Health IndicatorsUninsured Residents by Zip Code:

• This map displays the number of uninsured residents (adult and children) in each zip code, based on US Census rates at the county level multiplied by the population of each zip code.

• The Healthy Community is outlined in orange by census tracts.

• Note: this data is extrapolated using county uninsured rates; given the high proportion of low-income residents in this service area, it is likely that this map underestimates the uninsured population.

• This map displays the estimated number of adults diagnosed with diabetes in each zip code, based diabetes rates at the county level multiplied by the adult population of each zip code.

• The Healthy Community is outlined in orange by census tracts.

• Note: this data is extrapolated using county diabetes rates and was not adjusted for age, income or race. It is likely that this map underestimates the adult diabetic population.

Health IndicatorsAdults with no Usual Source of Care, by Zip Code:

• This map displays the estimated number of adults who self-reported that they do not have a regular medical home in each zip code, based on BRFSS rates reported at the county level multiplied by the adult population of each zip code.

• Note: this data is extrapolated using county rates; given the high proportion of low-income residents in this service area, it is likely that this map underestimates the population lacking a medical home.

Resources and Methodology

• Poverty and Low-Income Population: Census American Community Survey (ACS) Estimates, 2006 – 2010 data by zip code, “Ratio of Income to Poverty Level”

• Penetration of Low-Income Population: Census ACS and UDS Mapper (www.udsmapper.org), FQHC patients as a proportion of low-income residents, by zip code.

• Low-Income Population Not Seen by an FQHC: UDS Mapper; mapped by zip code is FQHC patient base by zip code subtracted from low-income population. Look-Alike and “other” patient data not available at zip code level.

• Health Indicators:– Uninsured Population: ACS uninsured population by county for 2011. Rate of the uninsured by county

was multiplied by zip code population to estimate uninsured population by zip code. – Diabetes and Adults Without a Usual Source of Primary Care: Behavioral Risk Factor Surveillance System

(BRFSS) data by County for 2009 was gathered from the California Department of Public Health (CDPH) report “Diabetes in California”. Diabetes and medical home rates at the county level were multiplied by the adult population in each zip code (adult population by zip code obtained from ACS 2007 – 2011 estimates) to estimate the adult population for each indicator.

– Note: Given the high proportion of low-income residents in TCE’s Healthy Communities, data extrapolated using county rates applied to zip code data likely underestimates the uninsured and diabetic populations, as well as those lacking a medical home.

– Low Birth Weight CDPH, Improved Perinatal Outcome Data Reports by County http://ipodr.org/001/vs/2010objectives.html. Data available at zip-code level for 70% of zip codes in the state; county data utilized where zip code data not available.

• Other Map Elements:– FQHC, Look-Alike and “Other Community Clinic Sites”: California Office of Statewide Health Planning and

Development (OSHPD) Annual Utilization Data, 2011 http://www.oshpd.ca.gov/hid/Products/Hospitals/Utilization/PC_SC_Utilization.html
