This Directory aims to explain how you can choose the support you need to enjoy the best quality of life for as long as possible.

We can provide information to help you live independently in your own homes, including care in a care home or care home with nursing.

Young carers

A young carer is someone under the age of 18 who helps look after someone. The person cared for may be aparent, grandparent, brother, sister, other relative or a close friend.

The young carers project (www.warwickshire youngcarers.org.uk) supports children and young people in their caring role through the provision of information, advocacy, practical advice, a range of other support services and emotional support. The project employs experienced, staff who meet with young carers to find out what would best suit their situation. There are regular activity sessions which provide a chance to talk about any issues, or simply just to have a break and some fun.

Library service

The Warwickshire library service provides a wide range of books and materials for all age groups.

It provides:

• magazines and newspapers, DVDs, videos, music CDs, talking books, local studies material, maps and plays in our larger libraries;

• material suitable for adults, children and young people in a variety of formats, which reflect the cultural and ethnic diversity of the community;

• at least 90,000 new books and other items;

• reading and literacy for babies, children and adults through reading groups, and a range of activities and events;

• National campaigns such as a Black History Month, Adult Learners’ Week, Family Learning Week and the Summer Reading Challenge;

• a wide range of material about Warwickshire and surrounding areas; and

• a wide range of learning opportunities for all ages.

Being admitted to and leaving hospital

Your recovery

If you need to be admitted to hospital the nursing and medical staff will see you shortly after your arrival and ask you questions about your health and home situation. Once a treatment has been agreed you will be told how long you may be in hospital. However, this will be kept under review as the timing for your discharge from hospital may change depending on your recovery.

With your agreement, and where appropriate, a referral will be made to the hospital social work team who can make an assessment of your needs. It is important that you tell them your views and wishes for the future. If you have someone who helps to look after you, for example family or friends, their views, with your permission, will also be considered.

Sometimes we may need to visit your home. This is to allow us to assess what services and/or equipment will help you when you are discharged from hospital.

Some people may return home from hospital with support. This could be provided by Warwickshire County

Council and/or NHS health teams depending on your needs. You could be referred to the reablement service (explained further on page 8). If you are eligible for reablement you will be supported to safely and gradually undertake everyday living tasks in your own home, for example, cooking, dressing over a period of up to six weeks.

You will then be assessed and, if needed, your social care practitioner will work with you to produce a support plan.

If you are unable to return home, you may need to consider your longer term plans. However, you cannot generally stay in hospital whilst you are doing this, as the hospital will need to be able to provide care to others in need of urgent treatment.

Not everyone will need this detailed assessment of their needs as some people can go home from hospital without any support. Others may have family and friends who can help out for a short time until they feel able to cope on their own again.

Occupational therapy

Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs) work with people who find it difficult to maintain an independent life, particularly in the face of increasing disability. OT practitioners work with people with physical and/or learning disabilities from the age of 18 years upwards. They also take into consideration the needs of carers whether formal or informal.

The OT practitioners carry out specialist OT assessments in line with the ‘Fair Access to Care’ (FACS) framework, to ensure eligibility for services. For further information about social care assessments, see page 35.

OT practitioners will provide:

• advice and information;

• strategies on managing disabilities;

• therapeutic interventions;

• specialist equipment; and

• home adaptations.

Warwickshire reablement service

The reablement service is designed to help people regain the skills and confidence they need to live independently at home, particularly after an illness or after staying in hospital. It is available for up to six weeks.

The aim is to assist you to manage your daily activities like washing, dressing, cooking and shopping.

The reablement service will also advise you about safety issues to help you reduce the risks of falls.

At the end of the reablement period, you may be able to live at home without any support. If you do need ongoing care, a package of care will be agreed and a support plan will be drawn up. You will be offered a financial assessment at the end of your reablement programme and will be able to discuss your care.

This service is only available to people who are eligible. Reablement will not be suitable for everyone as some people may still need a higher level of care.

Equipment and adaptations

Help to live at home

Equipment and adaptations can help people live more productive lives, enabling them to carry out essential activities, with the aim of maintaining or improving independence. In Warwickshire we currently provide:

Assistive technology (Telecare) Assistive technology is a range of equipment that can help people with a physical, sensory, or mental health disability. It helps people to continue to live at home safely and as independently as possible. Our assistive technology service Telecare can offer protection from:

• intruders; • floods; • fire; and • falls.

The service makes a real difference to people’s lives, not only for those using the service to maintain their independence, but also their families and loved ones who really appreciate the peace of mind that the use of this equipment can bring.

Benefits of the Telecare service include:

• instant response - the highly skilled operators respond to your call and take the right action to get you the help you need, when you need it;

• personal service - friendly and dignified service;

• we will not leave you - the operators will stay on the line with you until help arrives;

• complete reassurance of a 24-hour link to the control centre; and

• easy installation - systems are easy to fit and often wireless, so no ugly wiring.

Examples of the type of equipment used in Warwickshire are:

• personal trigger - a call for help can be raised from anywhere in the home or garden;

• bogus caller button - a discreet button placed near a door, which can be used to call for help from the 24- hour control centre when a stranger asks for entry into your home;

• smoke/carbon monoxide detector - a life-saving device, the detector provides additional protection by raising an instant alarm call to the control centre if anything is detected;

• fall detector - automatically detects a serious fall and raises an alert with the 24-hour control centre;

• flood detector - this small radio sensor gives an early warning by alerting the control centre of potential flood situations in the home; and

• movement detector - this allows the 24-hour control centre to detect for inactivity and raise an alert, or alternatively to audibly verify and record the presence of an intruder.

Equipment Service Assess yourself on-line for equipment AskSARA provides an on-line, risk-free assessment service. This will help you find out what equipment is available to you. There are lots of items of equipment that could help you, for all parts of your home, including:

• grab rails; • raised toilet seats; • bath boards; and • tap turners.

Meals service

This Directory’s helpline 0800 389 2077 – information on choosing and paying for care

The meals service in Warwickshire is delivered by Apetito who produce and deliver ready-made meals, direct to your home. The service is for older and vulnerable people and aims to help you to live as independently as possible.

The meals are:

• tasty and nutritious;

• varied; and

• made using fresh ingredients and seasonal vegetables.

Frozen meals These meals are delivered together weekly, fortnightly or monthly. You can then choose which meal you want to eat on which day, at a time to suit you. The frozen service offers a choice of over fifty meals a day.

Hot meals Daily delivery of a hot meal:

• choose from a choice of four meals a day;

• available on the days of your choice up to seven days a week; and

• breakfast and tea time packs can also be delivered with your main meal.

Apetito can supply meals to suit any dietary need including:

• vegetarian;

• African Caribbean;

• Halal;

• Kosher;

• spicy meals;

• specific medical conditions; and

• soft food diets.

To receive community meals, telephone Apetito or visit one of their Customer Service Points. They can talk to you about what meals they can provide and what is the best kind of delivery for you.

Home care

Home care is personal and domestic care that helps keep you safe and comfortable in your own home. This could mean helping you to get up in the morning and go to bed in the evening. Home care does not provide nursing care, which requires the specialist skills of a qualified nurse.

Who can receive home care? We will work with you to assess your needs. If you are eligible, an appropriate package of care is put in place. For more information about assessments, see page 35. All our home care customers are regularly reviewed and care packages are updated accordingly. If you do not qualify for a service funded by us, we will offer advice and information about possible alternative help.

Organising your own home care If you are thinking about arranging your own home care it is a good idea to ask any agencies you approach the following questions:

• Are there different rates for weekends/evenings?

• Are there any extra charges i.e. for the carer’s travelling expenses?

• What sort of services do you provide?

• Will somebody visit me before the service starts?

• If you provide me with a service, can I contact you out of office hours? If so, how?

• Are your carers trained?

• Do you take up references on your carers?

• Have your carers been subject to a Disclosure and Barring Service (DBS) check?

• Are you registered with the Care Quality Commission (CQC)?

INDEPENDENT HOME CARE PROVIDERS

Care in your own home is often referred to as either domiciliary or home care.

Given the choice, many people prefer to stay in their own homes surrounded by their possessions and near to their family and friends. This, of course, applies to everyone whether they are elderly and frail, convalescing, have a physical or learning disability, or have dementia. The range of care available is varied and each client should be involved, and agree a care plan with the care provider, which states their needs and the expected outcomes.

Domiciliary care is usually arranged in a manner dependent upon needs:

Low level domiciliary care – this may be to assist with household chores such as shopping, cleaning and assistance with food preparation. It may also involve a carer accompanying clients on trips out and even holidays. Calls from carers can be flexible and assistance can be given for as little as 15 minutes or when the client needs.

Medium level domiciliary care – in addition to low level

care this will normally involve personal care (assistance with washing, dressing etc), and/or one carer to assist with mobility. Clients may wish to have a carer sleep in their home overnight for security and reassurance.

High level care – this involves visits by two or more carers who assist clients with high dependency needs such as being transferred from a bed to a chair etc. Carers may require specific training for the health needs of a client.

Live-in care – a carer lives in the client’s home, is allowed time off each day and must have a night’s sleep. Live-in care can be arranged as a short respite for the client’s main carer or a permanent arrangement to suit the client’s needs.

24 hour care – this is required when the client needs assistance or monitoring through the day and night; it differs from live-in care in that carers work on a roster to ensure that a carer is on duty and awake both day and night. >>

All service providers must be registered and inspected by the Care Quality Commission (CQC). It is important to be assured that all carers have Disclosure and Barring Service checks, have received necessary training, are properly referenced, and are eligible to work in the UK prior to them visiting your home. Ask the service provider to show evidence that the carers they introduce are indeed compliant with the legislation, and check their latest CQC report at www.cqc.org.uk or call the CQC on 03000 616161.

Paying for home care Domiciliary care for social needs can be funded in part or whole by the local authority, or alternatively private arrangements can be made with service providers. The majority of social services funded care is contracted out to private companies, rather than the internal local authority service.

Fees for services vary, a good service provider will be able to provide you with a detailed cost breakdown prior to the commencement of any service.

Most service providers will accept clients who pay using direct payments. You will still need to have an assessment of your needs, and if you are eligible, you can decide whether you would prefer to arrange the services yourself. If you do not meet your council’s eligibility criteria, you must organise your own care as a ‘self-funder’. Listings of home care providers start on page 19. See page 39 for advice on self-funding your care.

Vulnerable adults may be certain older people, people with learning disabilities, physically disabled people, people with mental ill-health or those with a short or long term illness.

Mental health

It is estimated that one in four people living in the UK will be affected by issues regarding their mental wellbeing at some point in their lives. If you have any concerns about your mental health you should visit your GP who will be able to offer you advice and support.

A small amount of people may experience mental health problems to such a degree that they are diagnosed as having a mental illness, requiring the involvement of specialist services and support. The majority of people will not experience mental illness, but will experience mental health issues at different times in their lives.

Dementia services

The term ‘dementia’ is used to describe the symptoms that occur when the brain is affected by specific diseases and conditions. There are over 100 different types of dementia. In England alone, there are currently estimated to be around 750,000 people living with dementia (source: Alzheimer’s Society).

There are a lot of myths and misconceptions about dementia. To help raise awareness of the condition and provide information on local services for people with dementia, Warwickshire County Council has led the development of the Coventry and Warwickshire Living Well with Dementia information portal (website). The portal contains the following information:

• information for people who have had a diagnosis of dementia. Practical hints, tips and links to resources and services that can ensure people are able to live well with their condition for as long as possible;

• information for supporting someone with a dementia

diagnosis. Practical information on taking care of themselves and effectively supporting a person with dementia;

• general information about dementia. Including the signs and symptoms of the conditions and the things that people are able to do to reduce their risks of developing dementia in later life; and

• information for Social Care and Health Practitioners about dementia diagnoses, the local pathways for Coventry and Warwickshire and services that can be accessed for people with a dementia diagnosis and their support.

There are a number of different support workers at the Alzheimer’s Society that can help people with dementia or their carers. Whether you need information about the diagnosis, emotional support to help adjust to the

The end of life care strategy

‘ How we care for the dying is an indicator of how we care for all sick and vulnerable people. It is a measure of society as a whole and it is a litmus test for health and social care services.1 ’

Although the subject of dying is often painful to contemplate and as a society we do not discuss death and dying openly, the way care professionals approach the process is incredibly important for the client, their family and carers. The Department of Health has published Strategies for organisations involved in providing end of life care which aim to promote high quality care for all adults at the end of life, whether this be at home, in a care home, in hospital, in a hospice or elsewhere. A care pathway approach for both the commissioning and delivery of services is strongly recommended by the Department of Health, and should encompass:

• Identifying people who are approaching the end of life and initiating discussions regarding their preferences for end of life care.

• Care planning: assessing needs and preferences, agreeing a care plan to reflect these needs and

preferences and reviewing these regularly.

• The coordination of care across all sectors (health, public and private).

• The delivery of high quality services in all locations.

• Support during the last days of life and care after death.

• Involving and supporting carers, both during a person’s illness and after their death.

Although it can feel difficult to broach this sensitive area, potential care providers always welcome questions and should be happy to explain their approach to end of life care. It is therefore worthwhile asking any questions you have with regards to end of life care, for example, whether they are following any of the national strategies recommended for implementing best practice within their home, and if so, how they are implementing it within their organisation.

Residents The best indication of a good home is that the residents appear happy and responsive. Are the residents:• Involved in activities or chatting? • Clean and comfortably dressed? • Encouraged to do as much for themselves as they

can?• Do they seem alert and interested? • Do they talk to you as you walk round?

Access Check whether:• The corridors and toilets are wide enough

for a walking frame or wheelchair. • Toilets and baths are suitably adapted.• There are ramps or a lift. • There is adequate signage and cues for different

parts of the home.

Bedrooms Can the person with dementia have a single room? Check whether:• Residents are encouraged to bring in some

of their own furniture and possessions.• The bedrooms are bright and pleasant. • Residents can go to their rooms when they wish to

be alone. • Residents are able to keep pets in their room

or in other areas of the home. • Staff respect the right to privacy and knock

on bedroom doors.

Toilets Check whether:• There are enough toilets within easy reach?• Staff are trained to spot the signs when

someone needs to go to the toilet? • Staff are cheerful and tactful about helping

residents use the toilet and changing them if they are incontinent?

Living areas Are chairs arranged in groups to encourage talking rather than placed in a circle round the outside of the room? Is there:• A TV or radio left on when no one is watching or

or where they can be quiet or see visitors? • A garden where residents can walk safely?

Health• Which doctor or doctors can residents see? • Can a relative stay overnight if a resident is unwell? • What happens if residents need help with taking

• Are changes in medication discussed with the family carers?

Visitors Make sure that there is good communication between relatives and the home and that phone calls and visits are encouraged. • Are visitors encouraged to take residents out

or join them for a meal? • Are relatives involved in care planning and

the general life of the home?

Activities Residents should be stimulated without feeling stressed. Are: • There opportunities for residents to help staff with

small tasks if they wish? • The activities individualised and peoples’

likes/dislikes recorded in care plans?• Activities available each day or are residents

left to sit in front of the TV? • Reminiscence activities available?• Residents able to compile memory boxes?

Staff It is important to note whether staff seem friendly and caring towards residents and whether they treat residents with respect. Do they:• Have any training and experience in

dementia care? • Make time to sit and chat to residents or talk

to them while they are helping them with physical tasks such as washing and dressing?

• Know about residents’ backgrounds, habits and interests?

• Ensure that the person with dementia has a member of staff particularly responsible for their care?

• Make sure there is a member of staff you can talk to about your own worries about the person with dementia?

Manager/head of home A manager who is caring as well as efficient can make all the difference to a home. Does the manager:• Have a friendly manner with staff and residents? • Answer your questions openly and seem to

understand your concerns? • Have a knowledge of dementia and can

‘Tell Us Once’ service

When someone dies, the last thing the loved ones left behind want to be landed with is extra paperwork and red tape. But from now on, thanks to a ‘Tell Us Once’ service being rolled out across the country, it will be easier for families to notify government authorities about the changes in their circumstances.

‘Tell Us Once’ has been launched by the Department for Work and Pensions in partnership with local authorities across England, Wales and Scotland. It enables key local and central government departments and services to be notified simply through one single appointment with your local registrar.

When you make an appointment to register the death in Warwickshire, you can extend your appointment and take up the ‘Tell Us Once’ service. This will mean that the registrar can send your information to other government organisations that need to know about the death.

Books on prescription

Reading Well Books on Prescription (BOP) is a scheme offering high quality self-help books, eBooks and audio CDs. They are available to borrow from libraries and our website. GPs and health professionals can also ‘prescribe’ titles in the collection.

The self-help books help with a range of commonly experienced mental health issues, such as depression, anxiety and stress.

How the scheme works:

• You can be given a book prescription by your GP or mental health professional.

• Take it to your local library, where staff will help you find the book, eBook or audio CD.

• BOP collections are located in 21 libraries, and they can be obtained at any library in the county.

• If you are not already a member of the library, you can join immediately, using the book prescription as identification and take the book(s) home.

• Books and audio CDs are issued for three weeks and can be renewed if no-one is waiting for them.

• The recommended books are available for anyone to read. You do not need a prescription.

• EBook versions of some titles are also available for three weeks for free.

BOP is currently available across Warwickshire for GPs, IAPT Practitioners, Mental Health Professionals within Warwickshire County Council and Coventry and Warwickshire Partnership Trust, Occupational Health within public bodies and Wellbeing Centres to prescribe for their patients and service users.

Extra care housing

Extra Care Housing is provided by local authorities, housing associations, or the private sector. It is social or private housing that has been built or modified to suit people with long-term health conditions or disabilities. People tend to move in to Extra Care Housing when living in their own home becomes difficult.

Extra Care Housing includes properties and purpose built accommodation such as retirement villages, apartments and bungalows. These can be for sale, rent or shared ownership with many schemes offering a mixture of tenures.

It gives people the opportunity to live independently in a home of their own, but with other services on hand if they need them. These extra facilities vary depending on the site, but can include:

• 24-hour access to emergency support;

• an on-site care team 24/7;

• reablement services;

• a restaurant, or some kind of meal provision;

• laundry;

• fitness facilities and classes;

• a base for healthcare workers;

• some have mini-buses for residents and provide cleaning and maintenance for the property and grounds. These additional services will be charged for; and

• Extra Care Housing residents still have access to means- tested local authority services

Who is it for? Extra Care Housing suits people with long-term health conditions, or disabilities that make living in their own home difficult. It can be a very real alternative to residential care, with some people having already made the decision to move out of residential care back into more independent living. Most residents are older people, but this type of housing is becoming popular with people with disabilities

regardless of their age. It is usually seen as a long-term housing solution, but certain developments offer short-term rehabilitation such as intermediate care services for residents, with some specialising in care for people with dementia or other mental health issues such as depression.

How much does it cost? Here is an example of weekly charges (taken from January 2014):

Farmers Court, Rugby Rent: £125.49 (1 bedroom) or £152.35 (2 bedroom) Service charge: (contributes towards the cost of providing and maintaining communal services): £26.05

Wellbeing charge (an essential aspect of Extra Care Housing, which provide 24 hours a day cover. A resident may never need to use the help available - but the knowledge that they can provides the necessary reassurance): £14.99

Rohan Gardens, Warwick Rent: £141.10 (2 bedroom) Service charge: £41.69 Wellbeing charge: £15.99

How do I get a place in an Extra Care Housing scheme? Once you have identified a number of Extra Care Housing schemes you are interested in, the application procedure and eligibility criteria can vary depending on the scheme. Schemes run by local councils will normally have an eligibility criteria which you need to meet in order to qualify to live there. You may need to be:

• above a certain age (usually over 55)

• a resident in the area (or have connections to the area)

• assessed as needing Extra Care Housing following an assessment carried out by Warwickshire County Council’s adult social care services team - in order to meet your care and support needs.

Choices if you cannot live at homeMany older people can end up living somewhere where getting about is difficult or they are not happy to be because they want to please others, or not be a burden to their family. Moving after an accident or an illness can mean there is not enough time to think about what you

really want. Having the opportunity to think about the sort of place one might find desirable and suitable before a decision needs to be made is important.

Care homes – definitions

If you decide that a residential care home is the best option for you we will advise you on the type of care home that will best meet your needs - this could be a residential home, or a home that provides nursing care. The home might also need to provide a specialist service, for example, dementia care. We will help you find out about homes, vacancies and prices.

Most residents have to pay for residential care, but the amount will be assessed according to your income and savings.

All care providers in the country must be registered with the Care Quality Commission (CQC). Homes are inspected by the CQC, who report on their findings. These inspection reports are available at the home or from the CQC (www.cqc.org.uk).

There are two types of residential home:

Care homes (personal care only)If you are reasonably active, but would like greater security and care, subject to a needs assessment, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving but it must be paid for if your capital/savings exceed £23,250*.

See page 35 for more information on paying for your care.

Care homes with nursingIf you think you may need nursing care in a home, you will need to be visited by a social worker or a care manager to work out what care you will need. This visit might be in your own home, or in hospital if you’ve been ill, or in a home. You will be fully involved in planning your care needs.

If a care home providing nursing care is the best solution for you, the social worker will give you information to help you find a home which meets your care requirements. The cost of the nursing care part of your fees is paid by the NHS to the home directly: the current amount is £110.89* per week.

Supported living for people with disabilities

Supported living is designed to help disabled people live more independently, in their local community. Supported living enables people with learning disabilities to live in their own homes as an alternative to shared residential care and is available to people who are assessed as eligible for services using the Fair Access to Care Services (FACS) criteria. In supported living accommodation, housing and support are provided separately, and the customer has a licence or tenancy. If the support package is not meeting their needs, the service provider can be changed, without having to move home.

Support can be commissioned from a few hours a week, up to 24-hours a day, depending on assessed needs. There are also several different models of supported living, available such as living in shared houses, having a flat within a group, or living in individual tenancies in the community.

groups and independent care organisations to arrange services. These services may be jointly or partially funded by us.

Are you eligible?

We help people with the greatest need for support by using the Fair Access to Care Services (FACS) eligibility criteria. We will discuss this with you at your assessment. If you are not eligible for help from us, we will give you information about other organisations that may be able to help you.

We would normally arrange support if you are not able to:

• get out of your bed or chair or move around without help;

• look after yourself because you cannot prepare meals and drink; and/or

• wash or dress yourself properly, and you need help to manage those essential tasks.

If you are at significant risk because:

• your main carers, relatives, friends or neighbours who provide a lot of help cannot continue to provide all the help you need;

• you have needs arising from severe dementia or depression; or

• you have suffered from physical, sexual, psychological or financial abuse, or if there are good reasons to suspect you might be at risk of this happening.

If you are a carer and:

• you are putting your own health and welfare at risk because of your caring tasks; or

• you might need support to continue in your caring role.

We would not normally arrange support for:

• housework;

• shopping;

• collecting pensions;

• collecting prescriptions;

• ironing or other household tasks; or

• bathing, if you can manage a shower or strip wash.

What is a social care assessment?

People who are thought to be eligible for support have a social care assessment, this is how we find out what people’s needs are and what outcomes they want to achieve with their support.

To achieve this we will need to ask you questions about your needs or problems and how they are affecting your day-to-day life. We will only ask questions which help us to understand your needs or problems. This will include basic information such as your name and address, date of birth, next of kin, and GP. We may also need to discuss your current health, medical history, family, housing and financial situation, and personal care.

The assessment is an opportunity for you to say what you think and to ask us questions. We will listen carefully to your views and take your wishes and feelings into account.

You will only need to give your personal details and information about your needs or problems once and, if you agree, we will then share this information with other people involved in your care. We may have to ask other people for information about you. For example, we may need to ask your doctor to tell us something about your health problems. We will not normally collect or record any information about you, or share it with anyone else, without your permission. We call this ‘consent to share and record information’ - the person who does your assessment will explain this to you.

If you contact us by telephone on 01926 410410, you will initially speak to a customer service adviser based in the customer service centre, who will be able to advise you of your eligibility for further assessment for services.

If you are eligible for help we will talk to you, and your family and carers with your permission, about the options available to you. We will then write a plan that sets out the support we have agreed with you. If you are caring

for someone on a regular basis and feel you would like to be assessed in your own right as a carer you can request a carer’s assessment.

What is the support plan?After your assessment is completed we will discuss with you how you wish to be supported, this can include your family, friends or carers if you want it to. The support plan will give the detail of when you will get support, the type of support, how often you have it and who will provide it to you.

What is a personal budget?We now give people who are eligible for long term support a personal budget, this is the money to buy the support identified in the support plan.

The personal budget can be used in two different ways, you can choose to take the money and arrange your own support, this is called a ‘direct payment’ which just means that we pay you the money directly rather than paying the person or organisation that supports you. This gives you a lot of control over how your support is provided and makes it simple for you to change the arrangements.

If you want to use a direct payment but are worried about making the arrangements we can provide you with support or this can come from family or friends if they are willing to help you.

The other way of receiving your personal budget is for us to arrange the support for you - this would be done by the person who completes your assessment and support plan. We will still involve you in deciding how to arrange your support but you will need to come back to us if you want to change anything.

You are also able to have a direct payment for some aspects of the support plan and have us arrange support for other parts of your plan.

Your financial assessment

By law we can ask people that receive adult social care services to contribute to the cost of the support they receive. These contributions help us to continue to provide adult social care services to the growing number of people who need them.

Everyone will be offered a financial assessment to work out how much they can afford to pay towards the cost of their care.

What does a financial assessment involve?We will ask you to provide us with information about your income and any capital that you have so that we can calculate how much you are able to contribute towards

your care.The financial assessments will make sure you keep a

basic level of income to include basic housing costs and agreed disability related expenses.

As part of this assessment, we will take account of any savings you have, as follows:

Savings / ContributionBelow £14,250 We will not take these savings into account in the assessment.

A tariff income of £1.00 for each £250 (or part of £250) and add this to your income used in the assessment.

Above £23,250 We will assess you as being able to pay the maximum contribution, no matter what other incomeand disability costs you may have. As part of the assessment we will also give you advice on welfare benefits so you may be able to increase your income by claiming all the benefits you may be entitled to.

You can choose not to have a financial assessment, but this means that you will automatically need to make the maximum contribution for your care.

What is self-funding?People arrange their own social care services for different reasons; some pay the full costs and others may pay a contribution. If you are paying the full cost of your care, you are known as a ‘self-funder’. This means that either you:

• have chosen not to approach us for help; or

• have been assessed but you are not currently eligible for services; or

• are eligible for services, but your savings are above £23,250.

If, following an assessment, you have been told that you do not currently meet the eligibility criteria for services, or are simply arranging your own support, a list of service providers through voluntary or independent organisations can be found in this publication or on our Warwickshire Directory (https://directory.warwickshire.gov.uk). This means that you can choose and pay for the kind of services you feel you need.

Transport Your transport contribution is based on the actual number of journeys you make with your transport service.

• Transport distance up to 5 miles (rate per journey) £5.17.

• Transport distance 5 to 10 miles (rate per journey) £8.63.

• Transport distance over 10 miles (rate per journey) £12.23.

Telecare £4.87 per week. Your telecare service contribution is based on the actual number of weeks or part weeks that you receive a telecare service.

Day care Your day care contribution is based on the actual number of days or part days you attend the day care service.

• Day care – older people and mental health (day or part day rate) £25.21.

• Day care learning disabilities (day or part day rate) £46.74.

• Day care physical disabilities (day or part day rate) £40.28.

Home care £14.24 per hour. Your home care contribution is based on the actual number of home care hours that you receive. If you wish to cancel or change your care you must give 24 hours’ notice or you may still be charged.

Respite care £51.80 per day. Your respite care contribution is based on the actual number of days you receive your respite care service.

Carers replacement services (sitting service) £14.24 per hour.

Personal assistant services £10.78 per hour. Your personal assistant contribution is based on the full cost of paying and employing personal assistants (hourly rate, via a direct payment).

Other chargeable services You will be required to pay the full cost of other chargeable services and support such as laundry services.

If you’re not happy with your assessment or support plan you can ask for your assessment to be reviewed by contacting the manager of the team that carried out the assessment. If you are still unhappy you can complain or make comments to the manager of the team that has been dealing with you.

Self-funding advice

If you have more than £23,250* in capital or savings you will have to pay for any care services you require, although if you need care in your own home or are considering moving into a care home, you still qualify for a Community Care Assessment by us. The assessment will make sure that your needs will be met. Also, if you expect your capital to fall below £23,250* as a result of paying for care, we may then help you with the cost.

If you are moving into a care home, you may be entitled to some of the following financial assistance and support:

Twelve-week property disregard

This applies if:

• your former home is included in your financial assessment;

• your other capital is less than £23,250*; and

• your income is not enough to meet your care home fees.

We can help with the cost during the first twelve weeks of permanent care in a home, provided our assessment has shown that this is the kind of care you need. This is called the ‘property disregard’ period.

Deferred payment agreements After the twelve-week property disregard period, we may be able to help you while your property is being sold. We will charge any financial help we give against the value of your home. This means we will take back the amount we have paid once your property has been sold. However, we may limit how much we will pay. It may affect your entitlement to Pension Credit if your property is not seen to be on the market and becomes treated as capital by the Department for Work and Pensions (DWP).

Attendance Allowance This is a non-means-tested, non-taxable benefit from the DWP. It’s paid at the lower rate of £54.45* a week if you need care by day or night, and at the higher rate of £81.30* a week if you need care during the day and night. Everyone who needs care can, and should, claim Attendance Allowance.

NHS Nursing Care Contribution Whether you are a temporary or permanent resident, if you live in a care home that provides nursing care you may be entitled to NHS Nursing Care Contribution towards the cost. It is not means-tested and is currently £110.89* a week for the standard rate. This is paid directly to the home.

What if you run out of money? If your capital is likely to reduce to £23,250* as a result of paying for care, you must tell us well in advance that this is going to happen. We will then be able to help with your care fees (provided your assessment has shown that you need residential care). We will carry out a financial assessment to confirm the date from which your funds fell below £23,250*, and the amount we will contribute towards your care. Third party payments are explained overleaf.

>> Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself.

Third party payments If the care home you eventually choose costs more than the rate we will contribute, someone will have to make up the difference. This extra payment is often referred to as a ‘top-up’ or ‘third party payment’. Except in limited circumstances, the law states that you are not allowed to make this additional payment yourself. The responsibility for this often falls to a member of your family or a benevolent sponsor such as a charity. Once this person or organisation has been confirmed, they must sign an agreement formalising the arrangement. It is important that whoever agrees to do this for you is made aware that the amount could increase in the future and they must be confident any increases will be met. Should payments stop for any reason, you should seek help and advice from us immediately. As the financing of care is a complex area, you are advised to contact us to ask for specific advice and individual guidance.

Healthwatch

Coventry LINk and Warwickshire LINk were abolished at the end of March 2013 and replaced by Coventry Healthwatch and Warwickshire Healthwatch.

The Clinical Commissioning Groups are working closely with both LINks during the transition to the new arrangements and will continue to work with Healthwatch, which now has stronger statutory duties to ensure local patient and public feedback is given to the CCG and that they are involved in commissioning and planning.

As part of the changes to the NHS brought about by the Health and Social Care Act 2012, Primary Care Trusts (PCTs) and Strategic Health Authorities (SHAs) ceased to exist in 2013. Their responsibilities were taken over by Clinical Commissioning Groups (CCGs) and the NHS Trust Development Authority.

The local CCGs have responsibility for planning and commissioning healthcare and will work with local people and organisations, including councils and the providers of health and social care, to plan and pay for health services in the area.
