“DEPARTMENTAL APPLICATION FORM” INSTRUCTIONS i. Your application for studies in this Department must be approved before you can

register at the University. You will receive an official letter from the Faculty of Health Sciences to confirm your status; please use this letter when registering for the first time.

ii. Your application forms (UJ Application and Departmental Application) must

please include the following and handed in in duplicate (UJ Application at the UJ Enrolment Centre and the Departmental Application at the Department of Sport and Movement Studies):

a) An official certified copy of your academic results with regard to all degrees,

diplomas and certificates (including your matric certificate).

b) A recent photo of yourself (please note ID photo’s are not acceptable).

c) A complete Curriculum Vitae with all relevant information regarding past experience and present involvement (please exclude school experiences / achievements).

d) International students must first contact the UJ International Office for

clearance, before making their application to the Department:

iii. Your completed UJ application form, ID, academic results and all other supporting
