Westfield Youth Assistance Program
Building a healthy Community for

In the youth of today.
Program is a collaboration between Westfield Schools, the City of Westfield, and the Hamilton
County Juvenile Courts.
Program provides coordination of services for families and children.
Process Overview

2. Intervention Plan
Early Intervention Advocate meets with family and a case plan with goals is developed.
Case plans are unique to each situation. Examples include referrals for mental health resources, tutoring,
mentoring, pro-social activities and financial assistance.
3. Plan Implementation
Early Intervention Advocate walks alongside the family for a minimum of six months as they work
through the case plan and goals.

Mentoring
Mentors make a 1 year commitment to spend 1 hour per week with a mentee.
• Children are from areas throughout the City of Westfield
• Children are from all socioeconomic lines
• Children range from 5-17 years in age
• These are wonderful children who are in need of the time commitment and attention from a caring adult mentor
