When Lucie had her PEG tube procedure, it was recommended that she use Resource 2.0 as her food source. However:

(a) She had always eaten organic foods, so she would have preferred not to take Resource.

(b) She had a bodily reaction to Resource and could not take it. She thinks the reaction was to the milk proteins in it. She had always limited her intake of dairy products, because they produced mucus and a bit of inflammation, which became a big problem when her swallowing abilities decreased. So this meal plan does not include protein powders based on Whey.

(c) Lucie also had a mild inflammation reaction to Soy, which had not been a problem in her life until her swallowing ability was affected. So this meal plan does not use soy products.

*Lucie slowly gained weight on this diet, from 90 to 95 pounds on the bathroom scale.*Equally importantly to her, she felt that the fresh foods and quality food powders and supplements she ate, maintained her essential vitality, a clear head. She was lucid and communicating until the day she died. Her cause of death was from breathing problems, not food deficiencies. A blood test by her doctor indicated no obvious nutritional deficiencies.

Aside from the Enteral feeding bags and PEG tube provided through the CCAC, the necessary equipment for this diet are:

1- A high quality blender. Lucie’s recipes refer to the Vitamix blender, a high quality, high capacity machine.

2- **A “nut bag” strainer, very fine mesh, available at health food stores. **Necessary to keep fibres from blocking the feeding tube. The mesh is finer than colander strainers.**

3- Velcro, to hold the end of the feeding tube in the stomach tube. Through constant use, the stomach tube end becomes somewhat dilated and the enteral sack end would come loose and leak or spill. Velcro was a home made solution that held them together.

4- Syringes, provided by CCAC, to inject water to flush the stomach tube after use.

Eggs - When using raw eggs, it is important to obtain them ‘farm fresh’ from a trustworthy source. Also, store them in the body of the fridge, which has a more constant temperature than the door. Lucie preferred the yolks only in some recipes, there is nothing wrong with the whites.

All raw foods, including sprouts, need to be washed. Sprouts are harder to wash thoroughly and in some countries they have caused a type of food poisoning on occasion. The person is already having health problems, so needs to get them from a reliable source and consume very fresh.

Water: Lucie lived in a rural area, with water coming from a well. As a precaution, she did not drink it. It was used in washing up, by the rest of the family, etc. If you are on a well, know that Coliform Bacteria contamination is advised anyway and the local Health Unit does it for free. City water is chlorinated so will not contain these bacteria. Sometimes she had access to water that had been energized, balanced for Ph and so on, but that is a subtle option, not a requirement.

Blender heat: The Vitamix blender (and possibly others) can actually heat soups and other mixes until they are very hot. So, Lucie made sure the blender didn’t run too long, because heat destroys some nutritional value. Room temperature was usually comfortable for her.

The Resource product has advantages: it is easy to use, compact, MUCH less trouble when you are away from home, and it keeps you alive. It can be used instead of, or in combination with, this diet.

This diet is a lot of work. About 1 hour per feeding or 5 hours per day, including prep time, feeding process and cleanup. To eat this way, probably most ALS patients and their caregivers would need to be highly motivated, or have help.

Some of the ingredients, such as broth or vegetable purées, can be made ahead of time and frozen. Siobhan did that in her video.

But Lucie always had the blender mix itself fresh, never stored for use later, because once blenderized it oxygenates quickly and loses its value.

This food was prepared in a normal kitchen with normal precautions about cleanliness, hand washing by the cook, etc. It was not a “sterile” laboratory environment. As an extra precaution, Lucie used ‘food grade’ hydrogen peroxide about once a week to clean everything, as an alternative to bleach which can affect some plastics.

Although Lucie did consult a nutritionist in devising this diet, this document does not constitute a recommendation, nor medical advice. It is not a substitute for common sense or ‘listening to your body’. It is one person’s experience. It worked for her. Anyone going down this path should have their diet reviewed by a Registered Dietitian to ensure it meets the individual’s medical and nutritional requirements.

It would also be possible to adopt a strategy of eating what is possible and taking other foods via the tube. As noted in Siobhan Rock’s video, there is a social aspect to it. She was able to eat many of the same foods as her family at meal time. Lucie’s recipes were not similar to the family meals.

**Perhaps the main “take away” point is that it is possible to take whole foods through the PEG tube, you can experiment, it is not rocket science. The main issues are to get enough nutrition and to filter or sieve in order to not block the tube. Exercise lots of care and caution for a person who already has a health problem. It is always recommended to get professional advice.

This table shows all the research she did. It contains some foods she was eating before the PEG tube and some which did not end up in her blender recipes. A person who has trouble swallowing should get professional advice and proceed with caution. If on a tube, use a “nut bag” or other filter or sieve which is finer than most colanders, to not block the tube with fibres.

DATA: there are many web sites with nutrition data. Some are commercial and promote a diet or product. Some are sort of hard to understand. Home made soups are estimated at 100/10/10/300ml for one serving, which may be one cup or at most 1.5 cups; pending more accurate research or a description of the contents of each soup.

Lucie’s nutritional goal each day:

2000 calories / 130 grams of carbohydrates / 100 grams of protein / 1500 ml of liquids

Some days, especially when away from home or dealing with acute symptoms, she did not meet these targets. But when in her routine, at home, she did.

Lucie generally had 5 meals per day, which she called:

o Breakfast Alpha Mix /

o Green Smoothie /

o Meat Broth /

o Alpha Plain Mix /

o Carbohydrate meal

The instructions for each meal, on the following pages, are written such that inexperienced friends or home care workers could do it, if the ingredients and equipment were clearly laid out and labelled for them.

Note on the format of this document in Word. We modified the format of the 5 meal recipes that follow, mainly by reducing page margins, so they would print on one page, then go in a ring book in the kitchen, inside plastic sheet protectors. Very convenient and protects the page from the inevitable food splashes.

NOTES: These instructions refer to a Vitamix blender, i.e. the reference to switches and control knobs.

The references to “Alpha” mean Alpha ENF powder. It is a protein mix, very easy to digest and absorb, because each Amino Acid is separated at source. It is not based on milk or soy. Lucie purchased it on line and had it shipped from B.C. : http://www.nutramed.com/alphaenf

As a rural resident, Lucie was able to get fresh GOAT milk WHEY, which increased the protein value compared to any other liquid, including goat milk itself. She did not the have mucus and inflammation reaction to goat milk that she had from cow milk.

The recipes don’t specify flushing the PEG tube with water from a syringe after each meal, but yes, she always did, it is important to keep the tube clear and avoid any food building up on the inside of the tube.

Many variations on this diet are possible, for each person’s nutritional and lifestyle goals. The main issues have to do with physics: to strain the foods so they don't block the tube, to liquefy it enough so that it flows, and that the food temperature be in the range between room temperature and body temperature, for stomach comfort.

LUCIE’S BREAKFAST MIX

Good morning! Do all the measurements with “relative” accuracy, they don’t have to be perfect. Speed is more important, Lucie is usually very hungry when she wakes. Put in the blender:

1 ½ cups of one of the following liquids, hopefully it was already set out last night so it is at room temperature: rice milk / oat milk / flax milk/ coconut WATER/ or as a last resort almond milk (NOT home made almond milk, it is too thick) (NOT water, it has no nutrition content) (not coconut MILK, it is too thick).

Put the measuring cup in the sink and you can put the other utensils that you use for the other ingredients into it, as you finish with them, to wash later.

2 scoops of Alpha ENF powder (white canister). The scoop is usually inside the canister. Each scoop is ¼ cup, total ½ cup.

2 scoops of Hemp Pro 70 powder (brown canister). The scoop is usually inside the canister. Each scoop is 1 tablespoon, total 2 tablespoons.

2 tablespoons of Olive oil, the metal measuring spoon should be on the tray with the bottle.

1 whole raw egg. Eggs from the carton in the fridge. If the egg shell is dirty, wash it off. Pour the egg into a little bowl to check there is no egg shell going into the blender.

PUT THE TOP ON THE BLENDER !! Flip the right hand switch up, to turn on low speed. While that is still on, turn the middle dial to 10 for medium speed. There is a clock on the wall near the door to the deck. After ONE minute, turn the middle dial down and flip the right switch off.

… the list above are the mandatory ingredients … what follows are things to add if there is time:

o Wheat Grass Juice, a green coloured liquid, 1 ounce. Pour THROUGH A SIEVE, otherwise the fibres will block the feeding tube. The little white sieve should be on the blue tray.

o Food supplements and vitamins: there is a little clear plastic basket, with “Lucie Supplements” written on the handle. Use a paring knife and cutting board to cut the small end off each capsule. OPEN one gelatine capsule from each bottle, and put the powder or oil into the blender when it is NOT turning. Don’t worry if you can’t perfectly empty the capsule. If you drop the capsule into the mix, use a spoon to take it out. If there is a bottle dropper, put 3 drops in. If a bag or sack of powder, add a “pinch”.

o Coconut milk, “Arroy D” brand, it looks like a juice box you take in your lunch. Before opening, or if it is open, holding your finger over the opening, SHAKE it vigorously, then pour in your estimate of ¼ of the carton approximately (we try to use one carton per day).

o “f3” food supplement (white plastic bottle): OPEN one gelatine capsule and pour the green powder in.

Re-mix everything at LOW speed in the Blender. Flip the right switch on, for 15 SECONDS.

Before you pour the mix into the feed bag, CLOSE OFF THE CONTROL VALVE below the bag, so it doesn’t run onto the floor. If you have strained the wheat grass, you don’t need to strain the rest, it is all powders that dissolve well and liquids. After you pour the mix in the bag, quickly rinse the blender so it is easier to clean later. Bring the bag to Lucie. Thank you!

LUCIE’S GREEN SMOOTHIE / This is often her second meal of the day, mid-morning.

Put 1 cup of LIQUID in the blender. This meal is naturally thick, so use WATER as a base. 1st choice is coconut WATER (not coconut MILK, it is too thick); or city water or bottled water or from a source you trust. (Lucie does NOT drink our tap water, from a well, as a precaution.)

You probably will need to blend a little as you go, so it will reduce down and all fit in. PUT THE TOP ON THE BLENDER before you turn it on!!

Flip the right hand switch up, to turn on low speed. While that is still on, turn the middle dial to 10 for medium speed. At some point, maybe only at the end, while still on medium speed, you will go to High Speed by flipping the left side switch up for ONLY 15 SECONDS (otherwise the mix will heat up and lose nutrient value). There is a clock on the wall near the door. To turn it off, go in reverse order: flip left switch down, dial middle switch down, flip right switch down.

Here is your choice of ingredients. Rinse under tap, chop in chunks if required. If they are solid like cucumbers or celery, cut into big chunks about 2 or 3 inches to make it easier for the blender to chop them, but don’t worry, on high speed it is a very strong blender that will chop anything. Note that the compost bucket is to your left.

If these are all in the fridge, it will be too much, so choose only 5 ingredients.

- Avocado, 1/4 only (it is fibrous and will block the feeding tube if you put more)- Celery, 1 stick from the bunch only- Cucumber, approximately the equivalent of 1 cup - Green beans, that have been cooked, 4 beans- Parsley, approximately 1 cup- Pepper, Green, ½ of the pepper, cut in chunks- Spinach, approximately 1 cup packed loosely (or a big handful)- SPROUTS ! Always try to have sprouts in the mix. Sunflower sprouts are usually in a

clear plastic bag, put a really big handful or 1.5 cups. Pea sprouts or shoots come in green plastic containers from a green house, they may be in the cold room instead of the fridge, put one container in. Put both Sunflower and Pea sprouts in, if both are available.

If the mix gets “stuck” and won’t go down into the blades of the blender:- take the clear plastic knob out of the top of the blender (DON’T remove the whole black top);- use the special round black plastic “stick” you will see on the counter. It has a built in ring

that prevents it from ever going too far and getting caught on the blender blades. Use it to poke the ingredients down into the mix while the blender is turning at medium speed.

At the end, or along the way, you may have to add more liquid. Remember NOT to use our tap water, which is from a well.

FILTER IT, otherwise the fibres will definitely block the feeding tube! Pour the mix into a 2 cup measure or other container, through the white plastic sieve. You will probably have to stop and scrape off the sieve, which will block. (You can eat the healthy fibres or compost them). Then you have to filter it again, through the white bag sieve.

Put 500 ml in the feed sack. It loses nutritional value quickly, so give it to Lucie right away.

Before you pour the mix into the feed bag, CLOSE OFF THE CONTROL VALVE below the bag, so it doesn’t run onto the floor. After you pour the mix in the bag, quickly rinse the blender so it is easier to clean later. There may well be extra mix that won’t fit in the bag. So the cook should have a small glass and offer a small glass to the nurse or whoever is there. Sip slowly. Thank you!

This is a meal for sometime in the middle of the day. / LUCIE’S MEAT BROTH MEAL

The broth is prepared in advance and frozen. Big batches from bones from butcher shop. Inexpensive nutritional meats like heart or kidneys. The book Joy of Cooking has broth explanations.

Hopefully someone took the broth out of the freezer and put it on the counter so it is thawed and at room temperature. If you are looking in the kitchen freezer, the white or clear plastic containers may have names like Bone Broth, Beef Broth, Heart Broth, etc. If not, ONE ADVANTAGE OF A VITAMIX BLENDER is that it will crunch fairly large ice chunks and it will heat the food if you blend on high speed.

1. Put ¼ cup of LIQUID in the blender. 1st choice is coconut WATER (not coconut MILK, it is too thick); or city water or bottled water or from a source you trust. (Lucie does NOT drink any water from a well, as a precaution.)

2. Add coconut milk, “Arroy D” brand, it looks like a juice box you take in your lunch. Before opening, or if it is open, holding your finger over the opening, SHAKE it vigorously, then pour in your estimate of ¼ of the carton approximately (we try to use one carton per day).

3. Add 2 tablespoons of “Udo” brand oil, from the fridge.

4. 1 whole egg. Eggs from the carton in the fridge. If the egg shell is dirty, wash it off. Pour the egg into a little bowl to check there is no egg shell going into the blender. Put the egg shell in a yogurt container, there is usually one on the window sill.

5. Now add the meat Broth.

PUT THE TOP ON THE BLENDER before you turn it on!!

Flip the right hand switch up, to turn on low speed. While that is still on, turn the middle dial to 10 for medium speed. As soon as that is going, move to High Speed by flipping the left side switch up.

BLEND FOR 2 MINUTES OR UNTIL IT FEELS WARM. There is a clock on the wall near the door to the deck. Put your hand on the outside of the blender near the bottom. Because this recipe has animal fat, it must be served warm (not burning) to the touch, so it will not congeal in the feeding tube.

To turn it off, go in reverse order: flip left switch down, dial middle switch down, flip right switch down.

Now add new ingredients:

LUCIE’S ALPHA ENF MIX

This is a generic mix that Lucie can have any time, and she can have it twice a day (or even thrice). Do all the measurements with “relative” accuracy, they don’t have to be perfect. Usually what you need is to the left of the blender on the counter or on the blue tray. If not, ask. Put in the blender:

1 ½ cups of one of the following liquids, hopefully we put on the counter so it is at room temperature:

o any one of these from a 1 litre carton: rice milk / oat milk / flax milk/ coconut WATER/ or as a last resort almond milk (NOT home made almond milk, it is too thick) (NOT water, it has no nutrition content) (not coconut MILK, it is too thick).

2 scoops of Alpha ENF powder (white canister). The scoop is usually inside the canister. Each scoop is ¼ cup, total ½ cup.

2 scoops of Hemp Pro 70 powder (brown canister). The scoop is usually inside the canister. Each scoop is 1 tablespoon, total 2 tablespoons.

2 tablespoons of any oil, e.g. Olive or Udo, the metal measuring spoon should be on the blue tray.

1 egg yolk. Eggs from the carton in the fridge. If the egg shell is dirty, wash it off. Crack the egg over a little bowl to check there is no egg shell going into the blender. There is a yellow spoon utensil to separate the yolk, the egg white goes in a jar in the fridge for the rest of the family.

Variety is good, so ask if there is anything special to add today. E.g. pre-cooked vegetables like squash, and you can add ½ cup approximately; or fruit like a half banana or a slice of papaya, BUT then you have to filter out the fibres with a sieve.

Ask if Lucie is taking any spices, like tumeric or whatever, and add a “pinch”.

PUT THE TOP ON THE BLENDER !! Flip the right hand switch up, to turn on low speed. While that is still on, turn the middle dial to 10 for medium speed. There is a clock on the wall near the door to the deck. After ONE minute, turn the middle dial down and flip the right switch off.

LUCIE’s starchy, low protein meal

Sometimes, especially in the evening, Lucie just eats starches and does not take protein.This recipe relies on having pre-cooked vegetables or grains, eg- squash or barley. Often there are home made containers in the kitchen freezer. Backup supplies may exist in the basement freezer. They will be labelled. Hopefully, someone took one out of the freezer already and it is on the counter at room temperature.
