Notes: Safety Data Sheet (SDS) for products used must be attached to the statement. The product must be part of the controls so it is risk assessed. The SDS sheet must be within a 5 year period. All plant must have a maintenance schedule, an operator’s manual to be on the machine & the operator appropriately certified. The required operator’s license type must be listed in the

control measures. The SWMS must be monitored at a predetermined period to confirm that the work is being carried out as described in the statement by using the attached check sheet, defects or non-

4 4 16 The position of the shadow vehicle must be the following: Directly behind the 40km/h sign if there is no workers in the job site. Used as a “safety buffer” between the workers and the live traffic. Example: Where the taper starts there will be the shadow vehicle placed 30m after the start. A further 30m distance between the workers and the “safety buffer” vehicle.
