General

Please read these notes and the Fleet Driver Notes for Guidance booklet before you fill in this form.

The theory test question bank may be obtained from the Driver and Vehicle Standards Agency at a price of £3.99. Credit/Debit card payments may be made by contacting 0115 936 6502, alternatively cheques and postal orders should be crossed and made payable to ‘Driver and Vehicle Standards Agency’ and sent to DVSA Swansea Finance, PO Box 716, Swansea, SA1 9QR .

ADI & Licence Details

On the form you will be asked to give details about your driving licence and ADI Registration. For the purpose of joining the Fleet Driver Register, you must be a fully qualified ADI. Fleet Driver Registration will last for 4 years, or until the expiry of your current ADI Registration. If you lose your ADI registration, for any reason you will automatically lose registration as a Fleet Driver Trainer.

Character Details

You will be asked to give details of any motoring or non-motoring offences and these will be taken into account in assessing your suitability. As you are an ADI, most motoring offences will be automatically notified to the Driver and Vehicle Standards Agency by DVLA but you are still expected to notify DVSA of any offences (including motoring offences) as soon as they occur. A very serious view is taken of any failure to do so. If you have declared an offence you should send a separate sheet giving details of the circumstances that led to the offence. It will assist the Registrar in coming to a quick decision regarding eligibility. The Rehabilitation of Offenders Act 1974 explains when a conviction becomes spent and when you do not have to declare it. You can get further advice from the Citizens Advice Bureau, public library, or your legal advisor.

Release of Information

We are frequently asked by outside organisations, including associations which represent driving instructors, for the personal information on our database. Before we can supply this information, we are obliged under the Data Protection Act 1998 to first ask for consent. If you give consent, we would make it a condition that organisations would not sell it on to anyone else. At part 6 of this application form you were asked to tick one of two boxes. These are explained below:

Full means that you are content for the Fleet Register Manager to release your name and address to any company or organisation or member of the public seeking Fleet Driver Training.

Nil means that you do not wish your name and address to be released to any company or organisation or member of the public.

If you did not tick any box, it will be assumed that you agree to Nil release.

Ethnic Origin

We would like to invite you to indicate your ethnic origin on the application form. This will help us to ensure that discrimination does not take place. We hope you will provide this information, but it is completely voluntary.

Fees

Current test fees are available at: www.gov.uk/become-a-fleet-driver-trainer.

Cheques and Postal Orders should be made payable to “Driver and Vehicle Standards Agency” – do not send cash. Postal credit/debit card payments can be accepted, details of which should be completed in the appropriate space on the application form.
