New Employee Form
The ‘New Employee Form’ was set up to help streamline the internal processes associated with hiring a new staff member.
Brad can fill out this form and submit it electronically, sending the request through an approval workflow process

Filling out the e-Form• Brad fills out Steve’s first
and last name• He selects Steve’s
department from the drop down– This field is a SQL query
that pulls in (and syncs) with the list of departments from the intranet Employee Directory application

Filling out the e-Form• Brad enters Steve’s title• He selects the agreed upon
start date from the calendar date tool
• He checks the following options for IT to create a user account for Steve Birk– Add to Active Directory– Create an email in Exchange– Create User in Payroll System– Set up HUB in The Buzz– Send out Welcome Package

Filling out the e-Form
• Brad refers to Steve’s resume then enters a quick blurb about the new employee– This info will be used in Steve’s profile on The Buzz– Steve can modify the info when he starts his new job
• Brad leaves the upload field for Steve’s photo blank– It will be added to the intranet after Steve gets his photo taken for
his security cardhttp://blogs.intranetconnections.com
User-Defined Form Approval
In the Approvals section of the form, Brad selects Helen’s name from the pre-populated list of managers at WCS. Helen will be Steve’s manager, and she needs to approve the info on the form before IT sets up the user accounts

Workflow Approval on FormsWhen Brad submits the New Employee Form, the request is routed to the User-Defined manager first (Helen Whithall)
Helen must approve the request, then it will be routed to Heather Jenkins, the HR manager to approve
Finally Karen Parson, the IT Manager has to sign off on it
The approval process is recorded and attached to the request
Greg Tolsten, the Network Admin, receives an email with the form information included, so that he can create the user accounts

Form Submission
Brad clicks on the submit button to send the form on its way, knowing that he will be notified once it has moved through all the approval stages.
