What is transition?

• Transition is the period of time when young people move from being a child to an adult.

• It can be a difficult time for people with disabilities.

• We want to make sure that transition in Wolverhampton is as smooth and successful as possible.

• If your child has a Statement of Special Educational Needs, transition planning will begin when they are in Year 9. This may seem early but, in order to ensure your child’s transition is smooth and successful, forward planning is essential. Your son/daughter will be making choices that ultimately impact on what they do beyond Year 11.

• Planning is ongoing and any decisions made will be reviewed regularly as your child moves through school to ensure their wishes and aspirations are taken into account.

Section 139 assessments

• The Local Authority has a duty to carry out a section 139a Assessment for those young people with a Statement of Special Educational Needs and who they expect to leave school to receive post 16 education, training or higher education. For those young people who are at School Action Plus, a section 139a Assessment may be carried out if the Connexions Personal Adviser feels it is needed.

• To enable young people with additional and complex needs to exercise choice and control over their life decisions and to be aware of the opportunities that are available to them

• To deliver high quality, multi-agency support• To ensure continuity of planning, care and support from childhood

into adulthood
