The major focus of The University of Melbourne Department of Surgery, Austin Health is the development and delivery of high quality surgical education and training. The Department provides a comprehensive surgical education program to both undergraduate and postgraduate students in parallel with the clinical surgical training delivered by the members of the Division of Surgery. In addition the department provides opportunities for scientific research in a wide variety of fields in a warm, friendly and collegial atmosphere. Welcome Message

We wish to welcome all the Surgical Trainees together with all members of the Division of Surgery to the General Surgery Education Program for the year 2009. This program is aimed at providing a comprehensive series of tutorials, seminars and journal club sessions to Surgical Trainees at all levels. In addition, it also provides participation to all members of the Division of Surgery through the Surgical Forums and the Annual Surgical Unit Audits. The strength of the program and its success over the past five years has been predominantly due to the commitment and dedication demonstrated by the members of the Division of Surgery who have volunteered their valuable time for the benefit of our trainees. Despite their heavy clinical workload, this support has enabled us to maintain a program of excellence.

The program for the year 2009 has been altered to a more streamline form, based on the general attendance, impact and feedback received in the past years from both trainees and consultants. In particular the Wednesday evenings Surgical Trainee Symposium sessions will now commence in April and go on till November alternating between the Journal Club and Surgical Trainee Presentations.

Finally, the Austin Research Prize will be held on Wednesday the 25th of November 2009.

We wish to emphasize that the program is open to anyone who is interested from the Austin and other centres, including all Surgical Trainees, Unaccredited Registrars, Surgical HMOs, Interns and International Medical Graduates. The success of this program is dependant on active participation from all Surgical Trainees and staff members.

Surgical Education Program

INTRODUCTION

All Surgical Trainees are expcted to participate in the Surgical Education Program for the year 2009. This program provides a continuous series of educational activities for trainees at all levels. Since its inception in 2003, the program has continued to evolve and at present encompasses a wide range of topics that is intended to provide a broad knowledge base.

SURGICAL TRAINEE REPRESENTATIVE

The HPB Surgery Fellow attached to the HPB+T Unit will co-ordinate the Surgical Education Program. It will be the HPB Fellow’s responsibility to communicate with the relevant tutors a week in advance to confirm the sessions or make alternative arrangements if necessary.

ATTENDANCE

Attendance is MANDATORY for all SET trainees rostered at the Austin Hospital. Surgical HMOs (2 & 3), Interns and advanced trainees from other hospitals in Melbourne are also strongly encouraged to participate.

ABSENCE

Trainees who are unable to attend a particular session are expected to inform the Surgical Trainee representative so that apologies may be noted. If trainees are unable to attend a session where they are expected to present, they must make alternate arrangements amongst themselves in consultation with the HPB Fellow.

Surgeons attached to Austin Health have consistently demonstrated their commitment to the Surgical Education Program and part with their valuable time for the betterment of our trainees. It is imperative that their commitment is recognized and reciprocated by trainees attending all sessions without exception.

Surgical Forum

Time: 0730 – 0830 (Light Breakfast at 0715) Day: WednesdayVenue: Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend BuildingContent: The Surgical Forum will be conducted in two parts as described below:

INVITED LECTURES (February – June)

This will be conducted in the first half of the year from the 4th February to the 24th June. Speakers have been invited by the department to deliver clinical updates on current topics. A mix of internal and external speakers have been chosen based on their expertise.

SURGICAL UNIT ANNUAL AUDITS (July – November)

This will be conducted in the second half of the year from the 1st July to the 25th November. Each unit within the Division of Surgery is allocated a single day for a presentation of their Annual Surgical Audit.

The Austin Surgery Annual Surgical Audits provide an opportunity for all surgical units to present their annual activity, trends, outcomes and benchmarking. The unit members are encouraged to present the figures for the unit activity and the general and specific morbidity and mortality rates for the previous 12 month period. The morbidity should preferably relate to both the operation and the indication if possible (criteria auditing). All deaths should be listed. Detailed discussion of complications and deaths should be avoided as this would have been performed at the weekly surgical audits. However, instances where morbidity or mortality had resulted in a change in practice should be highlighted and discussed if necessary. If available, other data on key performance indicators and comparative data from other comparable hospitals would be appreciated. The duration of the audit meeting should be for approximately 45 minutes.

All units are encouraged to supply an electronic version of the audit to the Department of Surgery archives. (See Kim McAughtry)

It would be ideal if the heads of units would encourage other participating staff to attend (apart from the unit members) to their unit audits. This may include ward, theatre and anaesthetic nursing staff, as well as staff from other departments such as physiotherapy, nutrition, etc. In addition, members of similar units from other Melbourne hospitals may be invited and encouraged to attend.

Surgical Trainee Symposium - Seminars & Journal Club

Time: 1800 – 1900 Day: WednesdayVenue: Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend BuildingContent: The surgical trainee symposium will consist of Surgical Seminars and Journal Clubs occurring on alternating weeks

SURGICAL SEMINARS:

Each Surgical Seminar will consist of two presentations presented by SET 2-5 trainees. The aim is to present a concise summary of the topic which should include an overview of current management principles, recent advances and future directions. Controversies in the subject should be discussed and audience participation encouraged. An essential part of the seminar should be a detailed study of the literature encompassing at least the past five years. Please do not attempt to present a text book based account of the subject! Trainees are advised to contact the chairperson of the session well in advance to discuss the direction and focus of the seminar. You should expect to begin preparation at least two to three months in advance.

JOURNAL CLUB:

Journal Club presentations are given by the five SET 2-5 trainees based at the Austin Hospital. Each trainee is assigned two journals and two sessions for each six month rotation. At each Journal Club session one trainee will discuss relevant publications from the assigned journal for a six month period. The Journal Clubs in the first half of the year should concentrate on publications from the second half of the previous year (July – December 2008) while those in the second half concentrate on publications from the first half of the current year (January – June 2009).

Division of Surgery Weekly Audit

Time: 0730 – 0830 (Light Breakfast at 0715) Day: ThursdayVenue: Lecture Theatre, Dept of Surgery, Level 8, Lance Townsend BuildingContent: The weekly audit meeting is attended by all units within the Division of Surgery.

Audit Content:

The audits should display the weekly activity of the relevant unit, including surgical and non surgical cases. Morbidity and mortality should be listed with all UR numbers and relevant cases discussed in detail highlighting specific issues and any suggested corrective measures. The period under review will cover Monday to Sunday of the previous week, i.e. Monday 26th January to Sunday 1st February for presentation on Thursday 8th February 2009.

Audit Format:

The surgical audit is presented electronically using the lectern computer and projector.

Audit Submission:

Electronic format of the audit should be sent to Kim McAughtry at the Department of Surgery (T: 9496 5468; E: kimem@unimelb.edu.au) to be received latest by 1400hrs on the Wednesday preceding the Audit. These will automatically be uploaded to the computer in the Lecture Theatre ready for presentation. Late submissions will NOT be uploaded.

If you are unable to submit the audit in time, you may bring a copy on USB Memory Stick and physically load it on to the computer on Thursday morning. If you plan to do this, please have the courtesy to arrive ahead of time (before 7.15am) to load the audit and avoid disrupting the smooth flow and conduct of the audit session.

Electronic Format for Submission:

Beginning 2009 if at all possible we require the surgical audits to be submitted in power point format. This will ease the logistics of uploading and subsequent presentations. Submit the audit and any discussions of morbidity or mortality as a single file and name the file after your parent unit.

If you are unable to submit it as a power point file then a PDF file will be acceptable.

Surgery and Scientific Research: Why do it?Research Opportunities & The Academic Research Program

Research activity has progressively increased in importance to the surgical trainee over the past years as emphasis has increased on a scientific approach to surgical problems. Increasing demands are made on trainees to conduct, present and publish on surgical research not only as a component of their accredited training but also to increase their career opportunities and successful applications for future fellowships. The Department of Surgery continues to strongly support research undertaken by trainees either as full time research higher degrees or on a smaller scale during their clinical tenure.

Trainees at all levels from HMOs to advanced SET trainees are encouraged to seek opportunities early in the year to begin work on at least one research project. If you have a particular interest in a subspecialty, then feel free to approach members of that unit for a potential research project. If not, the Department of Surgery will be able to provide a number of options for small, relevant clinical studies based on our ongoing prospective data collection service. The aim would be to present the results at the Austin Research Prize presentation and subsequently to have it published in a relevant peer reviewed journal.

Research for Surgical Trainees: Surgical trainees interested in an academic career will have strong backing from the department to enrol full time in one of the Research Higher Degrees. In addition we also support all surgical registrars, in both accredited and un-accredited positions, to undertake serious research concurrent with their clinical work. These are mostly small project based clinical studies involving data from our clinical databases, hospital medical records and where necessary, tissue from the Victorian Tissue Bank.

Expectations and Outcomes of Research: The departmental philosophy supports the timely completion of all research undertaken by providing the necessary facilities, mentorship, technical expertise and funding support. In general everyone who undertakes research activities in the department will be expected to present their results at local, national or international meetings and be strongly encouraged to publish in peer reviewed journals. All full time research students are expected to present at the annual Austin Medical Research Week while clinical research undertaken by surgical trainees and junior hospital doctors will be encouraged to be presented at the annual Austin Surgery Research prize.

Research Higher Degree (RHD) Program: The Department of Surgery strongly encourages the undertaking of RHD by research (PhD, MD M Surg and MSc). The main focus of such research is related to hepatobiliary and pancreatic disorders, organ transplantation and regulatory peptides. Students from both the medical and biomedical fields are welcome. There is also strong international interest from regional centres in this program leading to international post doctoral students undertaking this course.

Currently active research projects include the treatment of liver metastases by vascular disrupting agents, macromolecular drugs and hyperbaric oxygen, the role of liver regeneration in tumour growth, the role of microcirculatory disturbances and hyperbaric oxygen in severe acute pancreatitis, xenotransplantation and the role of regulatory peptides in the development of liver, colon, prostate and kidney tumours. In vivo and in vitro models for each of these projects are well established in the Department. Clinical material and tumour banks are extensively used. Details are available in our “Pathways to Discovery” booklet which can be obtained from the office or downloaded from our web site.

Research for Clinicians: The philosophy in the Department of Surgery encourages scientific research at all levels in clinical, basic science and translational aspects. We also endeavour to establish strong links with other departments to develop collaborative research including the Peter McCallum Institute, and clinical departments such as Anaesthesia, Radiology, Oncology and Intensive Care. Detailed surgical databases have been developed for both general surgery and specialist HPB disorders. Data collected within these provide a solid base for developing short term clinical studies. All such projects will aim to have the results published in a peer reviewed clinical journal within a year of completion.

Research for Junior Hospital Doctors: Recognizing the need for small project based clinical studies for junior hospital medical officers (Interns, HMO 2 and 3) we offer a range of clinical and mixed projects. These are available to anyone interested in developing a future career in surgery. They are structured to be completed within an year to enable a full time junior doctor to complete without enrolling for a formal research degree.

Austin Research Prize in Surgery & Anaesthesia

Austin Surgery Research Prize: Surgical Trainees in ALL surgical specialties (including all SET trainees, Surgical HMOs and Unaccredited Registrars) as well as Advanced Trainees in Anaesthesia, Radiology, Emergency, Intensive Care & Pathology at Austin Health are invited to submit abstracts relating to original scientific research.

Presentation Format: The candidates will be asked to perform an oral presentation using powerpoint on Wednesday, 25th of November at 1800 and will be judged by a panel of adjudicators.

Presentation Timings: Each candidate will be allocated a maximum of ten minutes for their presentation. Candidates should aim to limit their talk to seven minutes after which there will be three minutes of question time.

A one-minute warning will be given at the candidate at six minutes in to their presentation followed by a warning bell at seven minutes. A second warning bell will sound at ten minutes and the presentation will be halted immediately and the next speaker called.

Prizes: There will be two prizes for the categories of research performed on a fulltime basis and that carried out from a clinical position. The prize will include a gold medallion and an inscription on the Honour Board in the Level 8 Lecture Theatre, Lance Townsend Building. In addition there will be a $1000 stipend contributing to travel to an international conference.

The gold medallion with ribbon will be presented to the winners by the Johnson & Johnson representative at the end of the proceedings. Subsequently they will have to be returned so that they may be mounted on a special frame as shown.

PREPARATION OF ABSTRACT:

Abstracts should be in Times New Roman size 11 font under the headings Background & Aims, Methods, Results, and Conclusion and should be no more than 500 words. • Background & Aims: Brief description of the background should be followed by a clear statement of aims. If you wish to state a hypothesis it should be mentioned here.• Methods: Relevant methodology must be stated briefly. • Results: Quantitative and/or qualitative data should be presented concisely, with brief statistical analyses where appropriate. • Conclusions: Conclusions should be stated clearly and justified.

ABSTRACT SPECIFICATIONS:

Annual Courses in Hepatobiliary and Pancreatic Surgery

The Annual Courses in Hepatobiliary Surgery programwas conceived as a one day symposium to address the need for a focused educational meeting on the various aspects of HPB surgery. The aim was to provide a forum which addressed issues relating to HPB surgery at various levels including advanced surgical trainees, HPB fellows, practicing general surgeons and specialist HPB / UGI surgeons. The course was first held in 2004 and its success in the first two years led to a joint two day conference incorporating the inaugural annual congress of the newly established ANZHPBA in 2006. In 2007 this expanded to three days with the addition of the ANZGOSA annual conference. The course will continue as a one day symposium on an annual basis.

The course includes notable speakers from the various HPB units across Australasia as well as international speakers. All surgical trainees are strongly advised to attend the symposium as the educational value, particularly towards the FRACS exams is highly valued.

Location and Access

The Department of Surgery is located in the Lance Townsend (1) and Kronheimer Buildings (13) next to the Austin and Mercy Tower Complexes. Entry is from Burgundy Street (level 3) or from Studley Road (level 1) which is just across the road from Heidelberg railway station. It is just 20 minutes by car or 30 minutes by train / tram from The University of Melbourne, Parkville campus. A large underground car park with four levels services both hospitals and the university precinct. Entry can be gained from Studley Road (level B3) or Burgundy Street (level B2).
