    Union for International Doctorate in Transdisciplinary Global Health Solutions (Specific Grant

    Agreement 2013-1479), met als doel het behalen van een gezamenlijk doctoraat. Het proefschrift is

    voorbereid in de Faculteit Geneeskunde van de Universiteit van Amsterdam en in de ISGlobal,

    Instituto de Salud Global de Barcelona (ISGlobal) van de Universitat de Barcelona, Barcelona, Spanje.

    This thesis has been written within the framework of the Erasmus Mundus program of the European

    Union for International Doctorate in Transdisciplinary Global Health Solutions (Specific Grant

    Agreement 2013-1479), with the purpose of obtaining a joint doctorate degree. The thesis was

    prepared in the Faculty of Medicine at the University of Amsterdam and in the Barcelona Institute

    for Global Health (ISGlobal) at the University of Barcelona, Barcelona, Spain.
