Dear Colleague:

It is our pleasure to present the fifth edition of the Department of Pharmacy Practice Annual Report. Our

faculty members are dedicated to serving our students, residents, fellows, department, school, university,

clinical practice sites, patients, and our community. While it is impossible to capture all that we do, this

document briefly highlights the accomplishments of the faculty in our department in the past year.

The vision of our department is to provide an outstanding education to our students and residents, to

further our profession by engaging in scholarly activities and professional organizations, and to contribute

to the health of the patients and communities we serve.

The Department of Pharmacy Practice contributes significantly to the didactic and experiential education

of our students. Our department faculty members are responsible for coordinating 41 courses. These

courses are offered within the School of Pharmacy and other schools within Loma Linda University (LLU).

In addition to School of Pharmacy, our faculty members teach in the LLU Schools of Dentistry, Nursing,

and Allied Health Professions. The past year has also been productive in terms of scholarly activities.

Our faculty members have had 27 posters and 16 manuscripts, abstracts, and book chapters accepted

for publication. In the area of service, our faculty members serve patients in ambulatory care and acute

care settings, both within and outside LLU Health. The department offers several diverse specialties,

including infectious diseases, oncology, cardiology, endocrinology, respiratory, neurology, psychiatry,

internal medicine, critical care, and pediatrics. In addition to participating in teaching, service and

scholarly activities, faculty members are responsible for mentoring Introductory and Advanced Pharmacy

Practice Experience students, PGY1 and PGY2 residents, and fellows. Additionally, our faculty provide

guidance and mentorship to students during numerous community outreach events and other service

activities throughout the year.

Once again, we would like to express our sincere appreciation to our faculty and staff, as well as our

administrators, including Dr. Billy Hughes and Dr. Rashid Mosavin, for providing us with an opportunity to

serve our department. Thank you also to our colleagues at LLU Medical Center, under the leadership of

Dr. Paul Norris and Dr. Norm Hamada, and to our preceptors, for their commitment and dedication.

Loma Linda University School of Pharmacy

Department of Pharmacy Practice BIOGRAPHY Diana X. Cao, PharmD, BCPS Dr. Cao joined Loma Linda University School of Pharmacy (LLUSP) in July of 2012. After receiving her Doctor of Pharmacy degree from the University of Southern California (USC), she completed a general practice residency at the University of California, San Francisco (UCSF), and a specialty residency in cardiology and critical care at USC. Currently, she practices at the Loma Linda University Medical Center (LLUMC) Coronary Intensive Care Unit and serves as a preceptor for students and residents. She also serves as the co-faculty advisor for the LLUSP chapter of California Society of Health System Pharmacists (CSHP)/American Society of Health System Pharmacists (ASHP). Dr. Cao is the coordinator of LLU Post Graduate Year 2 (PGY2) Cardiology Pharmacy Residency Program. Rebecca J. Cheung, PharmD, BCPS-AQ Cardiology Dr. Cheung has been a faculty member at LLUSP since 2005 and was promoted to Associate Professor and Vice Chair of the Department of Pharmacy Practice in 2013. She received her Doctor of Pharmacy degree from the USC School of Pharmacy in 2002 and completed a pharmacy practice residency at Veterans Affairs (VA) Loma Linda Health Care System in 2003. After residency, she completed a cardiovascular pharmacotherapy fellowship through the University of the Pacific (UOP) and VA San Diego Health Care System in 2005. She teaches and coordinates the Integrated Pharmacology and Disease Management (IPDM) III: Cardiovascular Diseases I and Pharmaceutical Care Lab (PCL) III. She is also the coordinator of IPDM and PCL course series. Additionally, she has a Medication Education Clinic at the VA clinic in Rancho Cucamonga. She manages and titrates medications for patients with chronic disease states such as diabetes, hypertension, and dyslipidemia. Naomi R. Florea, PharmD Since joining the LLUSP faculty in 2003, Dr. Florea has been involved in the curriculum development and didactic teaching of Infectious Diseases, Oncology/Transplant and most recently International Medical Missions. Service to the university has included committee work, as well as International Medical Missions to the country of Romania. Her area of clinical practice is in Ambulatory Care, serving as a clinician and a preceptor for the site and LLUSP Advanced Pharmacy Practice Experience (APPE) students. Dr. Florea’s Ambulatory Care rotation is a required, six-week learning experience at Valley View Wellness Medical Center, a private practice led by two family practice physicians and specialists including Gastroenterologists, Anesthesiologists specializing in Pain Management, Orthopedic Surgeons, as well as Podiatry and Nutrition. The average number of patients seen per day per physician ranges from 25-50. She spends three days a week at her practice site. Her clinical responsibilities include: direct patient access and consult services provided on appropriate medication regimens, adverse drug reactions, drug interactions, drug-induced diseases, and regimen adherence of patients seen in clinic, data collection and drug information services. Dr. Florea became the Chair of the Department of Experiential and Continuing Education in March 2012. As Chair, she has worked to establish new rotation sites, develop existing programs and enhance professional development and networking opportunities for students and alumni. Most recently, Dr. Florea has worked to establish a Community Pharmacy Residency Program in collaboration with LLUMC and Pacific Pharmacy Group. Steven C. Forland, PharmD Dr. Forland received his Doctor of Pharmacy degree from UCSF School of Pharmacy in 1974. He completed a post-doctoral fellowship at the State University of New York. Dr. Forland has been with LLUMC since 1975 and is currently the Clinical Pharmacist, Infectious Diseases Specialist for the Medical Center and Director of Infectious Diseases Fellowship.

Norman M. Hamada, PharmD Dr. Hamada received his Doctor of Pharmacy degree from the UOP School of Pharmacy in 1980. He completed a Pharmacy Practice Residency at the Wadsworth VA Hospital in 1981. Dr. Hamada has worked as a staff pharmacist, Clinical Coordinator, and Assistant Director of Clinical Pharmacy Services since joining LLUMC in 1981. He is currently the LLUMC Director of Clinical Pharmacy Services. Dr. Hamada is the Director of the PGY1 Pharmacy Residency Program and the Pharmacy Administration PGY2 Residency Program. Alireza Hayatshahi, PharmD, BCPS After receiving his Doctor of Pharmacy degree from Midwestern University College of Pharmacy-Glendale in 2003, Dr. Hayatshahi completed a general practice residency (ASHP equivalent) at Scottsdale Healthcare in Arizona and a PGY2 infectious diseases specialty residency at State University of New York at Stony Brook Medical Center in New York. He became a board certified pharmacotherapy specialist in 2008. Prior to joining LLUSP, Dr. Hayatshahi was a faculty member at the Tehran University of Medical Sciences (TUMS) School of Pharmacy in the Department of Pharmacy Practice. He also served as Residency Coordinator at TUMS-affiliated Shariati Medical Center. Dr. Hayatshahi’s current clinical practice site is the Internal Medicine Unit of LLUMC. His research interests include antibiotic stewardship and fluids and electrolytes management. He is heavily involved in LLU Inter-Professional Education Collaborative (IPEC) activities. He is coordinating and lecturing at many schools across LLU campus, including Schools of Pharmacy, Dentistry and Allied Health Professions. Lisa Hong, PharmD, BCPS Dr. Hong is joining LLUSP as a faculty member in July of 2015. She received her Doctor of Pharmacy degree from the University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences in 2013 and completed her first year of residency training at the University of Colorado Hospital. She then completed a PGY2 in internal medicine at the University of Utah and became a board certified pharmacotherapy specialist. Dr. Hong will be practicing in the Internal Medicine Unit at LLUMC. She has a strong interest in interdisciplinary education and has conducted research in the area of antimicrobial pharmacokinetics. Jeany Kim Jun, PharmD, MPH, BCACP Dr. Jun received her Doctor of Pharmacy degree from UCSF in 2001 and completed a residency in Primary Care Pharmacy Practice at USC in 2002. She joined the faculty at Western University of Health Sciences (WUHS) College of Pharmacy in 2002 and taught diabetes and asthma until 2010. She joined LLUSP in July 2010, which was where she received her MPH in 1997 from LLU School of Public Health with a double major in Health Administration and International Health. With her combined love for public health and pharmacy, she served in Cambodia from 2010 to 2013 as a medical missionary and pharmacy advisor for a mission hospital. She led medical mission teams to Cambodia and took pharmacy students to the mission field from 2012 to 2014. At LLUSP, she coordinated and taught the IPDM IV Endocrine Course during the Winter Quarters from 2010-2015 and also coordinated IPDM XII Gastrointestinal Disorders & Miscellaneous Conditions course in spring of 2015. Her clinical practice site is at the Social Action Community Health System (SACHS) clinic in San Bernardino where she works in a multidisciplinary setting under a collaborative practice agreement, managing patients with diabetes and hypertension in English and Spanish, and precepting third and fourth year pharmacy students.

Nancy Edman Kawahara, PharmD, MSEd Dr. Kawahara received her Doctor of Pharmacy degree from USC in 1982 and her masters in health professions education from USC in 1986. Before joining LLUSP in 2002, she held faculty positions at WUHS, the University of Illinois at Chicago, and USC. Dr. Kawahara serves in the role of LLUSP Associate Dean for Assessment and Professional Affairs. She coordinates and teaches the introductory course in pharmaceutical care. In addition, Dr. Kawahara has administrative oversight and coordinates the Professional Development sequence that spans the first through third years of the curriculum. Dr. Kawahara has established a sustainable, annual summer volunteer experience at Camp Conrad Chinnock for students interested in learning more about the management of Type1 Diabetes. Lee H. Nguyen, PharmD, BCPS-AQ ID Dr. Nguyen received his Doctor of Pharmacy degree from UCSF School of Pharmacy in 2004. He completed a Pharmacy Practice Residency at UCSF in 2005 and a PGY2 Infectious Diseases Specialty Residency in 2006 at USC. His clinical practice site is St. Jude Medical Center (SJMC) in Fullerton, CA. SJMC is an adult-only acute care hospital with 380+ beds. The patient population is typically older, long-term nursing care patients. Services provided at SJMC include oncology, surgery, maternity, orthopedics, stroke, rehabilitation, emergency medicine, and intensive care. Dr. Nguyen is one of the leaders who developed and co-heads the antimicrobial stewardship program at SJMC. Paul M. Norris, PharmD Dr. Paul Norris has been a part of LLU for more than thirty years. Starting as a staff pharmacist shortly after graduating from UOP in 1980, he is currently the Executive Director of Pharmacy Services and several other clinical departments at LLUMC. He also serves as Associate Dean for Clinical Affairs at LLUSP. Dr. Norris’ support for a School of Pharmacy started fifteen years before the dream became a reality. His persistence in his collaboration with Dr. Behrens to open a School of Pharmacy came to fruition when interviews for the first entering class got underway. His support of the pharmacy program has continued through the years in the form of sponsoring events such as hooding ceremonies, retreats, local outreach efforts, scholarships, teaching, advising student professional organizations and mentoring. In whatever role he serves, Dr. Norris has a passion for motivating students and residents to become leaders in the profession who give back to their communities and make a difference. Dr. Norris’ commitment and passion for service extends beyond local communities and has taken him on several occasions to Sir Run Run Shaw Hospital in China, to support mission work there, to demonstrate and model the important role that pharmacists have as part of the healthcare team and to work with their Children’s Hospital to improve pharmaceutical care. He has also traveled to Trinidad and Guiana as part of a healthcare team, and his department has supported multiple mission trips in the form of donated medications totaling well over $200,000. In his spare time, Dr. Norris enjoys time with his wife, Cynthia, and their 13 grandchildren. Kristine A. Parbuoni, PharmD, BCPS Dr. Parbuoni received her Doctor of Pharmacy degree from the University of Maryland School Of Pharmacy in 2005. She then completed a PGY1 Pharmacy Practice Residency and PGY2 Pediatric Specialty Residency at The Johns Hopkins Hospital in Baltimore, Maryland. Prior to coming to LLU in 2013, Dr. Parbuoni was the Assistant Director of Clinical Pharmacy and Investigational Drug Services at the University of Maryland Medical Center (UMMC) and provided clinical pharmacy services to the Pediatric Intensive Care Unit at the University of Maryland Children’s Hospital. At LLU, Dr. Parbuoni’s clinical site is in the Pediatric Cardiac Intensive Care Unit of the Children’s Hospital. Dr. Parbuoni currently serves as the Residency Program Director for the PGY2 Pediatric Pharmacy Residency and the Residency Academic Coordinator for the Loma Linda University Pharmacy Residency Program. Dr. Parbuoni is actively involved in professional organizations including ASHP, CSHP, and Society of Critical Care Medicine (SCCM). Her research interests include the management of sedation in pediatric critically ill children, pharmacokinetics of antimicrobials, and anticoagulation in pediatric patients.

Tania L. Stewart, PharmD After receiving her Doctor of Pharmacy degree from LLUSP in 2009, Dr. Stewart completed a PGY1 Pharmacy Practice Residency with a focus on Oncology followed by a PGY2 Oncology Pharmacy Specialty Residency at LLU. In 2011, she completed a pilot project investigating the correlation between increased pharmaceutical expenditures and the lack of appropriate documentation/clinical indications for medication use at Loma Linda University Adult Cancer Center (LLUACC). Dr. Stewart’s work resulted in the justification of a clinical pharmacist position at the Cancer Center as well as the development of LLUMC’s first Pharmaceutical Optimization Task Force consisting of specialists from Pharmacy, Medicine, Nursing, Administration, Billing and Registration. She has served as a Clinical Oncology Pharmacy Specialist with a dual role as a Cost-Recovery Pharmacist at LLUACC and has been involved with didactic education at LLUSP in both oncology and religion courses. Dr. Stewart joined the LLUSP full-time as an Assistant Professor in June of 2012. She currently teaches and coordinates Pharmaceutical Care Lab II and Oncology/Transplant for Pharmacy Year 3 (PY3) students. Dr. Stewart’s practice site is the LLUACC, where she is involved in precepting students and residents, educating patients on chemotherapy, medication reconciliation, symptom management, weekly clinic rounds, and evaluation of chemotherapy orders. She also serves as the co-faculty advisor for the LLUSP chapter of the CSHP/ASHP. Javad Tafreshi, PharmD, BCPS-AQ Cardiology, F.A.H.A. Dr. Tafreshi received his Doctor of Pharmacy degree from USC in 1992. He completed a PGY1 Pharmacy Practice Residency in Tucson, Arizona and a PGY2 Specialty Cardiology Pharmacy Residency in Iowa City, Iowa. Prior to joining LLUSP in 2008, he was a faculty member at Midwestern University College of Pharmacy-Glendale, where he was the first faculty to receive tenure and promotion to the ranks of Associate Professor and Professor, and the University of Louisiana at Monroe, where he started as an Assistant Professor in 1997. He established the first ASHP-accredited PGY2 Cardiology Pharmacy Residency Program in the nation and is currently the Director of LLU PGY2 Cardiology Pharmacy Residency Program. He has been serving as the Acting Chair and Chair for the Department of Pharmacy Practice since September 2011. Dr. Tafreshi’s practice site is LLUMC Coronary Intensive Care Unit. Dr. Tafreshi is a Fellow of the American Heart Association (AHA) and serves as a reviewer for AHA continuing education programs. Dr. Tafreshi has a secondary appointment as Professor with LLU School of Medicine. Huyentran N. Tran, PharmD, BCPS Dr. Tran received her Doctor of Pharmacy degree from LLUSP in 2010. She completed a PGY1 Pharmacy Practice Residency at Desert Regional Medical Center in Palm Springs, California and a PGY2 Specialty Cardiology Pharmacy Residency at LLU. After two years of residency, she joined the LLUSP faculty in 2012. Dr. Tran has participated in a multidisciplinary collaborative team at LLU International Heart Institute and LLUMC to provide comprehensive care in managing patients with cardiovascular disorders and serves as a preceptor for students and residents on clinical rotations. Dr. Tran serves as the faculty advisor for the American Pharmacists Association (APhA) chapter at LLUSP. Andrew M. Williams, PharmD Dr. Williams earned his BS in Health Promotion & Disease Prevention Studies and Minors in Forensics & Criminality and Natural Sciences at USC. He received his Doctor of Pharmacy degree from USC School of Pharmacy in 2012. He completed a PGY1 residency in Acute Care at Keck Medical Center of USC, and completed a PGY2 in Psychiatric Pharmacy Practice at Los Angeles County + USC Medical Center. Dr. Williams’ primary clinical interest is psychiatry. He also has interests in neurology, geriatrics, and men’s health. Dr. Williams joined LLUSP in July 2014. His clinical practice site is at the LLU Behavioral Medicine Center where he attends multidisciplinary inpatient psychiatry rounds and assists with optimization of psychotropic agents. Dr. Williams coordinates and teaches the IPDM VIII, Psychiatry course.

Joycelyn Yamzon, PharmD, BCACP Dr. Yamzon completed her Doctor of Pharmacy degree and community pharmacy practice residency program at USC. Before transitioning to part-time faculty status, she practiced ambulatory care pharmacy at SACHS clinic in San Bernardino for underserved, uninsured/underinsured patients. Focusing on diseases of diabetes, hypertension, hyperlipidemia and asthma, she worked with physicians, physician assistants and nurse practitioners to manage and adjust medication therapy. She currently teaches immunizations, the PCL I course focusing on medication therapy management, respiratory diseases, and informatics. She is a national Certified Asthma Educator (AE-C) and advisor for the California Pharmacists Association (CPhA) student chapter. Farnoosh Zough, PharmD, BCPS Dr. Zough received her Doctor of Pharmacy degree from USC in 2011 and completed a PGY1 pharmacy practice residency at LLU in 2012. She joined the LLUSP faculty in 2012 in the area of Internal Medicine. Dr. Zough’s current clinical practice site is the Internal Medicine Unit of LLUMC where she serves as a preceptor for students and residents on clinical rotations. She also serves as the co-faculty advisor for the LLUSP California Pharmacy Student Leadership (CAPSLEAD). Additionally, she is taking part in the inter-professional development program with LLU School of Medicine, School of Nursing, and School of Allied Health Professions at LLU Medical Simulation Center. She is currently the faculty advisor for Class of 2017. Dr. Zough teaches and coordinates Pharmacist Self-Care I and Introduction to Pharmacology (IPDM I) to PY1 students. Dr. Zough serves in the Assessment Committee and University Faculty Council (UFC).
