The Province will refund the 8% provincial component of the HST (form HST-R-

02) or the 13% Provincial Vehicle Tax (PVT) (form PVT-R-01) with respect to a

motor vehicle purchased in the Province where:

the motor vehicle is specially equipped with a device to enable a

wheelchair or scooter to enter or leave the passenger vehicle (e.g.

hydraulic lifts); or

the motor vehicle is specially equipped with auxiliary driving controls that

are used to facilitate the operation of the passenger vehicle (does not

include spinner knobs); and

the motor vehicle is not operated by any person for the purpose of earning

a profit for any person or as part of any undertaking carried on for gain;

and

the claimant is not eligible for any other GST/HST credit or rebate related

to this purchase.

Applications for these refunds can be obtained at any Service New Brunswick

Centre or by contacting the Revenue & Taxation Division, Department of Finance

at 506-453-2404.

Along with the application the claimant must submit the following necessary

supporting documents:

a. a copy of Bill of Sale and New Brunswick Tax Receipt;

b. proof that the motor vehicle is specially equipped with a device to enable a

wheelchair or scooter to enter or leave the passenger vehicle; or

c. proof that the motor vehicle is fitted with auxiliary driving controls that are

used to facilitate the operation of the passenger vehicle, truck, or van; and

d. a letter from a medical practitioner certifying person is disabled.

Please note: Any inquiry related to all other types of Harmonized Sales Tax

(HST) refunds must be made to the Canada Revenue Agency at:

E3B 5H1 E-mail : [email protected] The New Brunswick Department of Transportation offers the following program: The Vehicle Retrofit Program will provide a maximum grant of 80% of the first $5,625 and 50% of next $7,000 to individuals, registered non-profit organizations, municipalities or private companies towards the cost of supporting and/or installing eligible accessibility features for a new or existing vehicle, renewable every 10 years. Organizations that provide transportation services for the disabled community will be eligible to renew their limit every five years. Some examples of retrofitting and accessibility features eligible for assistance under the program are:

wheelchair lifts and ramps hand controls, steering devices and left-foot gas pedals wheelchair restraint systems special needs seating roof, floor and door alterations (if part on an accessible retrofit) scooter lifts

CANADA REVENUE AGENCY Specially equipped motor vehicles

You may buy a new or used vehicle from a dealer which is already equipped with

either auxiliary driving controls to enable an individual with a disability to operate

the vehicle, or with a device designed to allow a wheelchair to be placed in the

vehicle without having to collapse the wheelchair. If so, the GST/HST applies to

the purchase price of the vehicle.

You can get a rebate for the GST/HST payable on the part of the purchase price

that relates to the modification of the vehicle and the installation of these devices,

either from the supplier or by sending us a completed Form GST518, GST/HST

Specially Equipped Motor Vehicle Rebate Application.

The rebate is also available if you paid GST/HST on modifications that were

made to specially equip your vehicle outside of Canada.

For more information on whether or not you qualify for any personal tax credits,

please contact the Canada Revenue Agency at 1-800-959-8281 or access their

website and search medical expenses.

Email: [email protected] The Adaptive Driving Service enables persons with disabilities to have safe and accessible transportation in their community. It is a provincial rehabilitation service. Services are directed towards individuals who, as passengers or drivers, may benefit from specially adapted vehicles and/or driver training. The Adaptive Driving Service is not covered by Medicare. Services Provided: Driver Assessment The driving assessment helps to determine the client’s potential to begin or resume driving. A comprehensive assessment is completed that considers the client’s physical, visual and cognitive/perceptual abilities. Specially adapted equipment may be suggested to help the client to access and control the vehicle. Information on equipment, suppliers and funding sources is provided. Driver training may be recommended. In some situations further testing or alternative transportation may be recommended. Driver Training Driver training may be recommended for those clients who need to learn a new skill and/or improve driving habits and behaviors. A driver assessment must be completed by the Adaptive Driving Service in order to confirm the client’s needs and abilities. Consultation Services The Adaptive Driving Service provides consultation services to clients, caregivers and health professionals regarding vehicle selection and modification. This may include information on ways to transfer in and out of the vehicle, equipment for lifting and stowing mobility devices (wheelchairs, scooters) and issues related to safety. Information on equipment, suppliers and funding sources is provided.

Services Location Clients may be seen at the Stan Cassidy Centre for Rehabilitation in Fredericton on an inpatient or outpatient basis. Referral Process Document required for driver assessment:

Physician referral (can be initiated by the therapist or client but a physician is required to sign a referral).

Documents requested for driver assessment (as appropriate) o Summary of relevant occupational therapy findings; o physiotherapy findings; o psychology reports; o speech language pathology reports; o Results of visual tests (e.g., optometry, ophthalmology).

MANUFACTURERS’ MOBILITY PROGRAMS Most manufacturers offer special rebate programs to assist in defraying the costs of adapting a vehicle for a person (driver and/or passenger) with disabilities. The following are a few examples of manufacturers and the programs they offer, but it is not a complete list. Please see your Chrysler, Dodge, Jeep® retailer for details. Ford Canada Mobility Program Increase Your Confidence Living with a health condition or physical disability shouldn't limit your mobility. The Ford Mobility program is designed to extend your abilities, allowing you to enhance your personal independence. Extend your Ability At Ford of Canada, we are committed to understanding and supporting individuals with unique mobility needs. When you purchase or lease a new Ford of Canada vehicle, you may be eligible to receive up to $1,000 toward the installation of adaptive driving aids or mobility assistance equipment. Enhance Mobility Start driving now by customizing a vehicle to suit your needs. Eligibility modifications and equipment include:

General Motors Canada Ltd. Mobility Program The people at General Motors of Canada Limited, and GM dealers nationwide, know just how important mobility is if you have a disability. We also know that finding appropriate sources of mobility-related information and financing can be frustrating. That's why, with the assistance of the Saint Elizabeth Health Care and the Bridgepoint Community Rehab Drive Assessment and Training Center, we've put together an extensive collection of information, resources and a reimbursement of up to $1,000 towards the cost of adapting your GM vehicle. Call the GM Canada Mobility Assistance Centre toll-free at 1-800-GM-DRIVE to find out how you can qualify for this special reimbursement of up to $1,000. We'll send you helpful information on the variety of GM vehicles available for adaptation and on how the reimbursement can be applied, even if you secure other funding assistance for your conversion. Which adaptive equipment that's appropriate for you depends on many variables. To meet your needs, you can take advantage of the information and resources brought together by:

and others available to assist you. This resource team can become an important part of your support system. To take advantage of those resources, we recommend taking these steps: 1. Obtain an evaluation. Contact a driver assessment facility to arrange for a complete evaluation. The evaluation should include an assessment of your sensory motor function, cognitive processing, general knowledge and an on-road performance evaluation in an appropriately modified vehicle. The evaluation should also include a review of your medical history, driving history and driver's licence status. The assessment report should include recommendations on driving and a complete list of vehicle modification recommendations. 2. Contact the Provincial Ministry of Transportation. In most provinces, people are required to report any changes in their medical status to the Provincial Ministry of Transportation. Some medical conditions may affect your ability to operate a motor vehicle and can produce a potentially dangerous situation, both for you and the general public. When you report a change in your medical status to the Provincial Ministry of Transportation, the

ministry may conduct a medical review, and may allow you to retain your driving privileges with restrictions. Failure to report a change in your medical status may put your driver's licence status in jeopardy.

3. Select the proper vehicle. Do not purchase a vehicle before consulting a professional. Although there is a broad range of vehicles and adaptive equipment available, finding the appropriate match can be difficult. Space limitations in vehicles, vehicle use, family size and the type of equipment required are all factors that must be considered. Your GM Canada dealer, an adaptive equipment installer and a driver assessment professional can work together to assist you.

4. Select a qualified equipment installer. When selecting a mobility equipment dealer, shop around and inquire about the qualifications, warranty, service practices and capabilities of the dealer. Consider more than just initial cost, and ask the company for a list of references. A driver evaluation facility and your provincial rehabilitation services agency are good sources of information about mobility equipment installers in your area.

5. Obtain proper training. The equipment installer and/or driver evaluator should train you in the proper use, care and maintenance of the adaptive equipment and vehicle control system. You should drive the vehicle under the supervision of a qualified driving instructor until you can demonstrate adequate driving skills.

Hyundai Canada Mobility Program The Hyundai Mobility Program assists disabled individuals in outfitting their new Hyundai vehicle with the necessary equipment required to drive. The assistance is in the form of a monetary rebate of up to $500. Individuals with a permanent disability who purchase a new Hyundai vehicle and that require an adaptive aid to drive a vehicle. In order to qualify, the person must be able to produce a doctor’s or Government Certificate detailing his or her qualifying disability. Qualified customers will receive a Hyundai Mobility Program Claim Form from the Sales Person at the time of delivery of their new Hyundai vehicle. The customer then has up to 4 months to mail the following information to Hyundai Auto Canada:

1. A copy of the Hyundai New Vehicle Sales Contract. 2. A doctor’s or Government Certificate detailing the customer’s disability. 3. Proof of vehicle modifications in the form of a payment receipt or work

order from the installer clearly stating the Hyundai vehicle’s V.I.N.#, Year, Make, and Model and that the modifications were completed within 4 months of delivery.

4. The completed Hyundai Mobility Program Claim Form.

All applicants should mail the required information to:

Once approved, Hyundai Auto Canada will mail a cheque for the cost of the modifications up to a maximum allowance of $500 directly to the customer at the address stated on the Mobility Program Claim Form.

KIA Motors Mobility Program Kia Canada welcomes the opportunity to assist individuals with disabilities or special needs by providing up to $750 towards the purchase and installation of specialty equipment on your Kia vehicle. If you think you qualify or would like to hear more, visit your local KIA dealer for details.

Mazda Canada Inc. Mobility Program Eligible vehicles include all new and unused passenger cars and trucks. Mazda Canada Inc. (MCI) will provide up to $750.00 (includes GST) towards the purchase and installation of adaptive driving aids or mobility assistance equipment on all eligible Mazda vehicles. Adaptive driving or passenger equipment includes wheelchair or other lift equipment, hand or other driving controls. Purchase and installation of equipment which costs less than the maximum $750.00 will be paid at the actual costs. Purchase and installation of equipment exceeding $750.00 will be paid a maximum of $750.00. Qualifying Process:

1. Determine your Province’s or Territory’s Driver’s License Requirements regarding the use of mobility equipment.

2. Evaluate your needs in conjunction with your medical service provider. 3. Choose a qualified mobile equipment installer.

Claiming Process: To qualify for payment, the purchaser/lessee must complete the Mobility Assistance Claim Form and forward it, along with required documentation, to:

All payments will be made directly to the retail purchaser or lessee. To be eligible for payment, the “Mobility Assistance Claim Form” must be received within three (3) months from date of purchase. Required Documentation: Complete the “Mazda Mobility Assistance Claim Form” and submit to Mazda Canada Inc. along with the following documents:

1. A legible copy, from an authorized Mazda Canada Inc. dealer, of a signed Bill of Sale or Lease Agreement for a new, unused Mazda vehicle;

2. A legible copy of the vehicle ownership certificate; 3. A legible copy of the paid invoice(s) for the adaptive driving aids/mobility

assistance equipment installed on the purchased vehicle by a qualified installation provider, and;

4. A physician’s statement identifying reason(s) for mobility assistance.

Other Requirements: 1. Only the original owner of the vehicle is eligible 2. Modifications and equipment installation must be completed for the

original owner or their immediate family. 3. There will be only one re-imbursement request per vehicle.

Exceptions include the following items:

All accessories, such as side step tubes.

Automatic transmissions.

Any mobility equipment that is not permanently affixed to the vehicle.

Any other equipment that, in the opinion of Mazda Canada Inc., does not meet the requirements of the Mazda Mobility Assistance Program.

The customer is solely responsible for the selection of both the mobility equipment manufacturer and installer. Mazda Canada Inc. or its Dealers bears no responsibility for the warranty of the mobility equipment and installation. Toyota Canada Mobility Program

To help cover the costs of installing adaptive driving aids or mobility assistance

equipment, the Toyota Mobility Program offers up to a $1,000 allowance to

physically-challenged customers who acquire a new or eligible Toyota vehicle.

Qualified customers may obtain a Toyota Mobility Program Allowance Application

from the salesperson at the time of vehicle delivery. The applicant must present a

record of their physically challenged condition, in the form of a doctor's note or

Government certificate. Customers then have up to twelve months to complete

the application and return it to Toyota Canada, along with proof of vehicle

modification in the form of a payment receipt or work order from the installer.

Adaptive driving or passenger equipment includes, but is not limited to,

wheelchair or other lift equipment, hand or other driving controls, and lifts or cart

storage. Upon verification of the request, a cheque will be mailed directly to the

customer. See your Toyota Dealer for further details and eligibility requirements.
