1. Introduction

1.1 Purpose

The purpose of this document is to communicate and clearly outline Seqwater’s mandatory WH&S expectations and requirements to contractors and suppliers in relation to conducting work at Seqwater sites and facilities. It must be read and understood by all contracted persons conducting work at any Seqwater operated sites and locations. Seqwater places great emphasis on safety, the environment and the quality of our work, and we expect that Contractors who work on our Sites to adopt these requirements. Failure to comply with these requirements may result in termination of the contract or the contractor no longer being engaged to carry out works for Seqwater.

1.2 Scope

This induction handbook applies to all contractors, suppliers, subcontractors and their employees engaged by Seqwater to undertake work at Seqwater facilities.

1.3 Definitions and abbreviations

Term Definition

BAC Breath Alcohol Content

Construction Project Any project that involves construction work where the cost is $250,000 or more. Each construction project will have a principal contractor who has specific duties under the WHS Regulation, in addition to their duties as a PCBU.

Construction Work For Seqwater, any work carried out in connection with the construction, alteration, conversion, fitting-out, commissioning, renovation, repair, maintenance, refurbishment, demolition, decommissioning or dismantling of a structure, other than planned maintenance work.

Contract Manager / Project Manager

An authorised representative of Seqwater responsible for selection of a Contractor or management of a contract or both.

Hazard Something that has the potential to cause harm, including injury or illness as well as damage to property. Substances, plant, work processes, bullying or other aspects of the work environment can be hazards.

PCBU Person Conducting a Business or Undertaking

Prime Contractor The contractor directly engaged by Seqwater herein called “The Contractor”. The contractor’s responsibilities apply to their employees and subcontractors.

Principal Contractor An organisation appointed by Seqwater as the Principal Contractor for Construction Work or a Construction Project as per the Work Health & Safety Regulations 2011 (Qld) s293

Term Definition

Risk Is a measure of the potential consequences (usually harm to people, the environment, property, production or product) arising from a hazard combined with the likelihood or probability of the hazard resulting in the actual occurrence of the potential consequences being considered, and is normally used to determine priorities for control.

Risk Management The practice of systematically identifying, analysing, evaluating risks and understanding the controls in place to manage the risks.

WHS Workplace Health & Safety

WHS Management Plan

A WH&S management plan for the workplace that is written before work on the project commences (ref to Work Health & Safety Regulations 2011 (Qld) s309 for further requirements).

2. Contract requirements

Seqwater’s Contract Manager or Project Manager will communicate contract-specific risks and WHS requirements to the contractor during the tender/procurement and pre-start phases of the working arrangement. The information provided in this Induction Handbook is designed to support other risk management documentation (e.g. WHS Management Plans, Work Method Statements, JSEAs) prepared for the work to be undertaken by the Contractor. Where there is any real or apparent conflict between the content of this Induction Handbook and legislative requirements, the legal requirement shall be followed and the Project Manager will be informed as soon as possible.

2.1 Expectations of Contractors – general

Seqwater expects all contractors, their employees and their sub-contractors to comply with, as a minimum, the same WHS Standards that apply to Seqwater employees, and to: demonstrate a knowledge and commitment toward health and safety, and work with

Seqwater to meet the health and safety obligations of both parties take all steps necessary to protect their employees, agents, sub-contractors and invitees

from any health and safety risks arising from performance of work under the contract co-operate with Seqwater and any other Contractors at the work site in the pursuit of

positive workplace health and safety outcomes.

All contractors must read and adhere to the conditions of this document unless they can provide an improved method of work to Seqwater’s policies, procedures, work instructions and conditions. These methods must be assessed and approved by Seqwater’s Contract Manager prior to any work commencing. All work on Site is subject to the Seqwater’s policies, procedures, work instructions and conditions.

2.2 Expectations of Contractors – specific

The specific expectations that Seqwater has of its Contractors include, but are not limited to, the following:

Perform work in a manner that minimises the risk to the health and safety of any person who might be involved in, or affected by the work.

Comply with all applicable requirements under:

— The relevant Qld WHS or related legislation (e.g. WH&S Act 2011, WH&S Reg 2011, Electrical Safety Act 2002, Electrical Safety Reg 2002, and Transport Operations (Marine Safety) Act 1994)

— Any relevant codes of practice (e.g. the five codes of practice supporting the Electrical Safety Act 2002)

— Any relevant industry standard.

Positively participating in the Seqwater induction process.

Ensure that only appropriately qualified, licensed and competent personnel undertake the work, and workers are authorised to carry out specified tasks.

Provide adequate resources to ensure that the work activities are carried out safely and without adverse impact on the environment or water quality.

Comply with access and security protocols, and site rules as directed by Seqwater, and not allow unauthorised access by persons to the work site.

Prepare and have approved by Seqwater prior to commencement of the work, risk management documentation (e.g. Construction Safety Plans, Work Method Statements, Emergency Response Plans, etc.) for the activities to be carried out, and ensure implementation and compliance in the workplace.

Where design work is to be completed by the Contractor, involve Seqwater technical and operational staff in the design review, identify and minimise or eliminate potential hazards and risks during the construction phase and for the end users.

Participate positively in consultation about potential hazards, risks and controls.

Provide copies of licences, certificates, documents and records (e.g. plant/machinery service records) as requested by Seqwater as evidence of appropriate management of WHS issues.

Ensure that all machinery, tools and equipment including personal protective equipment used to conduct the work, are inspected and serviced, maintained, tested, and/or calibrated in accordance with manufacturer’s instructions and any relevant Australian Standards.

Ensure competent supervision and monitoring of work and personnel, appropriate to the level of risk of the activities.

Comply with lawful instructions given by any Seqwater employee, including a direction by a Seqwater employee to suspend work because of unsafe practices, behaviours, and/or conditions.

Conduct pre-start, safety and toolbox meetings and participate in any meeting called by Seqwater to deal with WHS issues.

Maintain good housekeeping practices.

Participate in evaluations by Seqwater of WHS performance on site as a means to encourage continuous improvement for all parties.

Advise Seqwater immediately of any WHS incidents that occur, and the corrective/preventive actions that have been/will be taken to prevent recurrence.

Notifying Seqwater immediately of any changes in Contractor and sub-Contractor personnel details.

Not wilfully or recklessly interfere with or misuse anything provided for health and safety and environmental protection.

Not wilfully place at risk the health and safety of any person at the workplace.

Participate in the Seqwater Fit4Work program by undertaking mandatory alcohol and other drugs testing. Testing is performed randomly on an occasional basis by an external provider. Refusal to undertake a test, or the return of a positive result from a test, will result in the individual being removed from the site. Individuals who are removed from a site cannot return to work for Seqwater at any site until they show a negative test result.

2.3 Sub-contractors

If the there is a requirement for a Contractor to employ another organisation to assist with the task for which they have been employed, the company hired by the Contractor will be considered by Seqwater as a subcontractor of the Contractor and not of Seqwater. The Contractor will be responsible for the organising all of the inductions, training, site specific requirements and documentation required by Seqwater and the conduct of the subcontractor. The documentation must be: Relevant to the specific scope of the work being performed. Up to date and detailed to include all aspects of the work to be performed. Any breach of Seqwater’s systems by the subcontractor will be deemed a breach by the Contractor.

3. Process

The process Seqwater applies to the management of Contractors is described below. Other requirements, specific to the activities to be undertaken by the Contractor, will be included in the Contract.

3.1 Tender/procurement process

Seqwater will identify the hazards/risks relating to the work environment for the scope of work and provide this information to the prospective Contractors. The potential Contractors will be asked to provide example hazard/risk control plans for all of the identified hazards/risks associated with their activities. Potential contractors will factor implementation of the control measures into the price for the scope of work.

3.2 Pre-qualification

For major contracts and potentially high-risk activities, the potential Contractors are required to complete a pre-qualification questionnaire relating to their WHS management system and past performance. In making decisions on selection of a Contractor for other contracts and activities, Seqwater takes into account any previous performance evaluations undertaken by Seqwater on the prospective Contractor.

3.3 Preparation

3.3.1 Terms and conditions of contract

For major contracts, specific WHS requirements are included in the Terms and Conditions for the contract. For other contracts, Seqwater specifies requirements in a letter which is sent to the Contractor prior to commencement of work, and the Contractor signs a Declaration (refer Appendix A) of their understanding and commitment to meeting the requirements and returns the signed Declaration to Seqwater.

3.3.2 Plans, work method statements and permits

The Contractor shall prepare risk management documentation as relevant to the work, including preparation of: Construction Safety Plans (as required by WHS legislation for Construction Work) Traffic Management Plans Work Method Statements and Permits as required by legislation or as otherwise specified

by Seqwater Risk assessment and risk status reports for High and Extreme risks.

The Contractor shall submit the documentation to Seqwater for approval prior to works being undertaken. The Seqwater Contract Manager may require these to be varied or for additional detail to be added to meet Seqwater’s requirements.

3.3.3 Emergency preparedness

The Contractor shall prepare Emergency Response and Evacuation Plans specific to their work environment aligning to existing Seqwater emergency plans, and conduct training for their employees and sub-contractors in these procedures. Where a contractor is working on a Seqwater asset and an evacuation plan is already established, the contractor is to utilise the requirements of this evacuation plan. Specific emergency planning is required for activities such as working at heights, excavations and trenching, diving, working in or near water and entry into a confined space.

3.3.4 Design

If the work involves design: Seqwater provides the Contractor with information about identified hazards and risks

specific to the design. The Contractor consults with relevant Seqwater technical and operational personnel

about the design to ensure that it does not present a risk to health and safety during construction and to end users. The Contractor documents this review and provides Seqwater a copy of the review. The Contractor provides Seqwater with advice about risk controls to be applied during construction and information in the Design Report about safe use and maintenance of the end product for its intended purpose.

Where necessary the contractor will perform a HAZOP study to ensure all risks are identified and controlled.

The Contractor repeats the process for any change in the design or scope of work.

3.3.5 Construction work

If the work is construction work as defined by WHS legislation: Seqwater provides the Contractor with information about identified hazards and risks

specific to the site, and how the Construction Work might be undertaken in a way that could prevent or minimise risk to health and safety.

The Contractor plans and manages the Construction Work in a way that prevents or minimises risk to the health and safety of all persons on or near the site.

3.3.6 Kick-off (or start up) meeting

Seqwater will hold a kick-off meeting with the Contractor at the work site to: provide induction documentation complete a site specific induction finalise risk management documentation confirm Contractor’s arrangements for supervision of work and personnel establish a communications protocol about WHS and environmental issues determine a mobilisation and site establishment plan to ensure disruptions are

minimised, and work areas and requirements are clearly defined resolve any outstanding issues prior to commencement of work.

3.3.7 Details of personnel

The Contractor will provide to Seqwater details of their key employees and sub-contractors including positions and emergency contact numbers. Seqwater maintains a register of Contractors, their employees and sub-contractors.

3.3.8 Licences, certificates, qualifications, registrations, training and competency, insurances and other records

The Contractor will provide to Seqwater prior to the commencement of the work copies of: relevant licences, certificates and qualifications for their employees and subcontractors

and other health and safety records relevant to the work (e.g. training records where potentially high-risk activities are involved). Licences and certificates include driver’s licences, Occupational Licences/Certificates, and Licences to work in Prescribed Occupations. Registrations include personal Registrations such as Registered Professional Engineer Queensland (RPEQ).

3.4 Prior to commencement of work on site

3.4.1 Seqwater induction

All Contract Workers must complete a Contractor WHS Induction by reading this Induction Handbook and relevant Seqwater policies and procedures and sign off the Corporate Safety - Contractor WH&S Induction Acknowledgement Form (FRM-00174, refer Appendix B) before they commence work at Seqwater sites. The completed and signed form is to be provided to the Seqwater WH&S Team at [email protected] or PO Box 16146, City East QLD 4002. Once this information is received, Seqwater will issue a Seqwater Contractor Induction Card which must be carried with employees when attending Seqwater sites and produced upon request. The Seqwater Contractor Induction Card is to be renewed every 2 years.

3.4.2 Site-specific induction

Every worker must complete a site specific induction prior to commencement of work. Records of inductions are kept by the party carrying out the induction.

3.4.3 Work area induction

The Contractor shall induct all of their employees and sub-contractors into the requirements for risk controls including the requirements of the approved Safety Plans, Work Method Statements and Permits, and ensures that all personnel sign-off their understanding on the Plans and Work Method Statements. The Contractor shall induct all other persons required to enter or work on the site prior to those personnel entering the work site.

3.4.4 Personal protective equipment and clothing (PPE)

The Contractor must: provides PPE to their employees appropriate to the potential risks ensures that sub-contractors have appropriate PPE ensures that the PPE conforms to relevant Australian Standards and that the PPE is

inspected, maintained, cleaned and stored properly

3.5 During the course of the contract

3.5.1 Implement risk controls

The Contractor shall implement risk controls as identified in the risk management documentation (Plans, Work Method Statements, JSEAs, Permits, etc.) Additional risk assessments must be undertaken for any new or changed activities. The amended Work Method Statement/s and Plans must be approved by Seqwater, and information communicated to relevant personnel. The Contractor’s personnel and sub-contractors participate in emergency evacuation and emergency response plan testing as applicable to the work undertaken.

3.5.2 Interacting with the community

Seqwater is committed to maintaining positive relationships with the community and requires that contractors undertake the following whilst undertaking Seqwater work: Erect temporary signage at the work site, which will be provided by the relevant project

manager and returned once the work is completed. Signage is particularly important when water supply, traffic, access to properties or amenities is affected.

Treat private property respectfully and take care when undertaking work beside homes, (such as no loud talking, music, or bad language, avoid parking or access disruptions, take your rubbish away with you, etc.)

Follow local council noise regulations during construction work (no noise before 7.00 am and after 6:30 pm)

Take note of any issues that may impact on future work at or around your worksite (such as pets and other animals, fencing, access) and report them to the Seqwater project manager.

Always ensure property is left as found, including gates, and that no rubbish or work materials are left behind.

3.5.3 Supervision and monitoring arrangements

Where the residual risk assessment undertaken has resulted in: A high or extreme inherent risk rating (risk rating prior to the implementation of risk

controls) - the Contractor ensures that there is a high level of competent supervision of the activity on site at all times while the work is being undertaken. A Seqwater employee may remain on site or may regularly visit the site to monitor compliance with and effectiveness of the documented risk controls (e.g. Work Method Statement).

A medium inherent risk rating (risk rating prior to the implementation of risk controls) - the Contractor ensures that a person on site is designated as the job supervisor and that person understands their responsibility for health, safety and the environment, and that there is overview provided by a competent person nominated by the Contractor to monitor compliance with and effectiveness of the risk controls.

A low inherent risk rating (risk rating prior to the implementation of risk controls) – the Contractor ensures that there is general supervision of the activity including the implementation of the risk controls.

The Contractor implements the supervision and monitoring arrangements including undertaking regular inspections and observations of work practices to ensure compliance with the risk controls. The Contractor takes timely corrective/preventive action to address non-compliance or deficiencies in expectations, and implements additional or changed risk controls where existing controls do not reduce the risk to at least a Medium risk rating. All workers are prepared to stop their work or the work of others if anything is unsafe or they are unsure as to whether it is safe to continue the work.

3.5.4 Change in Contractor’s employees and sub-contractors

The Contractor will advise Seqwater in writing of any changes in personnel undertaking work on the contract, including details of names, positions and emergency contact numbers, and copies or evidence of relevant licences, qualifications, registrations and training/competency for new personnel. Prior to coming on site all new personnel must undertake the contractor induction and obtain a contractor induction card. Seqwater maintains the Contractor Register.

3.5.5 Additional training

The Contractor will provide additional training and information into the requirements for risk controls including agreed Plans, Work Method Statements and Permits to employees and sub-contractors, and ensures that all personnel sign-off their understanding with records of training kept.

3.5.6 Insurance renewals

For all Contractors, Seqwater will be provided with copies of relevant insurances every twelve months from the contractor.

3.5.7 Contractor performance evaluations

Where the duration of the work is five days or more or where the risk rating for any of the activities being undertaken is assessed by Seqwater as a potentially High or Extreme risk, Seqwater will undertake site inspections and evaluations of contractor performance. For long-term or construction projects this will commence with a start-up check in the first six weeks of the work.

3.5.8 Non-compliance

Where non-compliance against risk controls and safe work methods are identified by Seqwater, Seqwater will immediately advise the Contractor. The Contractor must comply with all lawful directions provided by Seqwater and implement timely corrective/preventive action in respect of the non-compliance, for example: review and improvement of the risk controls in the Plan or Work Method Statement; suspending work until corrective/preventive action is taken to resolve the non-

compliance/unsafe work practices; removal of personnel from site in the case of serious or repeated non-compliance.

Where the contractor becomes aware of non-compliances, the contractor must complete an incident notification form and provide it to Seqwater within 24 hours.

3.5.9 Consultation and communication

The Contractor shall conduct pre-start and safety/toolbox meetings with attendance by the Contractor’s personnel and sub-contractors. Records of the meetings shall be kept including attendance and any actions arising. For long-term or construction projects, Seqwater and the contractor will hold a combined project meeting at least fortnightly to address project and WHS issues Both parties (Seqwater and the Contractor) shall ensure that there is an appropriate level of communication about WH&S issues. The Contractor cooperates in attendance at any meeting called by Seqwater to discuss WH&S issues

3.5.10 Incident, near-miss incident and hazard reporting

All reputation, personnel injury, near-miss and incidents involving employees, sub-contractors or the public must be reported immediately to the Seqwater contact. The Contractor will cooperate with Seqwater in any incident investigations. Where an incident involves: a reportable injury or work-related illness to the Contractor’s personnel; or an incident resulting in a dangerous event,

the Contractor will report the incident to WHS Queensland, will provide a copy of the report to Seqwater.

The Contractor’s employees and sub-contractors must report hazards to the Contractor. The Contractor will take action to rectify the hazards under their control in a timely manner reflecting the potential risk. Where rectification of the hazard is outside their control, the Contractor will contact Seqwater to discuss action to be taken in response to the reported hazard.

3.5.11 Compliance notices or directives issued by a regulator

The Contractor will provide to Seqwater copies of any Notices or Directives issued to them in relation to the work being undertaken for Seqwater within 24 hours of the Notice/s or Directive/s being issued. When the Contractor advises WHS Queensland of the actions and completion dates they will take in response to the Notice/s or Directive/s, they will also inform Seqwater. The Contractor will advise Seqwater when the actions have been implemented and the Notices/Directives closed out.

3.6 Risk assessments

The Seqwater Risk Assessment Process is included in this document as an example to follow in risk assessment tasks. The contractor must demonstrate the successful implementation of a similar system, or adopt the Seqwater processes. To assess the level of risk, the likelihood of an event occurring (will it happen or could it happen?) and the extent of the consequences that could result (if it does occur, how serious will the outcome be?) must be considered. Likelihood should be estimated based on a realistic determination of the event occurring, while consequence should consider the most-likely credible scenario rather than the worst possible scenario. Both factors are equally important in establishing the level of risk however the consequence factor should be determined first when using the risk matrix as it will define the specific likelihood. A risk matrix is used to determine the risk level which is categorised as Extreme, High, Medium or Low. Immediate actions and controls must be undertaken to ensure the risk is eliminated or managed. The Table 1 Risk Tolerance and Action Priority and Table 2 Hierarchy of Controls shall be followed when developing and implementing control measure.

3.7 During and on completion of the contract

Seqwater will at times inspect, review or audit the documentation, work activities or other matters pertaining to its Contractors. Seqwater may issue the Contractor with a Non-conformance Report if the Contractor does not comply with the requirements of the WHS Expectations for Contractors and Suppliers, or breaches any other conditions found unacceptable to Seqwater. The Contractor must complete a corrective action for the recorded Non Conformance and a record of Investigation and return to the Seqwater Contract Manager within the period set in the Non Conformance Report. Seqwater will compile a summary of issues arising during the contract including outcomes of Contractor performance evaluations and will maintain these records for future reference.

4. Site rules

4.1 Security, Site Access and Work Controls (SAWCS)

4.1.1 General

Contractors working at Seqwater sites must ensure they meet the necessary site access requirements depending on the type of work to be undertaken (refer table below). This will require additional induction and training in the Site Access and Work Control System, Permit to Work, and other specific risk processes used on Seqwater sites.

4.1.2 Contractor personnel and visitors

The Contractor must establish a methodology for accounting for all personnel who are on Seqwater sites at any time. All visitors must be escorted.

Notify the relevant person. Contractors arriving at Seqwater worksites must notify the relevant Seqwater supervisor/manager on arrival.

Site specific induction: Contractors must have completed a site specific induction for facilities where such an induction system exists (e.g. dams and water treatment plants)

Sign in/sign out (where relevant). Contractors must sign in/sign out at the reception area or entrance to the site every time they enter and leave a Seqwater worksite where a sign in/sign out system exists (e.g. dams and water treatment plants).

4.1.3 Keys/access pads

Access by the contractor to remote and unmanned sites may be required, where a key, swipe card or access code will be required from the Area Coordinator. These may also be issued where on-going access is required to a manned site. Any keys, swipe cards and personal safety locks issued to the contractor must be signed for and returned upon completion of work. Keys, swipe cards and access codes issued to the contractor must be kept secure at all times and Seqwater must be notified immediately of the loss of any issued key or swipe card. Access codes must not be provided to others for access to the site, and will be changed upon completion of the work. Unmanned compounds and buildings must be locked prior to leaving the compound/building. Special access requirements will be advised to contractors if they are applicable.

4.1.4 Alarm systems

Alarm systems are installed in many Seqwater assets on access doors and hatches. The Contractor will be issued with instructions for temporary disablement and re-engagement of the alarm system as required.

4.1.5 Unauthorised and restricted access areas

All persons on the Site must comply with signage related to unauthorised and restricted access areas. Entry to Confined Spaces is permitted only to persons who have current competency in Confined Space Entry. Entry to Low Voltage and High Voltage electrical installations is not permitted unless accompanied by a qualified LV/HV authorised person or a licensed electrician.

4.1.6 Permit to Work

The Permit to Work (PTW) procedure defines the minimum requirements to provide a safe system of work when plant is taken from service for breakdown, maintenance or capital work and that is not controlled as Pre-Approved Work. The system also provides a trigger for the use of 2nd Tier Permits for high risk work such as hot work, work at heights, confined spaces work, excavation, high voltage, and removal of flooring, handrails and grid mesh. The PTW system provides a notification of intent and an approval process, to undertake a specific scope of work. A PTW is issued to: Ensure all hazards are identified and the associated risks are adequately controlled to

reduce the risk to an acceptable level. Detail how equipment should be positively isolated following the Energy Tag and Lockout

procedure. Identify and record the task being undertaken and the workers carrying them out.

Permits to Work would be required for any situation requiring positive isolation of all energy sources to protect the safety of individuals. Examples of such energy sources are: Electrical energy, e.g. Power supply to a pump. High Voltage Influx of water or liquids, e.g. Scour valve on a main inside a pit. Material under pressure, e.g. Steam and compressed air, water inside a rising main. Rotating or moving machinery, e.g. Sludge scraper in a sed tank. Toxic or caustic chemicals, e.g. Chlorine gas line. Potential energy, e.g. scour valve on an intake tower riser

As there is not a written procedure to cover every situation that arises, all workers dealing with potentially hazardous situations must be well trained, competent and capable of making informed decisions.

4.1.7 Energy tag and lockout

The Energy Tag and Lockout procedure (PRO-00014) details the requirements for effecting positive isolation of energy in order to prevent the possibility of inadvertent re-energisation of the isolation and minimise the risk of uncontrolled movement of equipment or release of energy that could lead to an incident. The procedure provides details in the type and use of hardware (e.g. locks, tags, etc.) used in the isolation of equipment and plant. Personnel undertaking isolations must complete Site Access and Works Control training prior to participating in any isolations.

4.1.8 Pre-approved work

The Pre-Approved Work procedure describes the minimum requirements and processes used to determine and undertake Pre-Approved Work. It provides guidance and authorisation to Seqwater workers on performing routine low risk tasks that will not impact on the safety of others or the ability to supply water to customers. Pre-Approved Work is work that has been assessed and approved by a team authorised by Level 3 Seqwater Management as a task that satisfies the following criteria: Is routine or scheduled Is low risk Does not impact on the safety of others or the ability to supply water to customers Is supported by a work instruction and JSEA for each operational site based task Is supported by appropriate training and competency assessments

If the above criteria cannot be met, the task defaults to the Permit to Work Procedure until such time as all factors have been undertaken and approved.

4.1.9 Access roads and gates

Caution must be exercised when accessing Seqwater site via single lane unpaved roads which do not have designated markings. The Contractor should consult with Seqwater to assess whether a 4-wheel drive vehicle is required for access. The Contractor’s personnel and sub-contractors must apply defensive driving techniques and limit speed to safely handle the road conditions.

4.1.10 Public safety

Where there is potential interaction with the public in the performance of the work the contractor must implement controls to reduce the risk such as through the use of cordoned/fenced/barricaded off work areas or implementation of traffic management on road ways.

4.2 Potential site hazards

4.2.1 General

The major hazards associated with the work on Seqwater facilities are detailed below. Notwithstanding that Seqwater has provided this information, the Contractor remains responsible for identifying all potential hazards and risks associated with the work under the Contract, and ensuring that appropriate risk controls are put in place. A list of the Generic Hazards associated with Seqwater sites is outlined in the Pre-Tender Hazard Identification Form.

4.2.2 Asbestos-Containing Materials Register

Asbestos or Asbestos Containing Material (ACM) is present at many Seqwater worksites and relevant information is contained in the Asbestos Register for the site. When conducting maintenance work involving drilling, cutting or altering part of a building where ACM may be present, contractors must consult the site Asbestos Register and perform work to standard equal to the Seqwater Asbestos Management Plan (PLN-00055). ACM can only be removed by a licensed contractor under the direction of Seqwater.

4.2.3 Hazardous chemicals

At Seqwater worksites there are a range of hazardous chemicals present. Information on the hazardous chemicals kept on site will be provided at the site induction. It is important to be aware of the requirements of working in the vicinity of hazardous chemicals, including PPE, first aid and emergency procedures. For more information refer to the Seqwater Hazardous Chemicals Procedure or specific procedures regarding hazardous substances e.g. chlorine or fluoride. The use of hazardous chemicals by contractors also presents a hazard to water quality. No substance is to be brought onto any Seqwater site without the full details having been supplied to the relevant Seqwater manager/supervisor by the contractor. The required details are listed below: A full description of the substance, including name, use, quantity, etc.; A summary of the hazards associated with the substance; A copy of the Safety Data Sheet (SDS); A copy of the risk assessment relating to the specific use of the substance on Seqwater

property; Work procedures covering the safe storage, use, disposal, first aid and emergency

response; and Evidence that people using the substance are aware of the above details.

Seqwater will prevent a contractor from using a hazardous chemicals if it is not satisfied that the chemicals has been adequately identified or that the hazards have not been adequately controlled.

Unlabelled containers are prohibited at Seqwater sites. Drink containers must not be used for any other purpose than as drink containers, and must not be used or kept near chemicals.

4.2.4 Manual handling

Manual handling issues have been identified as the major cause of injuries at Seqwater sites. The Contractor must instruct its personnel in the correct manual handling techniques to apply in any circumstances including use of mechanical aids.

4.2.5 Slip, trip and fall hazards

Caution should be exercised and appropriate risk controls applied in accessing Seqwater infrastructure to avoid slip, trip and fall hazards where: surfaces may be wet, water-logged, slippery, or covered in slime or algae there are gravel or stone surfaces there are steep slopes there is excessive aquatic or terrestrial weed growth there are structures that are not fully enclosed or have hand-rails

4.2.6 Working over or near water hazards

The hazards associated with working over or near water include: Falling into the water and drowning. Being swept away by fast moving water and being injured or drowned. Falling into water with electrical equipment and suffering electric shock. Falling into cold water and suffering hypothermia. Slips, trips and falls on wet, uneven ground near water. Manual handling injuries on wet uneven ground near water. When planning work over or near water, supervisors are to assess and address the risk of: People falling from structures into water (jetties, walkways, ladders). People falling into water from adjacent natural ground (banks, shoreline). Water current and temperature. Climatic conditions (e.g. rain and sun). Electrical equipment coming into contact with water. Injuries to people working in isolation. Rescue resources and access abilities.

The contractor must apply appropriate systems to prevent persons falling into water, and have adequate water rescue capability and Personal Protective Equipment.

4.2.7 Remote locations

The Contractor must implement communication protocols and journey planning for all activities involving travel and work in remote locations. Journey planning must include provisions for management of potential fatigue.

4.3 High risk works which require Second Tier Permit

Second Tier Permits are required for the following high risk work activities: Confined space work Hot work Trenching and excavation High voltage Fault finding Work at height Grid Mesh, Hand Rail and Flooring Removal If a Second Tier Permit is required the Second Tier Permit Authority must fill out the required permits and implement the appropriate controls as described in the JSEA for the work.

4.3.1 Confined spaces and restricted access areas

Many Seqwater infrastructure locations and components are either Confined Spaces or Restricted Access Areas, and are marked as such. A Confined Space is any enclosed or partially enclosed space that is at atmospheric pressure during occupancy and is not intended or designed primarily as a place of work, and is liable at any time to:

Have an atmosphere, which contains potentially harmful levels of contaminant: or Have an oxygen deficiency or excess; or Cause engulfment; or Could have restricted means of entry

Only persons with a recognised and current confined space entry qualification are permitted to enter confined spaces at Seqwater worksites. In order to perform work in a Confined Space at a Seqwater worksite you must: Provide evidence of a recognised and current qualification before entry is allowed; Conduct entry and work according to the Corporate Safety - Confined Space Entry

Procedure (PRO-00443); Complete a Corporate Safety - Confined Space Entry Permit Form (FRM-00107).

Further information about confined space requirements, including training requirements and permits, can be gained from the Seqwater Confined Space Entry Procedure (PRO-00443), the Confined Spaces Register, and by discussing with the relevant Seqwater supervisor or manager.

4.3.2 Work at height

There is a potential risk of fall from height of two metres or more, or between distinct levels where injury may occur at many Seqwater facilities. Appropriate risk controls for access to and work on those installations must be applied. The Scope of Works and hazard review supplied to the contractor will identify known work at height hazards, and specific emergency response requirements for persons who may be exposed to falls. Contractors must follow the requirements defined in the Seqwater Procedure – Working at Height (PRO-00015).

4.3.3 Removal of grid mesh, flooring and handrails

Falls through loose panels on grated walkways have resulted in fatalities at industrial facilities. When work involves access via, working from, or modification to grated walkways or pit covers, contractors must follow the requirements defined in the Seqwater Procedure – Removal of Grid Mesh, Flooring and Handrails (PRO-01510).

4.3.4 Trenching and excavation

Due to the likelihood of encountering underground services during any activity where the ground will be disturbed, the contractor must implement strict controls when undertaking any excavation or ground penetrating work. Non-damaging equipment such as high-pressure water jets or vacuum services may be used to manage exposure of underground services. Contractors must follow the requirements defined in the Seqwater Procedure – Penetration and Excavation Safety Procedure (PRO-00302).

4.3.5 Electrical Work

Personnel working on electrical equipment must be licensed Electrical Workers. The following procedures apply to electrical work in Seqwater: PRO-00006 General Electrical Safety PRO-00011 Low Voltage Electrical Testing and De-energisation PRO-00014 Energy Tag and Lockout PRO-00307 Electrical Testing and Tagging PRO-01238 Electrical Fault Finding PRO-00010 High Voltage Isolation and Access

4.4 Qualified and competent personnel

Only appropriately qualified, licensed, experienced and competent personnel (employees engaged by the Contractor or sub-contractor) are to undertake work on site. All Contractors working on a Construction Work site (as defined in the WH&S legislation) must have successfully completed the QLD General Safety Induction (Construction Industry) or equivalent issued by another state. The contractor must ensure that the holder of an interstate card is familiar with the local requirements. Work undertaken by apprentices, trainees and young workers (under the age of 18) must be closely supervised, inspected and approved by the Contractor. It is the responsibility of the Contractor to ensure that an appropriate level and frequency of supervision, relative to the level of risk, of all work.

4.5 Safe work practices

All personnel must follow safe work practices for all work conducted on Site. If personnel are unsure or are unable to comply with safe work procedures, the work must be ceased immediately until the issues are resolved.

4.5.1 Work Method Statements

Work Method Statements must be prepared for any high risk tasks, as defined in the WH&S legislation and Seqwater Procedures. All personnel involved in the work must be inducted into the requirements of the Work Method Statements and sign their understanding of the requirements. Refer Appendix C for further details.

4.5.2 Job Safety Environmental Analysis

Development of a JSEA is a systematic approach to breaking down job tasks into steps and assessing each step for the identification, assessment and control of safety and environmental hazards and risks. A JSEA is required for documenting the risk management process for non-routine tasks of medium to high risk. A JSEA will be reviewed when: the activity steps or conditions change, a new hazard is identified, an incident is reported, an activity re-commences on the site.

4.5.3 Real Time Hazard Assessment (RTHA)

Real Time Hazard Assessment is a documented hazard assessment, or infield review, performed at the intended place of work prior to a PTW being issued, or during tasks where there is a change in conditions. The RTHA process ensures that hazards are identified and controlled before beginning work activities. An RTHA must be completed before starting any task or during the task when there is a change in scope. Personnel working on the same task can perform a group RTHA.

4.5.4 Risk assessments – other

Where a Work Method Statement and Permit is not a mandatory requirement, the Contractor’s personnel and sub-contractors should undertake an informal risk assessment for the work, e.g. by application of a “Personal Risk Assessment e.g. Take 5” or approved contractor used process.

4.5.5 Housekeeping

Safe and hygienic housekeeping practices are to be maintained. Work areas must be kept clean and tidy and free of slip and trip hazards. Waste must be stored and disposed of appropriately and promptly.

4.5.6 Personal protective equipment and clothing

All persons undertaking work on/in on a construction site must wear:

Steel-capped safety boots of good quality and in good repair. Boots shall extend past the ankles with tread in good condition.

Long sleeved shirts and long pants Safety glasses and Hard Hats (recommended with wide brims) in signed areas. Safety gloves where manual tasks are to be undertaken or exposure is possible to rough

surfaces; Hi visibility vests or clothing when working in close proximity of roads and where signage

dictates. Other personal protective equipment and clothing to be worn must be identified by a risk assessment and be appropriate to the work activities e.g. eye protection, hearing, respiratory protection, chemical PPE requirements.

4.5.7 Safety signage

All persons on site must comply with safety signage. If any person is unclear about the meaning of the signage, they must ask the site supervisor to explain the meaning.

4.5.8 Vehicles, mobile plant, tools and equipment

Contractor personnel and sub-contractors are not to use Seqwater vehicles, mobile plant, equipment or tools at any time. Seqwater may authorise specific exemptions, upon request.

Service records for vehicles and mobile plant provided by the Contractor must be made available to Seqwater on request.

Seat belts must be worn at all times and be fitted to all vehicles in which passengers travel.

All tools and equipment brought onto site by the Contractor’s or sub-contractor’s personnel must be of good quality and maintained in a safe condition.

Electrical equipment brought onto site by the Contractor’s or sub-contractor’s personnel must be in current test and tag. No piggyback leads or double adaptors are to be used on site.

RCD extension boxes are permissible for use as long as the reset switches are tested each time the extension box is utilised.

4.5.9 Prohibitions

Inappropriate behaviours such as those listed below will not be tolerated and any person exhibiting these behaviours may be removed from site: harassment, bullying or intentional intimidation of any person practical jokes skylarking discrimination swearing disrespect of any person or discourtesy

theft or misuse of Seqwater property smoking in designated “No Smoking” areas or in any enclosed space on site. No pets or animals are to be brought onto Seqwater assets At no time are weapons or firearms permissible on Seqwater assets without prior

permission from the Seqwater WH&S Manager.

4.5.10 Fit 4 Work

All contractors have a responsibility to monitor and manage their personnel to ensure that they are fit to undertake their duties and are not affected by: Fatigue (PRO-00696) Stress (PRO-00700) Drugs or Alcohol (PRO-01105) Poor fitness (PRO-00020)

Seqwater prohibits any employee, contractor or visitor from commencing work, or returning to work while under the influence of alcohol or drugs. All staff and contractors have an obligation to ensure they arrive at work in a state that enables them to conduct their business activities in a safe and effective manner. To assist all staff and contractors to self-manage alcohol and other drugs in the workplace Seqwater provides self-testing equipment or devices at all sites to facilitate the Voluntary Testing Procedure (PRO-01463). To minimise risks in the workplace due to the misuse of Alcohol and Other Drugs, Seqwater also has an AOD Testing Program that all contractors and staff are required to participate in (PRO-01462). This procedure also defines criteria for testing and outcomes required if test results exceed 0.00 BAC or drugs are detected.

4.5.11 Amenities

Toilets are located only at Seqwater office buildings. Alternative toilet facilities shall be provided and maintained by the contractor as required. The Contractor should discuss the availability and location of other amenities (e.g. lunch area, water) with the Seqwater representative.

4.6 Emergency response

4.6.1 Emergency contact numbers

In the event of an emergency, contact of Emergency Services (phone 000, or 112 from mobiles) should be considered the first priority. Emergency contact numbers for the site are located within the Seqwater Emergency Response Plan for the site.

4.6.2 Evacuation procedures

The Contractor’s personnel and sub-contractors must be familiar with and follow evacuation procedures and any reasonable directions of a Seqwater employee in the case of an emergency.

The Evacuation Plan for each location is kept at the entry point to the asset or building. Contractors are to be aware of the specific evacuation assembly points which are located by a green evacuation sign.

4.6.3 Equipment

Contractors should provide their own First Aid facilities. Larger Seqwater facilities do contain first aid rooms and kits however these are tailored to specific risks at each site such as chemical exposure, and may not provide suitable equipment for all injuries. The emergency eyewash and shower facilities are located at sites where chemical exposure may be present. Fire extinguishers are located in all Seqwater buildings. Different types of extinguishers are available for the types of fires that may be encountered. If personnel have not been trained in the use of the extinguishers, or it is unsafe to use them, personnel should leave the vicinity of the fire immediately and call emergency services. In the event of a chemical spill, a spill kit is located at designated areas where chemicals are stored. If untrained in spill response the contractor shall raise the alarm and ensure safety of their employees before contacting the site Seqwater Operator of the incident.

APPENDIX C - Activities Requiring Work Method Statements Work which may require a Work Method Statement includes the following: 1. Work where a Second Tier permit is required

2. If people are handling, using or transporting a hazardous substance where a chemical risk assessment has not already been undertaken and controls identified.

3. Working with lead-based product.

4. If a structure or building is going to be demolished (also requires that the demolition workers are licensed).

5. If it is Construction Work, and: there is tilt-up and precast materials involved a person will work on or near structures that are being temporarily supported to

prevent collapse powered mobile plant and machinery is moving around the site.

6. If the work is on, near or over water where there is a risk of drowning (including work undertaken while a person is travelling in a boat on the water).

7. All work involving diving.

8. If the work is on or near a road (including channel roads) or railway.

9. If the work is near pressurised gas mains or near gas piping, or near a chemical, fuel or refrigerant line.

10. If the work is near an exposed energised electrical installation including where there is risk of coming into contact with overhead powerlines.

11. All work on cathodic protection systems.

12. All work with alternative power supplies, including UPS testing, removal and replacement; solar cells; and standby generators.

13. Where work is performed at distances from isolation points and involves various points of isolation (also requires a Permit to Work and Isolation Schedules).

14. Removal or installation of a pump and motor.

15. Removal or installation of large meters, pipes, valves etc.

16. All work on structures or components where Asbestos Containing Material is present or presumed to be present, including work on AC pipe.

17. All work involving abrasive blasting.

18. Dewatering water storage (reservoir, balancing storage etc).

19. All work in remote areas (being areas remote from populated areas due to distance, terrain, access, time, communication links or a combination of these and where emergency assistance is a long distance away and/or not readily available.

20. Operating a chainsaw.

21. Any other activity/job assessed as having High or Extreme Risk
