RE-Tender documents are available on line from 18.05.2018 (https://esictenders.eproc.in). Bidders have to deposit the Earnest Money Rs 250000.00 (Two lacs fifty thousand only) Deposit as detailed above through DD/ Bankers Cheques drawn in favour of “ESIC Fund A/C No. 1” payable at Varanasi.

The interested tenderers should upload duly filled tender form and their bids along with scanned copies of all the relevant certificates, documents, etc. in support of their technical & price bids all duly signed on the website https://esictenders.eproc.in latest by 08.06.2018 up to 01:00 PM.

EMD and Undertaking (Annexure-V) shall also be submitted physically (Physical hard copy) along with Demand Draft/ Banker‟s Cheque of Earnest Money Deposit (E.M.D.) as stated above in favour of ESIC Fund Account No.1.

The envelope containing EMD and Undertaking (Annexure-V) should be dropped in the Tender Box kept in Office of Medical Superintendent, ESIC Hospital, Pandeypur before closing date and time. The technical bids will be opened online on 08.06.2018 at 2.00 PM.

Hard Copy of EMD and Undertaking (Annexure-V) received late will not be

considered. Proof of postage/courier won‟t be considered as a claim for timely submission of tender. All the bids received will be opened on the same day at 2.00 pm. Bidders or their authorized representative (with authority letter & ID Proof) may be present if they wish to be. In case 08.06.2018 is declared a holiday, bids will be opened on next working day at the same time & venue. Bid without Earnest Money deposit will not be accepted.

Annexure I

TERMS AND CONDITIONS FOR THE SUPPLY OF HOSPITAL FURNITURE

1) Sealed Tenders will be received upto and including the date stipulated by the Medical Superintendent, ESICHospital, Pandeypur, Varanasi 221002 for the purchase of Hospital furnitures.

2) The sealed envelope containing the completed tender duly superscribed as 'TENDER FOR SUPPLY OF HOSPITAL FURNITURES should be addressed to The Medical Superintendent, ESIC Hospital, Pandeypur, Varanasi.

3) Postal delays if any, will not be condoned. 4.) Tenders will be opened at the office of the Medical Superintendent, ESIC Hospital, Pandeypur, Varanasi 221002 on the stipulated date & time in the presence of the tenderer s / representatives who choose to be present.

5) The rates quoted should be F.O.R Destination, freight prepaid in full, no Handling, Clearing or transport charges etc. will be paid. While quoting the rates it should be specifically stated that the rates are excluding the GST/Customs Duty. The delivery should be at the Medical Stores, Medical Superintendent, ESIC Hospital, Pandeypur, Varanasi between 10 AM to 3 PM except Saturday and holiday.

6) The tender should be type written and every correction in the tender should invariably be attested

by full signature by the tenderer with date before submission of the tenders to the authorities concerned failing which the tender is liable for rejection.

7) The rates quoted in the tender must be both in words and in figures.

8) A Certificate that the quoted rates are not been supplied to any other organization at a rate, lower than quoted here should be enclosed.

9) Copies of purchase order for having supplied to Government Hospitals / reputed institutions (of the quoted equipment) should be enclosed with certificate duly authenticated from any other users of the equipment on satisfactory performance (Anx.VI) is to be enclosed.

10) Firms should also give a certificate that spares/consumables/ of the items quoted will be freely available at least for five years after the warranty period.

11) Successful bidder shall not be entitled to any rate revision of price for any reason except that allowed by Government of India.

12) The firm service center to which correspondence during the Period of finalization of contract, warranty, could be made and accordingly notifies the address of Office.

13) Undertaking on Guarantee/ Warranty (Comprehensive) for 3 years after purchase has to be mentioned in the Document.

14) The firm should undertake to enter into Annual Maintenance Contract for at least for 5 years after completion of warranty and accordingly quote the rates in the tender document.

15) The rates for AMC (Labour) should not exceed 3% and for CMC should not exceed 10% of the unit cost and should remain fixed for five years.

16) The Medical Superintendent, ESIC Hospital, Pandeypur, Varanasi, will have the liberty to terminate the tender proceedings without assigning any reasons thereof at any level of tender proceeding.

17) The bidder will not be entitled for any compensation for whatsoever in respect of such termination.

18) One copy containing terms and conditions and schedule of this tender should be signed by the tenderer at the bottom of each page with the office seal duly affixed and returned along with the tender.

19) Declaration form and Company profile formats enclosed must be filled in and signed by tenderer and enclosed along with the tender. Tenders received without the Declaration form shall

not be considered. 20) No articles shall be supplied to the Hospital except on requisition in writing signed by the

Medical Superintendent or by an officer authorized by him in writing to do so. 21) The contract should not be sublet without the prior written permission of the Medical

Superintendent. 22) TENDER DOCUMENTS THAT ARE BEING SUBMITTED IN PERSON ARE TO BE

DROPPED IN THE TENDER BOX KEPT IN office of Medical Superintendent, ESIC Hospital, Pandeypur, Varanasi 221002.

23) Bidders in their own interest shall ascertain the eligibility of whatsoever concessions and

exemptions eligible and applicable to the hospital and shall advise the purchaser and quote accordingly. Bidders shall indicate the actual amount eg. Octroi, GST etc. which becomes otherwise payable in the extreme event of hospital authorities not in a position to release certificates like octroi exemption certificate, Form D etc..

24) Bid Security (EMD)

The tender must be accompanied by EMD (3% of total tender value) in the form of Demand Draft only drawn in favour of “ESI Fund account No-1”, payable at Varanasi. The tenders not accompanied by EMD is liable for rejection. Advance stamp receipt for the refund of EMD should be enclosed with the tender documents.

26) PERFORMANCE SECURITY The successful bidder should furnish Performance Security Deposit @ 10% of the value of the contract. 27) VALIDITY PERIOD OF TENDER. The rates quoted should be valid for one year from the date of approval of the Tender and the quantity mentioned is on the basis of present requirement which are to be supplied within the stipulated period mentioned in the tender. During the currency of Tender orders are to be executed by the successful Tenderer at the tender rate as per future requirements. 28) DEMONSTRATION OF FURNITURES The visit to manufacturer‟s factory/ plant if required will be undertaken by outside Inspection agency or team of expert by tendering authority to verify the capability, production facilities and to verify infrastructure if required can be taken by the Hospital. Demonstration of the quoted product should be mandatory and it can be carried out before price bid opening 29) Penalty Clause - Supply is to be made within three to four weeks after placement of supply order. For late supply cumulative penalty for every week @ 2% of value of order will be recovered. 30) The payment towards the supply is subject to receipt of inspection note/ performance certificate to the effect that the items supplied are conforming to the supply order placed. 31) Bidder should be ISO 9001: 2015, ISO 13485:2003, ISO 14001:2015 and & BS OHSAS 18001:2007 certified. (Valid certificate should be attached with technical bid). 32) All Mild steel parts should be In-house pretreated and anti-microbial epoxy powder coated 50 to 100 microns thickness. 33) Anti-microbial oven baked powder coating process for achieving smooth, uniform finish and superior bonding properties which prevent growth of bacteria. 34) Salt spray test, Impact test to be carried out in manufacturing premises. Incoming material inspection process for 100% of all raw materials. 35) Stainless steel should be 304 grade and validated by Govt. approved and NABL certified laboratory. Test reports should be attached with technical bid.

OTHER TERMS AND CONDITIONS Evaluation of technical and financial bid

1. The Tenderers are required to upload two separate Bids i.e. - Technical and Financial, as per preformed available online. Financial bid has to be uploaded online only.

2. Each attached document should be signed and stamped by bidder or its authorized representative of

Tenderer. 3. The competent authority of ESIC reserves the right to reject all or any tender in whole or in part,

without assigning any reason thereof. 4. The financial bid of the bidders, whose technical bid is found to be responsive as per the eligibility

and other tender conditions, will be opened in the presence of the bidders, who choose to attend the opening of financial bid. The date and time of opening shall be intimated.

5. Lowest financial bid will be evaluated based on overall lowest rate quoted by the bidder. 6. The earnest money shall be refunded to the unsuccessful Tenderers after finalization of the contract.

It shall be refunded to the successful Tenderer on receipt of performance security deposit. No interest is payable on the EMD.

7. The bid shall be valid for 180 days from the date of opening.

8. Tender incomplete or Conditional in any form will be rejected outright.

9. In case the successful tenderer declines the offer of Contract, for whatsoever reason(s), his EMD will

be forfeited. 10. A formal contract shall be entered into with the successful tenderer. In this contract, the successful

tenderer shall be defined as Contractor.

11. Tenderer shall have annual audited turnover of minimum two crore in last three AY 2015-2016, 2016-2017, 2017-2018.

12. Medical Superintendent, ESIC Hospital, Pandeypur Varanasi reserves the right to decrease/increase the quantity of any item or reject/accept any or all tenders without assigning any reason thereof at any time of tender process. No correspondence (verbal/written) will be entertained in this regard.

Performance Security Deposit The successful tenderer will have to deposit a Performance Security Deposit of 10% of total

tender value by way of Bank Guarantee from a scheduled commercial bank in favour of

“Medical Superintendent, ESIC Hospital Varanasi” valid for 60 days beyond the expiry of

contract. The successful Tenderer will have to deposit the Performance Security Deposit and commence

the work within 15 days of acceptance of tender. Otherwise the contract will be cancelled and

EMD will be forfeited.

The undersigned certifies that I have gone through the terms and conditions mentioned in the tender document including annexure and undertake to comply with them. The rates quoted by me/us are valid and binding on me/us for acceptance for the period of one year from date of opening of tender. 2. It is certified that rate quoted are the lowest quoted for any institution/hospital in India. 3. To forfeit the security money deposited by me/us if any delay occur on my /agent's part or fails to supply the article at the appointed place and time and of the desired specification (B) I/we undertake that I/we will be in position to provide annual maintenance contract and comprehensive maintenance contract (AMC and CMC), spare parts. And consumables for 5 years after completion of guarantee/warranty period. I /we also undertake to keep the equipment in running order throughout the years under warranty/ guarantee/ AMC/CMC and in case of equipment going out of order the fault will be attended within 24 hours of lodging the complaint. The firm shall ensure the machine is set rights within 7 days of intimation. However the tenderer have to arrange similar equipment as a standby at his cost and risk in case of breakdown of machine is not set in working condition. Failing which, a penalty of 0.25% of the total no cost of the equipment per day for the period equipment remains out of order be levied on me /us 5. There is no vigilance /CBI case pending against the firm /supplier. Our firm /company is also not blacklisted by Govt /Public /Pvt agency. 6. On inspection if any of the articles is not found as per supply order, it shall be replaced by me/us in time as asked for, to prevent any inconvenience at my/our own expenses. 7. I/we hereby undertake to supply the items as per specifications and directions given in supply order within the stipulated time period. 8. I/we undertake to provide guarantee /warranty as mentioned in specifications from the date of satisfactory installation and inspection. I also undertake that I will maintain the equipment during this period and replace the defected parts free of cost, if necessary. 9. The taxes quoted in price bid is as per Act/Law /Rule /Regulation applicable to the tendered equipment. 10. I/we understand that Medical Superintendent, ESIC Hospital, Pandeypur, Varanasi has the right to accept or reject any or all the tenders without assigning any reason (s) thereof. 11.
