Breast Cancer Screening
• Mammogram and Clinical Breast Exams
– Women age 40 and older
– Women younger than 40 at higher risk for
breast cancer (Attestation Required)
Cervical Cancer Screening
• Pap Tests and Pelvic Exams
– Women age 40 and older
– Women younger than 40 at higher risk for
cervical cancer (Attestation Required)
Colorectal Cancer Screening
– Men and Women age 50 and older – stool
sample test (FOBT, FIT)
– Men and women of any age who are at
increased risk for colorectal cancer

Working Together:
• To increase the number of health care
providers who are following best
practices for tobacco intervention.
• Funded by the New York State
Department of Health, Tobacco Control

Smokers:
– 70% made at least one unsuccessful quit
attempt
– 70% visit a physician each year
– Almost 50% try to quit each year
– Less than 10% are successful without assistance

first week after the quit date.

pharmacotherapy to help him or her quit.
