3. Financial aid to a maximum of: $(12,000); Twelve Thousand Dollars

* Receipt of Federal Athlete Assistance Program (AAP) funds will result in an equal reduction in the amount of this awardfrom Simon Fräser University.

4. NCAA regulations restrict the total amount of financial aid a student-athiete can receive. If a student-athlete receives a USfederal grant (refer to NCAA bylaw 15.2.5) or any other scholarship or financial aid (including earnings from a Job during aterm), s/he will notify SFU's Student Financial Aid & Awards Office and the Athletics Department {refer to NCAA bylaw 15.1).These funds may replace a portion of the student-athlete's athletics financial aid in order to satisfy NCAA and Simon FräserUniversity regulations. The approval of any athletic financial aid is subject to final review of the student-athlete's academicrecord.

5. Financial aid will not be increased, reduced or canceled during the period of its award on the basis of athletic ability,Performance or contribution to the team's success, or because of injury or illness that prevents the student-athlete fromparticipating in athletics, or for any other athletic reason. Should an injury or illness preclude participation in athletics, thestudent-athlete will be expected to assist the Athletics Department in other operational activities (e.g. coaching, teammanager).

6. Financial aid mav be reduced or cancelled by Simon Fräser University during the period of its award in the followingcircumstances:

a) The student-athlete renders himself/herself ineligible for intercoilegiate oompetition;b) There is a finding that the student-athlete has fraudulently misrepresented any infomnation on his/her

application for financial aid, application for admission, historical report, or letter of intent;c) The student-athlete voluntarily withdraws from a sport for personal reasons;d) Where an authorized University body recommends the revocation of the award äs part of the discipline

imposed for academic or non-academic misconduct;e) Failure to artend; classes, squad or individual meetings, study hall, assemblies, tutoring or study group

sessions, athletic practice sessions or scheduled contests äs specified by the sport coach;f) Failure to adhere to training rules and regulations;g) There is a determinatiort that the student-athlete has engaged in gambling activities on intercoilegiate

activities which are prohibited by NCAA legislation;h) There is a determination that the student-athlete has engaged in the use, possession, or traffic of an

illegal drug substance, or refused to take a drug test when requested to do so by the NCAA, or bynational or international sport governing bodies.

7. A financial award will be reduced or canceled if the student-athletea) Signs a Professional sports contract for this sport;b) Accepts money for playing in an athletic contest;c) Agrees to be represented by an agent and accepts money; ord) receives other financial aid that results in his/her exceeding the maximum allowable limits for financial

support.

8. To receive any athletic financial aid, a student-athlete must be a Student in good academic standing and meet NCAAacademic eiigibility Standards.

9. Athletic financial aid shall be granted for the period indicated in this Agreement and may be renewed at the discretion ofSimon Fräser University. It will not cover the summer Semester.

10. Termination of athletic financial aid during the period of its award can be made only with prior notice to the student-athlete, who has a right of appeal to the Director Financial Aid & Awards and the Faculty Athletic Representative.

11. Student-Athletes may appeal any actions taken with respect to the terms and conditions of the student-athlete Agreementusing the process set out in Appendix B.

"Consent to Release Student-Athlete Personal Information"

With respect to any bursary, scholarship, award, or other financial assistance l may receive äs a resultof my Status äs a student-athlete, l hereby consent to the disclosure of certain personal Information tointerested parties. These interested parties may include University academic faculties and departments,University administrative departments, award donors, provinciai or other government funding agencies,athletic associations and unions, and external media sources. Such personal Information may also beread äs part of an awards ceremony. The specific personal Information of mine that may be disclosedwill be limited to my name, award, award amount, GPA, academic program or major, team affiliation,athletic or academic accomplishments, and extracurricular interests and activities. IMPORTANT - READ CAREFULLY

It is important to read this entire document before signing it. One copy is to be retained by you and the other copy is to be returned tothe Institution, which will file a copy with the GNAC Conference Office. Copies transmitted by facsimile or electronically areconsidered to be valid. The GNAC NLI is a voluntary program wtth regard to both institutions and prospective student-athletes. Noprospective student-athlete or parent is required to sign the GNAC NLI for a prospective student-athlete to receive athletics aid andparticipate in intercollegiate athletics.

1. Initial Enrollment in Four-Year Institution. This GNAC NLI applies only to prospective student-athletes who will beentering four-year institutions for the first time äs full-time students at a GNAC member Institution. It is also permissible for 4-2-4 transfer student-athletes who are graduating from a two-year College to sign the GNAC NLI. No prospective student-athleteenroliing at midyear shall sign a GNAC NLI, with the exception of midyear two-year College transfer student-athletes infootball, who must graduate at midyear for this GNAC NLI to be valid.

2. Financial Aid Requirement At the time I sign this GNAC NLI, I must receive a written offer of athletics fmancial aid for theentire 2011-12 academic year frorn the Institution named in this document. The offer must list the terms, conditions and amountof the athletics aid award. (A midyear football two-year College transfer student-athlete must receive a written offer of athleticsfinancia] aid for the remainder of the 2010-11 academic year. If the Institution does not renew the athletics aid for the followingacademic year, the student-athlete must be released of the GNAC NLI). In order for this GNAC NLI to be valid, my parent/legalguardian and I must sign the GNAC NLI and I must also sign the offer of athletics aid (see institutional policy for parent/legalguardian signature) prior to Submission to the institution named in this document and any other stated conditions must also bemet. If the conditions stated on the financial aid offer are not met, this GNAC NLI shall be declared null and void.

3. Provisions of Letter Satisfied.a. One-Year Attendance Requirement. The terms of this GNAC NLI shall be satisfied if I attend the GNAC member

institution named in this document for one academic year (two Semesters or three quarters) äs a full-time Student.

b. Two-Year College Graduation. After signing this GNAC NLI while in high school or during my first year of full-timeenroliment at a two-year College, the terms of this GNAC NLI will be satisfied if I graduate from the two-year College.

4. Basic Penalty. I understand that if I do not attend the institution named in this document for one füll academic year and enroll inanother GNAC member institution, I may not compete in intercollegiate athletics until l have corapleted one füll academic yearin residence at the latter institution. Further, I understand l shall be charged with the loss of one season of intercollegiateathletics cornpetition in all Sports sponsored by GNAC member institutions. This is in addition to any seasons of competitionexpended at any institution.

5. Early Signing Period Penaities. Prospective student-athletes who will participate in footbail are prohibited ftom signing aGNAC NLI during the early signing period. A Student who signs a GNAC NLI during the early period in a sport other thanfootball will be ineligible for practice and cornpetition in footbail during the first year of enroliment at a GNAC member

Institution and shall forfeit one season of competition in football. In circumstances where a student's primary sport is notfootball, but the Student anticipates participating in football, the Student should delay signing a GNAC NLI until either thefootball signing period or during the regulär signing period for all other sports.

6. Release Request and Appeal Process. In the event I wish to be released from my GNAC NLI Obligation, the prospectivestudent-athlete may request a release to the GNAC member Institution. I understand that the GNAC FAR Committee has beenauthorized to issue interpretations, settle disputes and consider petitions for complete release from the provisions of the GNACNLI when extenuating circumstances are determined to exist and the GNAC signing Institution denies my request for release. Ifurther understand that the GNAC FAR Committee's decision shall be final and binding.

7, Letter Beeomes Null and Vold. This GNAC NLI shall be declared null and void if any of the following occur:

a. Admissions Requirement. This GNAC NLI shall be declared null and void if the institution named in this documentnotifies me in writing that I have been denied admission or, by the opening day of classes in fall 2011, has failed toprovide me with written notice of admission, provided I have submitted a complete admission application. It is myObligation to provide, by request, my academic records and an application for admission to the signing institution. If I failto submit the necessary academic credentials and/or application to determine an admission decision prior to September l,the GNAC NLI shall be declared null and void. If discovered I purposely failed to provide all necessary academiccredentials to the institution, the GNAC NLI Office per its review may determine the NLI remain binding.

This GNAC NLI shall be rendered null and void if I am eligible for admission, but the institution named in this documentdefers admission to a subsequent term. However, this GNAC NLI remains binding if I defer my admission,

b. Eligibility Requirements. This GNAC NLI shall be declared null and void if, by the opening day of classes in fall 2011,1have not met (1) the NCAA initial eligibility requirements; (2) the NCAA, Conference or institution's requirements forfinancial aid to student-athletes; or (3) the two-year College transfer requirements, provided I have submitted all necessarydocuments for eligibility determination.

(1) This GNAC NLI shall be rendered null and void if I become a nonqualifier (per Bylaw 14.3). This GNAC NLIremains valid if I am a partial qualifier per NCAA Division II Bylaw 14.3.2.1 unless I do not meet the institution'spolicies for receipt of athletics aid.

(2) It is my Obligation to register with and provide information to the NCAA Eligibility Center. If I fail to submit thenecessary documentation for an initial-eiigibiüty decision and have not attended classes at the signing institution, theGNAC NLI shall be declared null and void. If discovered I purposely failed to provide all necessary information to theNCAA Eligibility Center, the Conference office per its review may determine the GNAC NLI remain binding,

(3) This GNAC NLI shall be rendered null and void if I am a midyear football two-year College transfer and I fail tograduate rrom two-year College at midyear (only for Division I nonqualifier). The GNAC NLI remains binding for thefollowing fall term if I graduated, was eiigible for admission and financial aid and met the two-year College transferrequirements for competition for the winter or spring term, but chose to delay my admission.

c. One-Year Absence. This GNAC NLI shall be declared null and void if I have not attended any institution (two-year orfour-year) for at least one academic year, provided my request for athletics financial aid for a subsequent fall term is deniedby the signing institution. I may still apply this provision if I initially enrolled in a GNAC member institution but have beenabsent for at least one academic year. To apply this provision, I must file with the appropriate Conference office a Statementfrom the director of athletics at the institution named in this document that such athletics financial aid will not be availablefor the requested fall term.

d. Service in the U.S. Armed Forces/Church Mission. This GNAC NLI shall be declared null and void after serving activeduty with the armed forces of the United States or an offlcial church mission for at least 12 months,

e. Discontinued Sport. This GNAC NLI shall be declared null and void if the institution named in the document discontinuesmy sport.

f. Recruiting Rules Violation. If eligibility reinstatement by the NCAA student-athlete reinstatement staff is necessary dueto NCAA and/or Conference recruiting rules violations, the institution must notify me that I have an Option to have theGNAC NLI declared null and void due to the rules violation. It is my decision to have the GNAC NLI remain valid or tohave the GNAC NLI declared null and void, permitting me to be recruited and aot be subject to GNAC NLI Conferencerules and regulation penalties.

8. Recruiting Ban After Signing. I understand all participating Conference institutions are obligated to respect my signing andshall cease to recruit me after my signing this GNAC NLI. I shall notify any recruiter who contacts me that I have signed a
