September and October have provided me with an opportu-
nity to attend several affiliate meetings in both Dade and
Broward. I must comment on the overwhelming attendance
at all of the meetings thus far. It was very reassuring to see
both the professional and social interactions that transpired.
It is a mystery to me to think that we could have ever ex-
pected social media to even come close to mimicking the
human interaction that I witnessed at the those meetings.
They foster strength in our profession and must be preserved
at all cost. I begin by congratulating all of you who took the
time to make it out to the local meetings.
For years, I have heard many local dentists state that they feel
they cannot participate in organized dentistry because they
are too busy, or because the dues are too much, or because
they are satisfied by simply obtaining their continuing edu-
cation requirementsfrom a local study club, online course or
specialty organization. These same individuals are the same
ones I hear worrying about the patient flow or the cost of mar-
keting their practices. So much time, money and effort is de-
voted to marketing and promoting our small practices. Well,
I pose a very simple question to you. How many members of
the public at large will remember or recognize your practice,
your study club, or even your specialty organization? I would
venture to say that your average Joe (yes, that includes the
Joe you would love to have as a new patient) has no clue or
interest in “X Study Club” or even “Y specialty organiza-
tion.” These groups play a role in our professional lives, but
not a comprehensive one.
What is taken for granted is the fact that most of our patients,
or would-be patients, do recognize the American Dental As-
sociation. The ADA is the subject of news and other media
releases constantly. Many people purchase gum, toothpaste,
floss, mouth rinses and toothbrushes of specific brands be-
cause of the marketing professionals that have cashed in on
a concept we tend to overlook. The ADA Seal of Approval
and affiliation carries clout in the public eye. Therefore, aside
from the numerous product and service discounts, business
resources, library resources, informational podcasts and we-
binars that the ADA has to offer, the largest benefit our mem-
bership provides is the prestige of being associated with
American Dental Association, a name well respected and well
recognized by the public nationwide.*
With this, I conclude by explaining that this is why I renew
my membership without any second thoughts. I truly value
the relationships I have built, particularly during my
leadership years. Many thanks to all of you for being a mem-
ber of this tripartite, for it is
worth every penny!

Rich’s Ramblings
Introduction:This article represents an updated version of a very similar one I
had written and published in this newsletter 10 years ago, when I
was SFDDA president, under the original title, “28 Things I have
Learned in 50 Years of Living and 17 Years of Practicing Den-
tistry.”
I must give credit for the idea and inspiration in writing the first
rendition – to the well known nationally syndicated Miami Herald
humor columnist, Dave Barry, and a column he had written entitled,
“25 Things I have learned in 50 Years.”
In any case, I am now ten years older, and have practiced dentistry
10 years longer. Some of the items presented are the same or similar
to what appeared previously, and some are new things I have
learned or realized in my life, which I wanted to share with you in
this updated version. Similar to the format of the original article, I
divided the things I have learned into those applying to dentistry,
and the rest, those applying to everyday life.
First, the Dentistry Things…
1. If something can go wrong (whether a crown not fitting, an endo
file separating, a tooth breaking, excessive bleeding, etc.), it can
and will go wrong, and usually at the worst possible moment. In
other words, I am 100 percent convinced that Murphy was actually
a dentist.
2. Patients with multiple tattoos and body piercings are invariably
also the ones who are deathly afraid of our local anesthesia.
3. Despite what dental marketing reps and/or our pocketbooks may
tell us: A bone graft is not required for every, or even most, extrac-
tion sites. A 3-D CT scan is not required for every, or even most,
dental implants, locating of impacted teeth, orthodontic treatment,
nor a myriad of other dental procedures.
4. In South Florida, “Sue” is not so much a name – it is most often
a verb.
5. For some unexplained reason, many dentists are all too happy to
speak negatively of other colleagues and criticize their work in front
of patients and others. This may also result in hearing the name, I
mean verb, “Sue.”
6. The mere mention of the phrase, “dry socket,” or question raised
by a patient about the possibility, dramatically increases the likeli-
hood of that condition occurring.
7. Treating a close friend or relative dramatically increases the like-
lihood of a complication, or of a procedure not working as intended.
8. Patients who show up twenty or thirty minutes late for a large
and complex dental appointment are also invariably the ones who
also decide to ask a thousand and one last minute questions, often
with multiple unexpected family members accompanying them,
which clearly could have been asked days or weeks prior to the ap-
pointment.
9. There is apparently a disease known as “Mafunzalo” (which I
have since learned is also an entity defined in the Urban Diction-
ary), also known as “Schleuger’s Disease, in which a patient has
“lots of disease/dental problems, but suffers from a severe lack of
money.” There is also a condition whereby one is affected by “lots
of money, but no disease.” However, I am unaware of the official
name of that condition.
10. There exists a “Law of Inverse Proportion As Related to Dental
Implants” - Patients with a lot of bone have a low likelihood of
wanting dental implants, and those with little or no bone want lots
of implants.
11. There are a certain percentage of patients (happily a small per-
centage) who, for some unknown reason, take great pleasure in,
and in some cases make a career out of, coming into our offices
and making life for ourselves and our staff as miserable and horri-
fying as possible. I have also developed a subtle method of tactfully
inducing such patents to consider leaving the practice and going
elsewhere – and that would be a special instrument tray set-up con-
sisting of a Black and Decker drill, 3/4 inch auger bit, hack-saw,
vice-grip and other instruments purchased at Home Depot.
12. Research studies have confirmed that the percentage of suc-
cessful mandibular blocks one experiences in a given week is di-
rectly proportional to the number of hours spent in church, temple,
or other houses of worship during the preceding week.
13. The volume of Lidocaine (or Septocaine) with epinephrine re-
quired in order to achieve a satisfactory level of local anesthesia
for a given patient is almost always directly proportional to the ex-

tent of that patient’s underlying cardiovascular
disease and other significant medical prob-
lems.
14. Patients who choose to ignore our previous
recommendations for significant and complex
dental care are inevitably the same ones who
show up, after many months or years of delay,
insisting they be worked into our hectic sched-
ules at the last minute just prior to departing
on a cruise, leaving for camp or college, going
overseas to study, or join the Peace Corps, and
the list goes on…, but would never consider
appointing weeks earlier for treatment to be
performed in a more controlled and relaxed
manner.
15. Patients who experience serious medical
problems and require the services of a hospital
emergency room, whether for reasons of chest
pain, stroke, TIA, syncope, or near fatal car-
diac arrest, will be virtually guaranteed of hav-
ing at least one, and perhaps more than one,
ER personnel ask the patient and/or their fam-
ily the following question: “Did you by any
chance have dental treatment within the past
few days, or weeks (or even months)?” And if
so, “What [in heaven’s name] did the dentist
do, or given you??” In these situations, the
name, I mean verb, “Sue,” may again be
thought of or mentioned.
And Now, General Life Things…
16. No man’s life, liberty, security or basic
human rights are safe as long as the legislature
is in session.
17. When a piece of mail arrives with big,
bold, capital letters imprinted on the front of
the envelope exclaiming, “IMPORTANT, UR-
GENT, MUST OPEN IMMEDIATELY!!” –
it is almost assuredly unimportant, and a piece
of junk mail which can be immediately dis-
carded.

18. “If you don’t have anything nice to say
about someone, then don’t say it.”
19. Studies have isolated the major underlying
cause of time wasted (in man-hours), money
wasted (totaling more than billions annually),
other resources wasted, and virtually explains
why we as a society accomplish very little and
will never live up to our potential or solve our
nation’s problems. And that underlying cause
is - “meetings.”
20. If a man talks nasty to a woman, it’s called
sexual harassment. But for a woman to talk
nasty to a man… it costs $3.99 per minute.
21. It is virtually impossible in today’s day and
age to pick up the phone, call a place of busi-
ness and actually reach a live person. Instead,
the only option available in many or most
cases is a useless voice mail with an endless
series of number or voice activated prompts
that leads us virtually nowhere, usually result-
ing in zero help, and bearing little or no rela-
tion to the original reason for calling.
22. The single greatest mistake in the history
of man’s existence on this planet consisted of
Noah inviting onto the Ark, and then failing to
swat - the two mosquitoes.
23. The news media will occasionally report a
story of a convenience store clerk robbed at
gunpoint and shot to death, and then often
adding (something to the effect of), “… and
all the robber got away with was thirty two
dollars,” or some other very small amount.
The unfortunate, sad, and eerily strange effect
of this, I feel, is the fact that it somehow im-
plies to us, the news audience, that this tragic
and senseless murder would have been more
justified, or more “worth it,” had the perpetra-
tor come away with a much higher dollar
amount.
24. You should not confuse your career with
your life.
25. To this day, no one has ever been able to
give me a clear and compelling reason why we
observe Daylight Savings Time.
26. To this day, no one has ever been able to
give me a clear and compelling reason why
Kamikaze pilots wore helmets.

27. They can hold all the peace talks they
want, but there will never be peace in the Mid-
dle East. Billions of years from now, when
Earth is hurtling toward the Sun and there is
nothing left alive on the planet except a few
microorganisms, the microorganisms living in
the Middle East will be bitter enemies.

28. Despite all of our pathetic whining, com-
plaining, and moaning on a daily basis about
what we do, all of us are extremely fortunate
to be a part of dentistry - one of the greatest
professions in the world.

Dr. Harold Menchel limits his private practice to treatment of TMD andorofacial pain in Coral Springs.
TMD Headache Neuropathic pain Sleep disordered breathing (OSA) Dr. Menchel coordinates treatment with restorative dentists,orthodontists, endodontists, and oral surgeons for these complexpatients.
Dr. Menchel has been in practice in S. Florida since 1981. He received themajority of his training at the University of Florida Parker Mahan FacialPain Center under the tutelage of Drs. Mahan and Gremillion from 19921999. He achieved the prestigious Diplomate of the American Board ofOrofacial Pain in 2000.
Treatment includes: (partial list) Splint therapy, medical management, physical therapy,joint mobilization, diagnostic and therapeutic injections.

This four(4) hour course meets the requirements of the Florida Dental Pract Act, effective November 1, 1986, which provides mandatory current CPR training for all dentists and hygienists. Course completion offers two (2) years certification. Continuing education credits apply. As part of the CPR training at the Basic Life Support level, this course provides training in the use of either an Automated External Defibrillator or a Defibrillator and Electrocardiograph.
All classes listed above are at the SFDDA office in Coral Gables. Class size is limited and pre-registration is required. Theprice is $50 per person. Registrants will be sent driving and parking instructions. Dress code is full length pants and shirts to practice one and two-person CPR with manikins on the floor. Participants must be in good health and able to do strenuous activity. If a disability exists, please notify the SFDDA office and you must speak to the instructor before class starts. Cancellations with 24 hours prior notice of the class will be refunded - less $10 administration fees. No-shows and cancellations with less than a 24 hours notice will not be refunded, but may be rescheduled. No telephone reservations are accepted.
