52 year old male patient whowas an electrical engineer with7/10 low back and left calf pain.He had physical therapy

treatments and medications withno success. His Pre MRIindicated a 5mm x 13mm x 8mm(ap x tr x cc) L4/5 disc extrusionwith displacement andimpingement of the L5 nerveroot; moderate facet
