Mandatory Insurance

• The University of Iowa requires all international students or Students in the Health Sciences to have medical insurance.

– Medical care is expensive– No government assistance available

SHIP Coverage

• Undergraduate and graduate students

• $130 per month for single coverage • $135 per month starting 9/1/14

• Care through Alliance Select providers

SHIP Coverage

• Worldwide coverage– Office visits to physicians– Routine Physicals– Pap smears and pelvic exams– Immunizations – Hospitalization– Surgery– Imaging and laboratory tests– Prescriptions– Mental Health– Repatriation

SHIP Coverage

• Co-payments– $10: office visit, lab test, imaging– $50: emergency room

• Coinsurance– 10%: hospitalization after $300 deductible

Example: 2 day hospital stay = $3,000

You pay: $300 deductible10% of $2,700 = $270Your total cost: $570

UIGRADCare Coverage

• Prescriptions– $7 or 25%, whichever is greater

• Out-of-Pocket Maximum (OPM)– $1,200 Single– $1,800 Family

Dental Insurance

• Delta Dental of Iowa– Participating: File claims and accept payment

arrangements.

– Non-participating: Claims settled directly with you and you are responsible for paying provider.

• $21 per month for single coverage• $25 per month for single starting 9/1/14

Dental Insurance Plan

• Diagnostic and preventive services

– Cleaning, oral evaluation, imaging, etc.No cost

• Restorative services

– Cavity repair, tooth extraction, root canals, etc.$25 deductible/per person + 20%

• High cost restorations

– Crowns, inlays, dentures, and bridges$25 deductible/per person + 50%

• Maximum of $1,000 per individual per year

Graduate Assistants & Fellows

Assistantship of at least 25% timeFellowship of at least $9500.00 per year

• University contributes toward cost of policies

• If you and your spouse both have assistantships, you will need to indicate that on your Employed Grad application form.

• Application forms are required. It is NOT automatic.

Graduate Assistants & Fellows

• Benefits begin on the first day of the month following the first day of work AND completion of enrollment form.

• Premiums will be deducted from your paycheck.• June paycheck will pay for three premiums – triple dip.

Application Process

• Complete application during open enrollment period (Aug 1- Sep 5, 2014) if you are:

– Enrolling in UIGRADCare or SHIP– Enrolling in dental insurance– Adding family members (this will increase your

premiums)

• Changes can only be made– During Open Enrollment– Within 30 days of an event

• Marriage• Divorce• Birth (60 days)• Arrival in U.S.• Spouse beginning or ending job

• Changes take effect the 1st of the month following the event, except for birth.
