CASO CLÍNICO - Trombosis. Hipertensión Arterial y Palpitaciones
MOTIVO DE CONSULTA
• Evaluación rutinaria de hipertensión arterial (HTA) y palpitaciones en una mujer de 63 años.
HISTORIA CLÍNICA
Antecedentes personales
• No refiere reacción alérgica conocida a medicamentos.
• Hipotiroidismo subclínico en seguimiento.
• Hipertensión arterial de unos 8 años de evolución.
• No tiene diabetes mellitus ni dislipemia.
• Sin hábito tabáquico.
• Intervención quirúrgica de un quiste en seno maxilar derecho.
• Sigue tratamiento con: atenolol (50 mg/24 h), irbesartán HTZ (300/12,5/24 horas).
Antecedentes familiares
• Su madre falleció de una hemorragia cerebral.
Situación actual
• La paciente es vista en 2006 y diagnosticada de HTA.
• En la consulta actual se aprecia un mal control de la PA, con cifras habituales de 150/90 mmHg.
- Toma la medicación de forma habitual.
- No toma AINEs.
- Come con poca sal.

• Desde el punto de vista cardiológico, refiere:
- Palpitaciones frecuentes, de duración breve. No se ha registrado ECG con los episodios ni ha sido estudiada.
- Mareos leves ocasionales, con inestabilidad, pero sin síncopes.
- No refiere dolor torácico, edemas ni síncopes.
• Es roncadora habitual, aunque niega tener apnea.
EXPLORACIÓN Y PRUEBAS
• Presión Arterial: 147/93 mmHg (media de 3 lecturas separadas por un minuto).
- No hay diferencias significativas en la lectura realizada en el brazo izquierdo.
• Talla: 158 cm. Peso: 72 kg. Cintura: 103 cm.
• Hemiparesia izquierda residual.
• Cabeza: normal.
• PVY: normal. Carótidas: normales, no soplos.
• AC: rítmica; no soplos ni extratonos.
• AR: normal.
• Abdomen: sin hallazgos significativos.
• Extremidades: no edemas; pulsos presentes a todos los niveles, de morfología normal.

• Bioquímica
- Gluc: 102; Col: 126; Tr: 204; HDL: 62; LDL: 143; ácido úrico: 6,6; Cr: 0,6; GOT: 18; GPT: 25.
JUICIO CLÍNICO (INICIAL)
• Hipertensión arterial, sin cardiopatía estructural.
• Palpitaciones no documentadas en ECG.
• Holter de ECG: sin hallazgos significativos.
EVALUACIONES COMPLEMENTARIAS (INICIALES)
MAPA
• PA media 24 horas: 151/90 mmHg.
• PA día: 150/90 mmHg.
• PA noche: 151/87 mmHg.
Conclusión: mal control de la PA. Patrón “no dipping”.
Holter de ECG
• Ritmo sinusal durante toda la grabación.
• FC: 63 (149/43 lpm).
• ESV: 156/24 h. Rachas de TSV de 3-4 latidos.
• EV: 10/24 h.

EVOLUCIÓN CLÍNICA
Servicio de Neurología
Ha tenido dos ingresos en el Servicio de Neurología:
• El primer ingreso , en 2009 por ictus carotídeo derecho en ganglios basales con hemiparesia residual izquierda.
- Doppler TSA normal.
- Ecocardiograma 2D: normal.

• PA 24h: 148/84 mmHg.
• PA día: 149/86 mmHg.
• PA noche: 144/78 mmHg.

• El segundo ingreso en 2010 por Síndrome Neurovascular Transitorio en territorio carotídeo derecho.
- AngioRM de TSA: normal.
- No presenta cardiopatía estructural.
- Tiene pendiente un estudio de hipercoagulabilidad.
Juicio clínico
• Hipertensión Arterial.
• No cardiopatía estructural.
• Palpitaciones no documentadas ECG.
• Ictus isquémicos de repetición.
• No se ha documentado arritmia.
Tratamiento
• En Neurología se pautó:
- Clopidogrel 75 mg, un comprimido cada 24 horas.
- Telmisartán HTZ 80/25 mg, un comprimido cada 24 horas.
- Atenolol 50 mg, un comprimido cada 24 horas.
- Manodipino 10 mg, un comprimido cada 24 horas, por la noche.
Exploraciones complementarias (seguimiento)
MAPA
• PA media 24 horas: 151/90 mmHg.
• PA día: 150/90 mmHg.
• PA noche: 151/87 mmHg.
Conclusión: mal control de la PA. Patrón “no dipping”.

Holter de ECG
• Ritmo sinusal durante toda la grabación.
• FC: 63 (149/43 lpm).

Servicio de Urgencias
• En 2012, ante una situación de estrés notó palpitaciones. Fue a Urgencias, donde se objeti-vó una fibrilación auricular a 130 lpm, que pasó espontáneamente a ritmo sinusal.
Electrocardiograma
• Fibrilación auricular con fvm 77 lpm.
Tratamiento en Urgencias
• Se inició anticoagulación con dabigatrán etexilato (150 mg/12 h) que se ha mantenido hasta ahora.
• En seguimiento, desde entonces, no ha vuelto a tener episodios tromboembólicos.
• Tiene hemiparesia, que ha mejorado levemente tras realizar rehabilitación intensa.

Tratamiento actual pautado en Neurología
- Dabigatrán 150 mg, un comprimido cada 12 horas.
- Flecainida 100 mg, un comprimido cada 12 horas.
- Telmisartán HTZ 80/25 mg, un comprimido cada 24 horas.
- Atenolol 50 mg, un comprimido cada 24 horas.
- Elecor 25 mg, un comprimido cada 24 horas.
CONSIDERACIONES A TENER EN CUENTA
• La fibrilación auricular (FA) es la taquiarritmia más frecuente. Es responsable de la gran mayoría de ictus cardioembólicos.
• El ictus cardioembólico se asocia con un mayor grado de severidad y pérdida funcional.1
• Para pacientes con FA y sin enfermedad valvular, el tratamiento con anticoagulantes antivita-mina K (AVK) es más efectivo para disminuir el riesgo de ictus que el tratamiento antiagregante (64% vs. 22%), con una reducción del ictus con AVK del 61%, con respecto a la antiagregación.
• La tasa de recurrencia de ictus en pacientes con FA con tratamiento antiagregante es de un 15% (frente a un 19% en placebo) y con tratamiento anticoagulante se han reportado reducciones de tasa anual de un 40%.2,3
• Se ha estimado que uno de cada 6 ictus son debidos a la FA, y que en uno de cada 4 ictus se des-conoce su etiología, clasificándolos como ictus criptogénicos. Este tipo de ictus está asociado con una tasa de recurrencia del 14% a los dos años. Además, se sospecha que una proporción significativa de los ictus criptogénicos realmente son debidos a una FA no detectada.4
• El tratamiento anticoagulante está indicado en la prevención primaria de pacientes con FA sin ictus previo y riesgo tromboembólico significativo (valorado mediante el score CHADS2-VASc ≥1) y en prevención secundaria en pacientes con antecedentes de ictus secundario a FA.

• Posicionamiento de la Sociedad Europea de Cardiología ante la escala de valoración de riesgo tromboembólico recomendada (CHA2DS2-VASc) y el algoritmo de decisión a la hora de indicar la anticoagulación, con preferencia de NACO en FA no valvular.

• Las guías americanas y europeas de práctica clínica recomiendan la monitorización ECG durante 24 horas en pacientes con un ictus criptogénico cuando hay sospecha de FA.
• Mediante dicha monitorización electrocardiográfica se ha reportado una tasa de detección de FA de 1,4% - 3,2%.
• La monitorización más prolongada (>24 horas) del ECG ha observado un aumento conside-rable hasta 16,1% de la tasa de detección de FA.

• Recientemente, dos ensayos clínicos multicéntricos e internacionales demostraron la su-perioridad de la monitorización electrocardiográfica prolongada en pacientes con ictus criptogénico frente al Holter de 24 horas.
• Se han desarrollado varias herramientas para facilitar su despistaje mediante monitoriza-ción electrocardiográfica a largo plazo:
- Técnicas no-invasivas como el registrador activado por eventos de 30 días.
- Técnicas invasivas como el Holter implantable que permite análisis del ritmo cardiaco hasta 3 años.
Estudio EMBRACE8
• Se aleatorizaron 572 pacientes con ictus criptogénico a recibir una monitorización con un regis-trador activado por eventos de 30 días o a un manejo convencional con un Holter de 24 horas.
• En el grupo de monitorización prolongada se detectó FA de duración >30 segundos en un 16,1% en comparación con 3,2% en el grupo de control (p<0,001).
• Lo cual resultó en un mayor número de prescripciones de anticoagulantes en el grupo de monitorización prolongada (18,6 vs. 11,1%; p=0,01).

• Se aleatorizaron 441 pacientes con ictus criptogénico, aleatorizados al Holter implantable o Holter de 24 horas.
• A los 6 meses, un 8,9% de los pacientes con el Holter implantable había tenido ≥1 episodio de FA de duración de 30 segundos frente a 1,4% en el grupo de tratamiento convencional.

RESOLUCIÓN DEL CASO
• Se trata de una paciente con hipertensión arterial e ictus de repetición, sin cardiopatía es-tructural.
• Tiene antecedentes de HTA. Aunque refería palpitaciones breves, y desde el principio se sospechó fibrilación auricular paroxística, varios Holter de ECG de 24 horas no detectaron arritmias significativas.
• La paciente fue etiquetada inicialmente de ictus criptogénico y tratada con antiagregantes.
• Al final, presentó palpitaciones más prolongadas, por lo que pudo realizarse un ECG en el que se objetivó una fibrilación auricular.
• Se inició anticoagulación con NACO.
• Desde entonces ha permanecido asintomática, aunque tiene hemiparesia residual que li-mita significativamente su calidad de vida.
CONCLUSIONES
• La fibrilación auricular es una causa frecuente de ictus.
• El ictus cardioembólico tiene mayor mortalidad.
• Debe buscarse activamente la presencia de FA en pacientes que debutan originalmente como ictus criptogénico.
• El Holter de ECG de 24 horas es con frecuencia insuficiente para la detección de la FA pa-roxística en algunos pacientes.
• Varios estudios han demostrado la eficacia de la monitorización prolongada del ECG para detectar FA en pacientes que inicialmente habían sido diagnosticados de ictus criptogénico.
• Esto conlleva mayor indicación de anticoagulación, con el beneficio pronóstico consiguiente, al disminuir las recidivas.
