The Transition Team works with families and young people to help facilitate the smooth transition of (vulnerable) young people into adulthood.

Transition Team - Involvement

Involvement between 14-18yrs:
- Be ‘involved’ at yearly reviews (where appropriate).
- Learn about your likes, dislikes, needs, and aspirations.
- Hold ‘information sessions’ to explain who we are and what we do.
- Assess (at 17yrs) if you will be eligible for Adult Social Care Services.
*Not lead professional at this point(But we will be working closely with Children's Services where appropriate).
