Please be advised that your Nuclear Materials license 40-29299-01, will expire on June 30, 2018. In order to prevent termination of your license, you MUST submit your renewal application so that it is received by the NRC no later than June 30, 2018. If you need to fax in your request in order to meet the submission deadline, you may fax it, to my attention, to 817-200-1083; and follow up with a hard copy through the mail. If your renewal request is not received by COB June 30, 2018, your license will, subsequently, expire, and you will have to divest yourself of any radioactive material before the expiration date, or commit to not using the radioactive material in NRC jurisdiction. If you make the commitment to not use the radioactive material in NRC jurisdiction, please give us a timeline of submitting a new license application (with applicable fee) or NRC Form 314, Certificate of Disposition of Material.

Please keep in mind that no fees need to be sent with a renewal application.
