The 19489 square metre Sturgeon General Hospital, St. Albert, was constructed in 1992 with two stories and a basement. The facility is a community general hospital providing a full range of programs and services. There are currently 131 inpatient beds in operation distributed as follows: 56 medical inpatient beds on Nursing Unit 17 (24 beds) and Nursing Unit 18 (32 beds) 42 surgical inpatient beds on Nursing Unit 27 (22 beds) and Nursing Unit 28 (20 beds) 4 intensive care beds 10 cardiac care beds 17 women's health beds including labour delivery rooms on Nursing Unit 25 2 pediatric beds The second floor surgical suite with six operating rooms arranged around a central work area and adjacent to the recovery room; the main floor diagnostic imaging which has four X-ray rooms, CAT scanner, two ultrasound rooms and film storage; the main floor medical laboratory; the main floor rehabilitation with open gym area and separate treatment cubicles with plinths in space shared with the intravenous therapy program; the main floor out patient department with treatment and procedure rooms; main floor medical records with doctors dictation room; second floor cardiology clinic. The support departments are: administration, doctors overnight suite; central sterilizing department on the basement level connected to the operating suite on the second floor by means of dumb waiters for clean and a dirty supplies; dietary department for food preparation in the basement supporting the cafeteria and inpatient units; housekeeping department with rooms located throughout the facility; facility management and physical plant in the basement. the new 4620 m2 emergency addition for Emergency spaces and ambulance bays was added in 2009 There is also a 4510 m2 three storey Health Services Centre on the south of the site which was opened in 2005 and accommodates other health programs including home care, mental health, adult day program, human resources, occupational health and safety, volunteers, pre-admission clinic, diabetes education and tele-conferencing.

General Summary:

Structural Summary:The structure consists of 150mm concrete floor slabs poured over steel ties welded to the top flange of steel beam to form a composite structure spanning steel columns carried on belled concrete piles. The roof structure is open web steeljoists carrying metal roof decking. The AHS building foundations are concrete belled piles and grade beams. The building frame in hollow steel columns with wide flange steel beams and open web steel joists Overall the structure is in acceptable condition

Envelope Summary:The building envelope consists of pre-fabricated brick spandrel panels which form a sandwich with a pre-cast concrete back wall with rigid insulation and a vapour barrier between. There are also sections of a conventional brick skin in locations which do not lend themselves to pre-cast units such as stair wells and some confined exterior spaces. The aluminum windows have sealed units. Some of the window units have failed and require replacement. The inverted membrane roof leaks in periods of prolonged rainfall. The AHS building exterior walls are a mix of brick veneer, glazed curtain wall and precast concrete panels. The roof is a 2-ply SBS membrane. Windows are sealed units in aluminum frames

Overall the envelope is in acceptable condition

Interior Summary:Typical interior finishes consist of sheet vinyl floors, acoustic T-bar ceilings and painted drywall partitions. There are some areas of carpet in meeting rooms, medical records and administrative areas. Some sections of both the sheet vinyl, carpet and ceiling tiles are damaged and require replacement. The public furnishings in waiting areas are well used and will require replacement within the next 5 to 10 years. The AHS building interiors are gypsum board partitions. Ceilings are acoustic tiles in T-bar grid and gypsum board ceilings in washrooms. Floor finishes are vinyl tile in corridors, exam and treatment rooms and carpet in offices. Overall the interior is in acceptable condition

Mechanical Summary:Main Building The building mechanical systems include a hydronic heating system with perimeter fin radiation, ceiling radiant panels, unit heaters and fan cabinet heaters. Hot water is derived from 6 boilers located in the main Boiler room. Ventilation systems are made up of 11 air systems which supply heated and cooled air for occupant comfort. Plumbing systems include institutional fixtures. Building automation is made up of pneumatic controls and a Siemens DDC system. The facility is completely sprinklered by wet pipe and pre-action systems. AHS Building Plumbing fixtures include flush valve water closets, wall hung urinals, countertop and wall hung lavatories and stainless steel sinks. There are 2 main roof mounted heating and cooling units which provide ventilation and cooling to the facility, through a ceiling mounted VAV system. Perimeter heating is provided by ceiling mounted, hydronic radiant panels installed around the perimeter of the building. Fire protection consists of a sprinkler system throughout the facility, with wall mounted fire extinguishers and standpipesin the stairwells. Automatic control of the Building HVAC systems is provided by a Siemens DDC system. Overall, the building mechanical systems are in Acceptable condition.

Electrical Summary:Service for the Hospital is 2000A, 347/600V, 3 phase, 4 wire which constitutes the main power distribution system for the facility and accommodates the power supply to the major mechanical equipment. Demand is a little over 65% of the service capacity. Central and localized dry type transformers provide the secondary distribution voltages of 120/208V throughout. A 600kW diesel generator provides the emergency power. A centralized UPS system is provided for the critical hospital and computer equipment. Voltages of outlets at Patient Services Modules are all 120V grounded system, both normal and emergency power, except in CCU and OR which use Isolated Power Supply (IPS) systems on emergency power. Receptacles used for theprovision of health care are hospital grade. Facility staff have replaced most of the outdated T12 fluorescent lamps in the Hospital with 32 W T8 lamps; but the interior fluorescent lighting system still retains most of its magnetic ballasts - about 60% of the lighting fixtures have so far been replaced with electronic ballasts. The lighting system is dominated by valance lights in the corridors and public spaces; all patient rooms are equipped with patient room bed-lights of up and down lights. Exterior lights are high pressure sodium. The two stage, addressable and zoned fire alarm system comes with Emergency Visual and Audio Communication (EVAC) control with the control centre at the front entrance including the control panel, annunciator and graphic. Corridors and patient rooms are equipped with smoke detectors; those in patient rooms are separately annunciated at the nurse's stations. The recently replaced (2004) nurse call system from Australia is a state-of-the-art system providingaudio and visual indication of nurse calls and wireless two-way communications. Other communications and security systems include the PBX telephone system, synchronized clocks, public address, card access, video surveillance, duress alarm and the inevitable data systems. To Be done: 1- Upgrade remaining 40% of interior fluorescent lighting.

The electrical systems are in acceptable condition. Summary for AHS Building: Service to the Health Services Centre is 600A, 347/600V from the Utility-owned pad mounted transformer at the southwest corner of the property running to the Service and Distribution Switchboard in the Penthouse, rated 1200A, 347/600V, 3 phase, 4 wire, and via a 300kVA dry type transformer to an 800A, 120/208V distribution panel and branch circuit panelboards throughout the facility. The 347/600V power system is used mainly for large mechanical equipment including the elevator while the 120/208V is used for building power and lighting. An emergency power feeder is provided from the Hospital to the elevator machine room and is manually transferable. The interior fluorescent lighting is the energy efficient type using electronic ballasts, T8 and compact fluorescent lamps. Emergency lighting is provided by emergency battery packs with integral and remote lighting heads. Exterior lighting is metal halide, wall mounted, photoelectric cell controlled. The fire alarm, telephone, data services, card access, video surveillance is extended from the Hospital. The nurse call and duress alarm systems are separate from the hospital. The electrical systems are in acceptable condition.

The foundations consist of poured reinforced concrete piles with diameters ranging from 400mm to 900mm and bell diameters between 400mm and 2400mm supporting a system of 430mm wide grade beams which vary in depth from 850mm to 2200mm. The piles also support steel stanchions in some locations.

The foundations consist of reinforced poured concrete belled piles with diameter ranging from 400mm to 600mm and bell diameters ranging from 900mm to 1800mm. The piles carry reinforced poured concrete grade beams and internal pile caps.

The basement is a 150mm reinforced concrete slab on grade on 6mil poly vapour barrier on 1000mm compacted gravel fill with an underlay of geotextile. There are also 75mm mud slabs in the crawl spaces under the nursing units.

The slab on grade consists of 177mm and 200mm poured concrete structural slab on grade on a ploy vapour barrier on 1/8" hardboard on 100mm void form.

Consequences of Deferral:Unevenness in slab on grade will persist.

Basement walls are 260mm reinforced poured concrete.

The building frame consists of a composite steel beam and concrete slab system with steel columns encased in concrete on lower levels and gypsum board on the upper levels. The concrete slab is tied into a 150mm reinforced concrete slab by means of steel connectors welded to the top flange and columns.

The building frame consists of 150mmx150mm and 200mmx200mm hollow section steel columns (HSS) on 381mmx500mm grid. Wide flange steel beams span the HSS and carry 800mm deep open web steel joists.

The deck for the inverted membrane roof consists of Type X exterior gypsum board on a 38mm metal deck. The floor decks are concrete slab.

The floor decks consist of concrete topping on steel deck trowelled to receive floor finishes.

There is a steel grate mezzanine floor in the lower level mechanical room with steel grate floor, steel stairs and painted steelpipe handrails. There is also a mezzanine floor in the upper mechanical room where air handling units are located with concrete slab and stairs with painted steel pipe handrails.

There are canopies over the two public entrances: on the west side into the emergency department and the south side into the secondary entrance. The canopies consist of a painted steel frame with a system of runs of peaked glass roof panels. The hollow section steel columns are clad in brick. The fascia is precast concrete. The two canopies are connected by a covered pedestrian walkway which is a lean to steel structure with metal roof deck, steel columns clad in brick and precast concrete fascia.

S2 ENVELOPE

There is a band of precast concrete spandrel panels around the building to sill height. There is also a precast concrete fascia at roof level. The exterior walls below grade such as the lower level areas for the cafeteria and the loading and receiving areas are also clad in precast panels.

There are pre-cast concrete panels on the main floor level of 100mm pre-cast concrete 25mm air space 50mm rigid insulation on self adhesive membrane on 14mm exterior gypsum board sheathing on metal studs.

There is brick masonry skin on stairs and on end walls of the building. It typically consists of brick skin 25mm air space 50mm rigid insulation over self adhesive membrane on 14mm exterior gypsum board sheathing on 150mm metal studs. On the stairs the brick skin consists of brick 25mm airspace 50mm rigid insulation over a self adhesive membrane on an 200mm concrete wall.

The majority of the hospital is clad in prefabricated sandwich spandrel panels consisting of brick facing, 25mm air space, 75mm rigid insulation on an inner leaf of 100mm precast concrete.

There are areas of the exterior wall where a brick skin has been used in locations which do not lend themselves to the installation of prefabricated sandwich panels in irregular spaces or cladding vertical stair shafts. There is an air space behindthe brick skin, rigid insulation and vapour barrier on a back wall of concrete block.

There are prefinished insulated metal panels on all elevations supported on HSS framing with an interior back wall of gypsum board on metal studs.

The brick units and precast units have a foam rod and flexible caulking at the vertical joints.

Service compressed air is provided by two air compressors. Both are Quincy, model 350 and each is driven with a 7.46 kWmotor. Compressed air is passed through an air drier before being distributed to the plant Shop Air system. These compressors also act as back up to the main Controls compressed air system.

Oxygen is provided with a central storage tank located remote from the hospital located in the lower level service yard. It is piped into the hospital and distributed to emergency, patient rooms and surgical suite. A bank of cylinders containing a reserve supply of oxygen is located in the medical gas storage room. This is connected into the central oxygen system. Local alarm panels and isolation valves are provided for this system.

The reverse osmosis equipment is a model E4-8800DLX HGE Osmosis. It has a capacity of 23.2 liters per minute under normal use and 9.1 liters per minute under concentrated use. This system supplies R.O. water to the clean sterile supplies(CSS) unit and to the Steam boilers.

An above ground 14,000 L diesel storage tank stores the diesel fuel for the equipment operating on diesel fuel. Pumps transfer the diesel fuel from the under ground storage tank to the indoor day tank for use by the emergency engine generatorengine and another set of transfer pumps supply diesel fuel to a storage tank for the boilers. The tank is monitored for soil contamination.

Natural gas is the primary fuel source. Carbon steel piping is used for distributing natural gas to gas burning equipment.

A buried branch natural gas line from the municipal service south of the building enters the south face of the building where it is metered. Service from this location is provided to the upper level Mechanical room and the roof to service the natural gas fired equipment.

Three (2 dual fuel & 1 Nat. Gas) low pressure (103 kPa) steam boilers can operate on natural gas or diesel fuel. Each boileris a Cleaver Brooks model CB200-400 (1900). The maximum input to each boiler is approximately 4,900 kW and the maximum output is approximately 3,926 kW based on a fuel efficiency of 80%. High pressure (1034 kPa) steam is provided with two boilers. They supply steam to the laundry, kitchen and sterilizers. Boiler feed pumps return condensate from the deaerator to the respective boiler.

Low pressure (LP) steam distribution is primarily confined to the boiler room, supplying steam to the heat exchangers. A smaller amount of steam piping supplies steam to the humidifiers in the penthouse.

The secondary transformers are 600V - 120/208V, 3 phase, 4 wire, totally enclosed, dry type by Polygon Magnatek. They are typically floor mounted on concrete housekeeping pads (some are suspended from the ceiling) in electrical rooms throughout the Hospital. Sizes were two 300kVA, two 225 kVA, three 150 KVA, two 112.5kVA, one 45 kVA, one 30 kVA and one 15 kVA.

The free standing, front and rear accessible Service and Distribution Switchboard (Siemens) is rated at 2000A, 347/600V, 3 phase, 4 wire, full capacity neutral. The service is metered by the Utility but has its own "customer" digital meter (the digital meter not been worked 1992). The overcurrent protective devices of the switchboard are industrial Air Circuit Breakers (ACB), draw-out type, with solid state tripping devices - long time, short time, instantaneous and ground. The main breaker has a 2000A frame with 2000A trip (long time setting); the 12 distribution breakers are all rated 800A (frame) with varying trip ratings. Interrupting capacity of the breakers is at 50 kA, rms symmetrical.

Located in the Penthouse Mechanical/Electrical Room, the Service and Distribution Switchboard is a free-standing switchboard with ventilated enclosure and sprinkler shield, manufactured by Square D, rated 600A, 347/600V, 3 phase, 4 wire with a 600A main Breaker with solid state over current protection device and distribution breakers consisting of moldedcase, solid state circuit breakers ranging 100A - 250A. equipped with its own digital meter.

Both main and secondary power distribution systems are solidly grounded systems. Ground risers (grounding bars in electrical rooms connected to the building main ground) allow transformer secondary neutrals to be grounded directly to the main ground. There is a grounding grid of copper alloy ground rods underneath the main electrical room.

The distribution panels are set as central distribution panels at 600 volt and sub distribution panels at 120/208 volt. Panels are manufactured by Siemens and ITE. At 600 volt, panel sizes were three 600 Amp, two 800 Amp and one 225 Amp. 21 sub distribution panels rated at 225 Amp 347/600 volt are located as required in the building. Wiring method generally is cables in conduits (rigid or EMT), concealed in finished area and surface mounted in utility areas. Isolated Power System (IPS) is used in Surgical Suites and Coronary Care Units (CCU), where PVC conduits is employed. Using a 120V-120V isolating transformer, the 5 kVA, IPS is ungrounded and is constantly monitored for leakages. Each room has its dedicated IPS and its own customized Patient Service Module (PSM), complete with grounding jacks.. Receptacles are hospital grade for health care use and premium specification grade otherwise. Receptacles serviced from emergency power are separately identified (red). Furthermore, patient rooms are all fitted with a PSM per bed location - receptacles are 120V grounded, typically 4 normal power and 2 emergency power.

Branch wiring is predominantly cables in conduit, concealed in finished areas and surface mounted in utility areas. Pac poles are used sometimes to bring power and communication cables to the desk level. The elevator enjoys two sources of power supply (600V, 3 phase) - normal power from the Centre and emergency power from the Hospital - manually transferable by interlocking disconnects.

Lighting in the Hospital is locally controlled by line voltage (120V or 347V) switches. Corridor lights are controlled from the Nurses' Station. One exception is the switching of the patient bed-light, which is controlled by low voltage ( the relay is located in the ceiling space above). This permits switching of the patient bed-light via the pillow speaker of the nurse call bedside station and turning on the up-light portion of the bed-light from the corridor for bed checking.

The interior lighting system is controlled locally by line voltage switches. Some motion sensor controlled switching are usedin utility rooms.

Recessed incandescent fixtures are present in the Diagnostic Imaging area (X-ray Room). These are pot-lights with dimmable incandescent PAR lamps.

The fluorescent lighting system is comprised of 32W T8 and 34W T12 lamps and magnetic ballasts with some compact fluorescent lamps, also magnetic ballasts. T8 lamps are used to replace T12 lamps as they burn out. There are also electronic ballasts (approximately 60%) which replaced the magnetic ballasts when they failed. Exit signs are internally illuminated exit lights, polycarbonate housings, stencil faces, with LED lamps and built-in battery packs.

Examination lights are ceiling mounted in ICU and CCU - 2 each in ICU rooms & 1each in CCU rooms. They are the high intensity halogen lamp type, recessed in the ceiling. Examination lights in Procedure Rooms are specialized wall mounted incandescent lights with extension arms.

The Siemens Fire Alarm System is a two stage, addressable and zoned fire detection and alarm system with an Emergency Visual and Audio Communication (EVAC) feature. The system uses manual stations, heat and smoke detectors as detection devices and tones (first and second stage tones) through the fire alarm system loudspeakers as signaling devices - these tones will be silenced, after a predetermined period, on EVAC announcements. Patient rooms areprovided with their own smoke detectors individually annunciated at the Nurses' Station Annunciators. Ancillary devices include hold open devices for interconnecting doors and controlling devices for fan shut downs. Although zoned the system is able to address/diagnose the individual affected detection and signaling devices for alarm and trouble conditions. The control panel with its integral annunciator and EVAC control centre, accompanied by an active graphic (with colour coded zones and indicating LED lights) is located at the main entrance of the Hospital. The system is monitored by AHS central. The fire alarm system associated with the Halon activation procedure - smoke detector activation, time delay, discharge - isa Cerberus Pyrotronic system. There are only two of them - Electric and Telecommunication rooms. They are not part of the system but interfaced with the main fire alarm system to annunciate.

The fire alarm system is extended from the Hospital's Seimens system using manual stations, heat and smoke detectors (including duct smoke detectors) as detection devices and audio/visual (strobe/speaker) signaling devices.

The telephone system is a Nortel Meridian PBX system. It provides telephone services for the whole hospital, including the portable phones associated with the Nurse Call System but does not include the patient private phones, which is handled by a private company - Vista Care - who makes the connections/disconnections and is responsible for all the billings (a "Patient Room Phones" panel in the Telephone Room facilitates the procedure). The system is expandable.

The Hospital's telephone system (Nortel's Meridian) is extended from the hospital.

The Nurse Call system is a PC (Dell) based, all solid state nurse call system from Austco (manufactured in Australia), designed for health care facilities. A desk top master station provides two-way communications to the patient rooms and includes a free standing monitor, which registers all activities of the unit. Nurse call devices include wall mounted bedside stations with pillow speakers, staffcall buttons and Code Blue buttons, washroom emergency pull stations, which double as shower pull stations, staff presence buttons and corridor indicating lights. The pillow speaker provides the patient with more intimate communication with the master station, or with a portable telephone carried by care givers, having a loudspeaker and microphone of its own (duplicating the wall unit), radio station selections and volume control, and control of the bed-head light. Communication with the patient on the nurse call system may also be conducted through portable telephones (SpectraLink).

The original public address system is by TOA, a zoned distribution system with many small amplifiers (as opposed to having a large amplifier with zone relays). In 2002, the system was modified and expanded using Sony equipment, but retaining the original TOA equipment. The system provides all page or selective zoned paging to the Hospital. It is feasible to expand. Tied to telephone system.

The patient television rental is carried out by Vista Care, a private company. Television sets are small bedside sets on wall brackets with swivel extension arms.

A Chubb Security system provides the "Duress Alarm", a staff security system that transmits a duress signal from a pendant, carried by the staff, to a localized receiver which goes through the telephone line to Chubb Security. The transmission from the receiver identifies the location of the duress signal and Chubb Security is able to relay this message back to the Hospital's security personnel. The system is monitored 24 hours a day.

A wireless Duress Alarm system is used at Mental Health (extended to Adult Care - one pendant). The system is a Spider Alert System using pendants worn by staff. The control panel is located in the Communications room on the main floor. An alarm is activated when the pendant is pushed. When activated, the alarm goes to the Hospital's Security Office.

The emergency power system is a turbo-charged, diesel engine driven, direct coupled engine-generator unit (Waterous), providing 600 kW (750 kVA @ 0.8 power factor), 347/600V, 3 phase, 4 wire capacity. The unit has pneumatic and electric starting systems. The engine mounted control panel was replaced in 2004 by a wall mounted panel. The engine was rebuilt in 2010. The generator is approximately 85% loaded on essential services but is capable of being loaded to 100% by careful connections to the normal power supply. The physiotherapy department equipment includes: exercise steps, parallel bars, universal gym, 2 plinths, treadmill and a variety of free weights for arm and leg rehabilitation exercises. There is also a staff fitness room which is equipped with treadmills and a variety of resistance training machines.

There is a variety of fixed casework throughout the hospital. There are stainless steel counters with sinks in the laboratory, central sterile supply room, surgical suite, food services kitchen and other areas. There are plastic laminate shelves, counters, wall cupboards and based units in offices, nursing stations, work areas, store rooms for administrative and medical supplies. Estimates are based on gross floor area.

The prime consultant is ARUP DATTA ARCHITECT LTD. The evaluation date was December 17, 2013 Portions surgical and sterilized portions of the hospital were closed off during the site observation. The 2 storey and basement Hospital building is clad in brick masonry with concrete panels on the lower portion of the building. Interior finishes include resilient sheet flooring throughout. Interior partitions are steel stud construction with paintedwall finish, corners consists of plastic wall corner guards and wood stained guard rails. Ceiling finishes included t-bar throughout most of the hospital. Rooms that require sterilization have epoxy finishes with fiberglass paneling. The 3 storey Alberta Health Services building is linked to the hospital with a pedestrian link. The exterior wall consist of a mix of brick, glazed curtain wall and precast concrete panels. The interiors are predominantly gypsum board partition with some aluminum glazed screens. T-bar ceilings, vinyl tile and carpet flooring
