MONTHLY RENTAL: Monthly rental is $400.00/month. We will provide: Oxygen, propane, electric, utilities, garbage, internet access, security system, etc. You also get a 10% discount at the Conscious Glass Website for glass supplies; as well as a discount on classes we host. Weekly rental is $140.00, or $20 a day. We have a few torches available for day use for an additional $10 a day. There is a $25 deposit for a Door access card. $20 of this is refundable upon return of the card. First month's rent is due upon moving in. Thereafter rent can be split, and paid ½ on the 1st, ½ on the 15th. You have a 5 day grace period to pay rent, after the 5th day you will be charged a $25 late fee, and receive a strike. We are operating on a three strike system: After the third strike, if you are late again, we will ask you to leave. Strikes will be erased with full payment (if you have three strikes, and pay on time for three months, you will have zero strikes...) MOVING OUT: You must give notice if you plan to leave. Notice must be given with the same time parameters as your rental period, I.E. if you rent monthly you must give a month's notice, weekly rentals need a week of notice... Please note: if you leave for any period of time during your rental contract you must still pay rent for that time if you have not given notice and removed your equipment. Upon moving out, you agree to clean and leave your station in the same state it was when you moved in. This includes removing everything you have brought to the shop. We are not responsible for equipment/belongings you leave when you move out. CLASSES: We will be hosting classes from time to time. For the period of the class you will be asked to consolidate your equipment under your bench. We offer a discounted price for the class for all renters. Please note that we have security cameras. At any time you may be recorded while in the shop.

Please read this form carefully. By signing this release agreement you are waiving your right to seek compensation in case of injury, loss or damages. I, the undersigned, agree that: I understand, agree and acknowledge that in return for being allowed to rent space and participate in these activities, I have read and voluntarily signed this agreement. I understand that some of the equipment and activities that take place at Conscious Glass Creations Studio could cause economic damage to my property; and pain, injury, or damage to myself. I understand and acknowledge that my participation in activities at Conscious Glass Creations Studio is completely voluntary. I understand that I am free to choose not to participate in any or all activities at Conscious Glass Studio. I understand and acknowledge that I am an independent contractor. By agreeing to the above terms I understand that I must provide my own insurance for my operations at Conscious Glass Creations Studio, 300 Ewan Road, Mullica Hill, NJ 08062. Conscious Glass is not responsible for this coverage. For Myself, and for my spouse, parents, family, heirs, executors, administrators, agents and assigns, I hereby agree: I hereby knowingly and voluntarily assume the full risks of any and all physical or other injury, damage or losses during any activities organized by or on the property of Conscious Glass Creations. I hereby waive the liability of and agree to hold harmless Conscious Glass Creations and all of its employees, members, agents, staff, family, vendors, successors and assigns, and any sponsors, facilitators, organizers, contractors, volunteers and other participants. This agreement is binding upon myself, my spouse, parents, family, heirs, executors, administrators, agents and assigns. Please do not leave your torch on and unattended. Please ensure that your bench area is safe (no fire hazards, etc...) Please familiarize yourself with the locations of the fire extinguishers, first aid station, and exits from the building in case of emergencies. Please familiarize yourself with the shut off procedures for both the Oxygen and Propane lines.

CLEANING: Please clean your station weekly. Please clean up after yourself around the shop. Please be respectful of the fact that many people use the bathroom. Be clean. The coffee area is shared with the public and all renters. Please be respectful and clean up after yourself in this area. Items placed in the refrigerator must be marked with your name. If not marked, they are fair game and may be used or thrown out without notice. Smoking: There is no smoking cigarettes in the shop. Do not smoke by the propane tank or natural gas piping next to the shop. Please be courteous and do not leave butts or a mess on the ground. Alcohol: please refrain from drinking in the shop. You are not permitted to become intoxicated in the shop. If this presents a problem you will be asked to leave. Do not take other people's tools, glass, supplies etc. without written permission to do so. If you have a problem with another renter: Talk to a manager! Please do not address the renter until you have talked to a manager first. Visitors: This is a public shop. Please be aware that visitors may come into the shop. We are not liable for visitor's safety. Parking: There is a parking lot next to fire station. Please park there. Do Not park in front of the Studio Warehouse or Studio. Do Not park in front of the Oxygen/Propane tank storage area. We will be hosting classes occasionally. We will ask that your equipment be stored under the table for the duration of the class. We offer a discount on the class for monthly shop renters.
