Mooney Blvd., Visalia, CA 93277 Phone: 559.624.7471 Email: [email protected] Web: www.sptf.org Grief & Bereavement Counseling Voucher Program Project Scope: Fiscal Year 2015/16 The Tulare & Kings Counties Suicide Prevention Task Force (SPTF) has a program targeted at providing services to individuals immediately impacted by suicide loss. This program provides short-term, high intensity, early intervention services to decrease subsequent suicide risk by survivors of suicide loss. The SPTF will identify individuals eligible for vouchers, including LOSS Team members, and will refer them to a participating clinician. This program is designed to be a short-term, low intensity, early intervention to reduce immedi- ate risk for suicide. If additional mental health services are deemed necessary, clinicians will be responsible for making appropriate referrals, and is outside the scope of this program. This program will not provide long-term intervention or treatment. Clinicians receiving a voucher referral shall provide a timely response for treatment at no greater than 30-days from date of referral, and shall make contact within two (2) business days of the date of referral. The coordina- tors of the SPTF, or the Coordinator of the SPTF’s LOSS Team shall be the points of referral and are responsible for authorizing vouchers. Reimbursement for treatment will be at the fixed flat-rate of $70 per 1-hr session. Billing for the program will occur on a monthly cycle, with reimbursement occurring within 2-3 weeks of receipt of invoice. The clinician will provide six to eight, one-hour sessions, to voucher recipients. Requests for additional sessions will be handled on a case-by-case basis. Sessions can be individual, family, or multi-party, based upon the needs expressed by suicide loss survivors and treating clinician’s judgment. This program is on an “available funding” basis only. If funding is exhausted, or not appropriat- ed for a new fiscal year, service providers will be notified immediately, and agree to continue remaining sessions without reimbursement from any party for those that are already authorized and in treatment. Clinicians agree to forego any other form of reimbursement, including by not limited to: direct payment by voucher recipients, insurance, or grant funding. Required current documents: A) Scope of Work (this document on your letterhead) B) License C) Malpractice Insurance D) Curriculum Vitae or Résumé E) Current IRS W-9 Additional requirements: F) The SPTF may identify and require participation in professional training, which will be offered at no cost participating voucher program clinicians. Continued participation in the voucher program may be contingent on completion of identified training. G) Treatment cannot be provided without a fully executed Professional Service Agreement (PSA) with the County of Tulare.
