Final – and Very Important
When state scores are released, the scores can be imported so that each student’s record displays end-of-year scores on the same screen with all the Teachers’ Interventions and all the Administrative Interventions.
Scores can be analyzed by frequency of needed interventions/subject
This is the Main Menu. Teachers will select the FIRST button for entering data on students. Each entry will be for an INDIVIDUAL student.
At the Classroom Teachers Data Entry Menu, the two buttons labeled “Record Teacher Interventions for Individual Student” and “Create Office Referral” take you to screens where you enter data.
The two buttons labeled “View Teacher Interventions for Individual Students” and “Look up Office Referral” take you to screens where you view information previously entered.
The last two buttons take you away from this menu.
To begin recording an intervention for a student, press the first menu button: Record Teacher Interventions for Individual Student
A Find and Replace box will be on the screen. Type the last name of the student for whom you are looking. Records will be displayed. Press the Find Next button until the desired student’s record is displayed. Check the student demographics to make sure you have the correct student. When you have the correct student, select the X or the Cancel button to remove the information box.
To add an intervention, click into the row where the * appears. This signifies “new record”. When the “Approximate Date” field is activated (the cursor is in that block), a small calendar will appear to the right. Select the date you want.
Most of the fields have pull-down choices. For pure data, select from the list. (lists may be customized)Enter, tab, or the mouse will get you to the next field. Comments fields are free-text. The scroll bar moves the window.
When you are finished, press the “Return to Main Menu” button.
“View Teacher Interventions for Individual Students “ menu item works the same way as the first button, “Record Teacher Interventions for Individual Students”. The difference is that you will utilize the scroll bar under the interventions to view interventions that have been put into place for the student. Data will NOT be entered into this screen.
When you need to alert an administrator to a student’s need, you will “Create Office Referral”
Be sure to read the screen for information needed. Pay special attention to the choices labeled in RED.
When you use the “pull-down” box for Student Look Up, you will be able to find the grade, last, and first name of the student you need to refer. If you need the 7th grade list, press the 7 key on the keyboard to quickly get to the 7th grade list. The same applies for the 8th grade list.
When selected, all the student demographics fill in. You cannot edit any student information.
The date of the Behavioral Offense defaults to “today”. This can be changed.
Select Either MINOR or STATE REPORTABLE Offense. Use the up/down arrows in the field to scroll through the choices. When you click your selection, the selected option will be turn black, and the selection will pop into the “Behavior” field.Use the pull-down menus to fill in the other “Information Needed for Referral”.
“Additional Information” is free-text.
In the “Teacher Interventions Before Submitting Referral” box, you may select as many as have been used. All the selections show up in the field.
Upon completion, select one of the buttons for your next action. “Submit and Return to Menu” or “Create an additional Referral”.

“Look up Office Referral” is to view the remarks made by the administrators. The process works almost exactly like “View Teacher Interventions for Individual Students”.
The last two buttons move you away from this menu.
Type in the last name of the student. Be sure to check demographics for correct student. Cancel the “Find and Replace” box. Use the scroll bar to move within referral section. Use the “Return to Menu” button to leave this screen.
Note: The binocular button will bring up the “Find” box.
This is the Main Menu. Team Teachers or Reporting Teachers will select the SECOND button for entering and reporting data on students. Each entry will be for an INDIVIDUAL student.
You will use the first two buttons to perform a reporting task. The last two buttons will move you away from this menu.
Add Student to DatabaseYou will only need to use this feature if a new student moves into the team. All other students should be in the database.
Print Teacher Interventions for Individual Student – Student ID# RequiredWhen meeting with student/parent, a report can be printed displaying all the teacher interventions put into place for an individual student. Student ID# can be found on the Teacher Intervention Screen for that student’s record.
Each field will hold a different piece of information. Carefully read the message in red before entering data. It is standard practice to use upper/lower case for names. Example: Adam Gates Do not forget to put a check mark in IEP or SST if needed.
When printing a report displaying Teacher Interventions for an Individual Student, you are required to type Student ID Number. This can be found on the student’s record when Viewing Teacher Interventions. You may simply copy and paste the number.
To Print this report, click the button with the Printer picture. This action brings up a Print Dialogue Box.
When finished, the Home button takes you back to the menu.
Some schools have customized the database to include the feature of looking up Homeroom/Advisement students to monitor Teacher Interventions for each student. There are two views – Form View and Chart View. The form is more detailed, but the chart paints a good picture for the homeroom teacher.
Type in the Homeroom Teacher’s Name as it appears in the database.
Use the arrows to scroll through students
As you scroll through, the top part of the screen corresponds to the table view on the bottom section of the screen.The “close form” button at the top of the screen takes you back to the menu.
