Scheduling and Planning Your Appointment

Call our office at 616.949.2600 or 800.968.2600 Monday through Friday to schedule an appointment at any of our West Michigan locations. Evening and Saturday morning appointments are also available by request.

Most appointments, such as an initial consultation or complete exam, will take approximately 1-1/2 to 2 hours. Cataracts and retinal evaluations may last 2-1/2 to 3 hours. Both eyes will be dilated for complete exams, laser vision, and retinal evaluations. Dilation causes sensitivity to light and an inability to see things clearly at close ranges – such as words on a page. If your eyes will be dilated, take the precaution of having someone drive you home after your appointment.

Please bring your photo ID, insurance cards, Social Security number, and a list of your current medications for your record. Insurance copay and/or out of pocket fees are due in full at the time of service.

If patient is a minor, parent or legal guardian must be present at initial visit. A waiver may be signed for subsequent visits.

Please be considerate of our patient/doctor time when unable to keep your scheduled appointment; kindly give 48 hours notice. Failure to do so will result in a $35.00 fee.

For the consideration of others, please plan to arrive 10-15 minutes prior to your appointment time for administrative details regarding your visit. Patients arriving more than 15 minutes after their time may need to be rescheduled based on the specific schedule and day. Patients will be placed in the next available appointment with their doctor.

Billing and Insurance

Grand Rapids Ophthalmology participates with most vision and medical insurance plans. Vision plans cover exams specific to how your eyes see with glasses and/or contact lenses. Your medical insurance covers medical health issues affecting your eyes. Insurance co-pay and/or out-of-pocket fees are due in full at the time of service. If you have questions regarding your specific plan, feel free to call our billing department direct at 616.447.9450.

Emergencies

In case of emergency, Grand Rapids Ophthalmology maintains physicians on call around the clock. After normal business hours our answering service will page the doctor to answer your call as soon as possible. If you are experiencing a life-threatening medical emergency, please call 911 or proceed to the nearest hospital.

Kenyon Kendall, DODr. Kendall is a glaucoma specialist and performs small-incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium LifeStyle Lenses. He graduated from Michigan State Osteopathic School of Medicine, completed his glaucoma fellowship at Kentucky Lions Eye Research Institute, and is a graduate of University of Michigan. He has been in private practice since 1987.

Scott Weber, MDDr. Weber’s expertise includes Custom and BLADE-FREE LASIK as well as small-incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium LifeStyle Lenses. He also treats glaucoma and general eye care patients. Dr. Weber graduated from Indiana University School of Medicine, completed his residency at Bethesda Naval Hospital, and is a graduate of Notre Dame. He has been in private practice since 1987.

David Barrett, MDDr. Barrett specializes in laser-vision correction and performs small-incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium LifeStyle Lenses. He is a LASIK leader, having performed thousands of Custom and BLADE-FREE LASIK procedures. Dr. Barrett received his medical degree from Wayne State University School of Medicine, completed his ophthalmology residency at Jules Stein Institute at UCLA, and is a graduate of Michigan State University. Dr. Barrett has been in private practice since 1990.

Thomas Cowden, MDDr. Cowden is fellowship-trained in cornea, external diseases, anterior segment, and refractive surgery. He was involved in the initial FDA clinical trials for LASIK in 1996 and has performed thousands of procedures, including Custom and BLADE-FREE LASIK. Dr. Cowden also specializes in corneal transplant surgery, advanced anterior segment reconstruction as a result of trauma or congenital defects, and small-incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium LifeStyle Lenses. He received his medical degree from Wayne State University School of Medicine; completed his subspecialty fellowship at University of Minnesota and his ophthalmology residency at University of Wisconsin; and is a graduate of University of Michigan. He has been in private practice since 1997.

Yosef Gindzin, MDDr. Gindzin is a fellowship-trained retinal specialist, treating retinal problems such as age-related macular degeneration, diabetes, retinal detachment, and macular pathologies – often using advanced laser techniques and drug therapy. He received his medical degree from University of Michigan, completed his retina and vitreous surgery fellowship at Cleveland Clinic, and is a graduate of University of California at Irvine. Dr. Gindzin is conversant in Spanish and joined Grand Rapids Ophthalmology in 2002.

Marcus Muallem, MDDr. Muallem is fellowship-trained in cornea and refractive surgery, as well as glaucoma. He performs Custom and BLADE-FREE LASIK; Traditional LASIK; PRK; and small-incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium LifeStyle Lenses. He specializes in diagnosis, treatment, and surgical management of glaucoma and corneal diseases, including surgery and corneal transplants. Dr. Muallem completed his fellowship training at Bascom Palmer Eye Institute and his residency at University of Illinois, Chicago.

Michael Boyle, MDDr. Boyle is fellowship-trained in oculoplastic surgery and specializes in cosmetic and reconstructive eyelid surgery. He is also Medical Director of Skin Solutions by Grand Rapids Ophthalmology, providing patients the best possible results following their eyelid procedures. Dr. Boyle is a fellow of American Society of Ophthalmic Plastic and Reconstructive Surgery (ASOPRS) and is board certified by American Board of Ophthalmology. He received his medical degree from University of Illinois, College of Medicine; completed his fellowship at University of Alabama-Birmingham and his residency and internship at Indiana University.

Parin Gohel, MDDr. Gohel is a fellowship-trained retinal specialist using advanced laser techniques and drug therapies to treat age-related macular degeneration, diabetes, retinal detachment, and macular pathologies. Dr. Gohel received his medical degree from Wayne State University School of Medicine; completed his internal medicine internship at Lahey Clinic-Tufts University and his ophthalmology residency at University of Colorado, Rocky Mountain Lions Eye Institute; he also completed fellowships in vitreoretinal surgery at Associated Retina Consultants and Henry Ford Hospital; and is a graduate of Michigan State University. Dr. Gohel joined Grand Rapids Ophthalmology in 2014.

Robert Roosenberg, MDDr. Roosenberg specializes in providing post-operative care for cataract and other surgical patients along with routine eye exams. In addition, he performs eye exams on diabetic patients with the goal of maintaining their eye health and vision. Dr. Roosenberg received his medical degree from Loma Linda; completed his residency at University of Michigan and his surgical residency at Butterworth Hospital; and is a graduate of Andrews University. Dr. Roosenberg has been in private practice since 1979. Laura Piippo, MDDr. Piippo specializes in pediatric ophthalmology, treating children with crossed and lazy eyes as well as providing comprehensive eye exams for children. Dr. Piippo received her medical degree from Wayne State University School of Medicine; completed her ophthalmology residency at University of Illinois, Chicago and her fellowship in pediatric ophthalmology at Baylor College of Medicine; and is a graduate of Lake Superior State University. She joined Grand Rapids Ophthalmology in 2002.

Tara Golisch, MDDr. Golisch specializes in treating glaucoma patients in addition to performing small-incision cataract surgery utilizing traditional intraocular lenses, toric implants for astigmatism, and premium LifeStyle Lenses. She received her medical degree from Robert Wood Johnson Medical School; completed her ophthalmology residency at Wayne State University-Kresge Eye Institute and her glaucoma-fellowship training at Cincinnati Eye Institute; and is a graduate of Washington University. Dr. Golisch joined Grand Rapids Ophthalmology in 2014.

Thomas Dunning, ODDr. Dunning specializes in primary eye care for all ages with an emphasis in contact-lens fittings for difficult-to-fit eyes. He is a dry-eye specialist and has a dry eye clinic at Grand Rapids Ophthalmology— 7 Mile. He is the current President of West Michigan On-Site Eye Care, past Associate Clinical Professor for Michigan College of Optometry, and past Vice President of West Michigan Optometric Association. Dr. Dunning received his Doctor of Optometry from Michigan College of Optometry and is a graduate of Lake Superior State University. He has been in private practice since 1989.

Gerard Choryan, ODDr. Choryan specializes in contact lenses, dry-eye treatment, and urgent eye care. His contact lens work involves people with all types of vision problems including astigmatism, keratoconus, and corneal transplants. He has successfully treated many dry-eye patients allowing most to return to contact lens wear or experience renewed ocular comfort. He is also available for routine eye examinations and red-eye treatment involving foreign bodies, infections, and trauma. Dr. Choryan received his Doctor of Optometry from Michigan College of Optometry and is a graduate of Ferris State University. He has been in private practice since 1990.

Philip VanderLugt, ODDr. VanderLugt has been an optometrist with Grand Rapids Ophthalmology for over 20 years. In addition, he serves as Grand Rapids Ophthalmology’s Optometric Affairs Liaison. Dr. VanderLugt received his Doctor of Optometry from Michigan College of Optometry and is a graduate of Calvin College. He is the past President of West Michigan Optometric Association as well as an Associate Clinical Professor at Michigan College of Optometry.

Charlene Hamilton, ODDr. Hamilton provides routine eye care for patients of all ages. She is highly experienced with contact lens patients and providing pre- and post-operative care for laser and medical patients. Dr. Hamilton received her Doctor of Optometry and Bachelor of Science from Indiana University. She is a member of American Optometric Association and Michigan Optometric Association.

Gregory Patera, ODDr. Patera provides routine eye care for patients of all ages and is certified in the treatment of external eye diseases, injuries, and glaucoma management. Dr. Patera is highly experienced with custom contact lenses, including bifocal design, toric (for astigmatism), special-purpose soft lenses and rigid, gas-permeable lenses. He received his Doctor of Optometry from Illinois College of Optometry and is a graduate of Michigan State University. Dr. Patera practices at Grand Rapids Ophthalmology-Ionia.

David Den Braber, ODDr. Den Braber provides comprehensive eye care for all ages, specializing in contact lenses and the treatment and management of ocular disease. He especially enjoys working with children and co-manages eye muscle dysfunction. He received his Doctor of Optometry from Michigan College of Optometry and is a graduate of Ferris State University. Dr. Den Braber practices at Grand Rapids Ophthalmology-Ionia and at the East Beltline location.

Monica Fenton, ODDr. Fenton provides routine eye care for patients of all ages. She received her Doctor of Optometry from Michigan College of Optometry, completed her primary care residency at Hu Hu Kam Memorial Hospital in Arizona, and is a graduate of Arizona State University. Dr. Fenton practices at Grand Rapids Ophthalmology-Holland.

Rachel Hollenbeck, ODDr. Hollenbeck is available for routine and medical eye exams, contact lens exams, and pre- and post-operative exams. She received her Doctor of Optometry from Michigan College of Optometry and is a graduate of South Dakota State University. Dr. Hollenbeck has participated in several VOSH (Volunteer Optometrists Serving Humanity) vision missions, including missions to Dominica and Saint Lucia. She is an active member and past President of Greenville Rotary Club; and past President of West Michigan Optometric Association. Dr. Hollenbeck practices at Grand Rapids Ophthalmology-Greenville.

Annessa Dix, ODDr. Dix provides medical-based eye care, specializing in retina and glaucoma management. In addition, she is also available for routine eye exams. She received her Doctor of Optometry from Michigan College of Optometry. Dr. Dix joined Grand Rapids Ophthalmology in 2011 and practices primarily at the Walker location.

Lindsay Weatherhead, ODDr. Weatherhead is available for routine eye exams, pediatrics, contact lens patients, and pre- and post-operative medical exams. She is a member of American Academy of Optometry, Fellowship of Christian Optometrists, and Michigan Optometric Organization; in addition, she volunteers for Optometric Service to Humanity. Dr. Weatherhead received her Doctor of Optometry from Michigan College of Optometry; she completed her primary care residency at Nova Southeastern University College of Optometry where she gained experience in all specialties including retina, glaucoma, cornea, and neuro-ophthalmology; and is a graduate of Kalamazoo College. Lewis DeKuiper, ODDr. DeKuiper provides primary eye care for the entire family. He is experienced in all aspects of contact lenses, including premium lenses for astigmatism and presbyopia. He has a special interest in consulting with patients considering laser vision correction and providing appropriate pre- and post-operative care. Dr. DeKuiper also provides comprehensive management and treatment of ocular disease. He is members of Michigan Optometric Association and American Optometric Association; and is past President of West Michigan Optometric Association. He received his Doctor of Optometry from Illinois College of Optometry and is a graduate of Calvin College.

Angela Huizenga, OD Dr. Huizenga provides primary eye care for the entire family. She received her Doctor of Optometry from Michigan College of Optometry. Her externships included pediatrics, binocular vision, specialty contact lens fitting, and primary care; she completed a one- year residency specializing in ocular disease at Eye Center of Texas where she evaluated cataract, LASIK, glaucoma, cornea, and retinal patients; and is a graduate of Ferris State University. Dr. Huizenga joined Grand Rapids Ophthalmology in 2012.

Abigail Veldkamp, ODDr. Veldkamp specializes in complete eye care for pediatric patients, including routine exams, binocular vision, and contact lenses; she also provides comprehensive adult eye care. Dr. Veldkamp is a member of American Optometric Association, Michigan Optometric Association, and West Michigan Optometric Association. She received her Doctor of Optometry from Michigan College of Optometry and is a graduate of Calvin College. She practices at various Grand Rapids Ophthalmology locations. Shannon Stanek, ODDr. Stanek provides complete eye care for the entire family. She has completed advanced training and education in fitting specialty and unique contact lenses, including RGP, Hybrid, and Scleral lenses. Dr. Stanek is a member of West Michigan Optometric Association. She received her Doctor of Optometry from Michigan College of Optometry and is a graduate of Michigan State University. Dr. Stanek practices at the East Beltline office as well as other Grand Rapids Ophthalmology locations.

Holly Wall, ODDr. Wall provides routine eye exams for all ages, including contact lens patients, management of ocular disease, and pre- and post-operative medical exams. She is members of American Optometric Association and Michigan Optometric Association; and a fellow of American Academy of Optometry. Dr. Wall received her Doctor of Optometry from Michigan College of Optometry and is a graduate of Calvin College. Prior to joining Grand Rapids Ophthalmology, Dr. Wall served as an Optometrist for the United States Air Force in Texas.

Andrew Wilke, ODDr. Wilke provides eye care for the whole family, from routine eye exams and specialty contact lens fittings to the management of ocular disease and low-vision services. In addition, he provides pre- and post-operative care for surgical patients. Dr. Wilke received his Doctor of Optometry from Michigan College of Optometry and is a graduate of Michigan State University.

Claire Diekema, ODDr. Diekema provides routine eye care for all ages, including pediatric and contact lens exams, pre- and post-operative medical exams, and management of ocular disease. Bill Schultz, ODDr. Schultz specializes in primary eye care for all ages, including routine eye exams, preventative care, specialty contact lens fittings, medical eye care, and pre-and post-operative management. Dr. Schultz graduated from Michigan College of Optometry and has been in practice since 1989. He is available to see patients in many of the Grand Rapids Ophthalmology offices; his primary location is in East Grand Rapids at Lakeshore Eye – Grand Rapids Ophthalmology.

Ashley Tholen, ODDr. Tholen provides comprehensive eye care ranging from routine examinations to full-scope optometry to patients of all ages, including children as young as 6 months. Her experience includes fitting a variety of contact lenses, post-operative care, managing pathology, and treating myopia progression and amblyopia. Dr. Tholen received her Doctor of Optometry from Illinois College of Optometry and is a graduate of University of Iowa.
