Complex and Hybrid Care Manager Training: Training was held February 10-13, 2014.

Updates to the MiPCT-Approved Self-Management Programs: The February 10th, 2014 edition of the MiPCT Monday FLASH provided information on the most up to date Self-Management Programs that are approved by MiPCT. The article can be found at http://mipct.org/mipct-monday-flash.

Calculation for Care Manager Ratio Requirement to Remain Unchanged: The February 10th, 2014 edition of the MiPCT PO FLASH contained an article explaining the recent analysis of care manager staffing ratios, taking into account the effect of Priority Health member distribution on overall Care Management staffing sufficiency. Enhancement 7.1 for the MiPCT Dashboards & Reports Release: MDC posted Enhancement 07.01 of the MiPCT Dashboards & Reports during the week of February 10, 2014. This enhancement includes the addition of the 30-Day All-Cause Readmission Rate measure for IP utilization.

MDC’s February 2014 MiPCT All-Payer Patient Lists: The Michigan Data Collaborative posted the February 2014 MiPCT All-Payer Patient Lists the week of February 24, 2014. An email announcing the release was sent to Dashboard users. The MiPCT All-Payer Patient Lists are located on the Download PO Reports tab of the MDC MiPCT dashboards. The patient list .zip file includes a list of all MiPCT patients for the PO, formatted lists for each Practice within your PO, and a dropped patient list.

Webinar on Using Evidence to Improve Clinical Care: A webinar, titled “Improving Health Care Decision-making: Comparative Effectiveness Research,” was held on February 25, 2014. The webinar was hosted by the Michigan State Medical Society, Michigan Primary Care Transformation Project and the Federal Agency for Healthcare Research and Quality (AHRQ). You may access the presentation slides and transcript on the Michigan Primary Care Transformation Project’s Web site at http://mipct.org/resources/presentations/. The Webinar materials are located under the heading “Meetings, Q&As and Webinars” and listed under the first sub-heading, "AHRQ Webinar Materials 2.25.2014."

New PDCM Billing Code: Advance Care Planning and End of Life Counseling: The February 24th, 2014 edition of the MiPCT PO FLASH contained an article explaining a new billing code defined by BCBSM, the S0257 code for Advance Care Planning and End of Life Counseling. 2014 MiPCT Due Dates & Action Steps Document: The February 24, 2014 edition of the MiPCT PO FLASH contained a new 2-sided document which summarizes important program deadlines, and lists large group meetings, webinars and MiPCT/MDC incentive timing. Deadlines for quarterly report submission, annual Summit dates, and bi-monthly MiPCT webinar dates are included in the document, which is provided in the resource list below.

Care Team Connect (CTC) - Spotlight Training Webinar Recording: A CTC/ Spotlight recorded training webinar is now available on the mipctdemo.org website at http://mipct.org/resources/presentations/. This webinar includes an overview of CTC/Spotlight functionality, Admission, Discharge and Transfer (ADT) alerts and corresponding Care Manager work- flows.

Care Manager Conference Calls: Care managers, identified by their PO to be eligible to participate, attended the Care Manager Best Practice and Diabetes Mellitus Population Management (LMSW and Adult care managers) and Autism Revisited (Pediatric care managers) conference calls in February. Five conference calls were held (2/5/14 Adult CMs-South East region, 2/7/14 LMSW CMs, 2/12/14 Adult CMs-North region, and 2/19/14 Adult CMs-West region and 2/21/14 Pediatric CMs). The care manager conference calls provide an opportunity for discussion of application of concepts presented in webinars as well as sharing of best practices and networking. Materials reviewed during the conference calls are provided in the resource list below.

Pediatric webinar: There was no Pediatric webinar presented in February.

Adult webinars: One webinar was held February 26, 2014 titled Palliative Care Introduction Part I.
