Review Date: 2/2/17 The purpose of this assessment is to show how well Blackley’s teams are practicing health and safety for assisting in ensuring workers and others affected by our activities are protected to an acceptable level and identifying where improvements can be made to our practices and procedures. Alex Mist Contractors Nil Job Pack and Supporting Information A good standard of information was provided in the Job Pack at the time of job commencement with service plans on site. Tool Box Meeting A site walk over was discussed for cross referencing service plans and identifying street furniture meters etc before excavation work started. A good standard of service mark had been carried out by service providers. A good standard of information was recorded on the Tool Box Meeting Sheet with discussion taking place to manage any potential contamination of the storm water system for spills and when using the concrete saw. Traffic management was in place as per the TMP with the first sign off of the TMP being at 08.30 hrs. Reference to workers wearing full cover with a reminder to work safely inside the cone areas and to watch out for pedestrians. Other health and safety reminders were recorded on PPE and wearing of hard hats when working around the digger. The emergency evacuation point was Russell Street, Alex and Ali were the nominated First Aiders An emergency drill had been carried out on 3/3/15 David and Alex confirmed they were involved in the Tool Box Meeting. The Tool Box Meeting Sheet had not been signed off by the team as Ali had them start work while he completed his write up. The importance of the sign off was discussed. Ali also to ensure that all visitors are run through the Tool Box discussion and if it is their first time on site the Hazard Id Sheet. A very good standard of detail had been recorded. Job Pack and Supporting Information

A good standard of information was provided in the Job Pack at the time of job commencement with service plans on site.

Tool Box Meeting

Daily Plant Inspections

All Plant Daily Inspection and Vehicle Sheets had been completed to a good standard

Site Specific Hazard Identification and Master Register

All appropriate details in relation to site specific hazards had been covered with the following hazards identified:

Noise Dust Underground and overhead services Machinery and loading and unloading Traffic Other people and excavations Moving plant and Vacuum truck

Acknowledgment of plans, mark outs, exposure if significant services, permits and putting in place control measures was also signed off. Powerco had issued a Close Approach Consent of 1 m to overhead power and for excavation near strategic underground cables or equipment. Chorus has issued a permit and stand over certificate. This had not been signed off by Chorus A good effort by Ali and his team

Site Monitoring Requirements

On some of the older fences the mesh had detached and could be easily fixed with zip ties. This problem is being rectified as company fences are being up graded. In the mean time monitor fences to ensure the required standard id being maintained

Traffic Management

All temporary traffic controls were in place with good sandbagging of main signs, use of cones and barriers and fences. Ali is monitoring the site throughout the day and prior to departure at the end of the day Overall a very good standard.

Worker Competency

All workers hold the appropriate licenses and competency, skills and experience for the work being carried out

Tools and Equipment

Only one small plant item, a compactor plate was being prepared ready for use at the time of the visit and was in the process of being checked at part of the Daily Plant Inspection. It was in good condition

Incident Management

No incidents or near miss events have occurred on this project to date

General Comment

Overall this site was well managed by Sean, Shane and Ali, and well supported by David and Alex. The site was clean and tidy and well set out to protect the local Feilding community Three recommendations have been made. These are of a minor nature
