Membership Overview

Á LA CARTE PRICING

SMARTPALATE CREATION & 30-DAYS FREE

MEMBERSHIP TERM

Members and non-members can purchase products and services individually at any time with no limit on quantity. À La Carte Pricing can be seen on the À La Carte Pricing Sheet. Members receive exclusive À La Carte purchasing discounts.

Membership begins on the date that signed membership paperwork is received (Activation Date). Following the initial term, memberships will automatically renew on a month-to-month basis. Members may cancel their membership by filling out the Membership Cancellation Form then returning all DataLinks and accompanying equipment and packaging within 15 days after the 30-day free period. Cancellation fees are equal to 100% of the balance due in the initial 6-month period.

Members will submit a stone model of their upper palate to CompleteSpeech to have their custom SmartPalate made. This model can be made by a dentist or orthodon-tist for a fee generally under $50. CompleteSpeech will ship the finished SmartPalate within 5 business days of receiving a stone model. Accounting for shipping time, the entire startup process from ordering to receiving a finished SmartPalate usually takes 3-4 weeks. Members receive 30 additional days to their membership to account for the startup process. See the “5-Steps Smart-Palate” document for more information.

BILLING

The “Total Due On Activation Date” will be charged on the Activation Date. See order form for Total Due on Activation. Subsequent billing will begin the 1st of the following month regardless of your Activation Date. Each member has a 4-day grace period after which a $2/day late fee will be applied to the membership until the full amount due has been paid. Members may Prepay their membership and get a 10% discount applied only to their membership rate.

MEMBERSHIP UPGRADES

À La Carte purchases can be upgraded to a membership within the 30-day manufacturing period and the payment will be applied towards the membership rate.

SYSTEM COMPONENTS

The SmartPalate System is comprised of the following hardware and software components:

1. SmartPalate: A customized acrylic mouthpiece with sensors that read tongue-to-palate contact.

2. Practice Palate: A customized acrylic mouthpiece without sensors used to acclimate a user to wearing a SmartPalate. 3. DataLink: A processor which syncs palate contact with audio recordings and transmits the information to a PC. The DataLink is held by a lanyard and connects to the PC via a USB cord. 4. SmartPalate Software: An intuitive program that allows users to create articulation targets (tongue targets), monitor feedback, and receive scoring on their produc-tions. Software includes split-screen capability for two simultaneous users for modeling and instruction.

Each Membership comes with a SmartPalate (1), a Practice Palate (1), and a DataLink (1). Each membership comes with a license of SmartPalate Software for a computer (1). Additional licenses can be purchased for an additional monthly fee listed in the À La Carte section.

Special actions must be taken to create SmartPalates for individuals with orthodontia, missing teeth, or whose stone model has abnormalities such asbubbles or extra material that does not reflect the true palate of the individual. CompleteSpeech is not responsible for replacing loose or tight fittingSmartPalates for any of the following situations:

Braces: SmartPalates for individuals with braces are made by trimming the acrylic mouthpiece just below the brackets on the teeth. In most cases, the modified SmartPalate will not have the necessary grip on the teeth to stay firmly in place. If this occurs, the individual will need to use a small amount of denture adhesive to keep the SmartPalate in place. Individuals should consult with their Speech-Language Pathologist and Orthodontist to decide ifusing a SmartPalate should be delayed until after the braces are removed.

Palate Expander:length of time an expander is used, speech can be affected when it is either inserted or removed. Individuals should consult with their Speech-Language Pathologist and Orthodontist to decide if using a SmartPalate should be delayed until after the expander is removed.

Missing/growing in front teeth: SmartPalates require front teeth in order to properly align the sensors on the palate. When an individual is missing his or her front teeth or the front teeth are not fully grown in, SmartPalates are made by building up material as false teeth on the stone model. Thismay affect the fit of the SmartPalate and denture adhesive may be needed to keep the SmartPalate in place.

Other teeth missing/growing in: When teeth (excluding front teeth) are missing or have not fully grown in, an opening will be left in the SmartPalate to allow the tooth to grow. If too much material needs to be removed, denture adhesive may be required to keep the SmartPalate in place.

When teeth are very small or do not have a lot of contour, SmartPalates may not stay in place without the use of denture adhesive.

Stone Models:

Additionally, sharp edges of the model may be sanded down for easier handling.CompleteSpeech is not responsible for replacing SmartPalates due to apoor impression.

Medical adhesive acrylic or polyimide (Kapton) is used to make the SmartPalate mouthpieces. Individuals should not use a SmartPalate if they have known allergies to these materials.

The SmartPalate is a delicate device that requires special care and handling to ensure proper functionality. Users must follow the following guidelines:

CLEAN the SmartPalate mouthpiece gently after each use to prevent buildup of material on the sensors.

DO NOT allow water near the SmartPalate plastic housing cord.

NEVER submerge the SmartPalate housing in water.

NEVER bite, chew, or grind your teeth while wearing the SmartPalate.

NEVER remove the SmartPalate by pulling on the cord or plastic housing.
