Please review the 4hOnline Enrollment Forms for information regarding enrolling in 4-H. This document serves as a user guide to the 4hOnline Enrollment System. NOTE: Some counties do not allow on-line enrollment. Medical Release and Health History To ensure the safety of all 4-H participants, each 4-H member and volunteer must provide a medical treatment form upon initial enrollment. This ensures that at any given time, a 4-H volunteer in charge of an activity can reach the parents or guardians of all 4-H members participating in the activity, or can produce a medical treatment form to be used by emergency medical personnel. Additional Application Instructions: 4-H Adult Volunteers 4-H Youth Development Program Volunteers are appointed by the County Director. The decision to appoint or not appoint a volunteer (whether initially or upon renewal) rests with the County Director, in consultation with appropriate 4-H staff. All appointments should be made with the best interest of the 4-H Youth Development Program and 4-H members as the primary criteria. Adults are eligible to participate in 4-H if they are 18 years of age or older. A volunteer cannot simultaneously be a 4-H member. Chaperones must be 21 years of age or older (county age requirement may differ).
