project,

2.0 General Scope of Work

Work tasks will generally involve the removal of undersized culvert, replacement with open bottom arch

as described in plans meeting all requirements of the AOP following the attached plans. (Attachment A)

Construction will take place during dry weather and low flow prior to the deadline stipulated in theStream Alteration General Permit. Work hours and time frame will be agreed upon by the Contractor,

Project Coordination Team as reference in Section 3 prior to construction. All disturbed areas shall be

returned to pre-construction conditions and additional habitat stabilization implemented. Disturbance

shall be minimized during construction.

3.0 Owner's Project Representation

Oversite of the project will be conducted by the Project Coordination Team consisting of, US ForestService, US Fish and Wildlife Service, The Friends of the Mad River and the Town of Warren staff. The

Contractor will be obligated to comply with directives from the Project Coordination Team to ensurethat the all contract provisions, design specifications, and permit requirements are met. Team will assign

a point of contact to streamline this communication.

4.0 Sequence of Work

The final sequence of work will ultimately be determined by the Contractor and submitted to the ProjectCoordination Team for review and approval prior to Construction.

Technical specifications are included in the details and notes on the construction plans. Contractor will

be obligated to adhere to the Terms and Conditions in the Contract to be used for this project.

The project will be conducted in a manner which minimizes or avoids any discharge of sediment or otherpollutants to surface waters.

An onsite pre-construction conference is to be held with the awarded contractor and representatives

from the Project Coordination Team and permitting agencies.

In-Stream working dates for all General Stream Alteration Permit activities are through October 1st. Any

in-stream work outside these dates will require consultation with and prior authorized from the River

Management Engineer.

6.0 Sediment and Erosion Control

Work will need to comply with sediment and erosion control measures as detailed in the plans by

Milone & MacBroom.

7.0 Construction Notes

Construction notes are contained in the Plans.

8.0 Methods

Methods will be agreed on between the Contractor and Project Coordinator Team.

9.0 Construction Schedule

All project elements are to be completed by the deadline outlined within the AIMR Stream Alternationand Army Core Permit. Construction is planned for August or September 2019. All work must be

completed by October 1, 2019. A notice to Proceed will be issue by the Project Engineer prior to thestart of the work. All excavated fill/ pavement, guardrails, concrete and bedrock are to be trucked to

the Warren Town Pit.

10.0 Regulatory Requirement

Federal/ state and local permitting has been received for this project and conditions of each permit mustbe followed during construction. The Contractor must review permits in full to understand all regulatory

requirements prior to construction. All permits must be posted at the project site prior to the start of

Construction. .DAVIS BACON WAGE REQUIREMENTS: Bidders agree to abide by the Davis Bacon Wage

Rate Schedule. Current wage rates can be obtained from the AOT web site.

BUY AMERICA REQUIREMENTS: Buy America requirements of 23 CFR 635.410 are applicable to allFederal-aid construction projects. All steel or iron products permanently incorporated into Federal-aid

projects, shall be products that have been entirely manufactured within the United States. Allmanufacturing processes of the steel or iron material/ in a product, must occur within the United States

to be considered of domestic origin. This includes process such as rolling, extruding, machining, bending,

grinding/ and drilling. The action of applying a coating to a material is deemed a manufacturing processsubject to Buy America. Coating includes epoxy coating, galvanizing, painting, and any other coating that

protects or enhances the value of the material. This requirement does not prevent a minimal use of

foreign materials, provided the cost of foreign materials used does not exceed 0.1 percent of the total

Contract price or $2,500, whichever is greater. The cost of foreign steel or iron is defined as its value

delivered to the project.

11.0 Compensation

The construction contract will be between the selected Contractor and the Town of Warren. Payment

for the project will be made in two installments" One after 50% completion and one after projectcompletion and a final site inspection with the Project team. Prior approval is needed from the Project

Coordination Team for all change orders. The invoice will be submitted to the Town of Warren for

review and payment.

Note: This project is being funded thought multiple grants already awarded. If project is not completedby October 1, 2019, a fine of a $l/000/day will apply.

12.0 Bid Submittal Information & Format

The engineers' estimate the work is between $300,000 and $350/000 with a temporary bridge. Biddersmust be listed on the VTrans "Prequalified Contractor's List". Bids submitted from bidders who are not

on the VTrans "Prequalified Contractor's List" or who cannot provide written proof of prequalification

fromVTrans prior to the time receiving the bid will be deemed unresponsive and returned. We also

request that you cite experience with AOP projects in your bid documents. Please use the following bidform for submitting your bids.

Note on Add Alternative: Temporary Bridge - The Town will be weighing the option of closing the road

for 30 days vs a temporary bridge option. It is unknown at this time.

Bids and qualifications shall be presented on organizations official letter head and attached biddocument and shall include project reference for past work of a similar nature. Bid proposal must be

received by April 15, 2019, electronically by The Town of Warren, Town Administrator/ Cindi Jones at:[email protected] Notice of contract award will be by April 23, 2019. The Town of Warren and its

partners reserves the right to reject any or all bids on its own motion.

Electronic copies of the construction plans are attached to this bid request. Bidders will have until

March 22, 2019 to submit questions via email to both cjonesQwarrenvt.ore and
