Short Breaks – Monitoring information• Lots of great activities, good uptake and feedback from families
• Also good to understand how activity providers are adapting and changing when things don’t go as planned
• A requirement - and important to us:
- for panels who need to make decisions about how best to spend the short breaks budget
- for councillors who make decisions about what to spend money on overall – the councillors want to be sure short breaks money is used effectively, for Hampshire families who need it
Gateway Card• Thank you to activity providers who have encouraged families to get
Gateway Cards, and provided the Gateway Card numbers to us
• Families have other benefits e.g. additional borrowing at Hampshire libraries
• Many activity providers ask for the Gateway Card number at time of booking
• You only need to ask for the number once for your regular attendees!
GATEWAY CARD NUMBERS ARE ESSENTIAL
Short Breaks Grants – 2013-14
Changes for this year:• Piloting Local grant panels – local knowledge and parent /
carer involvement. Applications need to be separated by geographical area.
• Application process is now online on the standard Hampshire County Council grants form
Closing Date for applications:• Applications must be received by Tuesday 6th November

• For multiple activities, you can fill out one grant application and put the various clubs/sessions details on the Supplementary Question forms – these can be accessed when you are in the online application system, or from the short breaks funding pages
• Geographical areas need to be in separate applications
• You can request funding to help subsidise families in financial hardship
• Discuss your application with Bridging workers and other local providers to avoid duplication

• Please make sure you tell us:– What is the activity?– Where are you doing it?– How has the need been identified?– When will it take place?– Who will attend?
• Attach documents required with your application• Send in your application well before the deadlineAnd finally…
