2 Schedule Your HEALTH Screening
Primary Care Physician* Download and print the Physician Form and Instructions for Receiving Your Health Screening With Your Personal Physician. The instructions are listed on the form.
*Your physician may require a lab visit separate from an office visit. Contact your physician to see if this applies to you to ensure you complete your health screening by December 15, 2021.
LabCorp Download and print the LabCorp voucher and instructions. Take the voucher with you to your appointment. To find a location near you, visit labcorp.com/findalab or call 888-522-2677.
CVS MinuteClinicDownload and print the CVS MinuteClinic voucher and instructions. Take the voucher with you to your appointment. To find a location near you, visit minuteclinic.com or call 866-398-2727.
Onsite ScreeningLocations offering onsite screenings will be listed when you select this option.
Choose your health screening option.Forms and vouchers are available on the Healthy Reyes Portal.
*Find your VIC Employee Number on your payroll check, contact your local HR Representative, or call the HR Service Center at 800-298-9461, option 1.
Once logged in, update your profile with your preferred email or mobile phone information under My Profile in the top, right-hand corner.

Employees:
Visit ReyesConnect > Employee Center > Healthy Reyes Portal (under Single Sign On/Direct Access). The first time you log into the Healthy Reyes Portal, you will be prompted to register. You’ll need an email address, your date of birth and your VIC Employee Number.*
Spouses:
Go to healthyreyes.cafewell.com. The first time you visit the Healthy Reyes Portal, you will be prompted to register. You’ll need a unique/individual email address, your date of birth and your spouse’s VIC Employee Number.*
If you saw your Primary Care Physician before July 1, 2021, and your visit included the 5 health measures listed in Step 4, your visit can qualify as your 2021 health screening. Remember to submit the Physician Form!

Complete Your HEALTH ScreeningPrepare for your health screening
Ensure your health screening goes smoothly by
doing the following:
• Important: Fast for eight hours before — drink plenty of water, but no food or other beverages.
• If you are diabetic, pregnant, on medication, or have a medical condition that prevents you from fasting for eight hours, consult your physician.
Get your screening
Your health screening will include:
• A blood pressure reading
• Fast finger stick or blood draw
• Measurement of your height and weight

• If you complete your health screening and meet the good health threshold by October 15, 2021, you’ll see your discounted medical plan rates in VIC during Annual Enrollment.
• Eligibility requirements and process details are the same for enrolled spouses who received health screenings.

Your Health Screening: 4 Easy Steps
If you have registered on the Healthy Reyes Portal (see Step 1), your results appear there usually within 10 business days of your completed health screening. You must log into your Healthy Reyes Portal account to view results and ensure they were recorded.
• If you met the good health threshold, the Healthy Reyes Portal will inform Reyes Holdings that you qualify for discounted medical plan rates for 2022.
• If you did not meet the good health threshold, you will have the option to complete Reasonable Alternative ActionCard(s) to qualify for discounted rates.
4 Receive Your Results
To meet the good health threshold, you must meet at least four of the following measures:
• Blood pressure below 140/90
• LDL cholesterol below 130
• Body Mass Index (BMI) of less than 30
• Fasting blood glucose below 100
• Triglycerides below 150

Results Are Kept Confidential By law, your personal health screening results are confidential. Reyes Holdings will be notified that you qualify for the discounted rates. Reyes Holdings will not know how you qualified (whether by meeting the good health threshold or by completing a Reasonable Alternative ActionCard). CaféWell follows all patient privacy laws and is prohibited by law from sharing your individual results with Reyes Holdings. For the purpose of helping to plan future benefit offerings and programs, Reyes Holdings will receive a summary report that excludes individual health information. Additionally, CaféWell may collaborate with other Reyes
Holdings vendors to help promote the good health of our employees.

Meet the good health threshold on your health screening in 2021
You will be notified on the Healthy Reyes Portal that you met the good health threshold and a discount will be applied to medical plan rates for 2022.
Complete a Reasonable Alternative ActionCard(s)
You can still earn discounted rates by visiting the Healthy Reyes Portal (see Step 1) to view your Personal Health Itinerary and complete Reasonable Alternative ActionCard(s).
