A flow chart has been included to provide an overview of the medication management process whenusing Webster-pak as well as detailing many important steps in the process. This could be importantin explaining to Clients, Doctors and Carers how the process works and the importance ofcommunication.
Throughout this booklet you will find the tools and information that relate to the flowchart.
The purpose of this section is to outline some key issues you should consider when clients are startingWebster-pak.
It will help you to discuss patient profile information, frequency of supply, the right pack, delivery andreturn, prescription management, how to use the pack (and more) with your client and/or their carer.
This checklist can be kept as objective evidence that a comprehensive consultation has taken place.You can record key details and sign off when complete.
This section has been designed to answer any possible questions your clients may have aboutWebster-pak. This section can be copied for your clients’ information.
By providing clients with full information you can help overcome any fears or possible unknowns. Thisdocument is designed to answer all their questions and make them comfortable with using Webster-pak and the medication management process.
This booklet has been designed to give you all the resources you need to start newWebster-pak clients. The booklet provides an overview of the medication managementprocess, a checklist to guide you through the important questions to ask and usageinformation.

Webster-pak is one part of an integrated management process. This process is to ensure people take their medicationscorrectly. Taking medication, even the most complicated and confusing dosages, can be managed better when aWebster-pak is used. Webster-pak is recognised for its distinctive visual cues, clients can see at a glance if they have/haven’t taken a medication, and their carer can see if they are taking their medication correctly.
In addition to taking medication correctly it is equally important to manage the flow of medication information betweenall parties involved.

At this point we assume you have reached agreementwith the client and doctor re prescription managementand communicating medication changes. Some of thesethings are reiterated through this checklist.

Q. Why should I use it?A. Webster-pak makes taking medication safer.It helps to ensure you take your medicine asprescribed by the doctor and to maintain yourindependence. It’s easy to forget or double upeven with just one medication and as you getolder the effects can be much greater and youcould end up in hospital or even in a nursinghome. Webster-pak provides a visual cue soyou always know where you are up to.
Q. Will I still need to see my Doctor?A. Absolutely, Webster-pak is designed toensure you take the medication correctly thatyour doctor prescribes. If you take yourmedication correctly it helps the doctor tomake better clinical decisions. Visiting yourdoctor regularly gives them an opportunity toreview your medication and make decisions asappropriate based on your medical needs.
Q. What makes Webster-pak different fromanything else?
A. Webster-pak is recognised for its distinctivevisual cues. You can see at a glance if youhave/ haven’t taken a medication plus it is easyfor a carer or family member to see that you aretaking your medication correctly.
Webster-pak is tamper-evident. It can’t bespilled accidentally and nothing can bechanged without it being obvious. There is nochance of doubling up or missing doses andnot being able to tell.
The integrity and responsibility for the accuracyof the pack is guaranteed by the pharmacist.
Q. What if I go to Hospital?A. Webster-pak also provides life savinginformation if you are admitted to hospital. Itshows what medication you are taking. Thiscan be critical in an emergency.
Q. What if I need a new prescription?A. This can be discussed with your doctor todecide the best way to handle prescriptions.The doctor may wish to see you or may behappy for the pharmacy to request aprescription direct. The pharmacy can send areminder for a new prescription to you or thedoctor when the last repeat has beendispensed to ensure there is plenty of noticebefore your prescriptions run out. It is importantthat prescriptions are supplied by the doctorbefore your medicine runs out; otherwise thepharmacy cannot legally supply it.
Q. Where are the rest of the tablets andcapsules kept?
A. Only enough tablets/capsules to last oneweek are sealed inside the blister pack. Therewill always be some left over in the original(manufacturer’s) packaging which will be usedfor the following week. The “left over” tabletsare kept at the pharmacy in sealed plasticbags. These are dispensed as normal, eachlabelled with the person’s name. They arestored separately from other peoples’medication. If you wish you can always requestthese medications from the pharmacy.
Q. What if the doctor changes something?A. The doctor must tell the pharmacy directand we can then make the changes as soon asthe doctor thinks it is necessary.

Q. How do I inform my pharmacy ofchanges from my doctor?
A. Each time you pick up your pack yourpharmacist can provide an updated medicationlist. Your Doctor or specialist can write anymedication changes on your list and send it tothe pharmacy (depending on the agreedprocedure for Doctors to communicate withthe pharmacy).
The pharmacy will then update this list and re-issue with your next pack. This Medicationprofile should be kept in a secure place.
Q. What about the medicines that can’t bepacked in a Webster-pak?
A. These will be dispensed by the pharmacy asnormal with directions on the labels. Thepharmacy can list these on the front of theWebster-pak as a reminder for you and also incase someone else needs to know themedicines you are taking. For example; if youwere admitted to hospital. These may also beincluded on your medication profile sheet.
Q. How do I know what the medicinesare for?
A. The pharmacy can list indications on theback of the pack so you can see what eachone is for. We can also help you identify eachpill in case you don’t need to take one. Forexample; you may not want to take your fluidtablet if you are going out for the day.
Q. What do I do if I don’t want to take aparticular medication or the doctorhas told me not to take one for oneor two doses?
A. If you are uncertain about which medicationit is, check with your pharmacist first. Alwayscheck with your doctor as well to make sure itis alright not to take a particular medicationfrom time to time. Also there is a description ofeach medication on the back of the pack. Thiswill help you know which medication is which.You may also wish to go through this with yourpharmacist before starting the pack. It isimportant that you know as much as possibleabout your medications including how toidentify them if needed.
Q. What do I do if I miss a dose?A. Contact the Pharmacy or your doctor andyou will be advised. In most cases you wouldjust continue with the next dose, but this needsto be discussed.
Q. Are there any special storagerequirements for the Webster-pak?
A. It is important that all medicines are storedsafely away from children and are not exposedto high temperatures or sunlight. Yourpharmacist will advise if any special storage isneeded.
Q. What if I am going away or going outfor the day. Can I take the Webster-pakwith me?
A. Yes you can. Otherwise you might like to usethe Flexi-pak. This allows you to tear off individualdoses or a day or more of your medicines.

Q. What if I am going on holidays for a fewweeks?
A. Some people prefer the convenience of adisposable pack when travelling. This meansyou don’t have to worry about keeping thefolders together and bringing them into thepharmacy on your return.
There is a disposable Webster-pak to help inthis situation.
Q. How easy is Webster-pak to open?A. Select the right time and the right day andthen you simply push the pills through the foilbacking into a cup. There is also a simpledevice called the Pil-Bob that helps to breakthe foil from the back and allows the pills to fallinto it. This can be demonstrated if you wouldlike. It makes it easier if you have arthritis oryour eyesight is not so good.
Q. Most people have a weekly pack. Whatif I need something different?
A. Not everyone needs medications 4 times aday, every day. Your Webster-pak can beprepared to meet your needs so if you only havemedications 2 times a day your pharmacist mightsuggest a 2 weekly pack. The Pharmacist canlabel your pack to identify your specific dosagetimes, so it should always be as clear as possible.
Q. Can more than one pack be suppliedat a time?
A. The pharmacist simply prepares as manypacks as required. Normally each folder holdsone week’s supply. Four folders can be sent fora month’s supply.
Q. What about Safety?A. You have total information on the pack. Bothpacked and non-packed medicines. This isinvaluable in the case of emergency for theambulance or doctor if a you are admitted tohospital. There are descriptions to help withidentification of medication.
The Webster-pak and patient profile are both agreat reference point in this type of emergencythis is why we encourage you to keep themwith you.

Pharmacist Instructionsfor Doctors
Many pharmacists have a great relationship with their local doctors and good
communication makes managing Webster-pak easier.
From the onset it is important to get an agreement with the doctors you will be working
with in regards to Prescription Management and medication changes in particular.
The following page contains some frequently asked questions from doctors. You may
wish to forward this information to your doctor if they are unfamiliar with Webster-pak.
If your Doctors ask any questions not included here please let us know so we can
consider including them in future versions.

Half the battle in providing good medicine is ensuringyour patients take their medication as you haveprescribed. Webster-pak can help give you thisassurance. So you can be more confident whenassessing your prescribed treatment.
Q. What makes Webster-pak different fromanything else?
A. Webster-pak is recognised for its distinctive visualcues. Patients can see at a glance if they have or haven’ttaken a medication, plus it is easy for a carer or familymember to see if they are taking their medicationcorrectly. General Practitioners also find this featuremost valuable. If a patient does not take their medicationthe correct way, it is obvious and steps can be taken toassist them.
Webster-pak is tamper-evident. It can’t be spilledaccidentally and medication can not be changed withoutit being obvious.
There is no chance of doubling up or missing doses andnot being able to tell.

Q. Is it possible patients will visit me less often ifthey are using a Webster-pak?

Q. What if my patient needs a new prescription?A. You and the pharmacist can decide the best way tohandle prescriptions. It may be more appropriate foryour patient to visit you or you may be happy for thepharmacy to request a prescription direct. Thepharmacy can send a reminder for a new prescription toyou or the patient when the last repeat has beendispensed to ensure there is plenty of notice before theirprescriptions run out.
Packs are often prepared 1-2 weeks in advance in line
with the needs of individual patients. From time to time itmay seem like scripts are being requested early,however, the pharmacy needs to ensure they alwayshave sufficient medication/ scripts for the followingweek’s packs. This will always balance out over time.
Q. What if you need to make a change?A. It depends on the procedure you and your pharmacyagree to communicate changes.
There are a few ways we can co-ordinate this:

Each time your patient collects their pack the pharmacycan provide an updated medication list (Patient profile).That way you can write any medication changes ontothat list and send it to the pharmacy.
The pharmacy will then update this list and re- issue withthe next pack. This Medication profile should be kept ina secure place.
Q. What if I need the change made immediately?A. The pharmacy can implement the change when youneed it done. It helps if you specify the degree ofurgency. Is the change required “next dose” “next day”or with the “Next Pack” this helps to ensure thepharmacy allocate resources appropriately.
Q. What If my patient wishes not to take aparticular medication in certain circumstances?
A. If you advise your patients it is ok to not take aparticular medication under certain circumstances thiscan be achieved. If they are not sure how to identify thetablet they can talk to the pharmacy however, there is adescription of each medication on the back of the pack.This will help your patient to know which medication iswhich.
Alternatively this medication can be packed in a separatepack.

Pharmacist Instructionsfor Carers
Many carers are family members or friends who look after someone with a disability,
mental illness, chronic condition or who is frail. Most don’t recognise themselves as
carers. Webster-pak can be a very important tool for carers to use in monitoring if
medication is being taken correctly.
Good communication makes managing Webster-pak easier.
It is important to include carers in any arrangements relating to the Webster-pak. Their
role may be important in Prescription Management and communicating medication
changes.
The following page contains some frequently asked questions from carers. You may
wish to forward this information to your carer if they are unfamiliar with Webster-pak.

One of the tasks you may be responsible for whenlooking after someone is medication management.This can cover everything from picking up aWebster-pak for the person for whom you are caring,to monitoring how they use the pack. You might bethe one who takes them to the doctor or picks upmedication from the pharmacy.
Our Webster-pak service is designed to help makelife easier for all involved.
Q. What makes Webster-pak different fromanything else?
A. Webster-pak is recognised for its distinctive visualcues. Patients can see at a glance if they have or haven’ttaken a medication plus it is easy for you to see if theyare taking their medication correctly. One thing GeneralPractitioners find valuable is that if a patient does nottake medication in the correct order (according topackage labelling) you can tell. This is a good indicatorthat the person needs more assistance to manage theirmedication.
Webster-pak is tamper-evident. It can’t be spilledaccidentally and medications can not be changedwithout it being obvious.
There is no chance of doubling up or missing doses andnot being able to tell.
The integrity and responsibility for the accuracy of thepack is guaranteed by the pharmacist.
Q. Will the person still need to see the Doctor?A. Absolutely, Webster-pak is designed to ensure peopletake the medication that the doctor prescribes, correctly.If people take medication correctly it helps the doctor tomake better clinical decisions. Visiting the doctorregularly gives an opportunity to review medication andmake decisions as appropriate based on particularmedical needs.
Q. What if they need a new prescription?A. This can be discussed with the doctor to decide thebest way to handle prescriptions. The doctor may wishto see you or the patient, or may be happy for thepharmacy to request a prescription direct. Thepharmacy can send a reminder for a new prescription toyou or the doctor when the last repeat has beendispensed to ensure that there is plenty of notice beforeprescriptions run out. It is important that prescriptionsare supplied by the doctor before medicine runs out;otherwise the pharmacy cannot legally supply it.
Q. What if the doctor changes something?A. The doctor must tell the pharmacy direct and thepharmacy can then make the changes as soon as thedoctor thinks it is necessary.
Alternatively, each time someone picks up the pack thepharmacist can provide an updated medication list. TheDoctor or specialist can write any medication changeson the list and send to the pharmacy (depending on theagreed procedure for Doctors to communicate with thepharmacy).

Q. Are there any special storage requirements forthe Webster-pak?
A. It is important that all medicines are stored safelyaway from children and are not exposed to hightemperatures or sunlight. Place on the front of a fridge orcupboard (using a suction hook) so that the pack isalways obvious. A loop on the top of the pack allowsthis.

The Patient profile can also be referred to as a medications list. It is a current record of all medications your client istaking and other key details.
Each time your client collects their pack you can provide an updated medication list. This enables the Doctor orspecialist to write any medication changes on the list and forward it to you for your records (depending on the agreedprocedure established between you and the Doctors).
After receiving the list you can update your records and the Clients patient profile and re- issue with the next pack. ThisMedication profile should be kept in a secure place.
