Behavioral Health InnovationTarget’s Goal: reducing stigma of mental illness
Need: understand scope of depression & anxiety, identify & treat earlier.
Gap: team members weren’t open to sharing struggles.
Innovation: initiated PHQ-2 screening for every primary care patient since June 2017
Outcome: June 2017-August 2018 screened almost 5000 patients, 8.4% positive PHQ2, 21% positive PHQ9.
Results: best remission rates within HealthPartners/Park Nicollet network
