Job-Specific Safety Orientation Checklist

Use this checklist to document training to be completed by all workers with their supervisors.

This document applies to:

new and young workers,

workers returning after a lengthy absence,

workers who are relocated and require training on the hazards of the new work location, &

workers who require additional training when a change in their current workplace createsnew hazards.

Training Requirements

EHS offers an online SFU Safety orientation as well as several other general safety training

courses (some online, some in-person) for SFU employees. In addition, EHS offers online and in-

person safety training for laboratory workers.

Employees must complete all training sessions that are relevant to their positions. Use the table

below to document general safety training to be completed by the employee. For lab safety

training, use Appendix A.

Emergency procedures I have been advised of the emergency situations that could occur in my work location and the procedures to follow in the event of an emergency evacuation. I know the location of the assembly area for my building, the closest fire pull station and fire extinguisher, emergency eye wash and shower and exit routes.

g Safety program and procedures I have been oriented to SFU’s Occupational Health & Safety Policy and the departmental safety program elements and procedures.

h Local Safety Committee Members or Worker Safety Representative I have been advised of the local joint health & safety committee that covers my work area and how to contact them.

Working alone or in isolation (if applicable) I have been trained on the policies and procedures to be followed for working alone or in isolation. If I am required to work alone or in isolation then the procedures for monitoring and summoning assistance were fully explained to me and documented.

Prevention of Violence in the workplace (if applicable) I have been advised of the potential risk for violence or threat of violence in the workplace (e.g., from clients or the public). I have been trained on the procedures to follow in minimizing or eliminating the risk and in responding to an incident. I was informed of the University services available to reduce the risks such as the Safe Walk program and emergency blue light phones.
