ASH HANDLING PLANT DATE: 17.04.2013 APPROVAL NOTE: Subject: Administrative Approval for the work of providing of isolation valve along with

dismantling, fabrication & assembling of Adapter (Spool piece) from required size of M S Plate at ESP hopper Unit no.1, 2 of Ash Handling Plant.

During the R&M of 2 x 120MW sets in the year-2008-2010, new designed UCC make 20 Nos. Material handling valves (MHV) were installed in all ESP hoppers by BHEL. At present in the running unit of Boiler #1 & 2, we are unable to attend defects such as replacement of MHV seat, body seat, removing of foreign material from ESP Hopper etc due to non availability of Isolation between ESP Hopper & MHV. As a result fly evacuation system gets disturbed & field rectifier gets out of service which adversely affects stack emission. In a newly erected ESP Hopper of additional field, isolating valves are provided by BHEL as an R & M contract work. In view of these facts, it is proposed to prove isolation valves between existing ESP Hopper & MHV by dismantling of existing spool piece and providing of new Adapter (Spool piece) by cutting, drilling & grinding from M S Plate in both the units to facilitate the easy maintenance & isolation for repair/rectification/replacement of MHV or Body seat during running plant thereby avoiding the outage of field rectifier incase of Hopper filled with ash. The subjected works requires special skill in fabrication as accommodation of new isolation valves with new adaptor between fixed existing ESP hopper bottom & fly ash line. Due to paucity of staffs & engagement in day to day maintenance, it is proposed to carry out the subjected from well experienced & resourceful contractors for the smooth & better evacuation of fly ash system of Boiler #1 & 2. The rates are calculated as per prevailing labour charges which are taken from the circular ref. no. Labour Escalation No. CCUT/HR/Daily wages/Contractor labour/1972 dtd.03/04/2013 (For period from 01-04-2013 to 30-09-2013). The estimate cost for fabrication & erection of 01 No. of Adaptor with isolation valves worked out is Rs. 9027:00 per adaptor /spool piece as per attached detail estimate sheet & Annexure-I.

Therefore the total estimates cost for fabrication & erection of 20 Nos. of Adaptor is Rs. 1,80,540.00 (Rupees One lac Eighty Thousand Five Hundred Forty only).

As per DOP - 1a for Works (Individual & Committee levels), Competent authority being is requested to accord approval for the work of providing of isolation valves along with dismantling of existing adaptor and fabrication & erection of new Adapter (Spool piece) at an estimated cost Rs. 1,80,540.00 (Rupees One lac Eighty Thousand Fi ve Hundred Forty only) by inviting offers form experienced & recourse full contractor thro’ press tender enquiry.

Point No. 2): Review estimate & procure adaptor as supply items:

The estimate is calculated on min. labours required & labour rate is considered as per Labour Escalation No. CCUT/HR/Daily wages/Contractor labour/1972 dtd.03/04/2013 (For period from 01-04-2013 to 30-09-2013). It is to state that the new adaptor (fabricated) is required to be accommodating between existing ESP Hopper & Material Handling Valves (MHV) with providing of new Isolation Valve. The space between existing ESP Hopper & Material Handling Valves is not equal as it is vary from Hopper to hoppers. Hence it is very difficult to decide the exact length of adaptor, the minor difference in adaptor length/ error in fabrication will resulting misalignment between Hoppers to MHV & MHV to Fly ash carrying line (X- axis & Y-axis) and vacuum system gets disturbed. In the view of above outline fact, it is opined that subjected work if to be carried out by single agency, retrofitting/misalignment of various matching parts of valve & adapter can be practically eliminated & rectification if any to suit site requirement can be carried uot without any delay. Demand Draft drawn on Bank of Baroda or SBI payable at Ukai. Tender submitted without EMD are liable to be rejected. All tenderers are strictly instructed that Xerox copy of DD No. of EMD & Tender fee to be sticked on tender cover without which it will be presumed that EMD is not paid and tender will not be considered / accepted.

02. The tender is to be submitted in two separate envelopes, one for Technical Bid & second for Price Bid duly offered in sealed super- subscribed with Tender Number, Name of Work, Due date of opening, Photocopy of M.R. of EMD Tender fee shall be received by RPAD only.

03. Tenders received late after prescribed due date & time will not be accepted. Any alternation / modifications to the bids received after opening of the tender shall not be considered unless called for by the GSECL in negotiation.

PRE- QUALIFICATION CODITIONS FOR TECHNICAL BID

01) Tenderer should also certify that the tender submitted is unconditional tender and specifically remarks “Unconditional Tender” on cover itself.

02) For technical bid the bidder will have to submit following documents. a. Past executed order copies (experience certificate etc.) of same/similar work carried out in GSECL, other State Electricity Board, NTPC etc. b.Copy of separate P.F.Code number/ PAN No. and receipt of EMD paid. c.Service Tax Registration Certificate. d.List of vehicles, tools and tackles lying with them for carry out the work. e. Schedule-A Terms,& conditions duly signed on each page, deviation if any clearly be brought out. f. Evidence of having successfully/satisfactorily carried out the simillar type of work satisfactorily at the thermal Power Station of SEBS,NTPC & Public Sector within last 04 (four) years period. The experience certificate with order copy & satisfactory work completion certificate from such organization where the work has been carried out by the bidder should be submitted. g.Solvency certificate of minimum 20% of estimate cost from any scheduled bank. Technical bid without above will not be qualified % liable for rejection. 04) Chief Engineer (GEN) UKAI TPS reserves the right to either accept or reject any or all tenders without assigning any reason any thereof. 05) The offer without any of the above documents or non acceptance of any clause shall be liable for rejection. SCHEDULE-A

Scope of Work:

1) Dismantling of existing Adapter (Spool piece) from ESP hoppers, without damaging / disturbing the nearest parts of ESP hoppers.

2) Fabrication of newly Adapter (Spool piece) from required size of MS Plate for ESP hoppers.

3) Assembling of newly Adapter (Spool piece) and isolation valve at ESP hoppers. 4) Required steel materials i.e. 8” MS pipes /M.S plates size 08 mm to 12 mm for Adapter

(Spool piece) whichever is available to be drawn from main store for fabrication of Adapter (Spool piece) for ESP hopper #1 & 2. GSECL will issue MS plate & steel materials of required size free of cost from TPS main store.

respective specimen as per sample given and instruction of E.I.C. 7) The fabrication includes cutting of plates machining, drilling, & grinding etc. 8) All consumable items i.e. O2, DA, and cylinder; grinding wheels, Lathe machines,

grinding machine, drilling machine etc. are to be arranged by the party at their own cost. 9) All other consumable material like MS bolts, Nuts, Washers, Gasket, General

purpose welding electrodes etc. are to be arranged by the party at their own cost. 10) Party has to assemble & erection of newly fabricated Adapter & isolation valve in such

way that there is no ash, air leakage during commissioning of plant. 11) After assembling of newly fabricated Adapter, MHV & Fly ash pipe lines alignment are

under party’s scope. 12) Party has to observe all the laws including Factory/act/labour laws and all the safety laws

which are present in force. 13) Red oxide two coats & one coat of smoke grey color are applied on each Adapter (Spool

piece) assembly. 14) Any work related to contract awarded is not mentioned in the scope and is required to be

carried out than same will be in the party’s scope at no extra cost.

TEHNICAL TERMS & CONDITIONS

1) Necessary safety precautions are to be taken for safety of working personnel. 2) Party has to observe all the laws including factory/act/labour laws and all the Safety laws at

present in force. 3) After completion of work all the scraps are to be credited to the TPS Main Store. 4) Red oxide & smoke grey color to be applied on each fabricated items. 5) Above all works are to be carried out in GSECL UKAI TPS premises/Party’s premises. 6) 5 % of the order value will be retained against cleaning and clearing the site and handing

over the space occupied as per instruction of E.I.C. Party shall have to clean the area and remove the foul material, cotton waste and scrap in all respect in the area in which work is executed and get it certified through the Engineer-in-Charge looking after the job. Retained amount will be released only after ensuring that the respective work area is cleared by the party satisfactorily.

7) In case party fails to clean the area and clear the site, within five days after completion of work, same will be carried out at the risk and cost of the party from the amount 5% of the order value retained by the Company.

8) The validity of this A/T shall be 11 (Eleven) months from the letter of commencement issued by the competent authority. The GSECL reserve the right to extend this contract for further period of 06 months, considering the balance quantities yet to complete and it will be binding to the party. The rates and other terms and conditions will remain same as per original order for the extended period. Party should have complete the erection work of isolation valves & fabricated adaptor at identified ESP Hoppers & same should be commission within 25 Days of the available annual shut down or force shut down in each unit of Boiler unit 1 & 2. Party has to keep ready the fabricated adaptor in adequate number before the onset of AOH for which material shall be issued after the placement of order which shall be intimated by this office.

9) 10 days advance intimation will be given before AOH of Units and party will have to mobilize man, material and vehicles at site immediately on intimation.

10) Drawing is given for estimate purpose only & party shall arrange to fabricate the respective specimen as per sample given and instruction of E.I.C.

11) Party shall get the sample approved prior fabricating in mass. 12) Minor rectification if required while fitting, same shall be arrange by the party free of cost. 13) Welding should be carried out fully as per the fillet mentioned with good quality general

purpose welding electrodes. 14) Party shall get the welding electrode approved prior to be utilizing it. 15) In case of dispute, decision of C.E. (G), Ukai TPS shall remain binding to both user

department and the agency. 16) C.E. (G), Ukai TPS, reserves the right to reject one or all bid without assigning any reason

thereof. 17) After fabrication of the adapter, party shall get the adapter inspected through AHP Engineers

and there after same shall be carried in sub store. 18) Transportation of raw material either from the main store or department store falls in the

scope of contractor and the party shall credit the good material in GSECL Main Store or

AHP sub store as per the instruction of Engineer-in-charge and shall credited the scrap in GSECL Main Store in his quoted rate.

19) Party shall furnish the material account in detail in given Performa given by Engineer In charge along with the bill otherwise bill will not be accepted.

20) Party shall cut the raw material judiciously so that minimum wastage occurs. Undue wastage made by the party will be recovered from the bill of the party.

21) It will be the contractor’s responsibility to take extreme care while working lifting and shifting or any other such job related activity. Any damage to the GSECL‘s property or loss of material shall be charged appropriately to the party and recovered from the bills.

22) It will be the party’s responsibility to collect required size of plates & welding electrodes from main store Or sub store to his premises in his quoted rates for that no any extra cost will be paid by GSECL.

TERMS & CONDITIONS REGARDING INDUSTRIAL LAWS AND OT HER RELATED MATTERS TO BE INCORPORATED IN TH E TENDER DOCUMENTS

1.1 Wages to be paid and time of payment etc. by the Contractor. 1.2 The contractor shall pay minimum of wages per day or as may be specified hereafter or rates fixed

under the Minimum wages Act, whichever is higher. The wages of every contract labour employed by him under this contract shall be paid him before the expiry of 7th day of the last day of the month in respect of which the wages are payable ( i.e. wages of a month have to be paid by him in the first week of the next month). The payment shall be disbursed in the presence of management Representative during the working hours in factory premises and the contractor shall get the entires certified in the register of wages by the representative of the Corporation. Any default will result in cancellation of contract forthwith or else the contractor shall be punishable to the extent of Rs. 400/- fine per each day.

1.3 The contractor shall give his telephone number and address to the Corporation so that in case of labour troubles etc. the contractor can be contacted. The contractor shall arrange to have his office outside the factory premises and the contractor keep himself present through out the working hours “As contractor you will be responsible and liable to pay difference in wages if any or observed the revised service condition that may be awarded by the Honorable Industrial Tribunal effective from the date directly in the Award”.

02) Labour Laws: 2.1 Person below the age of 18 years shall not be employed for the work. 2.2 No female worker shall be employed in the night shift between 7 PM to 6 AM. 2.3 Contractor shall maintain a valid labour license under the Contract Labour (Regulation and

Abolition) act for employing necessary manpower to be required by him. In the absence of such license the contract shall be liable to be terminated without assigning any reasons thereof.

2.4 The contractor shall at his own expenses comply with all labour laws and amendment made from time to time keep the Corporation indemnifies in respect thereof. Some of the major liabilities under various labour and industrial laws which the contractor shall comply with are as under:-

2.5 Payment of contribution by way of Employees (contractor Labour)aswell as employer’s )contractor) Contribution towards Provident Fund, Family pension scheme, Employees Deposit linked insurance scheme, Administrative charges, inspection charge etc. at the rates made applicable from time to time by government of Gujarat/Government of India or other statutory Authority.

2.6 Labour engaged by you shall be entitled for Earn leave as per the provision laid down in factories Act. 1948. Contractor shall extended the facility of E.L. as per the provision laid down in factories Act. And shall also have to maintain leave records in prescribed form under factories Act.

2.7 No.labours are allowed to enter in power station premises who do not have Identity card prescribed under Factories Act.

2.8 Contractor shall have to comply with the Maternity Benefit if he happens to engaged female labours as per the provisions of the Maternity Benefit Act.

2.9 Contractors shall have to obtain Insurance of the labourers engaged buy him to carry out contractual work before commencement of work under workmen’s compensation Act. 2.10 P. F. code Number: The contractor shall have to submit the details of his own P.F code number along with offer from appropriate authority. Without P.F. code No. the tender is likely to be disqualified. 2.11 Provident Fund & Family Pension Schemes The Contractor shall submit along with his bill (month-wise) a statement regarding deductions

at the rate of 12% of the wages against employees provident Fund & Family Pension Scheme in respect of each concerned employee. (or at the rate made applicable by the Government from time to time) of the wages. The contractor’s contribution and his workers contribution towards Provident Fund and Family Pension Scheme shall be deposited by the Contractor with Regional Provident Fund Commissioner, Ahmedabad or to the authority prescribed under the Act. Along with the other charges applicable.

2.12 If any workman engaged by contractor meets with fatal or non-fatal accident while on duty then contractor shall be liable to pay compensation Act.

2.13 Employees Deposit Linked insurance Scheme: The contractor shall have to deposit ½% of the wages in respect of employees who is member of the Provident Fund as the contribution to the Deposit Linked insurance scheme with Regional Provident Fund Commissioner, Ahmedabad

2.14 The contractors should pay prevailing minimum wages to the labourers engaged by him as per the minimum wages Act. In presence of the corporation officer of representative.

2.15 The contractor shall deposited fifteen days salary for every completed year of service in case of each worker towards the liability of gratuity. 2.16 Administrative Charges: Administrative charges for maintaining provident fund A/c. shall be deposited by the Contractor with Regional Provident Fund Commissioner, Ahmedabad at the rates applicable. 2.17 Labour License for Labour Contract: The contractor shall have to obtain the copy of labour contract license under contract Labour (Regulation & Abolition) Act. from the appropriate authorities for commencement of work 2.18. Contractors shell have to issue the contractual appointment letter for a specified period to their

labour engaged during the contract period each and every instant & should narrated clear out terms and conditions for the liabilities of the concerned employees.

2.19. After completion of contract work, the work completion certificate is required to be produced before the DGM/IRO for the purpose of no dues certificate and cancellation of gate passes issued to the contractors work men at the time of execution of the contract work.

2.20. The contractor shell have to maintain all the relevant records under labour laws as mentioned here in above and keep ready for verification by the corporation authority as well as the authority of the Govt. official a and when asked for the checking.

2.21 The contractor shell have to produce/submit the original copy of wedges register/muster roll and relevant record under labour law for the contract work as & when demanded by the corporation statutory completion if arises in future.

The above are some of the major liabilities of the contractor in addition to other liabilities prescribed under the various labour laws in force from time to time from statutory Authorities like stage Government/Government of India which the contractor shall have to comply with.

03) Paid Leave Facility: Paid leave facility at the rate of one day for every twenty days worked by the contract labour shall be provided by the contractor to his workers. He shall maintain Leave Records/Leave Cards, for individual laborers which shall be duly verified and approved/certified by the authorized officer of the Corporation.

04) Workmen’s Compensation Fund & Employer’s Liability Insurance: The contractor shall cover all his employees under workmen’s compensation Fund & under the employer’s liability insurance. 05) Maintenance of registers & records: The contractor shall employ adequate number of experienced staff at site for daily supervision and for maintenance of various registers and records required under the law and contract. No payment for supervision shall be admissible. 06) Contractor to indemnify the Corporation: The contractor shall indemnify the Corporation and every member officer and employees of the

Corporation also. Engineer-in-charge and his staff against all action, proceedings, claims, demands, costs and expenses whatsoever arising out of or in connection with matters referred in herein above elsewhere and against all actions, proceeding claims, demands, costs and expenses which may be made against the Corporation or Government for or in respect or for performance of his obligation under the contract documents. The Corporation shall not be liable for or in respect of or in consequence of any accident or injury to any workmen or other person in the employment of the contractor of his sub-contractor, and the contractor shall indemnify and keep indemnified the Corporation against all such damage and composition and against all claims, demands, proceedings costs, charges and expenses, whatsoever in respect thereof or in relation thereto.

07) Workmen’s Compensation and Employer’s Liability Ins urance: Insurance shall be affected for all the contractor’s employees engaged in the performance of this contract. If any of the work is subject, the contractor shall require the sub contractor to provide workmen’s compensation and Employer’s Liability Insurance for the latter’s employees unless such employees are covered under the contractor’s insurance.

08) Corporation’s Right to Terminate Contracts: The Corporation reserves the right to terminate this rate contract at any time during its pendency without giving notice of termination or any reasons thereof.

09) Corporation’s Right to Deduct from Contractor’s bil ls: The Corporation will be entitled to deduct directly from the bills, to be paid to the contractor any

sum or sums payable by you & which sum/sums the Corporation is required to pay as a principal employee on account of your default in respect of all liabilities referred to in above clauses.

GENERAL TERMS AND CONDITIONS

01 The ‘Contract’ means the documents forming tender and subsequent agreement that may be entered into.

02 The tender should be submitted along with EMD without which no tender will be accepted. 03 Conditional tender will not be accepted Unilateral withdrawal of condition after opening of tender will not

be permitted. 04 The general rules & regulations of the corporation for tender and contract for works will apply to the

extent the same are not modified herein. Any other rules and regulations, conditions etc. that are in force at present and that may be formed be the corporation from to time in connection with contracts will be blinding and acceptable to the contractor.

05 Sub-letting or transfer of contract without prior written approval of corporation will be treated as a breach of contract it well be punishable by forfeiting the security deposit as well as termination of contract.

06 CHIEF ENGINEER (G.)TPS, GSECL, UKAI reserves the right to reject any tender (s) without assigning any reasons thereof.

07 The successful bidder has to pay total 5% of the order value as security deposit in cash or DD payable at Bank of Baroda or State Bank of India Ukai branch in favour of Gujarat State Electricity Corporation Ltd. The amount of Security Deposit will be refunded only after six months from the date of completion of work and after deducting any dues of the Corporation.

08 The contractor shall hold the corporation indemnified against and in respect of any claim (S) arising out of injury to any person(S), loss of life, workman’s compensation or any other claims whatsoever. The contractor shall be responsible for any damages to any property of the corporation due to negligence of the contractor or his employees. The corporation will be final and blinding to the contractor.

09 The contractor shall submit his Sales Tax registration number and same should be quoted on all his bills.

10 The contractor will have to enter into an agreement with the corporation and the cost of the stamped paper shall be borne by the contractor, the contractor shall also have to execute an indemnity bond on required value of stamped paper as per the corporation rules. The cost of the stamped paper shall be borne by the contractor.

11 Arbitration Clause: " All questions " disputes or differences what so ever which may at any time arise between the parties to this agreement in connection with the contract or any matter arising out of or in relation there to, shall be referred to the Gujarat public works contract dispute Arbitration tribunal as per provision of the Gujarat public works contract disputes Arbitration Tribunal Act, 1992”.

12 The party shall strictly observe all the Labour Laws and Factory acts in existence and amendments made from time to time. i)Party shall have to strictly observe all Safety rules as per the Factories act 1948 and whatsoever amendments made from time to time to avoid any chances of accidents. Contractor shall have to strictly follow the instructions of Safety Officer.

ii)The safety precaution issued vide this office Circular no, CCUT / Safety / 4 / 90 / CIR / 345A dtd.14-12- 90 are applicable to this contract also and to be followed strictly. iii)Before starting the contract, it is advisable that party should have the documents for insurance of

workers. 13 The work shall be completed within stipulated time. If required, party shall have to work round-the-clock

to maintain the work schedule. 14 Payment shall be made as per actual work done only. N. O. C for P.F. schedule of labour engaged for

the work is to be submitted along with bill verified by D.G..M. / I.R.O / L.W.O 15 The contractor shall have to take all necessary precautions to avoid fire during, cutting, welding etc. In

case of any shouldering fire, the same should be extinguished by contractor immediately using their own labour, tools water and buckets etc.

16 The party shall arrange his own adequate tools and tackles, welding accessories drilling machine, girders, scaffoldings lighting materials including lighting transformers as pre IS-1416 etc. required for the work included in Schedule-A.

17 Party shall have to arrange all consumable, including required kerosene, rustolene, paints, grinding wheels etc. at his own cost.

18 An authorized and technically qualified representative of the contractor must be available at site white the work is going on. The address of the representative should be submitted to this office so that party can be contacted whenever required in connection with execution of the work.

19 Scaffolding material, if available with GSECL will be issued on chargeable basis, otherwise contractor will have to arrange for the same at his own cost. After completion of work scaffolding is to be removed immediately.

20 After completion of work, working area and surrounding area should be made clean by removing M.S. debris and all types of scrap etc. Scrap should be credited to main store and should be dumped at place shown by E.E. (Stores).

21 Electrical connection & consumption will be given free of cost at one mutually agreed point. 22 Oxygen / Acetylene gas if available, will be issued on chargeable basis from GSECL main store,

otherwise the party has to arrange on its own. The empty cylinders should be returned to main store within stipulated time.

23 Party shall submit daily progress report for the work carried out by them. 24 The contractor, if desires, can seen the plant and site of work with permission of undersigned on any

working day during working hours. 25 Party has to carry out the work at site only. 26 The tenderer / contractor shall have to submit their own P.F. code number if allotted by RPF

commissioner / P.F. authority. In case of P.F. authority has not issued / allotted P.F. code number to you then you shall have to comply with provision of the P.F. Act and submit P.F. contribution of your Employees and contribution of yourself to the P.F. trus of the corporation.

27 All the employees engaged by you will be covered under P.F. Act and if you are denied the separate P.F. code number by the P.F. Authority than you will have to submit the P.F. contribution along with administrative charges to GSECL P.F. Trust without fail. If you will not comply with this condition the final bill will not be released

28 “OH & Management System Requirement: It is to be noted that GSECL Ukai TPS is establishing and implementing Occupational Health & Safety

Management System complying with OHSAS 18001:2007. You as a part of the GSECL Ukai team and as defined by the scope of mentioned standard, all

contractors are required to comply with and participate to prevent the occupational Health and safety hazards and risks including observation of safety rules. Further you are also to comply with all applicable legal requirements while executing the work, in case the job is assigned to you.

UTPS – SAFETY - 002 REV - 00 15\12\2008 Ukai TPS “GENERAL SAFETY RULES TO BE OBSERVED BY THE CONTRAC TORS”

All the contractors working at Ukai thermal Power Station shall have to strictly observe following safety rules. The contractors shall be responsible for informing and observing these rules by their supervisors / employees /labours as well as the supervisors / employees / labours of their Sub-agencies / sub-contractors engaged, if any. Prior to commencement of the work, contractor shall have to submit a written assurance on their letterhead to the concerned Sectional Head / Engineer-in-charge that they have thoroughly gone through these rules, have educated their employees / employees of their sub contractor and will strictly observe the said rules while execution of work under work contract awarded to them. They will have to indemnify the corporation for any loss or damage / accident / injury to the corporation’s property / employee or employee of their own in default of non - observing these rules.(01)Persons to be employed for carrying out the work shall possess required qualification, be fully trained and conversant for works to be done. All persons should have gate pass. Register consisting the full details (i.e. address, phone no, details of nearest relative, etc.) of all persons is to be maintained .during the work execution, one trained & competent supervisor should always remain present at site.

(02) The contractor shall all the required safety measures prior to commencement of work on dangerous substances, machineries or area at which cautionary notice is displayed and obtain “line clear” or “work permit” through the concerned Department / Section.

(03) Prior to carrying out welding, gas cutting, furnace heating or any other hot work job, the contractor shall remove all the inflammable material lying at nearby worksite or cover it properly by suitable protective covrin. Also, special care shall be taken before carrying out such job & see that all possible contributing factors to set fire shall be removed / vanished prior to commencement of the work. Advance intimation shall be given to concerned section / fire section to commence the work in fire prone areas. they should also keep ready all the first aid fire extinguishers / equipments & fire extinguishing media / material like sand / water buckets or other appropriate equipment at such place.

(04) While carrying out work in confined space or inside vessel, the contractor shall obtain necessary “confined space / vessel entry permit” from concerned department prior to commencement of the work.

For lighting in such areas, 24 volt (ISI certified & with proper guard) hand lamp shall be used. For taking care of the persons working inside the confined space / vessel, a supervisor / person capable

to keep continuous watch on person(s) working inside, assist them incase of emergency or arrange to get immediate outside help. Shall remain present at entry point shall remain present at entry point and shall use full body safety belt without fail.

While working inside sewage, trench or in-depth, a person to warn outsiders / entrants/ passers etc shall remain available near entry point or the entry point shall be cordoned by a barricaded tape with a cautionary notice. After completion of the works, all the lids / covers / grills / grits opened, shall be re-fixed / re- placed in the original position as it were prior shall be re- fixed / re-fixed / re-placed in the original position as it were prior to commencement of the work and leave the work place in safe condition in all respect, so as to prevent accident to fellow workers.

(05) The contractor shall see that he / his persons do not block (by stacking material, spare parts, tolls-tackles, equipments etc), any passages / walkways / gangways / aisles / staircases / ladders / lift or any other approaches / roads leading to plants or its auxiliaries, on which there is a traffic movements in case of emergency . if it is utmost necessary to carry out work in such area with blocking of passage, prior permission of competent authority or the engineer- in- charge shall be obtained. To demarcate /declare the area as UNSAFE, cordon it using barricading type & display suitable caution notice or keep a person to restrict / divert the traffic route through other safe passage.

(06) Prior to use power / electrically operated hand tools / equipments / machines / gadgets like welding machine, hand grinder , hand drill etc, ensure its safe operation & use it only if it is found safe to use . do not use defective, unsafe or improperly maintained equipments.

The electrical power supply required to run such equipments shall not to be taken directly at their own but shall be obtained through concerned electrical maintenance departments or their authorized persons or under their observations / guidance only. The electrical section shall provide temporary electrical connection up to contractor’s mains board on which it is compulsory to install mains switch, ELCB & fuses of adequate capacity. all such equipments shall invariably be earthed adequately to prevent electrical shock, sparking, short circuit etc. power cord to be used shall be of adequate capacity, without any joint & shall consist of earth wire also. Hence, it is necessary to use adequate

capacity 3-wire power cord for single & 5-wire power cord for three phase power connections. The plugs, receptacles, pins, holders etc shall be of adequate capacity & safe to use.

All electrical & mechanical equipments / tools-tackles viz. welding machine, cutting machine, grinder drill, chain pulley blocks, hook chooks etc required to be used during work execution shall be of standard make & bear ISI certification mark on it. The consumables like welding electrodes, grinding wheels / discs etc which has specific prescribed life span shall not be used in any case if its expiry date is over.

(07) It is compulsory to use standard make personal protective equipments (P.P.Es.) as per the job requirement. Do not work without use of required P.P.Es contractor is responsible to provide standard make (ISI approved) personal protective equipments / safety gadgets suitable to give sufficient protection against hazards involved in their work / job to their staff, as per the job to their staff, as per the job requirement and insist / enforce their work / job to their staff, as per the job requirement and insist / enforce their staff to put on the same while at works.

The Ongoing work is liable to be stopped at any time if the contractors’ staff is found working without P.P.Es. Following is the list of various P.P.Es. To be used for various works / worksites. 01 Industrial Safety Helmet For protection of Head against falling objects or during fall of

person from height. 02 Safety Goggles (Grinding, Welding,

(08 )Before using lifting machines / tackles (like C.P. Bs., Hook chooks, winch, forklift, mobile crane, EOT crane etc.) & its attachments (like D-shackles, slings, U-clamps, Eye bolts or any fixtures), it shall be checked and used only if found safe to use. Also, ensure that these are tested, examined & certified in form no.9 / 10 by competent person as per the Factory act-1948 and its validity is not expired. Further, it shall be fixed properly and firmly prior to lifting the weight.

(09)Scaffoldings to be used for working at height shall be adequate size & capacity. Obtain the work permit when working at height. While climbing on such scaffolding or working on any structure at height, use of full body safety belt & helmet is compulsory. It is also necessary to fasten chinstraps of the helmet.

(10) Contractor or their employee shall not interfere in day-today routine plant activities / works except the work assigned to them, shall not loiter in the areas other than their work jurisdictions, as well as shall not temper / operate / touch the machineries / equipments / auxiliaries with which they are not concerned. Also, the contractor shall strictly instruct their staff not to sit or take rest at / near / below running plants, auxiliaries, systems or any place which is risky, hazardous & prone to accident.

(11) The cylinders containing poisonous / toxic or inflammable / explosive gas like oxygen, acetylene, LPG, hydrogen, ammonia, chlorine, co2 shall be handled safety taking due care. To handle / shift such cylinders a special trolley / cage meant for it must be used but in no case it should be rolled. Domestic LPG cylinder shall not be used / permitted.

(12) On completion of the work, cotton waste, spilled oil / grease, pieces of welding rod & other waste material shall be removed from work site and the area shall be left safe, neat & clean.

(13) In case of any injury / accident while working, it shall immediately be reported to safety department through concerned sectional head / Engineer. The prescribed from no.21 may be obtained from concerned section or safety officer.

For any incident occurred but have no injury to any persons should also be informed to safety officer as “Near Miss Incident.”

(14) In all risky jobs, before starting work, contractor should obtain general safety work permit from concerned section well in advance.

(15) In case of noticing smoke or fire during their work execution, they shall make immediate efforts to extinguish / control it and simultaneously inform the fire brigade on phone no.4444 or 4344 or 5555 or 02624-290049,shall shift the casualty to nearby hospital after rendering first aid in case of accident.

(16) Over & above these, contractor shall have to follow all the safety requirements / rules & regulation / norms and legal provisions laid down in various statues, particularly the provisions of The factories act-1948 & The Gujarat states factories rules-1963 (Amended up to date)shall be followed strictly. The contractor shall also obey the rules / regulations / instructions of the local competent authority for safety requirements.

(17) No women or young person shall be allowed to clean, lubricate or adjust any part of a prime mover or of any transmission machinery while the prime mover or any transmission machinery is in motion. Examination or operation of motion machinery shall be made or carried out only by a specially trained adult male worker wearing tight fitting clothing as per section 22 of factories act 1948.

(18) No women or young person shall be employed or permitted to work in lead-compound area like battery room etc. as per schedule VI of GFR 1963.

(19) All the relevant labour and industrial laws shall also be followed compulsorily. (20) It is felt necessary to deploy safety officers / safety supervisors by contractors / agencies involved in

carrying out hazardous activities / operations inside TPS to have better and constant supervision in terms of health safety and environment activities.

(21) in case, it comes to the notice of GSECL UTPS management that the safety guidelines / safety rules / safety norms are not being followed then a minimum sum of ropes one thousand shall be penalized on the contractor / Agency and for subsequent such violation, a server penalty / action as deemed fit shall be imposed, which may please be noted.

(22) The above rules shall be scrupulously followed and where required, they may contact the safety officer for any ambiguity / further guidance in this regard.

(23) For performance evolution of contractor, safety factors of work accident, fire incident & near miss accident will be considered. Steps can be taken to review the job assignment up to cancellation for negligence.
