RATION CARD MEMBER ADDITION( BIRTH)

The Ration Card is issued by the State Government. It is a very important document that serves as an identity and

nationality proof in India. It helps categories families into Above Poverty Line (APL) and Below Poverty Line (BPL) on the basis of their economic status. It also provides citizens with the right to purchase goods from rationing shops at subsidized rates.

This Service is used for Adding New Members in Ration Card. The Processing Steps and Mandatory Documents for Applying Member Addition as shown below.

Note: The asterisk (*) denotes mandatory requirement of documents. Apply for Ration Card Member Addition (Birth): This section contains instructions for the operators of the Meeseva web application to apply for Ration Card Member Addition (Birth) Service.

1) In Meeseva home screen, under List of Services, select Civil Supplies Department as depicted in Figure 1.

2) Select Ration Card Member Addition (Birth) Service in Civil Supplies Department as depicted in Figure 2.

33) Now Ration Card Member Addition (Birth) Request Screen will be displayed as depicted in Figure 3.

4) Enter Ration Card no and click Get Details button as depicted in Figure 4.

5) It Fetch data from Department Server and display details related to that ration card no. as depicted in Figure 5.

6) Enter New Member details such as Member Name, Mother Name, Date of Birth (DD/MM/YYYY), Gender, Age, Father Name, Spouse Name, EID, UID, Relation with Head, Reason, Is New Member, and Action and click Add New member Button as depicted in Figure 6.

7) Enter all the Informant details such as informant Name, Relation, Delivery Type, Mobile No and Proof Document as depicted in Figure 7.

8) Collect the necessary supporting documents as indicated in the House Hold Head Modifications s request page. Scan the documents and upload into the system, as depicted in Figure 8. Note:- Here Proof documents should be uploaded based on Selection.

Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the system. 9) Click Show Payment. Another window appears requesting to Confirm Payment as depicted in figure 10.

11) Collect the amount from the Citizen and click Confirm Payment to submit the request as depicted in figure 12.

On confirmation, a receipt will be generated as depicted in the following figure. The receipt will contain the Certificate delivery date. Note: Ensure that you have sufficient balance with your service provider (SCA), else the system will not accept the request.

Note: - On submission of the request, Citizen Receives Message. Like your Request for Ration Card Member Addition has been

entered vide Application Number, Transaction Id and sent to Dept.

16) On submission, the request will be sent to the work flow of the respective Department for further processing.

13) Once the request is approved, then the citizen need to collect the Ration Card from franchisee where he/she applied. Note: - Print Ration Card Service is used for printing the Ration Card.
