FACTORS BLOOD AND NEONATAL HEALTH
Kalenteva S.
MD, PhD, Kemerovo State Medical University, Kemerovo
Abstract
Comparative assessment of indicators of the health of newborn boys and girls depending on the mother’s Rh-
factor of the. There was a statistically significant dependence of the frequency and severity of complications of
neonatal period from mother’s rhesus blood group.
Keywords: factors blood, the Rh-factor, newborns, health.
The impact of group affiliation to the social heter-
ogeneity still remains one of the major topics of discus-
sion among scientists. In recent years, there is evidence
of the relation RH supplies human with the develop-
ment of diseases. In China [4], in particular, didn’t find
such association in women with breast cancer; not dis-
covered this relationship and Behcet’s disease [3].
However, in the Kursk medical University [1] relation
of the II blood group and presence of RH-factor with
the development of chronic prostatitis that is associated
with a reduction of strength, deformability of erythro-
cyte membranes and their metabolic activity. In the
United States at 20-year-old observation about 90,000
persons turned out in particular that people with I blood
group the risk of developing heart disease is much
lower in carriers of the second group, it is more by 6%
III - 15% IV - 23%, combined with increasing levels of
factors of hemostasis, inflammation and low-density
lipoprotein [5].
Obviously, morpho-functional differences of the
body there are people with different RH accessory. So,
no S-antigen system of Rhesus erythrocytes combined
with a large development of patient hemolysis after car-
diopulmonary bypass in heart surgery [6].
The aim of the paper study was to conduct a com-
parative evaluation of indicators of the newborns health
depend on the mother’s Rh-factor.
Materials and methods. By the method of con-
tinuous sampling we performed a retrospective analysis
24466 stories of childbirth (form 096/u) and 24622 his-
tories of newborns (form 097/u).
The initial stage of the statistical processing of the
obtained information provides for the application of the
program Microsoft Office 2013 Pro EN to spreadsheet
(academic license Open License 62007606).
Using the above program was carried out the for-
mation of a database. On the basis of the generated da-
tabase was audited, sorting, and coding (encryption) of
the information received. In the process of the next
stage of information processing was formed pivot ta-
bles. For a visual image of materials of the research ap-
plied different types of charts.
To prove statistical significance of the results of
the research were used the software package IBM SPSS
Statistic Base Campus Edition Campus Value Unit Li-
cense v. 24 (license agreement No. 20160805-1 from
30.08.2016 with JSC "Predictive Solutions").
Statistical processing of data was built taking into
account the nature of the distribution of the received
data. The nature of distribution of variables in the con-
sidered complexes was determined using the criterion
Shapiro-Wilk.
The obtained data did not meet normal distribu-
tion. Therefore, to determine the statistical significance
of differences mapping populations used non-paramet-
ric evaluation criteria of study results. To assess statis-
tical significance in the comparison groups (non-paired
populations), we used the criteria Mann-Whitney and
χ2. The estimate of the influence factor on the fre-
quency of complications was carried out according to
the odds ratio (OR) with calculation of 95% confidence
interval (CI). The critical significance level was taken
as 0.05 [2].
Results. The first phase was carried out compara-
tive characteristics of newborn children of the female
sex. For the comparative assessment of health indica-
tors infant girls depending on the Rh-factor of the
mother of the children were divided into 2 groups:
group IF - 9919 female children born to mothers with
Rh-positive blood group and IIF group - 1833 female
children born to mothers with Rh-negative blood (table

On the first day of life and at discharge for new-
borns IF group compared with newborns IIF groups
were more frequent in cerebral ischemia (OR=1,70,
95%[CI=1,41-2.04], p=0.000). Such severe forms of
cerebral ischemia as intraventricular (p=0,028) and
periventricular hemorrhage (p=0.000) was observed in
girls born to mothers with Rh-positive blood group.
Also newborns IF group significantly more often were
diagnosed with congenital malformations (OR=2,55,
95%[DI=1,70-3.97], p=0.000), and only in this group
occurred congenital malformations of the central nerv-
ous (p=0.000) and urinary systems (p=0.000).
Compared with girls born to mothers with Rh-neg-
ative blood, girls born to mothers with Rh-positive
blood group was rare complications such as pulmonary
edema (OR=0,27, 95%[CI=0,16-0,47], p=0.000), the

differences between groups in the frequency of congen-
ital malformations of the central nervous system ac-
cording to the present study is not established
(p=0,183).
Conclusion. According to the conducted research
it is established that the frequency and severity of com-
plications of neonatal period depends on the RH factor
of the mother. So RH-positive blood group of the
mother increases the risk of cerebral ischemia up to
brain edema and intraventricular hemorrhage. RH-
negative mother's blood increases the incidence of such
complications of the neonatal period as respiratory dis-
tress syndrome up to pulmonary edema, intrauterine in-
fection. Installed interesting fact. The risk of malfor-
mations does not only depend on the RH factor of the
mother, but also from the sex of the newborn. So, the
risk of various malformations in male newborns was
significantly higher in RH-negative affiliation of the
mother, and the newborn female with RH-positive af-
filiation of the mother.

CHRONOBIOLOGICAL PREDICTION OF PERINATAL MORTALITY
Kalenteva S.
MD, PhD, Kemerovo State Medical University, Kemerovo
Abstract
In this work established a significant effect of seasons of the calendar year and individual year trimesters of
women on perinatal mortality rates. It shows the relationship of seasonal changes in perinatal mortality rates in
connection with the patterns of the individual annual cycle women. It is established that the result of the joint
effects of two biological rhythms seasonal and annual individual – depends on the coincidence in time of different
phases of these rhythmic processes. When matching with each other unidirectional influences the risk of adverse
perinatal outcomes is more pronounced when the same individual favorable period of the year with a poor season
and, conversely, decreases the severity of the effects of these biological rhythms.
Key words: perinatal mortality, seasonal changes, individual year, forecasting risks.
In recent years increased the interest of biology
and medicine to the chronobiological aspects of health
[1, 5].
The main contribution to the reduction of perinatal
losses contributes to reduction in early neonatal mortal-
ity 49.6%, and to a lesser extent, the stillbirth rate -
34.7% [7]. Hence stillbirth rates depending primarily
on the health status of the mother and the adaptive ca-
pacity of the body, is the main reserve for reduction of
perinatal losses.
The aim of the study was to develop an effective
system for predicting adverse perinatal outcomes based
on the identified chronobiological regularities.
Materials and methods.
At the first stage by way of targeted sample mate-
rials 408 history of childbirth in women with reproduc-
tive losses and 409 case histories of newborns con-
ducted a comprehensive analysis of perinatal mortality.
Phase II of the study based on the data developed
and implemented a computer program "Prediction of
the risk of adverse perinatal outcomes on the basis of
chronobiological factors," and assessed its sensitivity
and specificity.
To develop the risk prediction of perinatal mortal-
ity depending on the month of the calendar year and in-
dividual of the mother, in which conception occurred,
based on the obtained data were used in the calculation
of the coefficients of the likelihood ratio [6]. To build a
mathematical model for predicting the risk of adverse
perinatal outcomes and the creation of the computer
program used the method of multinomial logistic re-
gression [4]. In regression analysis, the level of inclu-
sion in the model was set at p=0.1.
Analysis of the accuracy of predictive methods of
risk was conducted by calculating sensitivity and spec-
ificity, the relationship of the ratio of the likelihood [6].
Results.
Analyzing the seasonal variations of perinatal
mortality (table 1) in different periods of individual
year (IG) found that in any trimester IG the highest rel-
ative risk of adverse perinatal outcomes was observed
in case of coincidence of the trimesters with the spring
period of the calendar year.
The coincidence of the most unfavorable for con-
ception season – spring calendar year with the most un-
favorable period – the fourth trimester – individual
women's year was an increase in risk of adverse perina-
tal outcomes. The risk reduction observed at the com-
bination of two fertile periods - the summer season of
the calendar year and the first trimester of the women’s
individual year (R2 = 0, 5745). There was also the in-
creased risk of perinatal mortality with the combination
of IV trimester of women’s individual year from all pe-
riods of the calendar year (R2 = 0, 9143).
Speaking of individual annual changes in the rate
of prenatal mortality (table 2) in different seasons, it
should be noted a significant reduction in risk of ad-
verse perinatal outcomes in case of coincidence of the
first trimester with all periods of the calendar year, es-
pecially when combined with the most favorable sea-
son is autumn.

The most unfavorable period for conception on fe-
tal death is IV trimester of the women’s individual year
and the spring of the calendar year, and the coincidence
of these periods, the risk of adverse outcomes increases
substantially. When the coincidence of the date of con-
ception in the fourth trimester of the women’s individ-
ual year with any season of the calendar year the risk of
fetal death increases.
At conception in the most favorable period for in-
dicators intranatal mortality (table 3) – III trimester of
the women’s individual year observed a reduced risk of
adverse perinatal outcomes in all seasons, especially in
coincidence with the most unfavorable periods – spring
and winter.

The combination of III trimester with favorable
seasons (summer and autumn) of the calendar year the
risk of intranatal fetal death decreasing. Conversely, the
coincidence of unfavorable conception of the seasons –
spring and winter – from the fourth trimester of the IG
women significantly increases the risk of intranatal
mortality.
At conception in all seasons of the calendar year
the risk of early neonatal mortality decreases from the
first to the fourth trimester (table 4). However, in favor-

On the basis of the developed mathematical model
predicting the risk of adverse perinatal outcomes-devel-
oped a predictive map.

riods of conception (table 5) for the patient with the

ity observed at conception in February, April and May
of the calendar year. To periods favorable for concep-
tion or low risk according to the results of the calcula-
tion include January, June, August, September, and Oc-
tober.

Based on the findings of this report, calculation of
the likelihood, then the effective index the algorithm of
the computer program for determining periods of
conception risk assessment of perinatal mortality.
With the help of a computer program calculated
the effective risk factors for 120 patients of the main
group and 100 in the comparison group (table 6).
The analysis of the obtained results revealed that
there were significant differences between the groups
in the calculation of high and low risk.

the forecasting periods the risk of perinatal mortality on
the basis of chronobiological factors for the main group
and the comparison group (table 7).

The comparison
group 14 86 100
In total 90 130 220
For evaluation of the results was used to calculate
the sensitivity (Se) and specificity (Sp) method.
Se = 76 / (76 + 14) * 100% = 84%
84% of women whose conception occurred in the
period of high risk will have a perinatal mortality rate
in this pregnancy.
Sp = 86 / (44 + 86) * 100% = 66%
This method in 66% of cases at conception in the
period of low risk or lack of influence of
chronobiological factors allows predicting a favorable
outcome of pregnancy in women at risk of perinatal
mortality.
Of sensitivity and specificity to calculate the
likelihood ratio for a positive test result (LR+) and for
negative test result (LR-): LR+ = sensitivity / (1 – specificity) = 84% / 34% = 2, 5;
LR- = (1 – sensitivity) / specificity = 16% / 66% = 0, 24;
LR+ / LR- = 2, 5 / 0, 24 = 10, 4.
Thus, the likelihood of adverse perinatal outcome
for the calculation of the relationship-likelihood (LR+
and LR-) have a core group of women at 10.4 times
higher than women in the comparison group.
Evaluated the risk of adverse perinatal outcomes
with the help of the program "Forecast risk of adverse
perinatal outcomes on the basis of chronobiological
factors" in the validation group (table 8).
Table 8
The contingency risk of perinatal mortality with the period of conception
Groups Perinatal mortality
In total risk factor “+” risk factor “-”
A group of women who had adverse
perinatal outcomes 55 25 80
A group of women with favorable
pregnancy outcome 15 67 82
In total 70 92 162
Upon evaluation of the results the calculation of
sensitivity (Se) and specificity (Sp) method.
Se = 55 / (55 + 15) * 100% = 79%
Sp = 67 / (25 + 67) * 100% = 73%
LR+ = sensitivity / (1 – specificity) = 79% / 27% = 2,9;
LR- = (1 – sensitivity) / specificity = 21% / 73% = 0,28;
LR+ / LR- = 2,9 / 0,28 = 10,4.
Thus, the likelihood of adverse perinatal outcome
in the calculation of likelihood ratios (LR+ / LR-)
women who had adverse perinatal outcomes in 10.4
times higher than in women with favorable pregnancy
outcome.
Conclusion. Annual changes in the indices of per-
inatal mortality depend on the orientation of the influ-
ences coincides phases and individual calendar year.
Seasonal and individualnostyu changes reinforce each
other at the coincidence of favorable periods an indi-
vidual women's year – first trimester – with the best
season – summer (b = 0,03), and in combination unfa-
vourable – the fourth trimester individual of the year
with the worst time of year – spring (b = 0,087). The
coincidence in time of two phases opposite effect on
perinatal outcomes (adverse season with the best tri-
mester, or, on the contrary, a favorable season critical
trimester individual years) individualnostyu and weak-
ens seasonal effects.
Predictive maps and a computer program for cal-
culating the risk of adverse perinatal outcomes, devel-
oped on the basis of established chronobiological pat-
terns have a high sensitivity (79%) and specificity
(73%) of the forecast when testing them on an inde-
pendent sample.

MODERN EDUCATION IN THE PROCESS OF SOCIO – PHILOSOPHICAL PARADIGM
Siniavskii D.
4th year student
Saint – Petersburg University of the Humanities and Social Sciences
Abstract: this article explores the concept of formation of personality. Special attention is paid to self-devel-
opment and self-improvement of personality in the educational process. The proposed material can be of interest
for specialists of psychological – pedagogical cycle.
Keywords: self-knowledge of personality, education, modern global issues of socio – philosophical para-
digm.
Contemporary global problems affect the vital in-
terests of mankind, and therefore requires urgent inter-
national cooperation within the world community. The
impending global crisis, experts associated not only
with the natural, social, psychological, political and
economic factors, but also anthropological reasons,
called a crisis of humanity. The changes occurring with
the person, acquire a crucial role in the global phenom-
ena and cannot be considered a consequence of social
or historical processes.
The international community is seeking global so-
lutions for the development of civilization without so-
cial shocks, which have implemented measures to pre-
serve life in the face of increasing threats of biological
degeneration and spiritual death of mankind. New socio
– philosophical paradigm and implements the concept
of sustainable and continuous development of man-
kind, which determine the real and worthy life through
the transformation process of anthropogenesis, culture
and education.
A. Peccei wrote that to rule the world, the need for
fundamental leap in human thinking, which would be
able to harmonize relations of the person with the clos-
est future. It is possible only if sustainable development
and self-improvement of man himself, his actions, as-
sociated with the philosophical, ethical and religious
life.
The process of human development has an onto-
logical nature as the content of education related to the
areas of human existence and human activity, which in
one form or another and the volume rather assigned the
individual in the course of his professional education.
Continuous improvement of the person and partic-
ipation in the educational process, which gives meaning
to life, the right choice, leading to equitable and sus-
tainable development of society. A truly humanistic
value is the social education that is focused on being
objective. Also put before human tasks of development
of abilities to mastering your own life and ready to be-
come a true subject of his existence.
Humanity through education is looking for the
foundations of a new global humanism, which could be
metaideology, including the major achievements of the
human spirit and unite a growing number of conscious
and socially active people. Human humanism preaches
against in the process of continuous education as a nat-
ural creature, a new look at the relationship to natural,
social, cultural potentials of being and of changing atti-
tudes towards the nature of man himself.
For humanization of the modern global educa-
tional space required by the reality in which human na-
ture is configured to antinucleosome substance of be-
ing. Therefore, modern philosophical paradigm of so-
cial education asks for reconsideration of the
ontological and axiological functions of education from
the point of view of public and personal significance,
change-oriented social order for the solution to the
global and personal challenges of humanity, i.e. socio
– cultural development and strengthening of spiritual
potential of man. Self-consciousness can be achieved in
which a person is able to relate his unique, dominat with
the needs, interests, and sense of global community and
to recognize the relativity, and not absoluteness of their
rights. This raises the need for social organization of
common life of people in cultural traditions as forces
that support the "link of times", the continuity of human
experience, the unity of human self-development.
By definition, John. Dune - the content of the
global social formation, the essence of which is "the
fullness of intelligent sense of human interests", based
on the notion of our world as miraclesto system, in
which a person acts as its regulator. In making its edu-
cational objectives and forms of organization of con-
tent, educational institutions, it is important to remem-
ber that every single subject of education is a repre-
sentative of humanity, and humanity, in the opinion of
V. I. Vernadsky, attempts to solve the mystery in his-
tory "from a human point of view, reasonableness and
good": "for the first time People really understood that
he was a citizen of the world and maybe – have to act
in a new aspect, not only in the aspect of the individual,
family or genus, States or their alliances, but also in the
planetary aspect".
Modern innovative global education rose to the
humane posture of communication to man as part of na-
ture and as a complex system, therefore, the educational
approaches are not divided into separate spheres, with
one-sided lead in development, and are provided by
methods, harmoniously uniting his nature, being and
consciousness. This approach at any stage of lifelong

education, helping to establish links between the per-
sonality of man as a harmony, in the opinion of the
American scientists J. R. Royce, A. Powell: "... man as
an individual tries to find or construct their own "I", and
culture – to create a meaningful image of social reality"
In the personality, as a combination of multilevel
and hierarchical systems, focus mechanisms of the hu-
man and professional adaptation, identify yourself and
your behavior with options in life and profession, it lays
the sources of self-organization and interaction with
other systems. This system engenders a sense of com-
munity of people, humans and other beings, the whole
biosphere as a whole, helps the humanization of all hu-
man activity and knowledge, understanding of their
meaning. As noted by M. K. Mamardashvili: "... the
self – form and a way of being, a particular state of life,
the discovery of its evolution. I would say that person
is "kupna the idea of nature." Socio-psychological sa-
marastroydetal its manifestations does not depend on
omniscience or any of the higher landmarks, this mys-
terious "cultural phenomenon" as any other "findings of
evolution." Human development at the individual level
is a prerequisite for addressing global challenges of our
time education in the establishment of harmonious re-
lationships with oneself, other people and the world.
The creation of continuous educational space of
genuine, existential community, enabling people, re-
gardless of the obstacles in the differences of culture, to
go beyond oneself and to understand each other, to al-
low closer approach to the resolution of problems of
globalization, ensuring integration of the "I" person
with "You" and "We", the transition group, national and
ethical interests to universal. It is impossible not to
agree with the opinion of contemporary German phi-
losopher R. Speman: "... the Objective of education re-
veals the value of the contents of real life, forming a
broad, matching with the reality of interest. Only bring-
ing their interests and desires with reality by measuring
their scale, can be compared with others, and then there
is the ability to understand others and try competing in-
terests"
"Openness" nature of man should be today human-
istic philosophical Foundation for lifelong social edu-
cation. This entity cannot be the same and given the
time, education should not focus on the complete pic-
ture of a person, as it obscures her view of the future.
Therefore, social education is recognized among
scientists as one of the major ways of sustainable sur-
vival of all mankind, as it determines the universal on-
tological and theological Foundation of progress. So-
cial education according to the new socio – philosoph-
ical paradigm of communication to man as part of
nature and challenging the system. Therefore, the edu-
cational approaches are not divided into separate
spheres with a significant lead in development, but by
methods of harmonization of combining nature, life and
consciousness. Contemporary global problems affect
the vital interests of mankind, and therefore requires ur-
gent international cooperation within the world com-
munity.

Abstract
The article is devoted to the development of speaking skills within the Federal State Educational Standard
(FSES). Special attention is paid to evaluation criteria in domestic (SFE, USE) and international (FCE, TOEFL,
IETLS) systems of assessing oral speaking skills. The article presents a comparative analysis of evaluation criteria
and the set of exercises directed to speaking skills development and preparing students for the oral part of English
exam.
Keywords: speaking skills, SFE, USE, English language, education.
First Certificate in
English (FCE), Test of English as a Foreign Language
(TOEFL),
International English Language Testing System
(IELTS).
вать следующим критериям: мотивированность;

FCE, TOEFL, IELTS.
FCE (First Certificate in English) –
TOEFL (Test of English as a Foreign Language) –
IELTS (International English Language Testing
звуков и ударения;
нирование;
ставленной темой;
ческих структур;
Логичность высказывания;
Аргументация своей позиции;
Композиция высказывания;
бок);
Беглость речи;

For speaking tasks that require you to summarize
someone else’s opinion the following introductions are
appropriate: This person believes that, This person
holds that, This person argues that, This person’s view
is that, The reading stated, This person’s point is, The
reading presented, The lecture stated, The lecture of-
fered.
Step 3: State what and why you believe.

statement?
“It is better for students to gain real-worlds expe-
rience than to spend their time in classroom”.
Before you begin speaking, make sure you will or-
ganize your response. You should include these parts in
every spoken response:
Introduction: State your purpose;
Body: Mention a topic sentence that introduces
the main point of the paragraph, examples, facts, and
evidence that help the author achieve his or her pur-
pose. Don’t forget to support each example with details.

Hurry up! They _____ (start) the film!
1. The writer says that we should learn to accept
ourselves as we are. In three minutes write 10-12 sen-
tences expressing your opinion. Tell them to the class.

Abstract
The article deals with the aspect of preschool children' sociocultural development, describing conditions that
affect the level of a child's playing activity as well as the degree of mastering the playing skills. The author de-
scribes the methods of monitoring the level of playing skills' development for 5-6 years old children as one of the
indicator of a child's sociocultural maturity. The article contains examples of educational situations that complicate
playing conditions, the author concludes that it is essential to replenish the child's sociocultural experience directly
in the process of play.

роко образован (73 %);
ностью (12 %);
этикета (10 %);
ссорится» (15 %);
доме порядок» (12 %);
дает руку» (10 %);
