Foreword

Dear Colleague,

On behalf of Croydon Council, Croydon Clinical Commissioning Group, and Croydon Local Pharmaceutical Committee and the South West London Academic Health and Social Care System, I am delighted to launch this Healthy Living Pharmacy prospectus. This signals the start of the journey towards the development of Healthy Living Pharmacies (HLP) across Croydon.

The Healthy Living Pharmacy concept was developed by the Department of Health as a mechanism by which improvements to the quality and effectiveness of community pharmacy and wider access to health and wellbeing services overall.

HLPs are now well established in many areas of the country. And HLP staff are proactive in supporting health and wellbeing as a core activity and are readily identifiable by the public and other healthcare professionals.

Community pharmacies are, by definition, embedded within their Communities. For people who are well or ill, the local pharmacy can be a more appropriate, inviting and readily accessible venue than a hospital or GP surgery.

We have the opportunity in Croydon to develop a group of highly motivated healthcare professionals based in Pharmacy to move to centre stage in our public health delivery system.

To be a HLP will require a commitment to, and demonstration of, proactive engagement with both the community you serve and with other healthcare professionals. It will also require a change in ethos and effective leadership both within and beyond the community pharmacy team. It is very encouraging to see the level of local interest already expressed by contractors who want to embrace the HLP status and several of you have already encouraged your team to develop Healthy Living Champions.

This document aims to provide you with more detail on how we see HLPs developing in Croydon and how the LPC will support you in developing this concept in an innovative way.

I would like to invite you to participate in this exciting programme. I believe it will prove to be both personally and professionally rewarding for you and impact positively on the health of the people of Croydon.

Croydon Healthy Living Pharmacy Prospectus - Introduction

Introduction

The public health White Paper “Healthy Lives Healthy People” published in 2010 recognised the contribution that Community Pharmacy can make to the delivery of public health services.

Healthy Living Pharmacies aim to inspire the provision of high standards of service delivered by dedicated teams working in pharmacies at the heart of each community.

The introduction of the Healthy Living Pharmacy programme in Croydon marks the beginning of an exciting journey which will give all local community pharmacies the opportunity to gain the quality mark1 that indicates they are a Healthy Living Pharmacy.

We, as commissioning partners, believe that Healthy Living Pharmacies are good for community pharmacy, good for contractors and good for the public.

Having seen the results of Healthy Living Pharmacy programmes elsewhere, Croydon Public Health, Croydon CCG and the South West London Academic, Health and Social Care System have agreed to provide the resources to lead implementation of the Healthy Living Pharmacy approach in Croydon, in partnership with Croydon Local Pharmacy Committee and its members.

The aim of all Healthy Living Pharmacy activity will be to benefit patients and the public who will remain at the centre of all that is offered at the pharmacy. Healthy Living Pharmacies in Croydon will deliver health and wellbeing services reliably and consistently based on an interventionist Health Promotion ethos instilled by the training required to become a Healthy Living Pharmacy. The entire pharmacy team will be informed and aware of all services offered by their pharmacy. When the need arises, teams will be able to offer advice and support patients effectively with accurate signposting and referral to services both inside and outside the pharmacy.

The whole team will work together to deliver important health messages and advice as part of their normal day-to-day activity. Healthy Living Pharmacies will display a quality mark of excellence that gives members of the public the assurance that anyone accessing services in a Healthy Living Pharmacy will enjoy high standards of service and care.

The Health Needs of People in Croydon – Lifestyle

Lifestyle has a significant impact upon the health and outcomes of an individual.

Within Croydon, the lifestyle factors and behaviours which are a cause for concern include:

Smoking

• 17.1% of adults smoke in Croydon and stopping smoking is the most effective intervention in improving health outcomes.

• Prevalence is highest in the wards of Addiscombe & Broad Green (East Croydon locality) Ashburton (Woodside & Shirley locality), Bensham Manor (Mayday locality), Coulsdon East (Purley locality).

• The prevalence of smoking is higher in people from routine and manual occupational groups, 29.4% of people in these groups currently smoke.

Poor diet

• 67.9% of infants are either totally or partially breast fed at the 6-8 week check. This is significantly higher than the England average (47.2%). The percentage of breast fed infants is lowest in the New Addington & Selsdon and Woodside & Shirley Localities (47.3% and 63.7% respectively)

• Only 27.8% of people eat the recommended 5+ portions of fruit and vegetables each day

• There is a correlation between fast food and obesity. Croydon has a high proportion of fast food outlets (114 outlets per 100,000 population compared to the England average of 77.9)

Physical inactivity

• 28.3% of adults in Croydon are inactive, this is similar to the London (28.4%) and England (28.9%) averages

• Croydon ranks in the bottom 10% of local authorities for physical activity (2011/12 data)

Substance misuse

• It is estimated that there are 1,676 opiate and/or crack users (OCU); this is equivalent to 6.94 per 1,000 population, which is lower than the regional (9.32) and national (8.60) averages

• 12.8% of the population drink alcohol at a level of increasing risk (hazardous) to their health, a further 5.0% drink at an even higher risk (harmful).

• There have been approximately 1,726 hospital stays for alcohol related harm.

• GP data shows alcohol dependence is highest in Waddon & Addiscombe (East Croydon locality), Woodside & Ashburton (Woodside & Shirley locality) and Bensham Manor (Mayday locality)

• It is estimated, approximately 30% of secondary school pupils in Croydon have been drunk.

• There is strong correlation between alcohol related recorded crime and poor sexual health outcomes (per 1,000) is 9.2 compared to 9.0 in London and 5.7 in England (2012/13).

Risky Sexual Behavior

• Sexual health is influenced by a number of factors including sexual behaviour and attitudes

• Unprotected sex can lead to poor sexual health, sexually transmitted infections (STIs) and unplanned pregnancy

• There is a strong correlation between alcohol and poor sexual health outcomes

- Croydon has higher rates of teenage pregnancy, HIV, Chlamydia (all ages) and other common STIs than the national average (2012/13)

- Abortions rates (26.9 per 1,000 women aged 15 -44) are higher than the London (21.7) and England (16.1) averages (2012/13)

- Repeat abortions in those under 25 (38.7%) is higher compared to London (32.6%) and England (26.9%) (2012/13)

Below we have begun to explore the health consequences of the lifestyle choices identified in the previous page as well as a range of diseases caused.

Cardiovascular Disease and Stroke

• Cardiovascular disease (CVD) is one of three most common causes of death in Croydon

• It is estimated that in Croydon 55% of cases of CVD are preventable either through modification of lifestyle and/or the use of medication (e.g. to control blood pressure, reduce cholesterol, anti-coagulant or anti-platelet therapy, anti-diabetic medication etc)

Diabetes

• Diabetes is associated with long-term complications including heart disease, stroke, blindness, amputation and chronic kidney disease

• Modifiable risk factors for diabetes include being overweight or obese, smoking and inactivity

• There is also a correlation with:

- Deprivation: those living in the most deprived areas have a higher risk

- Ethnicity: risk for people of South Asian origin is six times greater; and Black-African Caribbean origin is five times higher than that for white people. There is a greater risk of long-term complications in these groups

• Croydon has a 6.39% GP recorded prevalence rate of diabetes; this is higher than the regional (5.82%) and national (6.01%) averages (2012)

Cancer

• It is of note that cancer rates & ‘preventable’ deaths in men are statistically similar to London and England averages

Respiratory Disease

• Respiratory ‘preventable’ deaths are statistically similar to the London and England averages; the standardised mortality rate for ‘all deaths’ is statistically similar to the regional and national average

• The prevalence for COPD, for which smoking is the main cause, is statistically worse than the England average

Hospital admissions

• Tackling smoking will reduce smoking-related hospital admissions

Substance Misuse

• The World Health Organisation (WHO) defines the misuse of drugs or alcohol as “the use of a substance for a purpose not consistent with legal or medical guidelines”. It may also be defined as “a pattern of substance use that increases the risk of harmful consequences for the user”

• Substance misuse is associated with a range of adverse physical, mental health and/or social consequences

Drug Misuse

• Drug misuse is associated with a high risk of blood-borne viruses such as hepatitis C, hepatitis B and HIV. These infections may cause chronic poor health and can lead to serious disease and premature death

• The Health Protection Agency (HPA) has estimated that in England (2013) for current and previous drug users:

- 1.2% are Hepatitis B Positive - 17% are Hepatitis C positive - 50% are HIV positive - In 2011/12, it was estimated that there were

1,914 opiate and/or crack users in Croydon

Alcohol misuse

• Regularly drinking more than the recommended daily allowance, and particularly binge drinking (i.e. at least twice the daily recommended amount of alcohol in a single drinking session i.e. 8+ units for men and 6+ units for women), has health consequences which include:

- Liver disease: The under 75 mortality rate in 2010/12 was 15.4/100,000.

- Alcohol-related mortality (2012): This was 55.57/100,000 for males and 25.58/100,000 for females.

• GP recorded prevalence of alcohol dependence shows quite a variation within Croydon; with higher areas of prevalence in the Thornton Health, Woodside & Shirley and East Croydon localities

Sexual Health

• Sexually transmitted infections (STIs) and HIV can cause a range of illnesses which may lead to premature death:

- In 2013, the number of acute STIs diagnosed was 4,615 in Croydon

- The rate of STIs (including chlamydia) per 100,000 population was 1251 for Croydon; this compares to 1332.5 and 834.2 for London and England respectively (2013)

- The rate of chlamydia diagnosis, in those aged 15-24 years (per 100,000), was 2,704 for Croydon; this compares to 2,179 for London and England 2,016 (2013)

- In 2013, the gonorrhoea diagnosis rate (per 100,000) was 128.5

- Croydon has an HIV prevalence rate of 5.1 per 1,000 population. This is lower than the London rate of 5.5 but significantly

higher than the national rate of 2.1 per 1,000 population.

- 58.3% of HIV in Croydon is diagnosed at late stage (CD4 <350) in those aged 15+. This is statistically higher than the London (44.9%) and England (48.3%) averages (2010-2012 data)

• Unwanted pregnancy has a significant impact, particularly in young girls; and termination of pregnancy can have long term physical and psychological effects leading to health problems in the future.

• Teenage pregnancy often leads to poor health and social outcomes for mother and baby.

- In 2012, the rate of under 18 conceptions (per 1,000) was 28.6, in Croydon. This is slightly higher than the regional (25.9) and England (27.7) averages

Mental Health

• At least one in four people will experience a mental health problem at some point in their life

• One in six adults has a mental health problem at any one time

• Common mental health disorders include anxiety, depression, phobias, obsessive compulsive and panic disorders

• In Croydon:

• The percentage of GP registered patients diagnosed with a common mental health condition is significantly higher than national averages

• An estimated 38,620 adults (aged 16-64) will be suffering from a common mental health disorder by 2015

• The recorded suicide rate in Croydon (6.2) is significantly lower than the national average (8.5)

Older People

• The frequency of ill health rises with increasing age and older people generally need to use health and care services more frequently than younger people

• Older people are particularly vulnerable to: - Depression: Especially those living alone,

those in care homes and those with physical illnesses and disabilities. The diagnosed prevalence (in the last 5 years) is around 20,120 (5.5%) of the population

- Dementia: The diagnosed prevalence in Croydon is 1,575 (0.43%) of the population. It is predicted that prevalence is actually twice as much in this age group

- Falls: The rate (per 100,000) of older people, who sustained an injury due to a fall was 2,318 for those aged 65 – 79; this is higher than the London (2,242) and England (2,011) average

Seasonal Influenza

• Seasonal influenza may cause severe illness and complications in vulnerable groups including children aged under 6 months; older people; pregnant women and those with underlying disease especially chronic respiratory disease, cardiac disease and immunosuppression. Seasonal influenza vaccine is recommended for people falling into these clinical groups

• Each year, the Department of Health sets targets for seasonal influenza vaccination. For 2013/14, the target was 75% or higher for both the over 65 years and those aged under 65 who fall into ‘risk’ groups (including pregnant women)

• With respect to seasonal influenza vaccination in 2013/14:

- For the over 65s, the vaccination rate was 65.7%; this is lower than the London (70.0%) and England (73.2%) averages. Vaccination rates were particularly low in the Mayday, Thornton Heath and Woodside & Shirley localities (64.8%, 62.1% and 64.7% respectively).

- For those aged 6 months to 64, in all ‘at risk’ groups, the vaccination rate was 47.3% compared with 52.0% and 52.3% for London and England, respectively. Vaccination rates were below the Croydon average in the Thornton Heath, Woodside & Shirley and Purley Localities

Childhood immunisation

• A priority is to achieve ‘herd’ immunity against infectious diseases (e.g. 95% of the eligible population should be immunised against the disease)

• Croydon is not meeting the national vaccination targets for childhood immunisations; and performs below the regional and national levels:

- DTaP/IPV booster uptake at 5 years is 75.6% compared with 79.9% in London and 88.9% in England

- MMR2 uptake at 5 years is 77.4% compared with 80.8% in London and 87.7% in England

- HPV vaccinations (girls aged 12-13 years) is 77.4% compared to 78.9% in London and 86.1% in England.

A significant amount of the burden of ill-health could be avoided if the right interventions were made at the right time.

Community pharmacies have an important role to play in engaging with the public, either to provide these interventions or to assist the public in their awareness and use of these services elsewhere. Community pharmacies are well recognised and are easily accessible to local people, including those who do not seek other sources of health care.

A Croydon Healthy Living Pharmacy will provide more than just a range of health and wellbeing services; it will be a place where the public feel they are guaranteed high quality advice, consistent service delivery and have access to a highly-qualified healthcare professional when they need it.

The public will recognise that the team working in a Healthy Living Pharmacy will provide good, reliable advice about leading a healthier lifestyle coupled with a commitment to improving general health and wellbeing.

Those pharmacies awarded Healthy Living Pharmacy status will be able to display a logo that is becoming increasingly recognised by the public as an indicator of quality.

• has trained Healthy Living Champions

• consistently delivers a range of contracted and commissioned NHS services of high quality

• maintains premises to a high professional standard, with private consultation facilities and with appropriate IT availability and accessibility

• works closely with other healthcare providers, proactively and reactively.

Croydon Vision for Pharmacy

An established ‘first port of call’

We wish to see community pharmacy widely recognised, and used, as a first port of call, reducing demand on other services particularly General Practice, A&E and unscheduled care providers.

We envisage that this may include building upon existing, and potentially commissioning new, pharmacy based services

An enhanced role in Self Care

Pharmacy is well placed to support Croydon residents with self-care.

This may include enhancing the role of pharmacists in helping people to manage long term conditions and facilitating them living independently at home

A wider role within primary care

There are opportunities to maximise the role which community pharmacy undertakes within primary care, with a view to enhancing choice for our residents, providing care closer to home and optimising use of skill mix.

This may include commissioning a wider range of pharmacy-based services to be provided by pharmacists (or their staff) or through other healthcare professionals from working within pharmacy premises

A network of Public Health Practitioners

A key ambition is to create a network of public health practitioners, using our Healthy Living Programme as a solid foundation upon which to expand the range of public health activities undertaken within pharmacy.

We envisage that through a more integrated offering, that we will maximise opportunities to make “Every Contact Count”

Taking pharmacy to Croydon Residents

We believe there are opportunities to provide more pharmacy services on an outreach basis - whether this is directly to people in their own homes or in other settings e.g. the work place

There are many services provided by community pharmacies across Croydon; some of these are provided as part of the current national contractual framework, and others are commissioned locally to meet identified needs - the Locally Commissioned Services (LCS).

Below is an overview of the services provided by Croydon Pharmacies, if you wish to obtain an in-depth knowledge of the services commissioned within the Borough, please view the Croydon 2015 – 2018, Pharmaceutical Needs Assessment on the Croydon Observatory (this will be made available from April 2015)

Essential Services must be delivered by all pharmacies, as detailed in the current contractual framework:

• Dispensing

• Repeat dispensing

• Supply of appliances

• Disposal of unwanted medicines

• Support of public health

• Signposting

• Support for self-care

• Clinical governance

The provision of these services sits as a core requirement of the Healthy Living Pharmacy framework.

Advanced Services are delivered by pharmacists who are appropriately accredited. Healthy Living Pharmacies will embrace the opportunities to maximise the benefit of Medicines Use Reviews (MURs). MURs can help meet treatment goals by supporting patients in gaining the intended benefit from their medicines. Effectively delivered and

targeted MURs have demonstrated improved adherence to treatment. Evidence from five anonymised MURs will be required for accreditation.

The New Medicines Service provides support for people with certain long-term conditions newly prescribed a medicine, to help improve medicines adherence. Healthy Living Pharmacies will embrace this new service to ensure the public gets the most from their new medicines.

Locally Commissioned Services - a range of services are commissioned locally by both local authorities (through public health) and by CCGs. We want to increase the number of people making community pharmacies their provider of choice for the management of certain ailments, the promotion of self-care and support to live healthier lifestyles. Pharmacies are also well placed to offer service provision opportunistically and at a time that is right for members of the public. We will expect Healthy Living Pharmacies to be active participants in a number of these services.

A Healthy Living Pharmacy will be required to provide two core services (currently the minor ailments service and the stop smoking service), the Londonwide Flu Vaccination service, Medicines Optimisation Service and at least one other locally commissioned services.

Healthy Living Pharmacies will use the opportunities in nationally or locally commissioned services as a platform to offer and refer to other relevant services as well as delivering public health awareness messages and healthy lifestyle advice.

The most important person in a pharmacy is the first point of contact.

Implementation of the Healthy Living Pharmacy framework requires education of the whole pharmacy team. Healthy Living Pharmacies will have highly skilled, informed teams that can speak with knowledge and understanding of the services they offer at their pharmacy, provide appropriate advice, and signpost correctly. The team will be able to deal appropriately with requests of a personal nature. Services will be offered in a non-judgmental manner and according to service specifications. In the event that their accredited pharmacist is absent, Healthy Living Pharmacy teams will know where services can be accessed and signpost accordingly. It is important to remember that continuity of service is important to the public and commissioners. Therefore, pharmacy managers and contractors are expected to take responsibility for ensuring that new pharmacists and staff are appropriately trained to continue the leadership and health promotion activities.

Leadership in the pharmacy

To lead the team and ensure the adoption and development of the ethos of the Healthy Living Pharmacy, the lead pharmacist / Non-Pharmacy Manager or Supervisor will undertake appropriate leadership training to increase their skills in workforce development. Two training courses designed for Healthy Living Pharmacy are available; pharmacy managers and contractors can choose which to use. Pharmacy leaders interested in developing their leadership skills may wish to complete Further training.

Croydon is also researching avenues to provide further training support through colleagues in the Health Education South London team.

Role of the Healthy Living Champion

Pharmacy managers will need to identify non-pharmacist members of the pharmacy team who have the motivation to become Healthy Living Champions. Healthy Living Champions will actively promote healthy living and lifestyle support services to both the public and other members of staff.

The pharmacy manager will work with the Healthy Living Champion(s) to identify the activities which will best match the needs of their community with services offered by the pharmacy. Pharmacy managers are encouraged to train more than one Healthy Living Champion to ensure continuity, particularly where part-time staff are employed.

Croydon has been working closely with colleagues in Health Education South London, and will continue to do so over the coming year to support training needs.

Why do you need Healthy Living Champions?

Healthy Living Champions will have a nationally recognised qualification allowing them to deliver brief interventions (where appropriate), healthy living advice and refer or signpost into specialist services as appropriate. Healthy Living Champions also provide an excellent opportunity not just to engage with customers, but reach out to the communities and community organisations in the areas you serve.

Training of a Healthy Living Champion

The minimum qualification for Healthy Living Champions in Croydon is the

“Royal Society of Public Health Level 2 Award Understanding Health Improvement”.

Motivated staff may wish to complete further training to complement this core requirement.

Managers within a pharmacy should consider what activities they should ask their Healthy Living Champion to undertake to best meet the needs of their community and services offered by the pharmacy.

To help with this, it is expected that a Healthy Living Champion would be engaged in some or all of the following:

• Work with the whole pharmacy team to develop ideas on how the pharmacy can be more proactive in improving health and wellbeing; making the most of every interaction as an opportunity for an intervention

• Take an active role in delivering the health and wellbeing services provided by the pharmacy; be available to advise customers and patients about health and wellbeing; be able to signpost customers and patients to health and wellbeing services, both within the pharmacy and outside

• Develop different health and wellbeing campaigns in line with local health priorities and develop a health zone in the pharmacy; take responsibility for keeping health promotion materials up to date and sourcing new leaflets and posters from organisations with an interest in public health

• Develop window displays to attract the public into the pharmacy and increase use of health and wellbeing services; consider whether the pharmacy is a healthy workplace and propose ideas to make it healthier for the staff that work within it

• Complete an audit of the difference they are making.

Before a pharmacy can be considered as a Healthy Living Pharmacy, it must already be achieving the standards required and be active within the contractual framework for Essential and Advanced Services.

The pharmacy must also adhere to all standards set out by the General Pharmaceutical Council.

In addition there will be a set of locally assessed quality criteria that pharmacies must meet in order to be accredited as a Healthy Living Pharmacy. The details of these requirements will be set out in a separate Quality Criteria and Service Requirements document that will be available in the coming weeks. However, the broad requirements are set out below.

Healthy Living Champion (a non-pharmacist staff-member)

Certificate from approved training organisation (i.e. CPEE or NPA), as detailed on page 14 of this prospectus.

Internet Connection to relevant computers Self-declaration.

Medicines Use Review Active and consistent delivery – self declaration and evidence.

New Medicines Service Active and consistent delivery – self declaration (to be reviewed in line with national review of NMS).

Locally Commissioned services A Healthy Living Pharmacy will be required to provide two core services (currently the minor ailments service and the stop smoking service), the Londonwide Flu Vaccination service, Medicines Optimisation Service and at least one other locally commissioned services.

Community engagement Your pharmacy must demonstrate engagement with local healthcare teams and the local community, for example, GP Practices and community groups.

In order to be awarded Healthy Living Pharmacy status the pharmacy must meet the criteria detailed in the Quality Criteria and Service Requirements, which will be published in February 2015, along with the HLP application form.

When you as a pharmacy team feel ready to become accredited as a Healthy Living Pharmacy you may complete the application form to become a Healthy Living Pharmacy. The Healthy Living Pharmacy Accreditation Panel will give you a date of assessment for your application within one month of receipt.
