The major focus of The University of Melbourne Department of Surgery, Austin Health is the development and delivery of high quality surgical education and training. The Department provides a comprehensive surgical education program to both undergraduate and postgraduate students in parallel with the clinical surgical training delivered by the members of the Division of Surgery. In addition the department provides opportunities for scientific research in a wide variety of fields in a warm, friendly and collegial atmosphere. Four separate research groups with international repute provide excellent oppportunities for advancement in both basic science and clinical research. Researchers and students have easy access to supervisors and support and close supervision at all times. Modern facilities and easy access to transport ensure a pleasant working environment.

Welcome Message

We wish to welcome all the Surgical Trainees together with all members of the Division of Surgery to the General Surgery Education Program for the year 2010. This program is aimed at providing a comprehensive series of tutorials, seminars and journal club sessions to Surgical Trainees at all levels. It also provides participation to all members of the Division of Surgery through the Surgical Forums and the Annual Surgical Unit Audits. The strength of the program and its success over the past five years has been predominantly due to the commitment and dedication demonstrated by the members of the Division of Surgery who have volunteered their valuable time for the benefit of our trainees. Despite their heavy clinical workload, this support has enabled us to maintain a program of excellence.

The program continues to evolve and improve each year based on the general attendance, impact and feedback received in the past years from both trainees and consultants. The major change for 2010 involves the Surgical Trainee Symposium sessions previously held weekly on Wednesday. This will be replaced by a monthly operative surgery video session and a monthly journal club both held on Tuesday nights. The trainee presentations have been moved to a monthly session held on Saturday afternoons which comprises of an operative surgery quiz session followed by four trainee presentations and ending with case presentation by one of the surgical fellows. This format should enable most non metropolitan trainees to attend these sessions. A new SET1 refresher course has been included to support trainees preparing for the surgical sciences and clinical examinations.

On the 12th of November 2009 an informal dinner was held by the Austin Surgical Trainees to demonstrate their appreciation of the time and effort imparted to them by the Surgeons and Fellows of the Austin Hospital. This will become an annual feature in the future.

Finally, the Austin Research Prize will be held on Tuesday the 30th of November 2010.

We wish to emphasize that the program is open to anyone who is interested from the Austin and other centres, including all Surgical Trainees, Unaccredited Registrars, Surgical HMOs, Interns and International Medical Graduates. The success of this program is dependant on active participation from all Surgical Trainees and staff members.

Thank you to all the members of the Division who participate in the surgical education program and we look forward to another successful year.

Surgical Education Program

INTRODUCTION: All Surgical Trainees are expcted to participate in the Surgical Education Program for the year 2010. This program provides a continuous series of educational activities for trainees at all levels. Since its inception in 2003, the program has continued to evolve and at present encompasses a wide range of topics that is intended to provide a broad knowledge base.

SURGICAL TRAINEE REPRESENTATIVE: A representative based at the Austin Hospital will be chosen (usually a SET4 trainee) to co-ordinate the Surgical Education Program. It will be the trainee representative’s responsibility to communicate with the relevant tutors a week in advance to confirm the sessions or make alternative arrangements if necessary.

ATTENDANCE: Attendance is MANDATORY for all SET trainees rostered at the Austin Hospital. All trainees including those on regional rotations are expected to attend the Saturday Surgical Seminars. Strict roll call will be maintained. Surgical HMOs (2 & 3), Interns and advanced trainees from other hospitals in Melbourne are also strongly encouraged to participate.

ABSENCE: Trainees who are unable to attend a particular session are expected to inform the Surgical Trainee representative so that apologies may be noted. If trainees are unable to attend a session where they are expected to present, they must make alternate arrangements amongst themselves in consultation with the trainee representative.

SURGICAL TUTORS: Surgeons attached to Austin Health have consistently demonstrated their commitment to the Surgical Education Program and part with their valuable time for the betterment of our trainees. It is imperative that their commitment is recognized and reciprocated by trainees attending all sessions without exception.

Operative Surgery Video Sessions

OPERATIVE SURGERY VIDEO SESSIONS :

Time: 1800 – 1900 Day: Tuesday (Monthly)Venue: Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend BuildingFacilitator: M Nikfarjam

This is a new addition to the surgical education program which aims to utilise the wide availability of digital recoding devices in the operating theatre. Mr Mehrdad Nikfarjam will act as facilitator for the session. The session is composed of two separate sections, a presentation by the trainee followed by a presentation by the invited surgeon.

Trainee presentation: The surgical trainee allocated to the session is expected to present a short video of any surgical procedure he has undertaken recently. This will usually be a laparoscopic procedure. The trainee is expected to take the audience through the procedure while explaining specific areas of the operation.

Examples of appropriate procedures include: laparoscopic appendicectomy •lap cholecystectomy •laparoscopic hernia repair •

It does not matter if the procedure being demonstrated by the trainee has already been shown by another trainee at a previous session. What matters is that each trainee presents a procedure that they have performed. If trainees are able to produce a video clip of an open procedure they have performed then all the better.

Trainees should confer with the facilitator while preparing their videos to determine details such as duration, degree of editing and relevance.

Surgeon presentation: The presentation by the surgeon is on a specific topic. It will be in the form of an operative surgery tutorial with the aid of video clips.

Role of the facilitator: The faciltator will be avilable for discussion with the trainees prior to the session and will help in their preparation. He may also elect to provide additional material in discussion with the allocated surgeon.

Journal Club

JOURNAL CLUB & OPEN FORUM:

Journal Club presentations are given by trainees at all levels of training including SET1 and unaccredited trainees based at the Austin Hospital. The sessions will be held over dinner at Cellini’s Restaurant, courtsey of the attending surgeons.

Each trainee is assigned one journal and one session for each six month rotation. At each Journal Club session three trainees will discuss relevant publications from the past six months from the journal assigned to each of them.

The Journal Clubs in the first half of the year should concentrate on publications from the second half of the previous year (July – December 2009) while those in the second half concentrate on publications from the first half of the current year (January – June 2010).

Surgical Forum

Time: 0730 – 0830 (Light Breakfast at 0715) Day: WednesdayVenue: Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend BuildingContent: The Surgical Forum will be conducted in two parts as described below:

INVITED LECTURES (February – June)

This will be conducted in the first half of the year from the 4th February to the 24th June. Speakers have been invited by the department to deliver clinical updates on current topics. A mix of internal and external speakers havs been chosen based on their expertise.

SURGICAL UNIT ANNUAL AUDITS (July – November)

This will be conducted in the second half of the year from the 1st July to the 25th November. Each unit within the Division of Surgery is allocated a single day for a presentation of their Annual Surgical Audit.

The Austin Surgery Annual Surgical Audits provide an opportunity for all surgical units to present their annual activity, trends, outcomes and benchmarking. The unit members are encouraged to present the figures for the unit activity and the general and specific morbidity and mortality rates for the previous 12 month period. The morbidity should preferably relate to both the operation and the indication if possible (criteria auditing). All deaths should be listed. Detailed discussion of complications and deaths should be avoided as this would have been performed at the weekly surgical audits. However, instances where morbidity or mortality had resulted in a change in practice should be highlighted and discussed if necessary. If available, other data on key performance indicators and comparative data from other comparable hospitals would be appreciated. The duration of the audit meeting should be approximately 45 minutes.

All units are encouraged to supply an electronic version of the audit to the Department of Surgery archives. (See Kim McAughtry)

It would be ideal if the heads of units would encourage other participating staff to attend (apart from the unit members) to their unit audits. This may include ward, theatre and anaesthetic nursing staff, as well as staff from other departments such as physiotherapy, nutrition, etc. In addition, members of similar units from other Melbourne hospitals may be invited and encouraged to attend.

Division of Surgery Weekly Audit

Audit Content: The audits should display the weekly activity of the relevant unit, including operative, endoscopic and non operative cases. Morbidity and mortality should be listed with all UR numbers and relevant cases discussed in detail highlighting specific issues and any suggested corrective measures.

Audit Format: The surgical audit is presented electronically using the lectern computer and projector.

Audit Submission: Electronic format of the audit should be sent to Kim McAughtry at the Department of Surgery (T: 9496 5468; E: [email protected]) to be received latest by 1400hrs on the Wednesday preceding the Audit. These will be uploaded to the computer in the Lecture Theatre ready for presentation. Late submissions will NOT be uploaded.

If you are unable to submit the audit in time, you may bring a copy on USB Memory Stick and physically load it on to the computer on Thursday morning. If you plan to do this, kindly arrive ahead of time (before 7.15am) to load the audit and avoid disrupting the smooth flow and conduct of the audit session.

Division of Surgery Meeting

Saturday Surgical Seminars

SATURDAY SURGICAL SEMINARS:

Each surgical seminar will consist of three parts as described below:1400 – 1430: Operative Surgery Quiz by the panel1430 – 1530: Trainee presentations x 2 1530 – 1600: Afternoon Tea1600 – 1700: Trainee presentations x 2 1700 – 1730: Case presentations by Specialist Surgical Fellow & Discussion

The aim of this format is to concentrate the fortnightly sessions conducted in previous years in to a monthly session on a Saturday afternoon to enable trainees from metropolitan and regional as well as interstate rotations to participate.

Each Surgical Seminar will consist of four presentations presented by SET 1 and 2-5 trainees. The presentations should be approximately 20 minutes each with 10 minutes reserved for questions and discussions.

The aim of each presentation should be to present a concise summary of the topic which should include an overview of current management principles, recent advances and future directions. Controversies in the subject should be discussed and audience participation encouraged. Presenters are encouraged to question trainees in the audience and vice versa. An essential part of the seminar should be a detailed study of the literature encompassing at least the past five years. Please do not attempt to present a text book based account of the subject!

The panel will consist of surgeons with special interest in the topics as well as non surgical specialists where appropriate. Trainees are advised to contact the chairperson of the session well in advance to discuss the direction and focus of the seminar. You should expect to begin preparation at least two to three months in advance.

All trainees except those posted interstate are expected to attend. Given the monthly frequency of the sessions, we expect trainees to be able to adjust their on call commitments in order to attend. Interstate trainees are strongly encouraged to attend where possible.

All non Austin SET trainees are also welcome.

Surgery and Scientific Research: Why do it?Research Opportunities & The Academic Research Program

Research activity has progressively increased in importance to the surgical trainee over the past years as emphasis has increased on a scientific approach to surgical problems. Increasing demands are made on trainees to conduct, present and publish on surgical research not only as a component of their accredited training but also to increase their career opportunities and successful applications for future fellowships. The Department of Surgery continues to strongly support research undertaken by trainees either as full time research higher degrees or on a smaller scale during their clinical tenure.

Trainees at all levels from HMOs to advanced SET trainees are encouraged to seek opportunities early in the year to begin work on at least one research project. If you have a particular interest in a subspecialty, then feel free to approach members of that unit for a potential research project. If not, the Department of Surgery will be able to provide a number of options for small, relevant clinical studies based on our ongoing prospective data collection service. The aim would be to present the results at the Austin Research Prize presentation and subsequently to have it published in a relevant peer reviewed journal.

Research for Surgical Trainees: Surgical trainees interested in an academic career will have strong backing from the department to enrol full time in one of the Research Higher Degrees. In addition we also support all surgical registrars, in both accredited and un-accredited positions, to undertake serious research concurrent with their clinical work. These are mostly small project based clinical studies involving data from our clinical databases, hospital medical records and where necessary, tissue from the Victorian Tissue Bank.

Expectations and Outcomes of Research: The departmental philosophy supports the timely completion of all research undertaken by providing the necessary facilities, mentorship, technical expertise and funding support. In general everyone who undertakes research activities in the department will be expected to present their results at local, national or international meetings and be strongly encouraged to publish in peer reviewed journals. All full time research students are expected to present at the annual Austin Medical Research Week while clinical research undertaken by surgical trainees and junior hospital doctors will be encouraged to be presented at the annual Austin Surgery Research prize.

Research Higher Degree (RHD) Program: The Department of Surgery strongly encourages the undertaking of RHD by research (PhD, MD M Surg and MSc). The main focus of such research is related to hepatobiliary and pancreatic disorders, organ transplantation and regulatory peptides. Students from both the medical and biomedical fields are welcome. There is also strong international interest from regional centres in this program leading to international post doctoral students undertaking this course.

Currently active research projects include the treatment of liver metastases by vascular disrupting agents, macromolecular drugs and hyperbaric oxygen, the role of liver regeneration in tumour growth, the role of microcirculatory disturbances and hyperbaric oxygen in severe acute pancreatitis, xenotransplantation and the role of regulatory peptides in the development of liver, colon, prostate and kidney tumours. In vivo and in vitro models for each of these projects are well established in the Department. Clinical material and tumour banks are extensively used. Details are available in our “Pathways to Discovery” booklet which can be obtained from the office or downloaded from our web site.

Research for Clinicians: The philosophy in the Department of Surgery encourages scientific research at all levels in clinical, basic science and translational aspects. We also endeavour to establish strong links with other departments to develop collaborative research including the Peter McCallum Institute, and clinical departments such as Anaesthesia, Radiology, Oncology and Intensive Care. Detailed surgical databases have been developed for both general surgery and specialist HPB disorders. Data collected within these provide a solid base for developing short term clinical studies. All such projects will aim to have the results published in a peer reviewed clinical journal within a year of completion.

Research for Junior Hospital Doctors: Recognizing the need for small project based clinical studies for junior hospital medical officers (Interns, HMO 2 and 3) we offer a range of clinical and mixed projects. These are available to anyone interested in developing a future career in surgery. They are structured to be completed within an year to enable a full time junior doctor to complete without enrolling for a formal research degree.

Austin Research Prize in Surgery & Anaesthesia

Time: Presentation: 1800Day: Tuesday 30, November 2010 Venue: Lecture Theatre, Department of Surgery, Level 8 LTBSponsorship: Johnson & Johnson Medical Pty Ltd.

Austin Surgery Research Prize: Surgical Trainees in ALL surgical specialties (including all SET trainees, Surgical HMOs and Unaccredited Registrars) as well as Advanced Trainees in Anaesthesia, Radiology, Emergency, Intensive Care & Pathology at Austin Health are invited to submit abstracts relating to original scientific research.

Presentation Format: The candidates will be asked to perform an oral presentation using powerpoint on Wednesday, 25th of November at 1800 and will be judged by a panel of adjudicators.

Presentation Timings: Each candidate will be allocated a maximum of ten minutes for their presentation. Candidates should aim to limit their talk to seven minutes after which there will be three minutes of question time.

A one-minute warning will be given at the candidate at six minutes in to their presentation followed by a warning bell at seven minutes. A second warning bell will sound at ten minutes and the presentation will be halted immediately and the next speaker called.

Prizes: There will be two prizes for the categories of research performed on a fulltime basis and that carried out from a clinical position. The prize will include a gold medallion and an inscription on the Honour Board in the Level 8 Lecture Theatre, Lance Townsend Building.

The gold medallion with ribbon will be presented to the winners by the Johnson & Johnson representative at the end of the proceedings. Subsequently they will have to be returned so that they may be mounted on a special frame as shown.

PREPARATION OF ABSTRACT:

Abstracts should be in Times New Roman size 11 font under the headings Background & Aims, Methods, Results, and Conclusion and should be no more than 500 words. •Background & Aims: Brief description of the background should be followed by a clear statement of aims. If you wish to state a hypothesis it should be mentioned here.•Methods: Relevant methodology must be stated briefly. •Results: Quantitative and/or qualitative data should be presented concisely, with brief statistical analyses where appropriate. •Conclusions: Conclusions should be stated clearly and justified.

University of Melbourne Department of Surgery Web Site

The clinical, educational and research activities of the Austin Department of Surgery is supported and complemented by a comprehensive educational and informative web site. The site contains all the information required for trainees interested in a career in surgery, ranging from the Surgery education program, research opportunities, presentations and statistical tools. Trainees are advised to drop by and browse the content to help them in the many aspects of their future career. Some of the more notable and useful sections of the site are described below:

Education: This section leads to a number of links. Medical undergraduate education lists the departmental commitments as well as links to the lectures in surgery series for medical students. All powerpoint presentations are available as pdf files for download. The Specialist surgical training section details the Surgical Education Program, Austin Research Prize andrResearch pportunities. The CME section leads to the annual HPB symposia which contains all the presentations of the past years.

Research: The basic science section details the three major research groups in the department. The research opportunities section details all the possibilities available for students and graduates from both the science and medical streams.

SSSA: The department inaugurated the Students Surgical Society of Austin in collaboration with The University of Melbourne and the Austin Hospital to encourage students towards a career in surgery.

Events: Links to major events including research related events as well as local, national and international conferences.

Departmental Publications

The Department of Surgery has produces a number of regular annual publications relating to surgical education, research and training. These are aimed at providing all relevant information to trainees and surgeons alike and encourage participation in the educational and training activities. They may be readily downloaded from the departmental web site or may be requested as hard copies from the departmental office.

Pathways to Discovery: This is the flagship publication for the research arm of the de-partment. Printed as an 88 page A4 full colour booklet it con-tains details of all undergraduate and post graduate research degrees offered by the department along with information on research groups and research projects available to science students and surgical trainees to undertake.

Although a large number of research projects are described in the book that is only the tip of the iceberg when it comes to research opportunities.

Austin Surgery Education Program:

This is printed as an A5 sized 40 page full colour booklet for ease of storage. It contains all relevant details about the sur-gical education program, surgical rosters and details of de-partmental activities.

Austin Research Prize Book:

This booklet is the official publication for the Annual Aus-tin Research Prize in Surgery & Anaesthesia. It is published with all the abstracts being submitted in that year along with the abstracts of the winners from previous years.

Below are a few of the conferences and courses that trainees may find useful during the course of the year.

RACS Annual Scientific Congress: 4 - 7 May 2010, Perth, WA.The Annual Scientific Congress (ASC) is the College’s major educational annual activity and is the largest multi-disciplinary surgical meeting held in Australia and New Zealand. It provides Fellows with a valuable opportunity to present their experience, wisdom and opinions to their colleagues in a challenging and truly international forum. The many different scientific programs offer participants the opportunity to learn and experience first hand the advances in surgical specialties. The ASC consists of plenary and concurrent sessions for specialties. Each participating specialty invites an eminent authority as a Distinguished Visitor around whom the specialty program is built. Distinguished Visitors are selected from overseas or within Australia. The ASC is held over four days and provides ample opportunity for inter-disciplinary as well as specialised discussion.

The Dunedin Basic Science Medical Courses: 3 May - 28 May 2010, Dunedin, New ZealandThe Dunedin Basic Medical Sciences Course Trust was set up to support surgical education and research. For many years the Trust has been running annually in Dunedin, a four-week course for candidates taking the FRACS Surgical Sciences Examination (SSE) (formerly the Basic Science Examination). Tutors are members of staff of the Otago School of Medical Sciences and Dunedin School of Medicine, Faculty of Medicine, University of Otago, together with several visiting lecturers. This postgraduate course aims to revise relevant topics in anatomy, physiology and pathology (including immunology, microbiology, pharmacology and statistics), as defined in the textbooks listed by the RACS. Candidates can also take computerised tests, with multiple-choice questions similar in style to those in the FRACS SSE. The Examination is designed to test the candidate’s knowledge, understanding and application of the sciences basic to the practice of surgery.

Victorian Trainees Weekend: 24 - 25 July 2010, Mornington VIC.An annual weekend event conducted by the GSA members of Victoria providing a mix of practical information and surgical teaching in a relaxed environment. Content has previously included presentations by trainees who have complete their FRACS exams as well as fellows presenting on relevant clinical topics.

Victorian Annual General Scientific and Fellowship Meeting : 10-12 September 2010; Victoria. (Contact: [email protected])“Gateway to the North” Bell City, Preston.This is the major annual event held by the Victorian State Committee. It is a professional development event that provides a forum for Fellows to share research and knowledge. It also provides the opportunity for Trainees to fulfil training requirements and present research findings to network with other trainees and senior surgeons of all specialties. The meeting is convened by appointed regional representatives and hosted alternatively between City/Metropolitan and Rural Regional areas. The program includes a ‘formal’ dinner and lecture (GJ Royal Lecture) in memory of Geoff Royal. The program is accredited and contributes towards Continuing Professional Development points for all Trainee and Fellows who attend. Calls for abstract are usually announced early in the year, February or March with a close for submission around mid year to allow for these to be evaluated and offered either a poster or presentation within the program. To honour our Fellows the Victorian State Committee have a number of Awards presented on the last day of event and Prizes also are presented for the presentations and posters which have been judged by a selected panel for the event. Prizes include the D R LESLIE Prize awarded for the best clinical Registrar paper and the R C BENNETT Prize awarded for the best research paper presented at the AGSFM. Winners are presneted with engraved bronze medals. In addition sponsored prizes are also available and may include a Poster Prize or a Case Study Prize rewarding the efforts of trainees.

General Surgeons Australia Annual Scientific Meeting & Trainees Day: 17-19 September 2010; Gold Coast, QueenslandThe annual scientific event of the GSA usually commences with a trainees days followed by the actual scientific meeting. GSA aims to run two Trainees’ days each year, for all General Surgery Trainees. One of these days will be attached to the RACS Annual Scientific Congress (ASC), and the other to the GSA ASM. A module in Transplant / Vascular Surgery was prepared for the RACS ASC in May 2009. Eventually there will be six modules that will rotate over a three year cycle, so that all trainees will have an opportunity to attend all modules. The modules would focus on issues that are not well covered in other fora.

Austin Surgery Trainees Dinner: November 2010 Melbourne, VictoriaThis is an event hosted the Austin surgical trainees in appreciation of the education and training given them by the surgeons at the Austin Hospital. The initial event was held in 2006 and after a hiatus of two years recommenced in 2009. It provides an informal setting for trainees and surgeons to meet as well as allow introduction of new SET trainees in the future.

Location and Access

The Department of Surgery is located in the Lance Townsend (1) and Kronheimer Buildings (13) next to the Austin and Mercy Tower Complexes. Entry is from Burgundy Street (level 3) or from Studley Road (level 1) which is just across the road from Heidelberg railway station. It is just 20 minutes by car or 30 minutes by train / tram from The University of Melbourne, Parkville campus. A large underground car park with four levels services both hospitals and the university precinct. Entry can be gained from Studley Road (level B3) or Burgundy Street (level B2).
