Carers provide unpaid care by looking after ill, frailor disabled family members, partners or friendswho could not manage without their help.

Caring can be very rewarding, but it can also beisolating, physically and emotionally tiring, andtrying to find the information you need cansometimes be frustrating.

Warwickshire County Council hascommissioned Guideposts Carers Support Serviceto provide information, advice, quarterlynewsletter, training, group and one to one

support. This service also administers the Carers'Emergency Card scheme which providesemergency contact numbers in the event of carersbeing temporarily unable to continue in theircaring role eg due to sudden illness or travel delayspreventing their return home.

Every year an estimated £740 million in carers’allowances goes unclaimed. The benefit system iscomplicated and finding out what you are entitled

A young carer is someone under the age of 18 whohelps look after someone who has a physical orlearning disability; a serious illness; mental ill health;a dependency on alcohol or drugs or anyone withHIV/Aids. The person cared for may be a parent,grandparent, brother, sister, other relative or a close friend.

The young carers project(www.warwickshireyoungcarers.org.uk) supportschildren and young people in their caring rolethrough the provision of information, advocacy,practical advice, a range of other support servicesand emotional support. Young carers can contact

the service themselves or can be referred by theirparents or someone who knows them, for examplea teacher, social worker or health professional. Theproject employs experienced, friendly staff whomeet with young carers to find out what wouldbest suit their situation. There are regular activitysessions and a chance to talk about any issues orjust to have a break and some fun.

Getting out and staying activeDay servicesWe understand that our customers want to stay asactive and independent as possible for as long asthey can. Our aim is to promote independent livingby providing a broader mix of services to enablecustomers to have more choice and control in theirlives. If an individual is assessed as needing a daycare service, their needs will still be met but theywill be supported in a different way. This meansusing a Personal Budget, which you will controldirectly, or they can ask Warwickshire CountyCouncil, or a provider, to manage their budget. Thiswill give greater flexibility about how, when, andwhere that care is provided.

Being admitted to and leaving hospital If you need to be admitted to hospital the nursingand medical staff will see you shortly after yourarrival and ask you questions about your health andhome situation. Once a treatment has been agreedyou will be told how long you may be in hospital.However, this will be kept under review as thetiming for your discharge from hospital maychange depending on your recovery. If you knowthat you need to go into hospital, it will beimportant that you make arrangements for the careof your property and pets while you are away. Youshould also consider:

• how your benefits may be affected. The Department of Work and Pensions provide information about how a hospital stay affects benefits (www.dwp.gov.uk) ;

• how you can ensure that your essential bills are paid e.g. your gas, electricity, rent, while you are away, so that you do not encounter problems when you return home; and

• how to organise your return home.

With your agreement, and where appropriate, areferral will be made to the hospital social workteam who can make an assessment of your needs. Itis important that you tell them your views andwishes for the future. If you have someone whohelps to look after you, for example family orfriends, their views, with your permission, will alsobe considered. They will also have the opportunityto have their own needs assessed through a carer’sassessment.

Sometimes we may need to visit your home. Thisis to allow us to assess what services and/orequipment will help you when you are dischargedfrom hospital.

Some people may return home from hospitalwith support. This could be provided by adult socialcare and/or NHS health teams depending on theirneeds. You could be referred to the reablementservice (explained further on page 8), if you areeligible, where you will be supported to safely andgradually undertake everyday living tasks in yourown home, for example, cooking, dressing over aperiod of up to six weeks. You will then be assessedand, if eligible, and there is an ongoing social careneed, your social care practitioner will work withyou to produce a support plan.

If you are unable to return home, even with help,you may need to consider your longer term plans.However, you cannot generally stay in hospitalwhilst you are doing this, as the hospital will needto be able to provide care to others in need ofurgent treatment. By continuing to occupy a bed ina hospital when you no longer need it, you arepreventing someone else receiving the treatmentthey need.

Not everyone will need this detailed assessmentof their needs as some people can go home fromhospital without any support. Others may havefamily and friends who can help out for a short timeuntil they feel able to cope on their own again.

Recovery

A range of services may be provided that will helpyou recover when you leave hospital. These could betherapists, care assistants or nurses and they may beprovided in your own home, or another supportedenvironment, such as a care home for a short period.

What if I can't go home immediately?You may not be able to return home immediatelybecause of practical difficulties, or important decisionsneeding to be made about your future. You will not beable to stay in hospital whilst these things are beingsorted out. The team looking after you will explainwhat arrangements can be made in the meantime.

What if I need a permanent place in a care home?If it has been agreed that your needs would be bestmet in a care home (including care homes withnursing) you will need to find a home that your canmove to immediately. This may not be your firstchoice of home and you may want to considermoving again at a later date.

Sometimes it is not possible to find a vacancy inyour preferred area immediately and in thesecircumstances you will be expected to move furtherafield. This applies to everyone whether they fundtheir own care or if it is funded by the local

authority. Your social worker or the dischargeliaison nurse will provide you with any assistance/support in finding a home, or you can getinformation about homes in Warwickshire in thisDirectory (lists start on page 40) or by contactingthe Care Quality Commission (CQC), which hasnational as well as local information(www.cqc.org.uk).

Will I have to pay?There is no cost to the individual for anyassessment. If we provide a service, or arrange it foryou on leaving hospital, you may have to pay forthe service in full or make a contribution to the costof this service. You will need to have a financialassessment if you need or want us to fund all/partof the service and your social worker will be able toexplain any charges to you.

We understand that some people may not wishto provide details of their financial situation and inthese cases the services will still be offered, but youwill asked to pay the maximum charge. If you arelikely to require a significant amount of health careon a continuing basis you will receive anassessment and information about your rights andentitlements.

Warwickshire reablement service The reablement service is designed to help peopleregain the skills and confidence they need to liveindependently at home, particularly after an illness orafter staying in hospital.

It is available for up to six weeks. The aim is to assistyou to manage your daily activities like washing,dressing, cooking and shopping. The reablementservice will also advise you about safety issues to helpyou reduce the risks of falls, for example.

Reablement may well reduce your need forongoing support services by improving your practicalabilities so that you can do more for yourself. It hasbeen introduced nationally and aims to keep peopleaway from hospital services and the need forongoing support, such as residential care services.

At the end of the reablement period, you may beable to live at home without any support. You mayneed ongoing care, if so, a package of care will beagreed and a support plan will be drawn up. You willbe offered a financial assessment at the end of yourreablement programme and will be able to discuss

with your care practitioner how your ongoing careand support needs can be met and find out if anyfinancial assistance is available.

This service is only available to people who areeligible. Reablement will not be suitable for everyoneas some people may still need a higher level of care.

Occupational therapyOccupational therapists (OTs) work with adults(people over 18 years, or if in full-time education,over 19 years) and older people, alongside otherprofessionals. OTs will assist you as long as youmeet the ‘Fair Access to Care Services’ (FACS)criteria and if you are having difficulty with day-to-day tasks or if you might be at risk of harm, forexample, falling, or losing your independence.

Occupational therapists are regulated by theHealth Professions Council (www.hpc-uk.org). TheHealth Professions Council holds a register of allthose people who are eligible to practice.

Firstly, they, or one of their team, will talk to youto get full information about your needs. This mayresult in:

• referring you to a more appropriate service;

• giving you advice over the telephone;

• arranging to visit you at home; or

• a combination of these actions.

The occupational therapist or occupational therapyassistant will work with you to agree a plan ofaction that meets your needs. This may be through:

• suggesting alternative ways of completing a task;

• loaning specialist equipment;

• recommending minor building adaptations such as grab rails; or

• recommending grant support for major adaptations to make your home more suitable for independent living.

Occupational therapy services for children andyoung people are provided by your local HealthService through the Children’s IntegratedOccupational Therapy Service. This service can becontacted via NHS Warwickshire or by calling:

Independent Appointee providing Personalisation Support

As a Corporate Appointee Service, approved by the

Department for Work and Pensions, we provide an

Appointee and Money Management Service for anyone

receiving benefit who is unable to manage their money

appropriately and for whom there is no one else deemed

more suitable to take on this role.

This service can also be purchased by individuals or other

organisations. It can be tailored to specific client needs

where we can purely accept responsibility for claiming and

managing benefit entitlement, managing benefits, pensions

and pay bills on someone’s behalf. We can arrange support

schemes to help clients retain their financial independence

wholly or partly.  We can support the opening of bank

accounts, agreeing budgets and setting up payment plans.

Independent Advocacy

We represent the interests of people who may find it

difficult to be heard or speak out for themselves. These

can include over 65s, adults with physical or learning

disabilities and those with mental health problems.

With the support of trained staff, we provide an advocacy

service, which gives a voice to people in need. This

enables them to make their own choices and to have their

individual needs and views respected and acted upon.

We offer a free professional service to clients referred

from Warwickshire County Council, Warwickshire NHS

Trust, Coventry City Council (Brokerage) and Solihull

Care Trust.

We have over 17 years of expertise and, for those people

who do not meet the contracted criteria, our services can

be purchased by individuals and other organisations.

Home care

Help to live at home

Home care is personal and domestic care that helpskeep people safe and comfortable in their ownhome. This could mean helping customers to get upin the morning and go to bed in the evening. Homecare does not provide nursing care, which requiresthe specialist skills of a qualified nurse.

Who can receive home care?We will work with you to assess your needs. If youare eligible, an appropriate package of care is put inplace.

All our home care customers are regularlyreviewed and care packages are updatedaccordingly.

If you do not qualify for a service funded by us,we will offer advice and information about possiblealternative help.

Who pays?If you have savings of £23,250 or over, or are acouple with £46,500 or over, you will have to paythe full cost of your care.

Organising your own home careIf you are thinking about arranging your own homecare it is a good idea to ask any agencies youapproach the following questions:

• Are there different rates for weekends/evenings?

• Are there any extra charges i.e. for the carer's travelling expenses?

• What sort of services do you provide?

• Will somebody visit me before the service starts? If you provide me with a service, can I contact you out of office hours? If so, how?

• Are your carers trained?

• Do you take up references on your carers?

• Have your carers been subject to a Criminal Records Bureau (CRB) Check?

• Are you registered with the Care Quality Commission (CQC)?

Independent home care providersCare in your own home is often referred to aseither domiciliary or home care.

Given the choice, many people prefer to stay intheir own homes surrounded by their possessionsand near to their family and friends. This, ofcourse, applies to everyone whether they areelderly and frail, convalescing, have a physical orlearning disability, or have dementia. The range ofcare available is varied and each client should beinvolved, and agree, with the care provider, a careplan, which states their needs and the expectedoutcomes.

Domiciliary care is usually arranged in a mannerdependent upon needs:

Low level domiciliary care – this may be to assistwith household chores such as shopping, cleaningand assistance with food preparation. It may alsoinvolve a carer accompanying clients on trips outand even holidays. Calls from carers can be flexibleand assistance can be given for as little as 15minutes or when the client needs.

Medium level domiciliary care – in addition tolow level care this will normally involve personalcare (assistance with washing, dressing etc),and/or one carer to assist with mobility. Clientsmay wish to have a carer sleep in their homeovernight for security and reassurance.

High level care – this involves visits by two ormore carers who assist clients with highdependency needs such as being transferred froma bed to a chair etc. Carers may require specifictraining for the health needs of a client.

Live-in care – a carer lives in the client’s home, isallowed time off each day and must have a night’ssleep. Live-in care can be arranged as a shortrespite for the client’s main carer or a permanentarrangement to suit the client’s needs.

24 hour care – this is required when the clientneeds assistance or monitoring through the dayand night; it differs from live-in care in that carerswork on a roster to ensure that a carer is on dutyand awake both day and night.

Some agencies are also registered as Nurses’Agencies, meaning they can provide registerednurses if needed.

All service providers must be registered andinspected by the Care Quality Commission (CQC).It is important to be assured that all carers haveCriminal Records Disclosures, have receivednecessary training, are properly referenced, andare eligible to work in the UK prior to them visitingyour home. Ask the service provider to showevidence that the carers they introduce are indeedcompliant with the legislation, and check theirlatest CQC report at www.cqc.org.uk or call theCQC on 03000 616161.

Paying for home care Domiciliary care for social needs is often funded inpart or whole by the local authority, oralternatively private arrangements can be madewith service providers. The majority of socialservices funded care is contracted out to privatecompanies, rather than the internal local authorityservice.

Fees for services vary, a good service providerwill be able to provide you with a detailed costbreakdown prior to the commencement of anyservice.

Most service providers will accept clients whopay using Direct Payments (discussed further onpage 29). You will still need to have an assessmentof your needs, and if you are eligible, you candecide whether you would prefer to arrange theservices yourself. If you do not meet your council’seligibility criteria, you must organise your owncare as a ‘self-funder’. Listings of home careproviders start on page 14. Equipment and adaptationsEquipment and adaptations can help people livemore productive lives. It enables people to carry outessential activities, with the aim of maintaining or

improving independence. In Warwickshire wecurrently provide:

Assistive Technology (Telecare)

Assistive technology is a range of equipment thatcan help people with a physical, sensory, or mentalhealth disability. It helps people to continue to liveat home safely and as independently as possible.

Our assistive technology service, Telecare, cancan offer protection from:

• intruders;• floods;• fire; and• falls.

The name Telecare describes the wireless sensorsthat are linked to a control centre, or to a familymember’s mobile phone or pager. The sensorsmonitor your home and can raise the alarm shouldan emergency occur.

The service makes a real difference to people’slives, not only for those using the service tomaintain their independence, but also their familiesand loved ones who really appreciate the peace ofmind that the use of this equipment can bring.

Benefits of the Telecare service include:

• instant response - the highly skilled operators respond to your call and take the right action to get you the help you need, when you need it;

• personal service - friendly and dignified service;

• we will not leave you - the operators will stay on the line with you until help arrives;

• complete reassurance - 24-hour link to the control centre; and

• easy installation - systems are easy to fit and often wireless, so no ugly wiring.

Our aim is to:

• assist people to maintain a safe home environment;

• assist carers to care for someone at home;

• reduce the risk of falls and accidents within the home and reduce the waiting time for assistance following an incident;

• prevent avoidable admissions to hospital and speed up discharge; and

• prevent or delay admission to a residential home.

The service involves the provision of a telephone link24 hours a day between your home and the controlcentre. All that is required is a telephone line and anelectrical power point (within six feet of each other).Should a sensor be activated, a message is sent to thecontrol centre where trained operators take the mostappropriate action, such as contacting a familymember, neighbour, doctor or the emergency services.

What types of equipment are there? Examples of the type of equipment used inWarwickshire are:

Personal trigger - a call for help can be raised fromanywhere in the home or garden

Bogus caller button - a discreet button placed near adoor, which can be used to call for help from the 24-hour control centre when a stranger asks for entryinto your home

Smoke/carbon monoxide detector - a life-savingdevice, the detector provides additional protectionby raising an instant alarm call to the control centre ifanything is detected

Fall detector - automatically detects a serious fall andraises an alert with the 24-hour control centre

Flood detector - this small radio sensor gives an earlywarning by alerting the control centre of potentialflood situations in the home

Movement detector - this allows the 24-hour controlcentre to detect for inactivity and raise an alert, oralternatively to audibly verify and record thepresence of an intruder.

How do I get Telecare?If you are already supported by a social worker or anoccupational therapist you can simply speak to themabout the Telecare service.

Integrated Community Equipment Service (ICES)

The Integrated Community Equipment Servicehelps people to live in their own homes more easilyby providing a range of equipment. This can rangefrom a simple walking stick, to a more complexitem like a mobile hoist. To receive this service youmust be assessed and eligible for social caresupport (explained on page 28).

There is no charge for equipment provided by ICES.

What equipment is available?The equipment service has a range of equipmentavailable in stock. The assessor has to ensure thatyou meet the criteria for that piece of equipmentbefore being able to issue it. If the assessorrecommends a non-stock item of equipment, forexample, a specialist shower chair, this will have tobe individually ordered and may take longer toprovide

How do I get my equipment?The equipment service will make an appointmentwith you to deliver and fit the equipment. Theassessor will demonstrate and instruct you in thesafe use of the equipment.

ICES currently do not provide:

• wheelchair services;• school equipment;• sensory equipment; • communication equipment; or • telecare.

How long can I keep the equipment? The equipment will remain with you for as long asyou need it. You will be contacted to see if you arestill using the equipment at regular intervals.

If your needs change you can ask for a re-assessment, which may mean your equipment isexchanged for a different piece of equipment.

How do I return my equipment?To return equipment not being used telephone0800 915 7744 and they will arrange a time tocollect it from you.

How do I get my equipment repaired?You are expected to routinely check, clean, andkeep equipment in good working order. If theequipment breaks or becomes faulty do not use itplease contact us immediately on 0800 915 7744.

Smartcare - assess yourself on-line for equipment

We want to reduce the risks of falls and accidents inthe home and enable people to live moreindependently. We are currently working with anorganisation called ADL Smartcare to provide anon-line, risk-free assessment service. Here you willbe able to find out what equipment is available tohelp you such as grab rails, raised toilet seats, bathboards, or tap turners.

Warwickshire residents will also be entitled to adiscount on equipment ordered through thisservice. There are lots of items of equipment thatcould help you - for all parts of your home.

Meals service The meals service in Warwickshire is delivered byCounty Enterprise Foods who produce and deliverready-made meals, direct to your home. The serviceis for older and vulnerable people and aims to helpyou to live as independently as possible.

The meals are:

• tasty and nutritious;

• varied; and

• made using fresh ingredients and seasonal vegetables.

Frozen meals

These meals are delivered together weekly,fortnightly or monthly. You can then choose whichmeal you want to eat on which day, at a time to suityou. The frozen service offers a choice of over fiftymeals a day.

The meals are for any elderly person and also foradults who have:

• a physical disability;

• a visual impairment or poor eyesight;

• a learning disability;

• mental health problems; or

• a deteriorating or life-threatening condition or are nutritionally at risk.

Hot meals

Daily delivery of a hot meal:

• choose from a choice of four meals a day;

• available on the days of your choice up to 7 days a week; and

• breakfast and tea time packs can also be delivered with your main meal.

We can supply meals to suit any dietary needincluding:

• vegetarian;

• African Caribbean;

• Halal;

• Kosher;

• spicy meals;

• specific medical conditions;and

• soft food diets.

If you prefer, someone else (such as a friend,relative, neighbour, health or housing worker) canget in touch with us on your behalf.

• assisting service users to get to their dining tables;

• opening bottle tops or tins of food for later;

• setting a tray and guiding clients to where the meal is;

Many adults, because of illness or disability, may beunable to protect themselves from abuse. Vulnerableadults may be certain older people, people withlearning disabilities, physically disabled people,people with mental ill-health or those with a short orlong term illness.

Abuse can be defined in many ways however theterms most commonly used are:

• physical;

• emotional;

• neglect;

• sexual;

• institutional; and

• verbal.

Abuse happens in situations where one person in arelationship has more power than the other. In adultabuse cases this could be a vulnerable adult's paid orunpaid carer, a worker, relative, in fact almostanyone.

Services for people with sensory impairments

We work in partnership with the WarwickshireAssociation for the Blind to ensure that a number ofservices are available for people who have problemswith their sight. These services include:

• registering you as a person with visual impairment, following referral from a health practitioner

• carrying out an assessment to find out what needs you may have for equipment, services and benefits;

• providing rehabilitation services, as appropriate, which could include: mobility training, daily living skills, provision of low vision aids, and development of communication skills; and

• providing information about activities and organisations that may be helpful to you.

We offer a range of services to help meet the needsof deaf and hard of hearing adults and children, aswell as their families and carers. These include:• social workers being available to help and advise

on a wide range of personal and practical problems;• registering people as deaf and hard of hearing;• monitoring care of deaf adults in residential care;• participating in specialist assessments for a wide

range of settings such as residential care, P.A.C.E. interviews, transitional reviews for deaf children at 14 plus;

• providing information about deafness, sign-language, deaf clubs and support for

deaf children;• providing information about regional and

national resources for deaf people;• service development - we ask deaf people their

views and what they want to improve and develop services;

The term ‘dementia’ is used to describe thesymptoms that occur when the brain is affected byspecific diseases and conditions.

Dementia is a common condition and there areover 100 different types. In England alone, there arecurrently estimated to be around 750,000 peopleliving with dementia (source: Alzheimer’s Society).

In Warwickshire we are proud to be taking the leadon a partnership approach to dementia services andinformation and have been instrumental in

establishing the Living Well with Dementia PartnershipCoventry and Warwickshire. This is a collection of LocalAuthorities, NHS, Private and Voluntary Sectororganisations dedicated to raising awareness ofdementia, improving post diagnosis support foreveryone who has a dementia diagnosis and theircarers, and improving the quality and access thatpeople with dementia, their carers, social care andhealth practitioners have to important local, service-related, information about dementia.

A small amount of people may experiencemental health problems to such a degree that theyare diagnosed as having a mental illness, requiringthe involvement of specialist services and support.The majority of people will not experience mentalillness, but will experience mental health problemsat different times in their lives.

If people are given support and information, thoseexperiencing mental health problems can makepositive changes and improvements. Only a smallminority of people do not respond to appropriate helpand need more specialist involvement.

‘How we care for the dying is an indicator of howwe care for all sick and vulnerable people. It is ameasure of society as a whole and it is a litmustest for health and social care services.1’

Although the subject of dying is often painful tocontemplate and as a society we do not discussdeath and dying openly, the way care professionalsapproach the process is incredibly important for theclient, their family and carers. The Department ofHealth has published Strategies for organisationsinvolved in providing end of life care which aim topromote high quality care for all adults at the endof life, whether this be at home, in a care home, inhospital, in a hospice or elsewhere.

A care pathway approach for both thecommissioning and delivery of services is stronglyrecommended by the Department of Health, andshould encompass :

• Identifying people who are approaching the endof life and initiating discussions regarding theirpreferences for end of life care.

• Care planning: assessing needs and preferences,agreeing a care plan to reflect these needs andpreferences and reviewing these regularly.

• The coordination of care across all sectors (health,public and private).

• The delivery of high quality services in alllocations.

• Support during the last days of life and care afterdeath.

• Involving and supporting carers, both during aperson’s illness and after their death.

Although it can feel difficult to broach this sensitivearea, potential care providers always welcomequestions and should be happy to explain theirapproach to end of life care. It is thereforeworthwhile asking any questions you have withregards to end of life care, for example, whetherthey are following any of the national strategiesrecommended for implementing best practicewithin their home, and if so, how they areimplementing it within their organisation. 1 ‘End of Life Care Strategy - promoting high qualitycare for all adults at the end of life’ April 2008.Department of Health.

‘Tell Us Once’ service

When someone dies, the last thing the loved onesleft behind want to be landed with is extrapaperwork and red tape. But from now on, thanksto a new ‘Tell Us Once’ service being rolled outacross the country, it will be easier for families tonotify government authorities about the changes intheir circumstances.

‘Tell Us Once’ has been launched by theDepartment for Work and Pensions in partnershipwith local authorities across England, Wales andScotland. It enables key local and centralgovernment departments and services to benotified simply through one single appointmentwith your local registrar.

When you make an appointment to register thedeath in Warwickshire, you can extend yourappointment and take up the ‘Tell Us Once’ service.This will mean that the registrar can send yourinformation to other government organisationsthat need to know about the death.

Extra care housing

Choices if you cannot live at homeMany older people can end up living where gettingabout is difficult or they are not happy to bebecause they want to please others, or not be aburden to their family. Moving after an accident oran illness can mean there is not enough time tothink about what you really want. Having the

opportunity to think about the sort of place onemight find desirable and suitable before a decisionneeds to be made is important.

One such possibility that is becoming a majordevelopment across Warwickshire is extra care housing.

Extra care housing gives you the opportunity to liveindependently in your own home, access 24-hourcare to help with your personal care needs and bepart of a community.

Extra care housing is different because:

• you are living at home - not in a residential care home;

• you have your own front door - you decide whocomes in;

• couples and friends can stay together;

• there is a mix of able and less able people;

• 24-hour care services are available - if you need them;

• you get support to keep your independence;

• you can join in social activities - or you can beprivate;

• you have control over your finances; and

• you have security - a buzzer entry system and alocked front door.

Extra care housing offers a new way of supportingyou to live independently for as long as you possiblycan. It gives you the security and privacy of a home ofyour own, a range of facilities on the premises, with24-hour care services available if you need them.

Extra care housing is sometimes called ‘verysheltered housing’. The look of the accommodationvaries enormously: some are new purpose-builtretirement villages, others are modernised shelteredhousing schemes with extra services on site. Manyoffer a choice of accommodation such as flats orbungalows, for sale or rent. What all residents of extracare housing have in common is the security ofowning or renting your own home, control over yourfinances and the peace of mind that goes withhaving 24-hour care and support available on site.

You may be thinking of moving or having toleave your old home because of life changes or

health problems. Or you may simply be unhappywith where you are living now. If you value yourindependence and privacy and want control andchoices in your life, think about extra care housing.

What facilities do extra care schemes have?

Each scheme is different and what is available variesfrom site to site. However, each scheme will have:

• Housing - a range of modern and affordable oneor two bedroom homes available to buy, rent orpart own which are purpose-built or adapted foryour care and support needs

• Communal areas - these might include a cafe,hairdressers, library, guest accommodation foryour visitors, a day centre with organisedactivities and outings.

• Neighbourhood facilities - extra care schemes arebased within local communities so the usualrange of neighbourhood facilities are close bysuch as leisure centres, shops, GP surgeries andpublic transport.

What support can I get?

Home care staff can offer support with yourpersonal care, including:

• help with washing, dressing and getting to andfrom the toilet;

• reminders to take medication;

• preparing snacks and heating up meals;

• shopping;

• laundry; and

• some domestic tasks.

There is access 24-hours a day to support withpractical care tasks. Some extra care sites havedementia units which provide specialist care in acommunity setting.

Care homes - definitions• Care homes with nursing

If you think you may need nursing care in a home,you will need to be visited by a social worker or acare manager to work out what care you will need.This visit might be in your own home, or in hospital ifyou’ve been ill, or in a home. You will be fullyinvolved in planning your care needs.

If a care home providing nursing care is the bestsolution for you, the social worker will give youinformation to help you find a home which meetsyour care requirements. The cost of the nursing carepart of your fees is paid by the NHS to the homedirectly: the current amount is £108.70 per week.You can contact the Nurse Care Management Teamsfor your area at your local Primary Care Trusts (PCTs).

If you decide that a residential care home is thebest option for you we will advise you on the typeof care home that will best meet your needs - thiscould be a residential home, or a home thatprovides nursing care. The home might also needto provide a specialist service, for example,dementia care. We will help you find out abouthomes, vacancies and prices.

All residents have to pay for residential care, butthe amount will be assessed according to yourincome and savings.

All care providers in the country must be registeredwith the Care Quality Commission (CQC). Homes areinspected by the CQC, who report on their findings.These inspection reports are available at the home orfrom the CQC (www.cqc.org.uk).

There are two types of residential home:

• Care homes (personal care only)

If you are reasonably active, but would like greatersecurity and care, subject to a needs assessment, acare home offering only personal care may be thebest option. Personal care includes bathing,feeding, dressing and help with moving but it mustbe paid for if your capital/savings exceed £23,250.This figure may change after April 2013, check withus after this date. See page 31 for more informationon paying for your care.

People who need specialist nursing care (such asdementia) frequently or throughout the 24-hourperiod, beyond the level available from theCommunity Nursing Service, may need specialistnursing home care (explained further below).

How much does extra care cost?

There are three parts to extra care costs:

• housing - the cost of renting, buying or sharedownership of your home;

• service charge - home maintenance and communalfacilities; and

• care and support costs - these vary depending onyour needs. You may be eligible for support withthis following an assessment by a social worker.

The amount you pay will depend on your individualcircumstances. We will conduct a financialassessment and tell you exactly how much it will cost.

If you are at significant risk because:

• your main carers, relatives, friends or neighbours who provide a lot of help cannot continue to provide all the help you need

• you have needs arising from severe dementia or depression

• you have suffered from physical, sexual, psychological or financial abuse, or if there are

What is a Social Care Assessment?People who are thought to be eligible for supporthave a social care assessment, this is how we findout what people’s needs are and what outcomesthey want to achieve with their support.

To achieve this we will need to ask you questionsabout your needs or problems and how they areaffecting your day-to-day life. We will only askquestions which help us to understand your needsor problems. This will include basic informationsuch as your name and address, date of birth, nextof kin, and GP. We may also need to discuss yourcurrent health, medical history, family, housing andfinancial situation, and personal care.

The assessment is an opportunity for you to saywhat you think and to ask us questions. We willlisten carefully to your views and take your wishesand feelings into account. If you need an interpreterbecause your first language is not English, or youuse sign language, we will arrange this.

You will only need to give your personal detailsand information about your needs or problems once

and, if you agree, we will then share this informationwith other people involved in your care. We mayhave to ask other people for information about you.For example, we may need to ask your doctor to tellus something about your health problems. We willnot normally collect or record any information aboutyou, or share it with anyone else, without yourpermission. We call this ‘consent to share and recordinformation’ - the person who does your assessmentwill explain this to you. If you contact us by telephoneon 01926 410410, you will initially speak to acustomer service advisor based in the customerservice centre, who will be able to advise you of youreligibility for further assessment for services.

If you are eligible for help we will talk to you, andyour family and carers with your permission, aboutthe options available to you. We will then write a planthat sets out the support we have agreed with you. Ifyou are caring for someone on a regular basis andfeel you would like to be assessed in your own rightas a carer you can request a carer’s assessment.

What is the Support Plan?After your assessment is completed we will discusswith you how you wish to be supported, this caninclude your family, friends or carers if you want it

to. The Support Plan will give the detail of whenyou will get support, the type of support, how oftenyou have it and who will provide it to you.

What is a Personal Budget?Warwickshire now give people who are eligible forlong term support a Personal Budget, this is the moneyto buy the support identified in the Support Plan.

The Personal Budget can be used in two differentways, you can chose to take the money and arrangeyour own support, this is called a Direct Paymentwhich just means that we pay you the money directly

rather than paying the person or organisation thatsupports you. This gives you a lot of control over howyour support is provided and makes it simple for youto change the arrangements.

If you want to use a Direct Payment but areworried about making the arrangements we canprovide you with support or this can come from

Your financial assessmentWe are not able to subsidise the whole cost of carepackages in Warwickshire and so we are required tocarry out a financial assessment to find out whether youhave to make a contribution towards your support.

To find out if you need to contribute to the cost ofyour care, you will be visited from one of ourspecialist finance visiting officers who will look atyour income and any capital or savings you have,and will make sure you are receiving, or haveclaimed, all the benefits you are entitled to.

The visiting officer will also discuss with you anyexpenses you have as a result of your care needs ordisabilities. We call these ‘disability related expenses’and can include things like exceptional laundry costsor paying for certain items of equipment. The visitingofficer will discuss these items with you and agreeweekly amounts that can be included as expensesthat relate to your disability or care needs.

Once this information has been collected andagreed, the visiting officer will then be able tocalculate whether or not you will need to make acontribution towards your support. If you are happyto proceed with the visit it would be helpful to haveyour financial information available when the visitingofficer calls. This will include things like:

• benefit award letters including state pensiondetails and housing benefit award andoccupational pension details;

• rent/mortgage and council tax details;

• details of all bank/building society statements,savings accounts, PEPs, ISAs etc; and

• Trust funds, bonds and any other income.

Any recent receipts you may have for the followingwould also be useful:

• fuel bills;

• recently purchased disability equipment;

• invoices for privately arranged care purchaseddirectly by you; and/or

• essential purchases that enable care to beprovided in your own home.

How is the amount I need to pay calculated?

The total amounts of your income, capital andsavings will be added together and from this we willdeduct your housing costs such as mortgage/rentpayments and council tax. In line with Governmentguidance we will also deduct an amount equivalentto the basic level of Income Support or PensionCredit that the Government says that you need tolive on, plus 25% and also deduct the amount wehave agreed with you as ‘disability related expenses’.

How much will I have to pay for my social care services?

If you have savings of £23,250 or over, or are acouple with £46,500 or over, you will have to pay thefull cost of your care.

You will be sent a letter from the financialassessments and benefits advice team advisingwhen the visiting officer would like to visit.

The result of your financial assessment will be sentto you either by your Personal Budget administrator orthe finance assessments and benefits advice team.

You can ask for your assessment to be reviewed bycontacting the manager of the team that carried outthe assessment. If you are still unhappy you cancomplain or make comments to the manager of theteam that has been dealing with you.

Charges for social careBy law we can ask people who receive adult socialcare services to contribute to the cost of thesupport they receive. These contributions help us tocontinue to provide adult social care services to thegrowing number of people who need them.

Everyone will be offered a financial assessment towork out how much they can afford to pay towardsthe cost of their care.

As part of your financial assessment, we will takeaccount any savings you have, as follows:

• less than £14,250 - we will not take these intoaccount;

• between £14,250 and £23,250 - we will apply atariff income of £1.00 for each £250 (or part of£250) and add this to your income used in theassessment; and

• above £23,250 we will assess you as being able topay the maximum contribution, no matter whatother income and disability costs you may have.

The financial assessment will make sure you keep abasic level of income to include basic housing costsand agreed disability-related expenses. As part ofthe assessment we will also give you advice onwelfare benefits so you may be able to increaseyour income by claiming all the benefits you maybe entitled to.

What happens if I do not want a financialassessment?

You can choose not to have a financial assessment,but this means that you will need to make themaximum contribution for your care.

2. Developing ‘My Support Plan’

How you want to use this money (‘estimatedpersonal budget’) will be written in a plan called ‘MySupport Plan.’ This describes the support you wantto meet your social care needs and how you plan toachieve your outcomes. This must also keep yousafe, well and be legal.

A social care practitioner will meet with you tohelp you with your plan. You can also have a friend,family member or a trustee to help you as well.

The final amount of money is only confirmedwhen your support plan is agreed. This is called your‘Personal Budget’.

Your care, your choice

Self-directed support is about you being in control ofthe assistance you need to live the life you choose. Ifyou have been assessed as being eligible for social

care support, we want to put you at the centre ofthis support and give you control over that support.

There are seven main steps to helping you be incontrol of your support:1. Completing a supported self assessment

questionnaire

2. Developing a support plan

3. Agreeing the support plan4. Using a personal budget5. Paying towards a personal budget6. Keeping records and7. Reviewing.

1. Completing a self assessment

This helps us find out:• if you are eligible for social care support; and• about your day-to-day life; and• your social care needs.We will arrange to visit you to help you completethe assessment. If you want, you can involve afamily member, carer, friend or someone else whois important in your life to help with this too.

We also want to support carers. We will alsoassess someone who regularly cares for you, forexample, a family member, friend, neighbour etc. ifyou would like us to, provided they are not beingpaid for caring for you.

You can tell us about the changes you would liketo make, the things you would like to stay the same,and what you would like to achieve - we call theseoutcomes.

It may be that you are able to make some choicesabout how you want to be supported. If you areunable to make your own decisions about yoursupport, we will work in line with the Mental CapacityAct 2005 to make sure we meet your best interests.

After we have completed the assessment we willlook at what you have told us and work out anestimated amount of money you may need to payfor your support (we call this your ‘estimatedPersonal Budget’). This will help you as a guide toplan your support.

4. How do I use my personal budget?

There are a number of ways you can use yourPersonal Budget:

Direct payments

Direct payments provide greater choice andflexibility in the way you arrange the support youwant, and you can include things we (the council) donot organise and directly pay for.

Choosing direct payments is the only time you willuse your personal budget as money.

You will receive the money every four weeks paidinto your designated bank account to pay for yoursupport.

If you have capacity to agree to direct paymentsand you do not want to manage it alone, you maywish to nominate a person you know, or a trust, toopen a bank account and receive the money on yourbehalf. They could also arrange the support followingyour support plan. You will still be accountable forthe way the direct payment is managed.

Trust - this is usually a small group of people whomanage your direct payment on your behalf and caninclude anyone you agree to, such as members ofyour family.

If you support someone who does not have thecapacity to agree to direct payments you may beable to act as a Suitable Person. If you are appointedas a Suitable Person you will be accountable forreceiving and managing the direct payment. The lawtells us who can be appointed as a Suitable Personand we must follow a process to understand whocan act as this person.

There are only a few things you cannot use yourdirect payment for:

• you cannot purchase long term residential carebut you can take short breaks;

• you cannot buy a service from us, as the council isnot allowed to sell their services this way;

• you cannot use it for health related services i.e.dentist;

• you cannot use it for your own personal bills i.e.personal clothes, food, household bills, rent, andmortgage;

• you cannot use it for gambling, buying alcohol orcigarettes;

• the law tells us you cannot pay someone who livesin the same household unless we agree; and

• you cannot use it for anything that is illegal ordoes not keep you safe and well.

Sometimes people under certain mental healthlegislation cannot be offered direct payments.

A personal managed budget

You may decide that you prefer not to receive yourpersonal budget as money, and instead you want us(the council) to make your arrangements for you.

If you choose to do this, we will use the money topay for your support. Your support will be limited tothe organisations that we contract with, and theservices we provide.

If we agree with you that residential care is thebest way to meet your support needs, this will bearranged as a managed budget for legal reasons.

A combination budget (a mixture of a directpayment and a managed budget).

You may decide that you want a direct payment

3. Agreeing ‘My Support Plan’

Your personal budget must be spent on meetingyour eligible social care support needs and theoutcomes in your support plan. You can use yourpersonal budget on many different things within theamount of budget available to you, for example:

• employing someone to support you in day-to-daylife (we call this person a personal assistant);

• buying support from a care agency;

• meeting other people, perhaps at a local club;

• keeping well by having regular exercise, perhapsswimming, or being part of a local walking group;

• having help to run your home;

• having a break so that your family carer can havesome time off; or

• learning a new skill or maintaining an existing skill.

Often talking about this with someone else can helpyou think about the different possibilities.

7. Reviewing

We arrange review meetings with you to see if youneed a different amount of money and support.However, you can tell us at any time if you feel yourneeds have changed.

The purpose of these review meetings is to look atyour support plan and see if your support is stillhelping you to achieve your outcomes. We will lookat what you want to change, or keep the same, andcheck that the allocated funding is meeting yourneeds. A large underspend may trigger a review toensure the resource relates to your support need.

Your direct payments could be at risk if you breakthe rules and spend the money for something illegal,gambling, or something outside of the agreed

outcomes. We will work with you to stay within therules and discuss alternative arrangements tomaximise the choice available to you.

You will only continue to receive a personalbudget if you are eligible for social care support. Ifyour circumstances change and you are no longereligible you will be supported to close down yourpersonal budget responsibilities.

5. How much will it cost me?

We will carry out a financial assessment with you tosee how much money you can afford to pay towardsyour support. Depending on the outcome of thefinancial assessment you may be asked to pay

towards your personal budget to buy the supportyou need.

You will be told how much you need to pay beforebeing given your personal budget.

6. Keeping records and spending the personal budget

If you are going to receive direct payments, weadvise you to open a separate bank account in yourname, or the name of the person you have agreedwill help you with managing the money.

We will advise you how to keep simple recordsand how to get the types of help and support tomake things easier for you.

At first, we will need to look at your bankstatements for the separate account for directpayments every three months to help you manageand keep on top of things and we will write andrequest these from you.

If you find that you are spending more moneyeach month than you have coming in, you must tellus. We can help you to find out why. You may need

to spend your money in a different way to meetyour agreed outcomes. However, we cannot giveyou more money if you run out.

We will look at how much you have in yourpersonal budget. If you do not spend the entirepersonal budget, we will ask you to pay backanything above the agreed reserve amount.

We support the flexibility of personal budgetsand we will allow you to build up an amount ofmoney to help you to plan for such things as youremployer responsibilities and achieving youragreed outcomes. Usually around eight weeks offunding would be acceptable. Anything savedabove this must be agreed during your supportplan or review.

Individual service funds

Individual service funds are an option for people whentheir personal budget is known. We will work with youto choose a provider that will help you meet the needs

(some, or all of it, depending on what has beenagreed) to the provider on your behalf. The providercan only use your budget for your support and mustaccount to you. This gives you some flexibility andcontrol as you agree with the provider the exactdetail of your support.
