Toledo Bend Project – Part 12D Section 7 Safety Inspection Report Assessment of STI Document Rev. 0a Page 7-1 03/31/2005 SECTION 7 ASSESSMENT OF SUPPORTING TECHNICAL INFORMATION DOCUMENT 7.1 POTENTIAL FAILURE MODE ANALYSIS STUDY REPORT The PFMA Report was reviewed and found to be adequate. It is included in the final submitted form in the accompanying volume of the Supporting Technical Information (STI). 7.2 PROJECT DESCRIPTION AND DRAWINGS The Project Description, Section 2.0 of the STI was reviewed and found to be adequate. 7.3 CONSTRUCTION HISTORY A summary of the construction history of the Dam appears in Section 3.0 of the STI. Unfortunately design specifications, the original material testing reports, and the plans for the Powerhouse could not be located. The remainder was reviewed and found to be adequate. 7.4 STANDARD OPERATING PROCEDURES A summary of the standard operating procedures for TBPJO appears in Section 4.0 of the STI. It was reviewed and found to be adequate. In fact the operation procedures have been reviewed and found to be appropriate for flood releases as a result of the collaborative process and the litigation that ensued after the last major flood release in 1999. 7.5 ASSESSMENT OF GEOLOGY AND SEISMICITY A summary of the geology and seismicity of the TBPJO appears in Section 5.0 of the STI. It was reviewed and found to be adequate. 7.6 ASSESSMENT OF HYDROLOGY AND HYDRAULICS A summary of the hydrology and hydraulics for TBPJO appears in Section 6.0 of the STI. It was reviewed and found to be adequate. 7.7 ASSESSMENT OF INSTRUMENTATION A summary of the instrumentation in place at the TBPJO appears in Section 7.0 of the STI. It was reviewed and found to be adequate with the following exceptions. Some of the instrumentation data is not available either in past records or in the Damsmart database. It is assumed that various instruments have been abandoned and monitoring was suspended over the years for various reasons. Some of these reasons may include lack of response from the instrument, instrument malfunction, no need to continue collecting that particular type of data, etc.

SECTION 7 ASSESSMENT OF SUPPORTING TECHNICAL INFORMATION DOCUMENT

Unfortunately it is difficult to trace why an instrument was initially installed but is no longer monitored through the history of the project.

The STI guidelines have also called for instrumentation threshold levels or action levels to be defined. No information was found to indicate that any formal calculations have been performed to this end. These threshold levels should be determined through a comprehensive analysis, which was not a part of the scope for this inspection.

It would be useful to have a detailed mapping of the location of the crack gages. The format used to collect that information appears to be by eye and leads one to questions its accuracy.

7.8 ASSESSMENT OF STABILITY AND STRESS ANALYSES OF PROJECT STRUCTURES

A summary of the stability and stress analysis of the TBPJO structures appears in Section 8.0 of the STI. It was reviewed and found to be adequate.

7.9 ASSESSMENT OF SPILLWAY GATES A summary of the technical information related to the spillway gates appears in Section 9.0 of the STI. It was reviewed and found to be adequate. However, the STI document requests motor load test data including amperage draw, line-line voltage, etc. This information is not collected by TBPJO staff currently.

7.10 PERTINENT CORRESPONDENCE RELATED TO SAFETY OF PROJECT WORKS

A summary of the pertinent correspondence related to the TBPJO appears in Section 10.0 of the STI. Some correspondence is missing, but it appears the majority is included. It was reviewed and found to be adequate.

7.11 STATUS OF STUDIES IN PROCESS AND OUTSTANDING ISSUES The inspection team was not aware of any studies in process or any outstanding issues that were not being addressed.

7.12 REFERENCES The references section appears to be complete.

7.13 CONCLUSIONS Section is adequate
