2014-2020 YEAR ACTION PLAN
TARGET GROUPS
Children deprived of parental care
(including infants), who are at risk and

The strategic aim - to create system of a comprehensive range of services that enable every child, person with
disabilities or their families (guardians) to receive individual services according
to their needs and necessary assistance in the community, and
every child deprived of parental care to grow in a safe environment in his
biological or foster family.
1 aim - to ensure a sustainable environment and conditions for each child (and disabled child) to grow in his own family, and for every child deprived of parental care to grow in foster family or family households and to get assistance in the community.
2 aim - to enable adults with
disabilities and their families (guardians) to get community-based services according to their individual needs.
3 aim - promote the change of moral values in the society, while forming a positive public attitude towards restructuring of the system and to ensure the transparency of these processes.

THE PRINCIPLE SCOPE OF TRANSITION
FAMILY AND COMMUNITY BASED SOCIAL SERVICES(ACCORDING TO REGIONAL PLANS FOR SERVICES AND INFRASTRUCTURE DEVELOPMENT):
A foster-family care, custodial familiesChild care in a small community foster care homes for children (up to 8 children)
Protected housing/independent living homes, guiding/assistance services for children leaving foster care, etc. Social work and individual help services for the child / young person with behavioral / emotional disorders

needs of every service

To determine motivation, the need for a new knowledge and
skills of the staff
Plans:
1) individual care plan for every
resident of a social care institution,
2) institution's plan for the transition,
3) Regional plan of services and
infrastructure
Create methodological basis for a new
type of the services:
methodological documents,
legislation, etc.

• The main task is to - on the basis of the Regional Service andinfrastructure development plans to develop infrastructure of communitybased social services
• RESULT – INFRASTRUCTURE IS IN PLACE TO PROVIDE COMPLEX AND COORDINATED
COMMUNITY BASED SERVICES FOR THE TARGET GROUPS

Regional round table discussions with the representatives of municipalities, NGOs, social care institutions at the Regional Councils for the Development on the “DI process”

Summarizing up round table discussions : drawing up the visions on how the social care services could be organized in the region

Monitoring group for the implementation of the “DI Plan” has been set up May 2014 ( amendments in November 2014 )
