RuLES & ELIGIBILITYn All nominators and nominees must
be affiliated with the SQF program. Examples of appropriate affiliations include, but are not limited to registered auditors, consultants or trainers, licensed certification bodies, registered and certified suppliers, licensed training centers, etc.
n All nominees must have shown ongoing and dedicated support for the Safe Quality Food Program during the 2013 and/or 2014 calendar year.
n The entry form must accompany your nomination and all sections must be filled out legibly.
n Narratives must be 1,000 words or less.n Nominators must obtain permission
from the nominee prior to submitting a nomination.
n Self-nominations will be accepted.n Specific evidence and supporting
documentation must be included for all comments and accolades given by the nominator on behalf of the nominee. Examples of supporting documents include published articles, industry awards, news articles and 3rd party testimonials. These materials will not be returned to you. No videos will be accepted. Applications must contain only the allowed number of references.
JudGING CRITERIAn Only complete, legible entries will be
considered.n Entries will be scored and judged by
a panel consisting of a member of the SQFI staff, a past SQF Award winner and an executive from a food retailer.
n Final award decisions will be made by members of Safe Quality Food Institute for each specific award category.
n Finalists will be notified on or before September 1, 2014. All finalists will be reviewed to ensure they are in good standing with SQF and/or their customers.
SCORING CRITERIAEntries will be scored on overall content and the nominee’s demonstrated achievements in:
n Contributing significantly to achieving the SQF Mission.
n Communicating the goals, mission and benefits of the SQF program to both internal and external stakeholders.
n Proving his/her/their understanding and commitment to the SQF program.
n Demonstrating an ongoing personal commitment to improving the awareness of food safety within the global community.
ENTRY SuBMISSIONAll nominations must be submitted in the requested format no later than 12:00 AM EST (USA), July 21, 2014. Email is the preferred method of receipt. Send to Amanda Bond-Thorley at [email protected]. All nominations must include the evidence as outlined. Incomplete submissions or those that do not follow the submission rules will not be evaluated.
PRIzESn We will honor the top finalists from each
category at an awards luncheon on Wednesday, October 29, 2014 during the 2014 SQF International Conference, where award winners (one in each category) will receive a crystal award.
n The award winners will receive complimentary registration to the 2015 SQF Conference.
n All finalists will receive a reduced rate registration to the 2014 SQF Conference.
n The award winners will be recognized in a press release and on the SQF website.
AWARd CATEGORIES ANd NOMINATION CRITERIA
SQFI CertIFICatIon Body (CB) oF the year
Awarded to a licensed and accredited Certification Body that has contributed significantly to achieving the SQF mission of delivering consistent, globally recognized food safety and quality certification programs based on sound scientific principles, consistently applied across all industry sectors, and valued by all stakeholders.
SQFI traInIng Center oF the year
Awarded to a licensed Training Center that has contributed significantly to achieving the SQF mission and supporting the SQF stakeholders in implementing sound food safety and quality practices.
The nomination submission for either these awards must include:
•Anexecutivesummary(1000wordmaximum)thatoutlinesthepractices,cultureandmission of the organization and the contributions to SQF.
•PerformancemetricsmeetingthegoalsandobjectivesofSQF.
•Examplesastohowthenomineeworkswiththeindustrytodrivefoodsafetyandqualityin or along the supply chain.
•Evidenceoftrainingactivitiesfocusingoneither:
a) auditor competency (CB of The Year)
b) implementing food safety and quality along the supply chain (TC of The Year).
•Maximumtwolettersofreferencefrom:
- CB of The Year: SQF certified facilities or buyers in the system
- TC of The Year: SQF certified facilities, consultants or other stakeholders in the system.

SQFI ManuFaCturer/dIStrIButor oF the year
Awarded to a certified supplier that has demonstrated excellence in the SQF system and has promoted SQF.

SQFI PrIMary ProduCer oF the year
Awarded to a certified supplier that focuses on agri-business (primary producer) and has shown a commitment to excellence in the SQF system.
The nomination submission for either these awards must include:
•Anexecutivesummary(1000wordmaximum)thatoutlinesthepractices,cultureandmission of the organization and the contributions to SQF. The executive summary must be from the organization’s leader such as the owner, CEO, President, or the Senior Operations Manager and include information exemplifying continuous improvement and management commitment within the organization.
•Evidencethatthesupplier/producerhasmaintainedSQFcertificationfora minimum of 3 years.
•Examplesofhowthenomineeworkswithintheorganizationtodrivefoodsafety and quality.
•Maximumthreelettersofreferencewithatleasttwolettersfromabuyer.Allreferencesshall include the contact name, address and phone number for verification purposes.
Other considerations may include:
•ActivitiesoutsideSQFpromotingfoodsafety,suchasworkwithGFSI,regulatorybodies,or other stakeholders.
SQFI audItor oF the year
Awarded to an individual who has demonstrated outstanding performance as a credentialed SQF Auditor.
•Anexecutivesummary(1000wordmaximum)thatillustrateshowthenomineedemonstrates leadership, acts as a mentor and generally strives to improve the auditing credential and promotion of auditors.
•Evidenceoftrainingactivitiestopromoteprofessionaldevelopmenttodemonstratecontinuous improvement within the industry.
•Examplesofhowthenomineeworkswithinaswellasoutsidetheorganizationtodrivefood safety and quality.
•Maximumtwolettersofreferencewithonereferencefromacertificationbody. All references shall include the contact name, address and phone number for verification purposes.

SQFI outStandIng aChIeveMent award
This award category is open to all individuals currently, registered, licensed, or certified with SQFI or those who are affiliated with SQFI and have achieved a level of excellence above and beyond their duties in support of food safety and the SQF Program. This category has two submission categories: Industry Achievement and Supplier Achievement.
Industry achievement award
This award is given to an individual who has freely shared his/her time and expertise throughout the industry in the areas of food safety.
The nomination submission must include:
•Anexecutivesummary(1000wordmaximum)thatdescribestheactivitiesthattheindividual has accomplished as a leader, mentor and overall promoter of food safety, quality and SQF.
•Examplesofhowthenomineeparticipatedinoutsideevents.
•Examplesofhowthenomineeworkswiththeindustrytodrivefoodsafetyandqualityinthe supply chain.
•MaximumtwolettersofsupportfromstakeholdersintheSQFsystem.Allreferences shall include the contact name, address and phone number for verification purposes.
Other considerations may include:
•ActivitiesoutsideSQFpromotingfoodsafety,suchasworkwithGFSI,regulatorybodies,or other stakeholders.
Supplier achievement award
This award is given to an individual within a facility or company, who has demonstrated accomplishments in driving continuous growth and has taken their facility or company above and beyond the SQF program.
The nomination submission must include:
•Anexecutivesummary(500-1000)wordsthatdescribestheactivitiesthatthenomineehas accomplished in driving continuous improvement within the organization beyond the SQF Code.
•Examplesofhowthenomineeworkedwithintheirorganizationactingasateacher,trainer, or mentor in support of food safety and the SQF system.
•MaximumtwolettersofsupportfromstakeholdersintheSQFsystem.Allreferences shall include the contact name, address and phone number for verification purposes.
Other considerations may include:
•ActivitiesoutsideSQFpromotingfoodsafety,suchasworkwithGFSI,regulatorybodies,or other stakeholders.
