prize. It is published with all the abstracts being submitted in that year along with the abstracts of the winners from previous years. This booklet provides a good idea of the quality and breadth of research being presented by general and sub specialty trainees.
LaminasThis is the Austin General Surgery Trainees E-zine which was launched as a quarterly publication with the express purpose of improving communications between our SET and Non-SET trainees as a step towards developing a surgical community at the Aus-tin.

The General Surgery Trainee Representatives have played a major role in liaising between train-ees and their teachers to ensuring the smooth running of the Surgical Education Program and as mentors to junior surgical trainees. In 2013 a formal General Surgery Trainees Education Com-mittee (ASTEC) was established to further facilitate the process of education and learning. The roles and membership were expanded in 2015 as trainees took on more proactive roles in the delivery of surgical training and education. Membership for 2018 is as follows:

Surgery Tutorials

Surgical Forum: Invited Lectures & Annual Audits

INVITED LECTURES (February – July)
This will occur in the first half of the year from the 7th of February to the 18th of July. External speakers have been invited by the department to deliver clinical updates on current topics.
SURGICAL UNIT ANNUAL AUDITS (July – November)
This will be conducted in the second half of the year from the 25th of July to the 28th November. Each unit within the Division of Surgery is allocated a single day for a presentation of their Annual Surgical Audit.
The Austin Surgery Annual Surgical Audits provide an opportunity for all surgical units to present their annual activity, trends, outcomes and benchmarking. The unit members are encouraged to present the figures for the unit activity and the general and specific morbidity and mortality rates for the previous 12 month period. The morbidity should preferably relate to both the operation and the indication if possible (criteria auditing). All deaths should be listed. Detailed discussion of complications and deaths should be avoided in general as they would have been dicussed at the weekly surgical audits unless events led to a change in practice.

Units are encouraged to deliver a 15 minute update on any subject within their subspecialty relating to advances in treatment, changes in the unit activity, new technologies or future directions.
It would be ideal if the heads of units would encourage other participating staff to attend (apart from the unit members) to their unit audits. This may include ward, theatre and anaesthetic nursing staff, as well as staff from other departments such as physiotherapy, nutrition, etc. In addition, members of similar units from other Melbourne hospitals may be invited and encouraged to attend. All units are kindly requested to supply an electronic version of the audit to the Department of Surgery archives. (See Kim McAughtry)

Division of Surgery Weekly Audit

Content: The weekly audit meeting is attended by all units within the Division of
Surgery.
Audit Content: The audits should display the weekly activity of the relevant unit, including
operative, endoscopic and non-operative cases. Morbidity and mortality should be listed and
relevant cases discussed in detail highlighting specific issues and corrective measures.
Audit Format: The surgical audit is presented electronically using the lectern computer
and projector.
Audit Submission: Electronic format of the audit should be sent to Kim McAughtry at the
Department of Surgery (T: 9496 5468; E: [email protected]) to be received latest by
1500hrs on the Wednesday preceding the Audit. These will be uploaded to the computer
in the Lecture Theatre ready for presentation. Late submissions will NOT be uploaded.
Late comers: We ask those who are unable to email their audits to Kim, to kindly arrive 15
minutes early and upload your presentation on to the lectern computer. Uploading it during
the audit presentations is extremely inconsiderate towards the audience.
Multi-Unit Involvement: When specific complications or management issues involve
more than one unit then the presenting unit should contact the other units prior to the
audit to allow time for them to discover and prepare information. This would allow a more
productive audit meeting as the issues will then likely not need to be revisited repeatedly.
Training Component: Although not high on the priorities for surgical trainees the audit
provides an excellent platform on which to hone their oral skills. It would be useful for the
trainees to prepare their presentations so that they are able to focus on the relevant aspects
succinctly and deliver the information as if they were presenting at an oral exam.

Clinical Cases and Surgical Anatomy Discussions

The Friday session have evolved over time and currently will consist of clinical case discussions and Surgical Anatomy sessions. Two of every four Fridays will be clinical case discussions while one in four allocated to Surgical Anatomy. Between April and November the fourth Friday will be free. This will be the Friday before a Saturday Surgical Seminar.
Case Discussions: This is an open format discussion. Trainees are welcome to present a medium case as practice or simply discuss topics of their choice.
Surgical Anantomy: The session will focus on surgical anatomy based on regions. Trainees, especially future exam candidates should prepare in advance.

Austin General Surgery Non-SET Registrar Selection & Interviews
Austin General Surgery NSET Posts: These are designed to be both service and training orientated posts. They will provide you with the required surgical foundation towards entry in the SET program of your choice.
Selection Process:
- Structured curriculum vitae (40%)- Interview (50%)- Independant MWU Reference (10%)- Three referee endorsements (Mandatory requirement but no points)Details are available on the Austin Web Portal.
Selection Interview: Please note that selection interviews will be held on ONLY two specific dates, one at Austin Health and the other at Northern Health as denoted below: Dates: Wednesday 11 July 2018 (NH) & Tuesday 17 July 2018 (AH) Time: 1300 - 1730 Venue: Education precinct
Format: On each of the two specified days there will be three panels for interviews. Each panel will consist of one surgeon, one SET trainee and one member of the MWU. The interview will consist of the following:- one general introductory question (10 points)- one situational management question (20 points)- two clinical management questions (20 points each)- rebuttal of CV (10 points)- global communications score (20 points)This results in a total of 100 points which resolves in to 50% of the total score.
Structured curriculum vitae: Template for submitting the structured CV is be available from the Austin MWU web page along with scoring guidelines.
Attendance at the Saturday Surgical Seminars and Presentation at the Austin Research Prize will score points on the CV.
Independant MWU Reference: You will be scored on a number of non operative skills based on the nine core competencies described by the RACS. The score sheet will be filled out by a member of the relevant MWU of the last hospital you worked in by direct or telephone interview.
Referees Endorsements: Three referees must endorse your application.

This workshop provides a forum to encourage opportunities for surgical research and improve the eligibility of junior doctors for surgical training. It will also become the forum for developing an interest in surgical research and participating in ongoing or new research projects including multi trainee participation by empowering like minded trainees.
Faculty: The faculty is formed by a small group of surgeons with subspecialty interestcovering General Surgery to act as a Research Interest Group.
CV Clinic Panel: These are trainees who are undertaking or have completed full timeresearch.
CV Clinic: JDOCs and NSETs are welcome to register and send in their CVs by email to Kim McAughtry who will distribute it to the panel for feedback. Generic (non identified) feedback will be given at the workshop and individual (private) one-on-one feedback available at the end of the session
Workshop: This will include a variety of presentations, research ideas and the formation of trainee groups to initiate collaborative research. These will expand to four sessions in 2019and become a forum where any surgeon wishing to commence a research project may have their proposed studies presented to allow trainees to sign up.

Victorian consortium for Education, Research, Innovation, Training
and Audit by Surgical trainees (VERITAS)
Trainee led research consortium have developed in Europe and the United States over the past decade. These are independant trainee led organizations demonstrating leadership in conducting prospecting clinical audits and clinical trials. Their development has been very rapid and has now developed an international reach.
In Australia two such trainee led organizations currently exist in Queensland and in New South Wales. They have successfully conducted a limited number of multi centre studies and are collaborating with groups based in the United Kingdom.
The Section of Academic Surgery has supported the development of Clinical Trials Network Australia & New Zealand to provide an overarching support structure for such trainee led groups.
Once the research consortiums are recognized as such contributors as well as authors will be pubmed listed in pubications and be searchable, hence adding value to trainee CVs. The landscape of publlications, research output and recognition of contributions is changing to accommodate multi centre, multi trainee collaborative research which has shown to produce large cohorts of patients previously not achievable.
VERITAS is Victoria’s first trainee led research consortium in General Surgery established with its base at Austin Health. The aim is to foster trainee led, multi centre prospective surgical research and audits by trainees.
VERITAS’ structure will be developed during 2018 through the Research Workshops. VERITAS’ trainee leadership will also be integrated in to ASTEC.
Initial research project will be trialled between Austin and Northern Health and after the pilot study is complete move on to state and national levels. The first study will be discussed at the upcoming Research Workshop on 28 April 2018.

Saturday Surgical Seminars
OPERATIVE SURGERY QUIZ: [First session 60mins; Second session 30mins]This is by far the most valued and sought after session by the trainees. This involves surgeons rapidly questioning trainees coming up for their FRACS exams. Each surgeon will question a different trainee and the trainee will have 5 minutes to answer the question and subsequent follow up questions. At five minutes we move to the next trainee. If the question is answered satisfactorily then the next surgeon asks a new question of the next trainee. If not the old question is passed to the next trainee.
Chair & Convenor will maintain time discipline and move the session along.
SURGICAL SPOTS: [30mins]Presented by a senior registrar, fellow or recent graduand of Austin. This is aimed at mimicking the exam by focussing on a photograph / radiograph and developing a discussion around it. A minimum of six spots should be presented with 5 minutes for each spot and the audience being quizzed by the presenter.
CASE PRESENTATIONS: [30mins]Delivered by Fellows or senior trainees and aimed at provoking discussions with the audience. Usually present three case presentations.
CLINICAL TRIALS & JOURNAL UPDATE: [30mins]The presenter will deliver a review of two items, each 10 mins duration including question time. They should include clinical trials or seminal papers relating to the main topic. The assigned surgeon will do a microteach session on the methodology of the second article.
FROM PREPARATION TO OPERATION: HOW I DO IT: [30mins]In this session the presenter describes his approach to a particular procedure. This includes preoperative preparation, anticipation of difficulties and a description of the entire procedure.
HOW I DO IT: [20mins]Short presentations outlining salient aspects of how the surgeons do it. Preferably with video displays but power point or verbal discussions and drawings are also acceptable.
TIPS & TRICKS: [20mins]This also relates to the main topic and the how I do it presentations. Surgeon should focuson no more than 3 technical or management aspects that deal with the clinical situation.
REFLECTION & SUMMARY: [15mins]Presented by the convenor. An important part of strengthening the learning process and emphasizing the core messages.

VIC TRAINEES WEEKEND 30 NOV - 2 DECRACV RESORT, TORQUAY
The GSA Victorian Trainees weekend will be held at the RACV resort in Torquay.
The weekend will be using a three day format for the second time. Friday will be aimed at Junior SET trainees and introduction to SET for new entrants. The program will transition over Saturday and Sunday progressively towards exam preparation.
Attending two of the three days will be counted as attending the weekend.
At least two attendances at the Trainees Weekend will be a requirement to be accepted in to the FRACS Exam Prep Course the Vic/Tas Training Committee runs in March each year. This will be applicable for those wishing to register fo the 2018 FRACS Course.
There will be no Saturday Surgical Seminar in November in order to reduce the weekend commitments for trainees so that they may attend the Trainees Weekend.

Trainee Co-ordinator: Henry DrysdaleThe aim is to provide interactive simulation and training in selected areas. These will be repeated annually depending on the success and participation by trainees. In 2014 two separate workshops were conducted in the Simulation Centre on level 7 HSB, Austin Health. Building on that success we held five separate simulation workshops in 2015 and consolidated them to four events in 2016 and down to three workshops in 2018 based on feedback and attendance. Note: The dates and workshop topics are all confirmed. However due to unforseen circumstances there may be some changes in the faculty and the content
NOTE ON REGISTRATION: The number of trainees who will be able to participate is limited between 16 - 20 depending on the workshop. We wish to maximise the exposure to our trainees so expect all those who register to attend the session.
INABILITY TO ATTEND: The sponsors contribute significant funding to run these workshops and non-attendance is a waste of resources and prevents another trainee from attending.

LAPAROSCOPIC SUTURE WORKSHOP FOLOW UP PROGRAM
In 2018 two Lap-Tab trainers have been purchased. These simple constructs allow laparoscopic training using an iphone or an iPad. One will be left in the registrar room and the other in the simulation centre and available for trainees to take home for a period of two weeks. They will need to be signed out and then signed in again.
All trainees participating in the work shop will have their ability to suture recorded at the end of the workshop. They will then be expected to undertake a predetermined number of practice sutures and knot tying and email specific episodes to the faculty. This will allow evaluation of proress at each individual trainees’ pace.

MORTAR Simulation Workshop

The scenarios and debrief will focus upon non-technical skills such as establishing leadership and role clarity, calling for help early, effective closed loop communication, and developing a shared mental model. The scope of MORTAR is broad and can be extended to: i) test existing hospital systems (eg. are clinical practice guidelines being followed), ii) trial new initiatives (eg. how effective is a new cognitive aid) , iii) learn from past mistakes (eg. conduct a simulation based upon cases from the Surgical Audit). MORTAR will be run fortnightly on Thursday mornings, from 0750-0830, coinciding with the Surgical Audit for medical staff and the OR in-service for nursing and technical staff. The proposed arrangement is for one surgical registrar to attend MORTAR instead of the weekly audit. Each week, one registrar is assigned to attend. A similar rotational system will be applied to the other members of the OR team, thereby ensuring that, except for once every few months, anaesthesia registrars will still be able to attend the surgical audit, and nursing and technical staff will still be able to attend the OR in-service.
TRAINEE COMMITMENT: These sessions focus on the Non-Technical Core competencies required by the RACS for surgical training. It is important that each of the designated surgical trainees attend the session to ensure the success of each of the episodes. It is your responsibility to ensure that on your assigned date that you have someone else present your units audit or arrange it so you present early in the audit and excuse yourselves to attend the MORTAR session. If you are unable to attend it is your responsibility to swap or find someone who could attend.

Austin Surgery Research Prize
Time: 0900Day: Saturday 08 December 2018 Venue: Lecture Theatre, Department of Surgery, Level 8 LTBSponsorship: Johnson & Johnson Medical Pty Ltd.
Eligibility Criteria: Trainees belonging to a training scheme within the Division of Surgery in ALL surgical specialties ( all SET and Non-SET registrars, Surgical HMOs (PGY2 & 3) as well as Advanced Trainees in Anaesthesia, Radiology, Emergency, Intensive Care & Pathology at Austin Health are invited to submit abstracts relating to original scientific research.
Exclusion Criteria: Anyone not on a surgical or specialty pathway, namely interns and medical students.
Presentation Format: Oral presentation judged by a panel of adjudicators.
Presentation Timings: Each candidate will be allocated a maximum of 15 minutes for their presentation. Candidates should aim to limit their talk to eight minutes after which there will be seven minutes of question time. A warning bell will be rung at eight minutes in to their presentation and a final bell at fifteen minutes to mark the end.
Selection: Selection for presentation will be made based on the submission of “Expressions of Interest” Twelve top ranking submissions will be invited to submit abstracts and present at the Research Prize presentation.
Special Criteria: All General Surgery SET3 trainees and all General Surgery NSET Trainees in their second year or more at Austin are mandated to submit an Expression of Interest.

Prizes: Gold medallion and name on the Honour Board in the department of surgery. All presenters will be awarded certificates of attendance.A participant’s choice award selected by the participating presenters will be awarded to the presenter with most votes.

“Gratias Cena” - Austin General Surgery Trainees Dinner

This is an event hosted by the Austin surgical trainees in appreciation of the education and training given them by the surgeons at the Austin Hospital.
The initial event was held in 2006 and after a hiatus of two years recommenced in 2009. The Austin Trainees conducted a very successful dinner event in December 2010 at Collin’s Kitchen, Grand Hyatt which for the first time included surgeons from many of the attached training hubs. The event moved to “The Residence”, Grand Hyatt, Melbourne in 2011 and 2012 on a Saturday night which resulted on more than 70 attendees. In 2013, the trainees hosted the event at the courtyard and the Mezzanine level of the award winning restaurant “The Italian”. In 2014 it moved to “The Boat House” in Moonee Ponds and returned to the city in 2015 at the “Moon under Water”in Ftizroy and the Taxi restaurant in 2016.
The event provides an informal setting for trainees and surgeons to meet. In particular trainees were able to meet in person some of their consultants for the following year. This event has progressively expanded both in scope and quality and is expected be continued perpetually by the Austin surgical trainees. It also serves as an important time for
for surgical valedictorians, particularly those who have successfully completed their training. Supervisors and surgeons from Austin and Northern hospitals as well as all other metropolitan, regional and interstate hospitals where our trainees rotate to are invited to attend.
